Rotation Curve Fitting Model
One key piece of evidence for dark matter is the flat rotation curve problem:
the disagreement between measured galactic rotation curves and their luminous
mass. A novel solution to this problem is presented here. A model of
relativistic frame effects on Doppler shifts due to the slightly curved frames
of an emitting galaxy and the Milky Way is derived. This model predicts
observed Doppler shifted spectra (in excess of the luminous mass) based only on
the observed luminous matter profile and one free model parameter. Fits to the
175 galaxies reported in the SPARC database of galactic rotation profiles and
accurate photometry measurements are compared between this novel model and dark
matter and MOND (RAR) models. We find on the SPARC sample of 175 galaxies; that
MOND-RAR has an average reduced chisquare of for 175 galaxies
fitted, the isothermal dark matter model has for 165 galaxies
fitted, and the new model we present has for 172 galaxies
fitted. Implications of this model are discussed.Comment: 27 pages, 9 figure
Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries
Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P < 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction. Opioid-free discharge analgesia should be adopted routinely
Omecamtiv mecarbil in chronic heart failure with reduced ejection fraction, GALACTICâHF: baseline characteristics and comparison with contemporary clinical trials
Aims:
The safety and efficacy of the novel selective cardiac myosin activator, omecamtiv mecarbil, in patients with heart failure with reduced ejection fraction (HFrEF) is tested in the Global Approach to Lowering Adverse Cardiac outcomes Through Improving Contractility in Heart Failure (GALACTICâHF) trial. Here we describe the baseline characteristics of participants in GALACTICâHF and how these compare with other contemporary trials.
Methods and Results:
Adults with established HFrEF, New York Heart Association functional class (NYHA)ââ„âII, EF â€35%, elevated natriuretic peptides and either current hospitalization for HF or history of hospitalization/ emergency department visit for HF within a year were randomized to either placebo or omecamtiv mecarbil (pharmacokineticâguided dosing: 25, 37.5 or 50âmg bid). 8256 patients [male (79%), nonâwhite (22%), mean age 65âyears] were enrolled with a mean EF 27%, ischemic etiology in 54%, NYHA II 53% and III/IV 47%, and median NTâproBNP 1971âpg/mL. HF therapies at baseline were among the most effectively employed in contemporary HF trials. GALACTICâHF randomized patients representative of recent HF registries and trials with substantial numbers of patients also having characteristics understudied in previous trials including more from North America (n = 1386), enrolled as inpatients (n = 2084), systolic blood pressureâ<â100âmmHg (n = 1127), estimated glomerular filtration rate <â30âmL/min/1.73 m2 (n = 528), and treated with sacubitrilâvalsartan at baseline (n = 1594).
Conclusions:
GALACTICâHF enrolled a wellâtreated, highârisk population from both inpatient and outpatient settings, which will provide a definitive evaluation of the efficacy and safety of this novel therapy, as well as informing its potential future implementation
Pharmacist-delivered perinatal mental healthcare: exploring stakeholder perspectives and training needs
Perinatal depression (PND) affects 1 in 5 Australian women with potentially long-term consequences on women and their families. PND screening enables identification of at-risk women for referral and support. Pharmacists are accessible primary care providers who interact regularly with perinatal women and may assist existing PND care providers in screening delivery. The aim of this thesis is to build the evidence base surrounding potential roles for pharmacists in perinatal mental healthcare.
A systematic review was conducted to identify international PND screening recommendations. Interviews with Australian medical, pharmacy and nursing program providers explored the extent of Mental Health First Aid (MHFA) training and assessment in curricula. Interviews with perinatal women explored their acceptability towards pharmacist-delivered PND care.
The systematic review identified that the majority of recommendations supported PND screening. Generic terms, including âcliniciansâ and âhealthcare professionalsâ, were often used when referring to the providers responsible for PND screening. Pharmacy program providers were more likely to implement MHFA training than their medicine and nursing counterparts. Addressing barriers to integration of MHFA training including cost, time and staffing availability may facilitate training delivery. Perinatal womenâs experience with existing PND services, patient-pharmacist relationships, knowledge of pharmacistsâ roles and pharmacistsâ visibility in PND care were factors influencing perinatal womenâs acceptability of pharmacist-led PND screening.
Although pharmacists are not widely recognised as PND screening providers, mental health training is increasingly embedded in pharmacy curricula and women are accepting of pharmacist-delivered PND screening, indicating the potential to expand pharmacistsâ roles in this area. Further research is required to explore the development and evaluation of pharmacist-specific PND screening training

Institutional trading before dividend reduction announcements
Using a large proprietary database of transaction-level institutional trades for the 1997-2011 period, we find that institutional investors are net sellers in dividend reduction firms during the two quarters prior to the announcements. They also trade more intensively in firms that do not prepare the market for dividend cuts or that have greater information asymmetry. Trading by both pension plan sponsors and money managers affects the market's reaction to the dividend reduction announcements. Finally, all institutional investors earn significant profits by trading in the two quarters prior to the announcements, and money managers outperform pension plan sponsors
Accounting for Small and Medium Sized Enterprises in Sweden
The area of measurement principles is considered one of the most controversial aspects of the Exposure Draft for Small and Medium sized enterprises. Although fair value measurement has been mandated for some assets and liabilities under IFRS for SMEs, the issues of when fair value accounting is appropriate and when is not is still far from resolved. Generally, the effect of application of fair value accounting varies among different countries. Based on IASBâs statement and Swedish economic and accounting environment, we find interesting to find out whether the fair value measurement principles in proposed IFRS for SMEs is suitable for companies in Sweden. The investigation will address specific questions about measurement simplifications that the board adopted on which the Exposure Draft seeks guidance. The main aim of this study is to examine the accounting measurement principles for SMEs under IFRS for SMEs and Swedish accounting standards. We are interested in finding out which measurement principles are suitable for small and medium sized enterprises in Sweden. Using Sweden as the case study, this paper offers data and useful information on the perceptions of internal users on accounting valuation issues and therefore has significant research implications for standard setters and policy makers. The findings from this research could also offer implications on financial reporting issues to countries with similar economic and accounting environment. As the nature of our thesis required direct contacts with our respondents and the fact that our subject considered as difficult for most people, we chose to use the Strategic Approach. This approach offered us with the possibility to work with a small number of respondents, which suited us well given the time limit of this thesis. Based on the character of our thesis, to get an understanding rather than to calculate, we chose to build our research strategy on qualitative interviewing. Of several interview methods that Malcolm Smith (2003)1 depicted, we applied the semi-structured interview which offers researcher to have a list of questions that cover a fairly specific topic. Outgoing from the practical analysis, the conclusion can be drawn that historical cost accounting is more suitable for non public private entities than fair value accounting in Sweden. In addition, most SMEs do not have sufficient accounting knowledge and practices, and they do not have resources for preparing financial statements in compliance with IFRS for SMEs. However, the implementation of historical cost accounting is not without any problems due to difficulties in revenues and expenses matching. In many situations, historical prices become irrelevant in evaluating companyâs financial position
