    Colonial Natal, 1838 to 1880: The Making of a South African Settlementsystem. (Volumes I and II).

    The study of colonial settlement policy is one way for human geographers to understand how people shape the places they inhabit. The major concern of this study was to determine the impact of Dutch-Afrikaner, Native, and British cultures on the Natal colonial settlement system between 1838 and 1879. A prolonged era (1844-1879) of British settlement policy was preceded by a brief period of Dutch-Afrikaner hegemony (1838-1843). I test the hypothesis that Natal\u27s British settlement system was a syncretic compromise between European and South African (Native and Dutch-Afrikaner) adaptations. My interpretation turns on frontier processes of transfer and borrowing. I suggest a threefold regionalization (sociogeographic entities), reflecting the relative importance of transfer and/or borrowing processes to explain the diversity of the Natal settlement system. British cultural transfer and persistence was dominant in coastal Natal and characterized the first sociogeographic region centered on Durban. The second sociogeographic region, centered on the capital Pietermaritzburg in midland Natal was a hearth of British experimentation. Here processes of transfer and borrowing (from Dutch-Afrikaner and Natives) coupled with adaptation co-existed. The third sociogeographic region in interior Natal (area north of and including Weenen) exemplifies the persistence of Dutch-Afrikaner settlement ideas and institutions (laagers, large erven sizes, dorp names, and extensive pastoral landholdings) and their adoption by the British. Here British borrowing of Dutch-Afrikaner settlement ideas outweighed the wholesale transfer and implantation of British ideas. The Natalian settlement system is considered a syncretic compromise between European (British) and South African (Native and Dutch-Afrikaner) adaptations--Natal was not a European (Dutch or British) transplantation from northwest Europe. The settlement of Natal was modelled after the compact agricultural settlement ethos of Wakefield and the colonial reformers. The successful settlement of Anglo agriculturalists on 20-acre allotments prescribed by the colonial reformers\u27 settlement model was, however, modified and adapted by the presence of approximately 100,000 semi-nomadic Bantu and Dutch-Afrikaner pastoralists.     Learning Deep Similarity Metric for 3D MR-TRUS Registration

    Purpose: The fusion of transrectal ultrasound (TRUS) and magnetic resonance (MR) images for guiding targeted prostate biopsy has significantly improved the biopsy yield of aggressive cancers. A key component of MR-TRUS fusion is image registration. However, it is very challenging to obtain a robust automatic MR-TRUS registration due to the large appearance difference between the two imaging modalities. The work presented in this paper aims to tackle this problem by addressing two challenges: (i) the definition of a suitable similarity metric and (ii) the determination of a suitable optimization strategy. Methods: This work proposes the use of a deep convolutional neural network to learn a similarity metric for MR-TRUS registration. We also use a composite optimization strategy that explores the solution space in order to search for a suitable initialization for the second-order optimization of the learned metric. Further, a multi-pass approach is used in order to smooth the metric for optimization. Results: The learned similarity metric outperforms the classical mutual information and also the state-of-the-art MIND feature based methods. The results indicate that the overall registration framework has a large capture range. The proposed deep similarity metric based approach obtained a mean TRE of 3.86mm (with an initial TRE of 16mm) for this challenging problem. Conclusion: A similarity metric that is learned using a deep neural network can be used to assess the quality of any given image registration and can be used in conjunction with the aforementioned optimization framework to perform automatic registration that is robust to poor initialization.Comment: To appear on IJCAR

    Segmental Kinematic Analysis of Planovalgus Feet during Walking in Children with Cerebral Palsy

    Pes planovalgus (flatfoot) is a common deformity among children with cerebral palsy. The Milwaukee Foot Model (MFM), a multi-segmental kinematic foot model, which uses radiography to align the underlying bony anatomy with reflective surface markers, was used to evaluate 20 pediatric participants (30 feet) with planovalgus secondary to cerebral palsy prior to surgery. Three-dimensional kinematics of the tibia, hindfoot, forefoot, and hallux segments are reported and compared to an age-matched control set of typically-developing children. Most results were consistent with known characteristics of the deformity and showed decreased plantar flexion of the forefoot relative to hindfoot, increased forefoot abduction, and decreased ranges of motion during push-off in the planovalgus group. Interestingly, while forefoot characteristics were uniformly distributed in a common direction in the transverse plane, there was marked variability of forefoot and hindfoot coronal plane and hindfoot transverse plane positioning. The key finding of these data was the radiographic indexing of the MFM was able to show flat feet in cerebral palsy do not always demonstrate more hindfoot eversion than the typically-developing hindfoot. The coronal plane kinematics of the hindfoot show cases planovalgus feet with the hindfoot in inversion, eversion, and neutral. Along with other metrics, the MFM can be a valuable tool for monitoring kinematic deformity, facilitating clinical decision making, and providing a quantitative analysis of surgical effects on the planovalgus foot

    Statin therapy in critical illness : an international survey of intensive care physicians' opinions, attitudes and practice

    Background Pleotropic effects of statins on inflammation are hypothesised to attenuate the severity of and possibly prevent the occurrence of the host inflammatory response to pathogen and infection-related acute organ failure. We conducted an international survey of intensive care physicians in Australia, New Zealand (ANZ) and United Kingdom (UK). The aims of the survey were to assess the current prescribing practice patterns, attitudes towards prescribing statin therapy in critically ill patients and opinions on the need for an interventional trial of statin therapy in critically ill patients. Methods Survey questions were developed through an iterative process. An expert group reviewed the resulting 26 items for face and content validity and clarity. The questions were further refined following pilot testing by ICU physicians from Australia, Canada and the UK. We used the online Smart SurveyTM software to administer the survey. Results Of 239 respondents (62 from ANZ and 177 from UK) 58% worked in teaching hospitals; most (78.2%) practised in ‘closed’ units with a mixed medical and surgical case mix (71.0%). The most frequently prescribed statins were simvastatin (77.6%) in the UK and atorvastatin (66.1%) in ANZ. The main reasons cited to explain the choice of statin were preadmission prescription and pharmacy availability. Most respondents reported never starting statins to prevent (65.3%) or treat (89.1%) organ dysfunction. Only a minority (10%) disagreed with a statement that the risks of major side effects of statins when prescribed in critically ill patients were low. The majority (84.5%) of respondents strongly agreed that a clinical trial of statins for prevention is needed. More than half (56.5%) favoured rates of organ failure as the primary outcome for such a trial, while a minority (40.6%) favoured mortality. Conclusions Despite differences in type of statins prescribed, critical care physicians in the UK and ANZ reported similar prescription practices. Respondents from both communities agreed that a trial is needed to test whether statins can prevent the onset of new organ failure in patients with sepsis

    Comparison between transperineal ultrasound and digital dectection of levator ani trauma : can we improve the odds?

    Aims To investigate the predictive ability of four digital assessment parameters to detect levator ani (LA) muscle defects (avulsion injury) and compare these to transperineal tomographic ultrasound images. Methods This was an observational study imbedded in a larger quasi-experimental cohort study for women with urinary incontinence. Seventy-two women, ≄60 years who had attended or were going to attend physiotherapy for treatment of urinary incontinence, were included in the study. Inclusion criteria from the parent study were symptoms of stress, urge or both types of urinary incontinence. The predictive ability of the following digital parameters: direct palpation of a discontinuity of the LA muscle from insertion on the pubic ramus; palpation of the distance between the muscle insertion sites; palpation of LA strength; palpation of LA tone, were analyzed against findings from tomographic transperineal ultrasound images. Correlation between methods was measured using Cohen's kappa for each of the individual parameters. Results Seventeen women (24%) presented with a complete or partial avulsion of the puborectalis muscle as diagnosed with tomographic ultrasound imaging. Nine women (13%) had complete avulsions, one of which was bilateral. The predictive ability of the digital assessment parameters varied from poor (k = 0.187, 95% CI [0.02–0.36]) to moderate (k = 0.569, 95% CI [0.31–0.83]). The new parameter of ‘width between insertion sites’ performed best. Conclusions Adding the parameter of “width between insertion sites” appears to enhance our ability to detect avulsion of the levator ani (LA) muscle by digital examination however it does not distinguish between unilateral or bilateral avulsion

    Effects of Spinal Fusion for Idiopathic Scoliosis on Lower Body Kinematics During Gait

    Objectives The purpose of this study was to compare gait among patients with scoliosis undergoing posterior spinal fusion and instrumentation (PSFI) to typically developing subjects and determine if the location of the lowest instrumented vertebra impacted results. Summary of Background Data PSFI is the standard of care for correcting spine deformities, allowing the preservation of body equilibrium while maintaining as many mobile spinal segments as possible. The effect of surgery on joint motion distal to the spine must also be considered. Very few studies have addressed the effect of PSFI on activities such as walking and even fewer address how surgical choice of the lowest instrumented vertebra (LIV) influences possible motion reduction. Methods Individuals with scoliosis undergoing PSFI (n = 38) completed gait analysis preoperatively and at postoperative years 1 and 2 along with a control group (n = 24). Comparisons were made with the control group at each time point and between patients fused at L2 and above (L2+) versus L3 and below (L3–). Results The kinematic results of the AIS group showed some differences when compared to the Control Group, most notably decreased range of motion (ROM) in pelvic tilt and trunk lateral bending. When comparing the LIV groups, only minor differences were observed, and the results showed decreased coronal trunk and pelvis ROM at the one-year visit and decreased hip rotation ROM at the two-year visit in the L3– group. Conclusions Patients with AIS showed decreased ROM preoperatively with further decreases postoperatively. These changes remained relatively consistent following the two-year visit, indicating that most kinematic changes occurred in the first year following surgery. Limited functional differences between the two LIV groups may be due to the lack of full ROM used during normal gait, and future work could address tasks that use greater ROM

    Pointcatcher software:analysis of glacial time-lapse photography and integration with multi-temporal digital elevation models

    Terrestrial time-lapse photography offers insight into glacial processes through high spatial and temporal resolution imagery. However, oblique camera views complicate measurement in geographic coordinates, and lead to reliance on specific imaging geometries or simplifying assumptions for calculating parameters such as ice velocity. We develop a novel approach that integrates time-lapse imagery with multi-temporal digital elevation models to derive full 3D coordinates for natural features tracked throughout a monoscopic image sequence. This enables daily independent measurement of horizontal (ice flow) and vertical (ice melt) velocities. By combining two terrestrial laser scanner surveys with a 73-day sequence from SĂłlheimajökull, Iceland, variations in horizontal ice velocity of ~10% were identified over timescales of ~25 days. An overall surface elevation decrease of ~3.0 m showed rate changes asynchronous with the horizontal velocity variations, demonstrating a temporal disconnect between the processes of ice surface lowering and mechanisms of glacier movement. Our software, ‘Pointcatcher’, is freely available for user-friendly interactive processing of general time-lapse sequences and includes Monte Carlo error analysis and uncertainty projection onto DEM surfaces. It is particularly suited for analysis of challenging oblique glacial imagery, and we discuss good features to track, both for correction of camera motion and for deriving ice velocities

    Muscle invasive bladder cancer treated by transurethral resection, followed by external beam radiation and interstitial iridium-192

    Purpose: To evaluate the results of transurethral resection (TUR), external beam radiotherapy (EBRT), and interstitial radiation (IRT) with iridium-192, using the afterloading technique in patients with muscle invasive bladder cancer. Methods and Materials: From May 1989 until September 1995. 66 patients with primary, solitary muscle invasive bladder cancer were treated with TUR, EBRT, and IRT, aiming at bladder preservation. According to the protocol, in three patients low-dose EBRT was applied, whereas 63 patients received high-dose EBRT. Immediately prior to IRT, 42 patients underwent a lymphnode dissection, and in 16 cases a partial cystectomy was performed. For IRT, two to five catheters were used and IRT was started within 24 h after surgery. The majority of patients received 30 Gy of IRT, with a mean dose rate of .58 Gy/h. In three patients, additional EBRT was applied following IRT. Follow-up consisted of regular cystoscopies, mostly done during joint clinics of urologist and radiation oncologist, with urine cytology routinely performed. The median follow-up period was 26 months. The Kaplan-Meier method was used for the determination of survival rates. Results: In seven patients, a bladder relapse developed. The probability of remaining bladder relapse free at 5 years was 88%. The bladder was preserved in 98% of the surviving patients. Metastases developed in 16 patients, and the probability of remaining metastasis free at 5 years was 66%. The cumulative 5-year overall and bladder and distant relapse free survival were 48% and 69%, respectively. Acute toxicity was not serious in the majority of cases; surgical correction of a persisting vesicocutaneous fistula was necessary in two patients, whereas a wound toilet had to be performed in another patient. Serious late toxicity (bladder, RTOG Grade 31 was experienced by only one patient. Conclusions: Interstitial radiation preceded by TUR and EBRT, in a selected group of patients with muscle invasive bladder cancer, yields an excellent bladder tumor control rate with a high probability of bladder preservation.
