    Knowledge and Attitudes Towards Breastfeeding Among Medical Students at Memorial University

    Breastfeeding has many short term and long term health benefits to mothers and children, yet not all mothers in Canada initiate breastfeeding. Physicians’ support has been shown to improve breastfeeding rates, yet many physicians have poor knowledge and skills about breastfeeding. Improving breastfeeding education at an undergraduate level in medical school may lead to improved breastfeeding knowledge amongst physicians. This thesis explores medical students’ knowledge and attitudes towards breastfeeding at Memorial University. A previously validated questionaire was administered to first and second year medical students at Memorial University to examine their knowledge and attitudes towards breastfeeding. A descriptive qualitative study was conducted with breastfeeding educators at Memorial University to examine knowledge gaps within the breastfeeding curriculum. Medical students were found to have positive attitudes towards breastfeeding, yet limited knowledge. Students from rural areas had the highest attitude scores, whereas second year students had the highest knowledge scores. Breastfeeding educators noted many curriculum gaps in breastfeeding education, and provided suggestions to improve the curriculum. This study was the first to examine medical students’ knowledge and attitudes towards breastfeeding in Canada. The study was conducted in the Province of Newfoundland and Labrador, which has the lowest breastfeeding initiation rate in Canada. During the time of this study, undergraduate curriculum refinement was occurring at the medical school of Memorial University. Thus the findings of this study may lead to potential curricular improvements at Memorial University

    Water immersion increases the concentration of the immunoreactive N-terminal fragment of pro-atrial natriuretic factor in human plasma

    Atrial natriuretic factor (ANF) N-terminal (ANF 1–98) and C-terminal (ANF 99–126) fragments were determined by radioimmunoassay in human plasma. Mean basal plasma ANF N-terminal concentrations in 9 healthy subjects were 461 ± 58 fmol/ml,significantly (p<0.0001) higher than ANF C-terminal concentrations ( 4.8 ± 0.5 fmol/ml). Central volume stimulation by one hour head-out water immersion (WI) induced a significant (p<0.01) increase of the C-terminal peptide levels to 11.6 ± 2.3 fmol/ml,paralleled by a significant (p<0.001) increase of the N-terminal fragment levels to 749 ± 96 fmol/ml. Increases of plasma concentrations of both fragments upon WI correlated significantly (r=0.71;p<0.05). These data suggest cosecretion of the N-terminal fragment with the C-terminal fragment of pro ANF 1–126 following a physiological stimulus of ANF release in man

    Walk Your Life to Health—Motivating Young Adolescents to Engage in a Brisk Walking Program

    Increasing numbers of overweight and obese children resulting from sedentary and abundant lifestyles are a health concern in Hong Kong and other affluent communities around the globe. The importance of physical activity is well documented for health and wellbeing, with sedentary behavior emerging as an independent risk factor for chronic diseases and mortality. In this study, Pender's Health-Promotion Model informed the development of an intervention program aimed to engage junior high school students in increased physical activity, specifically brisk-walking. The model set out to motivate participants to engage in behaviors to enhance their health across the life span, including developing self-efficacy with regard to brisk walking. The intervention featured four stages. First, participants were provided with information about the benefits and in stage two were trained in the technique of brisk walking. Next they engaged in the brisk walking program and in stage four were encouraged to serve as health ambassadors by introducing brisk walking and its associated benefits to others, thereby exercising peer influence to diffuse the practice of brisk walking more widely among members of the community. Motivational strategies were used as incentives in the program, including the involvement of a popular singing band to award certificates at the completion of the program. This 7-week intervention program including a 4-week brisk walking component was conducted in a high school context with 71 participants. Data were collected to enable paired-sample t-tests to be conducted to statistically analyze the data at pre- and post-intervention. Findings indicate significant differences among the mean Body Mass Index (BMI), Waist-to-Hip Ratio (WHR), knowledge and attitudes of the participants at pre- and post-intervention. These findings suggest that the intervention was effective as a strategy to reduce sedentary behavior with the concomitant effect of positive shifts in measurable indicators and attitudes.Full Tex

    Kansas Speaks 2016 Statewide Public Opinion Survey

    To assess attitudes and opinions of Kansans, the Docking Institute has surveyed a random sample of Kansas residents age 18 and older every year since 2009. For this year’s survey, the Docking Institute purchased from Scientific Telephone Samples a random sample consisting of Kansas landline telephone numbers and cell phone numbers. Interviewers are highly trained student researchers pursuing degrees at Fort Hays State University. From September 1st to October 13th, a total of 2,914 Kansas residents were contacted by telephone, with 1,043 of them completed the survey, resulting in a 36% response rate. The margin of error was 3% at the 95% confidence level. A margin of error of 3% means that there is a 95% probability that findings among the sample vary no more than +/- 3 % from the value that would be found if all adult Kansas residents were surveyed, assuming no response bias. Out of these 1,043 respondents, 892 respondents were identified as likely voters, which resulted in a margin of error of +/- 3.2%

    Wheel-running activity modulates circadian organization and the daily rhythm of eating behavior

    Consumption of high-fat diet acutely alters the daily rhythm of eating behavior and circadian organization (the phase relationship between oscillators in central and peripheral tissues) in mice. Voluntary wheel-running activity counteracts the obesogenic effects of high-fat diet and also modulates circadian rhythms in mice. In this study, we sought to determine whether voluntary wheel-running activity could prevent the proximate effects of high-fat diet consumption on circadian organization and behavioral rhythms in mice. Mice were housed with locked or freely rotating running wheels and fed chow or high-fat diet for 1 week and rhythms of locomotor activity, eating behavior, and molecular timekeeping (PERIOD2::LUCIFERASE luminescence rhythms) in ex vivo tissues were measured. Wheel-running activity delayed the phase of the liver rhythm by 4 h in both chow- and high-fat diet-fed mice. The delayed liver phase was specific to wheel-running activity since an enriched environment without the running wheel did not alter the phase of the liver rhythm. In addition, wheel-running activity modulated the effect of high-fat diet consumption on the daily rhythm of eating behavior. While high-fat diet consumption caused eating events to be more evenly dispersed across the 24 h-day in both locked-wheel and wheel-running mice, the effect of high-fat diet was much less pronounced in wheel-running mice. Together these data demonstrate that wheel-running activity is a salient factor that modulates liver phase and eating behavior rhythms in both chow- and high-fat-diet fed mice. Wheel-running activity in mice is both a source of exercise and a self-motivating, rewarding behavior. Understanding the putative reward-related mechanisms whereby wheel-running activity alters circadian rhythms could have implications for human obesity since palatable food and exercise may modulate similar reward circuits

    Measuring home environments across cultures: Invariance of the HOME scale across eight international sites from the MAL-ED Study

    The home environment provides the context for much of a child\u27s early development. Examples of important aspects of the home environment include safety, cleanliness, and opportunities for cognitive stimulation. This study sought to examine the psychometric properties of an adapted form of the Home Observation for the Measurement of the Environment (HOME; Caldwell & Bradley, 1984, 2003) across the eight international sites of the MAL-ED project (Dhaka, Bangladesh; Vellore, India; Bhakatapur, Nepal; Naushahro Feroze, Pakistan; Fortaleza, Brazil; Loreto, Peru; Venda, South Africa; Haydom, Tanzania), to identify a factor structure that fit the data at all sites, and to derive a subset of items that could be used to examine home environmental characteristics across sites. A three-factor structure (i.e., Emotional and Verbal Responsivity; Clean and Safe Environment; Child Cleanliness) was identified, and partial measurement equivalence/invariance across sites was supported. Overall, these findings lend support for the use of portions of this abbreviated and adapted version of the HOME for use among heterogeneous, cross-cultural groups in low- and middle-income nations

    Early life child micronutrient status, maternal reasoning, and a nurturing household environment have persistent influences on child cognitive development at age 5 years: Results from MAL-ED

    Background: Child cognitive development is influenced by early-life insults and protective factors. To what extent these factors have a long-term legacy on child development and hence fulfillment of cognitive potential is unknown.Objective: The aim of this study was to examine the relation between early-life factors (birth to 2 y) and cognitive development at 5 y.Methods: Observational follow-up visits were made of children at 5 y, previously enrolled in the community-based MAL-ED longitudinal cohort. The burden of enteropathogens, prevalence of illness, complementary diet intake, micronutrient status, and household and maternal factors from birth to 2 y were extensively measured and their relation with the Wechsler Preschool Primary Scales of Intelligence at 5 y was examined through use of linear regression.Results: Cognitive T-scores from 813 of 1198 (68%) children were examined and 5 variables had significant associations in multivariable models: mean child plasma transferrin receptor concentration (β: -1.81, 95% CI: -2.75, -0.86), number of years of maternal education (β: 0.27, 95% CI: 0.08, 0.45), maternal cognitive reasoning score (β: 0.09, 95% CI: 0.03, 0.15), household assets score (β: 0.64, 95% CI: 0.24, 1.04), and HOME child cleanliness factor (β: 0.60, 95% CI: 0.05, 1.15). In multivariable models, the mean rate of enteropathogen detections, burden of illness, and complementary food intakes between birth and 2 y were not significantly related to 5-y cognition.Conclusions: A nurturing home context in terms of a healthy/clean environment and household wealth, provision of adequate micronutrients, maternal education, and cognitive reasoning have a strong and persistent influence on child cognitive development. Efforts addressing aspects of poverty around micronutrient status, nurturing caregiving, and enabling home environments are likely to have lasting positive impacts on child cognitive development
