    The Rewards of Patience: An 822 Day Time Delay in the Gravitational Lens SDSS J1004+4112

    We present 107 new epochs of optical monitoring data for the four brightest images of the gravitational lens SDSS J1004+4112 observed between October 2006 and June 2007. Combining this data with the previously obtained light curves, we determine the time delays between images A, B and C. We confirm our previous measurement finding that A leads B by dt_BA=40.6+-1.8 days, and find that image C leads image A by dt_CA=821.6+-2.1 days. The lower limit on the remaining delay is that image D lags image A by dt_AD>1250 days. Based on the microlensing of images A and B we estimate that the accretion disk size at a rest wavelength of 2300 angstrom is 10^{14.8+-0.3} cm for a disk inclination of cos{i}=1/2, which is consistent with the microlensing disk size-black hole mass correlation function given our estimate of the black hole mass from the MgII line width of logM_BH/M_sun=8.44+-0.14. The long delays allow us to fill in the seasonal gaps and assemble a continuous, densely sampled light curve spanning 5.7 years whose variability implies a structure function with a logarithmic slope of gamma = 0.35+-0.02. As C is the leading image, sharp features in the C light curve can be intensively studied 2.3 years later in the A/B pair, potentially allowing detailed reverberation mapping studies of a quasar at minimal cost.Comment: Submitted to ApJ, 12 pages, 3 figure

    Inaccuracy of routine susceptibility tests for detection of erythromycin resistance of Campylobacter jejuni and Campylobacter coli

    In The Netherlands, both an increase in and regional differences in erythromycin resistance of Campylobacter jejuni and Campylobacter coli have been reported. To determine the accuracy of routine tests for erythromycin resistance, 48 erythromycin-resistant isolates from various laboratories that participate in the Dutch surveillance of Campylobacter infections were reinvestigated. Initial susceptibility testing for erythromycin had been performed by disk diffusion in six and MIC-based methods in two laboratories. Reinvestigation was carried out using broth microdilution as a reference standard, as well as E-test and genetic resistance testing. Of 36 C. jejuni isolates reported by the initial laboratories as erythromycin-resistant, four (11%) and five (14%) were confirmed as erythromycin-resistant using broth microdilution according to CLSI and EUCAST resistance criteria, respectively. Erythromycin resistance was found in eight of 12 (67%) C. coli isolates according to both criteria. Results of E-tests were in accordance with these results in all isolates. Resistance-associated mutations in the 23S rRNA gene (A2059G and A2058T) were found in all isolates showing high-level resistance, whereas none were found in susceptible isolates. Routine determination of the erythromycin resistance of C. jejuni and C. coli shows unacceptable interlaboratory variation. In the absence of standardized protocols and interpretive criteria for disk diffusion, and while we await the development of easily applicable and reliable methods for molecular resistance testing, the use of broth microdilution remains the best method

    Risk factors for indigenous Campylobacter jejuni and Campylobacter coli infections in The Netherlands: a case-control study

    A case-control study comprising 1315 Campylobacter jejuni cases, 121 Campylobacter coli cases and 3409 frequency-matched controls was conducted in The Netherlands in 2002-2003. Risk factors for both C. jejuni and C. coli enteritis were consumption of undercooked meat and barbecued meat, ownership of cats and use of proton pump inhibitors. Consumption of chicken was a predominant risk factor for C. jejuni enteritis, but many additional risk factors were identified. Unique risk factors for C. coli infections were consumption of game and tripe, and swimming. Contact with farm animals and persons with gastroenteritis were predominant risk factors for C. jejuni enteritis in young children (0-4 years). Important risk factors for the elderly (>= 60 years) were eating in a restaurant, use of proton pump inhibitors and having a chronic intestinal illness. Consumption of chicken in spring, steak tartare in autumn and winter and barbecued meat in rural areas showed strong associations with C. jejuni infections. This study illustrates that important differences in risk factors exist for different Campylobacter spp. and these may differ dependent on age, season or degree of urbanization

    The Variability of Sagittarius A* at Centimeter Wavelengths

    We present the results of a 3.3-year project to monitor the flux density of Sagittarius A* at 2.0, 1.3, and 0.7 cm with the VLA. The fully calibrated light curves for Sgr A* at all three wavelengths are presented. Typical errors in the flux density are 6.1%, 6.2%, and 9.2% at 2.0, 1.3, and 0.7 cm, respectively. There is preliminary evidence for a bimodal distribution of flux densities, which may indicate the existence of two distinct states of accretion onto the supermassive black hole. At 1.3 and 0.7 cm, there is a tail in the distribution towards high flux densities. Significant variability is detected at all three wavelengths, with the largest amplitude variations occurring at 0.7 cm. The rms deviation of the flux density of Sgr A* is 0.13, 0.16, and 0.21 Jy at 2.0, 1.3, and 0.7 cm, respectively. During much of this monitoring campaign, Sgr A* appeared to be relatively quiescent compared to results from previous campaigns. At no point during the monitoring campaign did the flux density of Sgr A* more than double its mean value. The mean spectral index of Sgr A* is alpha=0.20+/-0.01, with a standard deviation of 0.14. The spectral index appears to depend linearly on the observed flux density at 0.7 cm with a steeper index observed during outbursts. This correlation is consistent with the expectation for outbursts that are self-absorbed at wavelengths of 0.7 cm or longer and inconsistent with the effects of simple models for interstellar scintillation. Much of the variability of Sgr A*, including possible time lags between flux density changes at the different wavelengths, appears to occur on time scales less than the time resolution of our observations (8 days).     The time delay of the quadruple quasar RX J0911.4+0551

    We present optical lightcurves of the gravitationally lensed components A (=A1+A2+A3) and B of the quadruple quasar RX J0911.4+0551 (z = 2.80). The observations were primarily obtained at the Nordic Optical Telescope between 1997 March and 2001 April and consist of 74 I-band data points for each component. The data allow the measurement of a time delay of 146 +- 8 days (2 sigma) between A and B, with B as the leading component. This value is significantly shorter than that predicted from simple models and indicates a very large external shear. Mass models including the main lens galaxy and the surrounding massive cluster of galaxies at z = 0.77, responsible for the external shear, yield H_0 = 71 +- 4 (random, 2 sigma) +- 8 (systematic) km/s/Mpc. The systematic model uncertainty is governed by the surface-mass density (convergence) at the location of the multiple images.Comment: 12 pages, 3 figures, ApJL, in press (June 20, 2002

    Further Investigation of the Time Delay, Magnification Ratios, and Variability in the Gravitational Lens 0218+357

    High precision VLA flux density measurements for the lensed images of 0218+357 yield a time delay of 10.1(+1.5-1.6)days (95% confidence). This is consistent with independent measurements carried out at the same epoch (Biggs et al. 1999), lending confidence in the robustness of the time delay measurement. However, since both measurements make use of the same features in the light curves, it is possible that the effects of unmodelled processes, such as scintillation or microlensing, are biasing both time delay measurements in the same way. Our time delay estimates result in confidence intervals that are somewhat larger than those of Biggs et al., probably because we adopt a more general model of the source variability, allowing for constant and variable components. When considered in relation to the lens mass model of Biggs et al., our best-fit time delay implies a Hubble constant of H_o = 71(+17-23) km/s-Mpc for Omega_o=1 and lambda_o=0 (95% confidence; filled beam). This confidence interval for H_o does not reflect systematic error, which may be substantial, due to uncertainty in the position of the lens galaxy. We also measure the flux ratio of the variable components of 0218+357, a measurement of a small region that should more closely represent the true lens magnification ratio. We find ratios of 3.2(+0.3-0.4) (95% confidence; 8 GHz) and 4.3(+0.5-0.8) (15 GHz). Unlike the reported flux ratios on scales of 0.1", these ratios are not strongly significantly different. We investigate the significance of apparent differences in the variability properties of the two images of the background active galactic nucleus. We conclude that the differences are not significant, and that time series much longer than our 100-day time series will be required to investigate propagation effects in this way.Comment: 33 pages, 9 figures.     “I Wish I Had AIDS”: A qualitative study on access to health care services for HIV/AIDS and diabetic patients in Cambodia

    Financially stricken Cambodian patients with diabetes and HIV/AIDS typically encounter multiple, serious barriers to effective care. This process may extend over many years and involve numerous rounds of diagnosis and treatment as the disease progresses from initial symptoms to longer term complications. Living with both the impact of the disease and this ongoing struggle for care can severely disrupt the everyday life of both sufferers and their families. Our retrospective study adopted qualitative research methods to collect data from HIV/AIDS and diabetic patients enrolled and not enrolled in treatment programs at varying institutions in urban and rural settings. Using purposive sampling techniques, a total of 25 HIV/AIDS and 45 diabetic patients were recruited. Semi-structured and open-ended interviews were used to collect information on patient experiences of different phases in the on-going process of seeking care and treatment. The findings indicate that both HIV/AIDS and diabetic patients encounter multiple supply- and demand-side barriers to care at different stages of their illness. More strikingly, our research findings suggest that supply-side barriers, for example rationing systems or targeting strategies that limit access to free treatment or social assistance, are substantially higher for diabetic patients. This perceived inequity had a profound impact on diabetic patients to the extent that some “wished they had HIV/AIDS”. These findings suggest that there is an urgent need to widen the focus of health care to address the substantial and increasing burden of disease resulting from diabetes and other serious chronic disorders in Cambodia and many other low/middle income countries.
