    Development of a language and communication screening test and identification of protective and risk factors associated with communication and language disorders

    With the present research, we aimed to identify the principal risk and protective factors associated with the occurrence of a developmental language disorder and to develop a specific and sensible screening instrument, easy and rapid to use. We can understand language disorders as a condition that involves poorly developed grammatical patterns, odd conversational patterns, misuse of words with respect with their meaning, difficulty expressing or following ideas and an inability to follow directions (Matson & Neal, 2010). On the other hand, communication disorders refer to a break in the communication chain (Morris, 2005). We know that delays in communication are the most prevalent symptoms in young children with developmental disabilities (Wetherby & Prizant 1996) and speech and language delay affects 5 to 8 percent of preschool children, often persists into the school years, and may be associated with lowered school performance and psychosocial problems (U.S. Preventive Services Task Force, 2006). The recent investigation in this field tells us that the early identification and intervention of children with language disorders is a urgent situation, because of the related consequences of this kind of developmental disorder (reading disorders, emotional disorders, academic achievement). Developmental delays in infants and toddlers are significantly under identified, which means that the provision of early intervention is prohibited for many children and families in need of services (Wetherby et al, 2002). In a previous investigation, we concluded that there is a gap between the early concerns of parents and the intervention of 20 months (Peixoto, 2007). The research tells us that we can early identify language disorders trough prelinguistic Predictors of Language Outcomes, and they are: • Joint attention and intentional communication • Language understanding • Play • Phonological development. Early identification of most medical problems is advantageous in that it generally provides better opportunities for successful intervention, Yet, involves several considerations as economical, ethical and methodological However, a major problem in screening for communication and language disorder is that it implies a matter of degree. To implement a screening policy we have to decide on a proper “gold standard” instrument, as well as deciding on cut-off scores (Eriksson et al, 2010). Regarding this we have developed an epidemiological study, with a prospective cohort design, with 431 children between 12 and 36 months old (394 evaluated in kindergarten and 37 evaluated in child placement homes) Children were first assessed between February and July of 2011, and 33% of the children had a second evaluation between January and March of 2012. The children were first assessed with our screening instrument, and their parents completed a questionnaire with information about pregnancy, birth, ESE, development, temperament and occupation. Our screening instrument gathers information about the child's abilities from multiple sources; caregivers participate in the assessment process and measures milestones that provide important information about prelinguistic and early linguistic development. The second evaluation consisted in a gold standard language test. Every participant was informed of the aims and procedures of the study, and everyone agreed to participate. Because of this study, we believe that we will be able to recognize risk groups for developmental language disorders and then propose a screening policy for risk groups for the early identification of developmental language disorder

    Implementation of a process-scale adenovirus purification with a single-use platform

    Adenovirus vectors are finding increasing application within the vaccine and gene therapy industries. Companies developing adenovirus-based biopharmaceuticals will benefit from single-use process platforms that are quick and easy to install and have been demonstrated previously to produce purified adenovirus. The platform can be adapted to new adenovirus products with a minimum investment of time and resource in process development activities. We have assembled a platform from existing and proven technologies, available from lab to production scale, that meet the quality requirements of the vaccine industry. The platforms includes clarification, ultrafiltration/diafiltration, chromatography and sterile filtration steps. The approach has been successfully demonstrated for the purification of an Adenovirus serotype 5 vector, at the 20 L scale. It allows the complete purification of the vaccine in one day

    The aim of this paper is to present how to solve the problem of selecting a candidate up to his or her acceptance trough game theory. The originality this paper proposes is how this problem will be approached. It will be treated as a single game which is made up of two parts, going as far as to state that the payoffs in the first part of the game will be the mediators of the second part of the game. In order to represent the problem described above, a new form of game representation will be used – code form – consisting of a table which contains all of the information, without any suppression or “adulteration” with regards to the game

    Introduction: Airway infections (AI) in children constitute situations of relevance to Brazilian and global public health, especially those causes by Respiratory Syncytial Virus. Due to high morbidity and mortality in the early childhood period, especially infants who were born premature, it is relevant to study the adherence to Palivizumab (PVZ) of premature newborns under 28 weeks of gestational age (GA), infants with chronic lung disease (CLD) and congenital heart disease (CHD). Objective: To verify clinical aspects, and adherence to PVZ of premature infants born up to 28 gestational weeks and children with CLD and CHD (who have the right to receive prophylaxis) living in Uberlandia, Minas Gerais (MG), from 2011 to 2018. Methodology: This is descriptive and cross-sectional research. Information was collected from medical records of premature and infants with CHD and CLD who received the PVZ in the public health network. The supply of the PVZ is provided from a list of patients designated by the State Department of Health of MG. The inclusion criteria of this study were premature infants born and living in Uberlandia, MG, who have an indication to PVZ, as well as children with CHD and CLD who have the right to receive prophylaxis. The variables collect were: sex, GA, birth weight, underlying diseases, chronic diseases, twinning, hospital segment, hospital segment time, symptoms and drug therapy, and information about the use of PVZ. This study received approval from the Ethics Committee on Research with Human Beings under the number 3.268.624. Results: A sample of 253 medical records of live births was obtained from 2011 and 2018. The rate of male births is predominant, however, the highest rate of gemelars is female. Among the eligibility criteria of PVZ were found 150 premature, 144 CHD and 130 CLD. In addition to the association of the three chronic conditions, associations with other diseases were found. Regarding health care, most children had prematurity as a diagnosis, followed by CLD, CHD and AI. In the clinical management of infants treated at the Hospital of Clinical of Uberlandia during hospitalizations, the main drug therapies and symptoms of relevance to the study were categorized. The survey showed worrying data regarding the lack of full adherence to PVZ. Conclusion: The lack of full adherence to PVZ is an important finding. The incidence of AI was low, even considering children who did not have full adherence. It is extremely important that health professionals act in the screening and orientation of families without adherence.Trabalho de Conclusão de Curso (Graduação)Introdução: As infecções de vias aéreas (IVA) em crianças configuram situações de relevância para a saúde pública brasileira e mundial, principalmente aquelas causadas pelo Vírus Sincicial Respiratório. Devido à alta morbimortalidade no período da primeira infância, com destaque para os lactentes que nasceram prematuros, torna-se relevante estudar sobre a adesão de recém-nascidos prematuros menores de 28 semanas de idade gestacional (IG) ao Palivizumabe (PVZ), bem como de lactentes com doença pulmonar crônica (DPC) e doença cardíaca congênita (DCC). Objetivo: Verificar aspectos clínicos, e a adesão ao PVZ dos prematuros nascidos com até 28 semanas gestacionais e das crianças com DPC e DCC (que possuem o direito de receber a profilaxia) residentes em Uberlândia, Minas Gerais (MG), no período de 2011 a 2018. Metodologia: Trata-se de uma pesquisa descritiva e transversal. Foram coletadas informações de prontuários de prematuros e dos lactentes portadores de DCC e DPC que receberam o PVZ na rede pública de saúde. O fornecimento do PVZ é realizado a partir de lista de pacientes designada pela Secretaria Estadual de Saúde de MG. Os critérios de inclusão deste estudo foram lactentes prematuros que nasceram e residem em Uberlândia, MG e que possuem indicação ao PVZ, bem como crianças com DBP e DCC que possuem o direito de receber a profilaxia. As variáveis coletadas foram: sexo, IG, peso ao nascer, doenças de base, doenças crônicas, gemelaridade, segmento hospitalar, tempo de segmento hospitalar, sintomas e terapia medicamentosa, e as informações do uso do PVZ. O presente estudo recebeu aprovação do Comitê de Ética em Pesquisa com Seres Humanos sob o Parecer Consubstanciado número 3.268.624. Resultados: Obteve-se uma amostra de 253 prontuários de nascidos vivos entre 2011 a 2018. A taxa de nascidos do sexo masculino é predominante, contudo, o maior índice de gemelaridade é do sexo feminino. Dentre os critérios de elegibilidade do PVZ foram encontrados 150 prematuros, 144 DCC e 130 DPC. Além da associação das três condições crônicas, foram encontradas associações com outras doenças. Em relação aos atendimentos de saúde a maioria das crianças tiveram como diagnóstico a prematuridade, seguido por DCC, DPC e IVA. No manejo clínico dos lactentes atendidos no Hospital de Clínicas de Uberlândia durante as internações foram categorizados as principais terapias medicamentosas e os sintomas de relevância para o estudo. A pesquisa demonstrou dados preocupantes em relação a falta de adesão total ao PVZ. Conclusão: A falta de adesão plena ao PVZ é um achado importante. A incidência de IVA foram baixas, mesmo considerando as crianças que não tiveram adesão plena. É de suma importância que os profissionais de saúde atuem no rastreamento e orientações de famílias faltosas

    Retinal vascular occlusion and neovascularization in acute toxoplasmic retinochoroiditis in a patient with AIDS: case report

    To present a case of an HIV-infected patient, who presented with active chorioretinal inflammatory lesion suggestive of toxoplasmosis, that during the specific treatment developed a retinal vascular occlusion, and adjacent neovascularization. 35-year-old AIDS patient that, presented with toxoplasmic retinochoroiditis and after 4 weeks of the specific therapy (pyrimethamine and sulfadiazine) developed a vascular occlusion and retinal neovascularization that healed without the need for phototocoagulation or other adjunctive therapy. This case demonstrates that acute toxoplasmic retinochoroiditis that develops vascular complications may present a good result with the specific treatment without the need for adjunctive therapy.Apresentar e discutir o caso de paciente infectado pelo HIV, que desenvolveu quadro de obstrução vascular seguido de neovascularização retiniana e de papila, durante o tratamento específico para retinocoroidite por toxoplasmose e que evoluiu com resolução espontânea da neovascularização sem a necessidade de tratamento associado. Paciente com retinocoroidite por toxoplasmose, que após 4 semanas de tratamento específico (sulfadiazina e pirimetamina) desenvolveu quadro de obstrução vascular, seguido de neovascularização retiniana, apresentou involução espontânea, não havendo necessidade de fotocoagulação a laser, ou de outro tratamento adicional.
