Het omzeilen van monopoliehandel: Smokkel en belastingontduiking bij de handel in brazielhout, 1500-1674
In this article we take the example of smuggled brazil wood to study the complex relationship between smuggling and the development ofoverseas empires. We trace the legal framework provided by the Portuguese monarchy and the WIC for the exploitation of brazilwood, from the logging in Brazil to the import into the European consumption markets. We also provide a glimpse of the many groups and the different ways in which the legal frameworks were circumvented by the cunning of private entrepreneurs looking for profit and efficiency in the business of acquiring and distributing brazil wood from production outlets into consumption markets. Furthermore, in order to understand the different gradations and added complexities of avoiding established colonial monopolies, we showcase the entrepreneurial choices that contributed to the subversion of the legal frameworks imposed by the institutions of early modern empires
Uso de chupeta em crianças: contaminação fecal e associação com diarréia
A cross-sectional study of 354 children under two years of age was carried out in two periurban slums, with poor sanitary and socioeconomic conditions, located in Pelotas, southern Brazil. Most (79%) of the children studied were current users of pacifiers, 15% had never used one and the remaining 6% were ex-users. Among current users, 38% sucked a pacifier most of the time ("constant users"). Of the pacifiers in constant use, 93% were cultured for evidence of fecal contamination. Fecal coliforms were present in 49% of these. Diarrhoea was reported in 35% of all the children in the two weeks preceding the survey. Among constant pacifiers users, 40% had had diarrhoea in the preceding fortnight; this proportion was 32% for occasional users and 37% for non-users. These diferences were not statistically significant.Inexistem na literatura estudos sobre o possÃvel papel das chupetas na transmissão da diarréia. Realizou-se um estudo transversal em 354 crianças menores de dois anos em duas vilas da periferia urbana de Pelotas, RS, Brasil, com precárias condições socioeconómicas. A maioria das crianças (79%) usava chupeta, 15% nunca as haviam utilizado e 6% já haviam abandonado o hábito. Dentre os usuários, 38% passavam a maior parte do tempo fazendo uso da chupeta (uso intenso). Foram realizadas culturas para coliformes fecais em 93% das chupetas em uso, indicando que 49% estavam contaminadas. Nas duas semanas anteriores à entrevista, 35% das crianças apresentaram diarréia - 40% entre as de uso intenso, 32% entre usuárias em tempo parcial e 37% entre não usuárias. Apesar da forte presença de coliformes fecais, parece não existir associação entre uso de chupeta e diarréia
Fatores de risco para internação por doença respiratória aguda em crianças até um ano de idade
OBJECTIVE: To evaluate risk factors for acute respiratory disease hospitalizations in children under one year of age. METHODS: A case-control study was conducted in the city of Pelotas, Southern Brazil. Cases were children under one year of age who were hospitalized due to acute respiratory diseases from August 1997 to July 1998. Controls were same-age community children randomly selected without previous respiratory disease hospitalization. A questionnaire about risk factors exposure was applied to the mothers of cases and controls. Univariate, bivariate and multivariate analyses through logistic regression were carried out to evaluate risk factors for the outcome of interest. RESULTS: There were studied 777 children; 625 cases and 152 controls. In the crude analysis, the risk factors associated with the outcome were: being male, children under six months of age, household crowding, maternal education, family income, inadequate housing conditions, lack of breastfeeding, maternal smoking, use of pacifiers, and a previous history of hospitalization and respiratory symptoms. Maternal working was a protection factor associated with acute respiratory disease hospitalizations. In the multivariate analysis the following risk factors remained associated: maternal education (OR=12.5), previous history of wheezing (OR=7.7), lack of breastfeeding (OR=2.3), use of pacifiers (OR=1.9), maternal smoking (OR=1.7), children under six months of age (OR=1.7), and being male (OR=1.5). CONCLUSIONS: The study results show the importance of the family's social and behavioural aspects as well as previous respiratory disease as risk factors for acute respiratory disease hospitalizations in children under one year of age.OBJETIVO: Avaliar fatores de risco para hospitalização por doença respiratória aguda em crianças até um ano de idade. MÉTODOS: Estudo de casos e controles na cidade de Pelotas, RS. Os casos foram crianças de até um ano de idade, que se hospitalizaram por doença respiratória aguda, de agosto de 1997 a julho de 1998. Os controles foram crianças da comunidade, da mesma idade, sem hospitalização prévia por essa doença. Um questionário investigando exposição a fatores de risco foi aplicado à s mães de casos e controles. Os dados foram submetidos à análise univariada, bivariada e multivariada por meio de regressão logÃstica para avaliação dos fatores de risco sobre o desfecho de interesse. RESULTADOS: Foram analisadas 777 crianças, sendo 625 casos e 152 controles. Na análise bruta, os fatores de risco associados ao desfecho foram: sexo masculino, faixa etária menor de seis meses, aglomeração familiar, escolaridade materna, renda familiar, condições habitacionais inadequadas, desmame precoce, tabagismo materno, uso de bico, história de hospitalização e antecedentes de sintomas respiratórios. O trabalho materno foi fator de proteção para internação por doença respiratória aguda. Na análise multivariada, permaneceram associadas: ausência de ou baixa escolaridade materna (OR=12,5), história pregressa de sibilância (OR=7,7), desmame precoce (OR=2,3), uso de bico (OR=1,9), mãe fumante (OR=1,7), idade abaixo de seis meses (OR=1,7) e sexo masculino (OR=1,5). CONCLUSÕES: Os resultados mostraram a importância dos aspectos sociais e comportamentais da famÃlia, assim como morbidade respiratória anterior da criança como fatores de risco para hospitalização por doença respiratória aguda
A randomized, double-blind, placebo-controlled trial to assess the efficacy of topiramate in the treatment of post-traumatic stress disorder
<p>Abstract</p> <p>Background</p> <p>Topiramate might be effective in the treatment of posttraumatic stress disorder (PTSD) because of its antikindling effect and its action in both inhibitory and excitatory neurotransmitters. Open-label studies and few controlled trials have suggested that this anticonvulsant may have therapeutic potential in PTSD. This 12-week randomized, double-blind, placebo-controlled clinical trial will compare the efficacy of topiramate with placebo and study the tolerability of topiramate in the treatment of PTSD.</p> <p>Methods and design</p> <p>Seventy-two adult outpatients with DSM-IV-diagnosed PTSD will be recruited from the violence program of Federal University of São Paulo Hospital (UNIFESP). After informed consent, screening, and a one week period of wash out, subjects will be randomized to either placebo or topiramate for 12 weeks. The primary efficacy endpoint will be the change in the Clinician-administered PTSD scale (CAPS) total score from baseline to the final visit at 12 weeks.</p> <p>Discussion</p> <p>The development of treatments for PTSD is challenging due to the complexity of the symptoms and psychiatric comorbidities. The selective serotonin reuptake inhibitors (SSRIs) are the mainstream treatment for PTSD, but many patients do not have a satisfactory response to antidepressants. Although there are limited clinical studies available to assess the efficacy of topiramate for PTSD, the findings of prior trials suggest this anticonvulsant may be promising in the management of these patients.</p> <p>Trial Registration</p> <p>NCT 00725920</p
Trophic Relationships and Habitat Preferences of Delphinids from the Southeastern Brazilian Coast Determined by Carbon and Nitrogen Stable Isotope Composition
To investigate the foraging habitats of delphinids in southeastern Brazil, we analyzed stable carbon (δ13C) and nitrogen (δ15N) isotopes in muscle samples of the following 10 delphinid species: Sotalia guianensis, Stenella frontalis, Tursiops truncatus, Steno bredanensis, Pseudorca crassidens, Delphinus sp., Lagenodelphis hosei, Stenella attenuata, Stenella longirostris and Grampus griseus. We also compared the δ13C and δ15N values among four populations of S. guianensis. Variation in carbon isotope results from coast to ocean indicated that there was a significant decrease in δ13C values from estuarine dolphins to oceanic species. S. guianensis from Guanabara Bay had the highest mean δ13C value, while oceanic species showed significantly lower δ13C values. The highest δ15N values were observed for P. crassidens and T. truncatus, suggesting that these species occupy the highest trophic position among the delphinids studied here. The oceanic species S. attenuata, G. griseus and L. hosei had the lowest δ15N values. Stable isotope analysis showed that the three populations of S. guianensis in coastal bays had different δ13C values, but similar δ15N results. Guiana dolphins from Sepetiba and Ilha Grande bays had different foraging habitat, with specimens from Ilha Grande showing more negative δ13C values. This study provides further information on the feeding ecology of delphinids occurring in southeastern Brazil, with evidence of distinctive foraging habitats and the occupation of different ecological niches by these species in the study area.Peer reviewe
Metabolic Programming during Lactation Stimulates Renal Na+ Transport in the Adult Offspring Due to an Early Impact on Local Angiotensin II Pathways
BACKGROUND: Several studies have correlated perinatal malnutrition with diseases in adulthood, giving support to the programming hypothesis. In this study, the effects of maternal undernutrition during lactation on renal Na(+)-transporters and on the local angiotensin II (Ang II) signaling cascade in rats were investigated. METHODOLOGY/PRINCIPAL FINDINGS: Female rats received a hypoproteic diet (8% protein) throughout lactation. Control and programmed offspring consumed a diet containing 20% protein after weaning. Programming caused a decrease in the number of nephrons (35%), in the area of the Bowman's capsule (30%) and the capillary tuft (30%), and increased collagen deposition in the cortex and medulla (by 175% and 700%, respectively). In programmed rats the expression of (Na(+)+K(+))ATPase in proximal tubules increased by 40%, but its activity was doubled owing to a threefold increase in affinity for K(+). Programming doubled the ouabain-insensitive Na(+)-ATPase activity with loss of its physiological response to Ang II, increased the expression of AT(1) and decreased the expression of AT(2) receptors), and caused a pronounced inhibition (90%) of protein kinase C activity with decrease in the expression of the α (24%) and ε (13%) isoforms. Activity and expression of cyclic AMP-dependent protein kinase decreased in the same proportion as the AT(2) receptors (30%). In vivo studies at 60 days revealed an increased glomerular filtration rate (GFR) (70%), increased Na(+) excretion (80%) and intense proteinuria (increase of 400% in protein excretion). Programmed rats, which had normal arterial pressure at 60 days, became hypertensive by 150 days. CONCLUSIONS/SIGNIFICANCE: Maternal protein restriction during lactation results in alterations in GFR, renal Na(+) handling and in components of the Ang II-linked regulatory pathway of renal Na(+) reabsorption. At the molecular level, they provide a framework for understanding how metabolic programming of renal mechanisms contributes to the onset of hypertension in adulthood
Whole-Genome Sequencing Uncovers Two Loci for Coronary Artery Calcification and Identifies Arse as a Regulator of Vascular Calcification
Coronary artery calcification (CAC) is a measure of atherosclerosis and a well-established predictor of coronary artery disease (CAD) events. Here we describe a genome-wide association study (GWAS) of CAC in 22,400 participants from multiple ancestral groups. We confirmed associations with four known loci and identified two additional loci associated with CAC (ARSE and MMP16), with evidence of significant associations in replication analyses for both novel loci. Functional assays of ARSE and MMP16 in human vascular smooth muscle cells (VSMCs) demonstrate that ARSE is a promoter of VSMC calcification and VSMC phenotype switching from a contractile to a calcifying or osteogenic phenotype. Furthermore, we show that the association of variants near ARSE with reduced CAC is likely explained by reduced ARSE expression with the G allele of enhancer variant rs5982944. Our study highlights ARSE as an important contributor to atherosclerotic vascular calcification, and a potential drug target for vascular calcific disease
