    The recent release of Pope Francis’s much-discussed encyclical on the environment, Laudato Si’: On Care for Our Common Home, has reinforced environmental issues as also moral and spiritual issues. This anthology, twenty years ahead of the encyclical but very much in line with its agenda, offers essays by fifteen philosophers, theologians, and environmentalists who argue for a response to ecology that recognizes the tools of science but includes a more spiritual approach—one with a more humanistic, holistic view based on inherent reverence toward the natural world.     Examining National Trends in Educational Placements for Students with Significant Disabilities

    Using the least restrictive environments (LRE) data from annual Reports to Congress, this study examined national trends in placement between 2000-2014 for school-aged students considered to have significant disabilities from among the categories of autism (ASD), intellectual disability (ID), multiple disabilities (MD), and deaf-blindness (DB). Educational placement trends were calculated using a log ratio index, and students with significant disabilities were compared to groups of students from the other disability groups. Results confirmed that access to general education settings is lacking for this group of students. Implications for policies and practices as well as suggested future research are provided

    Impact of sleep disturbance on maternal and foetal outcomes

    Sleep is a vital factor that regulates the physiology of the body in various aspects. During pregnancy, sleep problems are common. Sleep deprivation has been suggested by previous research to be associated with more adverse birth outcomes that impact both maternal and foetal outcomes. The purpose of this review is to analyse the current evidence regarding the effects of sleep deprivation during pregnancy. Quantitative studies were included and for this review, Google Scholar, PubMed and CINAHL were used to perform a literature search. A total of 38 articles were chosen for this review. Sleep deprivation may increase the risk of adverse birth outcomes in female residents due to long work hours. The initial approach to treating sleep disturbances during pregnancy is non-pharmacological. Sleep position is a major component that influences sleep quality and pregnancy outcome. Poor sleep hygiene during pregnancy can lead to a worsening of the mother, especially in the third trimester, which can have an impact on the outcome of the pregnancy. Maintaining good sleep hygiene is crucial. It has been proven by research that several non-pharmacological measures can enhance the circadian cycle. Further research with longitudinal study designs is required to examine the effect of sleep deprivation on adverse effects on the mother and foetus. Furthermore, additional research is needed to confirm the impact of sleep deprivation on the postpartum period

    Mitigation of pattern sensitivity in full-field electronic dispersion compensation

    We investigate the pattern-dependent decoding failures in full-field electronic dispersion compensation (EDC) by offline processing of experimental signals, and find that the performance of such an EDC receiver may be degraded by an isolated "1" bit surrounded by long strings of consecutive "0s". By reducing the probability of occurrence of this kind of isolated "1" and using a novel adaptive threshold decoding method, we greatly improve the compensation performance to achieve 10-Gb/s on-off keyed signal transmission over 496-km field-installed single-mode fiber without optical dispersion compensation

    Chromatic dispersion compensation using full-field maximum-likelihood sequence estimation

    We investigate full-field detection-based maximum-likelihood sequence estimation (MLSE) for chromatic dispersion compensation in 10 Gbit/s OOK optical communication systems. Important design criteria are identified to optimize the system performance. It is confirmed that approximately 50% improvement in transmission reach can be achieved compared to conventional direct-detection MLSE at both 4 and 16 states. It is also shown that full-field MLSE is more robust to the noise and the associated noise amplifications in full-field reconstruction, and consequently exhibits better tolerance to nonoptimized system parameters than full-field feedforward equalizer. Experiments over 124 km spans of field-installed single-mode fiber without optical dispersion compensation using full-field MLSE verify the theoretically predicted performance benefits

    The molecular ruler method allows the precise control of the gap between a parent gold structure and a deposited daughter structure using a conveniently grown self-assembled molecular multilayer as a lithographic mask. However, we cannot choose a position where the gap should be placed, since the ruler attaches to all exposed gold surfaces. In this work, a convenient method of selecting the position of nanogaps by further patterning the molecular multilayer using low-energy electron beam irradiation and piranha etchant is described

    Impact of implementing a fast-track protocol and standardized guideline for the management of pediatric appendicitis

    Background: In 2017, a provincial guideline was created to fast track and standardize care for pediatric appendicitis in Alberta. We conducted a study to determine the impact of implementation of the guideline at our institution on length of stay (LOS), antibiotic stewardship efforts and costs. Methods: We performed a retrospective review of the charts of all patients younger than 18 years of age who underwent appendectomy at our institution in 2 periods: before guideline implementation (Dec. 1, 2016, to May 31, 2017) and after implementation (Dec. 1, 2017, to May 31, 2018). We compared LOS, duration of antibiotic therapy, 30-day postdischarge complication rates and variable cost between the 2 cohorts. Results: Of the 276 total appendectomy procedures performed, 185 were for simple appendicitis (81 before guideline implementation and 104 after implementation), and 91 were for complicated appendicitis (44 and 47, respectively). The median LOS was shorter in the postimplementation cohort for both simple and complicated appendicitis (15.5 h [interquartile range (IQR) 12-19 h] v. 17.0 h [IQR 13-22 h], p = 0.03; and 3.0 d [IQR 2-4 d] v. 3.0 d [IQR 3-5 d], p = 0.05, respectively). Patients with complicated appendicitis had fewer antibiotic days after guideline implementation; the difference was statistically significant for patients without diffuse peritoneal contamination or abscess formation (p = 0.02). There were no differences between the cohorts with respect to 30-day rates of complications, including emergency department visits, readmission and surgical site infections. After guideline implementation, the average variable cost per patient was reduced by 230,equatingtoatotalaverageannualcostsavingsof230, equating to a total average annual cost savings of 75 842 for our institution. Conclusion: The implementation of a provincial guideline aimed at standardizing care in pediatric appendicitis at our institution was associated with shortened LOS, improved antibiotic stewardship efforts and reduced cost of care. Other institutions may replicate our model of a standardized pathway in the management of pediatric appendicitis in an effort to improve the quality of patient care and reduce health care costs

    Synchrony of physiological activity during mother-child interaction: moderation by maternal history of major depressive disorder

    The family environment plays an important role in the intergenerational transmission of MDD, but less is known about how day-to-day mother-child interactions may be disrupted in families with a history of MDD. Disruptions in mother-child synchrony, the dynamic and convergent exchange of physiological and behavioral cues during interactions, may be one important risk factor. Although maternal MDD is associated with a lack of mother-child synchrony at the behavioral level, no studies have examined the impact of maternal MDD on physiological synchrony. Therefore, the current study examined whether maternal history of MDD moderates mother-child physiological synchrony (measured via RSA) during positive and negative discussions
