    On the saturations of submodules

    Let R ⊆ S be a ring extension, and let A be an R-submodule of S. The saturation of A (in S) by τ is set A[τ] = {x ∈ S : tx ∈ A for some t ∈ τ}, where τ is a multiplicative subset of R. We study properties of saturations of R-submodules of S. We use this notion of saturation to characterize star operations ⋆ on ring extensions R ⊆ S satisfying the relation (A ∩ B)⋆ = A⋆ ∩ B⋆ whenever A and B are two R-submodules of S such that AS = BS = S

    Extrapulmonary Tuberculosis: A retrospective Study at a tertiary care hospital in Palpa, Nepal

    Background: Extra-pulmonary tuberculosis (EPTB) is a significant global health problem. Related studies to it in different places and diff erent durations are indicated by many previous research findings. Findings of this study could be beneficial for its preventive and control strategies. Methodology: Retrospective analysis of clinical specimens submitted to Central laboratory of Lumbini Medical College and Teaching Hospital (LMCTH) for extrapulmonary tuberculosis was performed. Total 261 samples submitted from April 2011 to February 2013 were included for analysis in this study. Results: Total 20.7% (54/261) prevalence of EPTB was reported. Based on sites involved; lymph node 87.03%, pleural effusion7.40%, peritoneal5.55% were found. Genderwise equal prevalence was seen among male and females. Age-wise prevalence among patients between 21-40 years was reported. Conclusion: Our finding indicates great necessity for further large scale study on prevalence of EPTB in this location for its prevention and control

    Confirmatory factor analysis of the Pittsburgh Sleep Quality Index in women with hot flashes

    OBJECTIVE: Women, especially those with hot flashes, report poor sleep quality during various stages of the menopausal transition and postmenopause. Sleep measurements vary widely because of the copious instruments available. The Pittsburgh Sleep Quality Index (PSQI) is a frequently used questionnaire that produces a single score for sleep quality. This one-factor structure has not received consistent support in the literature. The goal of this analysis was to determine the best factor structure of the PSQI in women with hot flashes. METHODS: A confirmatory factor analysis was conducted on PSQI baseline data from three randomized controlled clinical trials enrolling perimenopausal and postmenopausal women with hot flashes (N = 849) from the Menopause Strategies: Finding Lasting Answers for Symptoms and Health network. Several a priori factor models were compared. RESULTS: One-factor and two-factor models did not fit the data. A three-factor model comprising sleep efficiency, perceived sleep quality, and daily disturbance showed good fit; however, the sleep medication item was dropped because of poor fit and low rates of sleep medication use. The three-factor model was examined in African-American and white subsamples and was found to be similar in both groups; however, two items showed small group differences in strength as indicators. CONCLUSIONS: Sleep quality in midlife women with hot flashes, as measured by the PSQI, seems to comprise three correlated factors. Minor measurement differences detected between groups are of research interest but do not necessitate different scoring practices. Additional research is needed to further define sleep quality and its associations with health-related outcomes

    Quantification of anandamide, oleoylethanolamide and palmitoylethanolamide in rodent brain tissue using high performance liquid chromatography–electrospray mass spectroscopy

    AbstractReported concentrations for endocannabinoids and related lipids in biological tissues can vary greatly; therefore, methods used to quantify these compounds need to be validated. This report describes a method to quantify anandamide (AEA), oleoylethanolamide (OEA) and palmitoylethanolamide (PEA) from rodent brain tissue. Analytes were extracted using acetonitrile without further sample clean up, resolved on a C18 reverse-phase column using a gradient mobile phase and detected using electrospray ionization in positive selected ion monitoring mode on a single quadrupole mass spectrometer. The method produced high recovery rates for AEA, OEA and PEA, ranging from 98.1% to 106.2%, 98.5% to 102.2% and 85.4% to 89.5%, respectively. The method resulted in adequate sensitivity with a lower limit of quantification for AEA, OEA and PEA of 1.4ng/mL, 0.6ng/mL and 0.5ng/mL, respectively. The method was reproducible as intraday and interday accuracies and precisions were under 15%. This method was suitable for quantifying AEA, OEA and PEA from rat brain following pharmacological inhibition of fatty acid amide hydrolase

    Prevalence Study of H. pylori infection in Dyspeptic patients coming to Nepal Medical College Teaching Hospital, Jorpati, Kathmandu

    ABSTRACT Helicobacter pylori (H. pylori) is one of the most common human infections worldwide particularly in the developing countries. It has been established as etiology of chronic gastritis and peptic ulcer disease, gastric adenocarcinoma and mucosal associated lymphoid tissue lymphoma (MALT). During this decade, there have been some reports showing a decline in global prevalence of H. pylori infection and peptic diseases including many Asian countries. Hence to determine prevalence of Helicobacter pylori infection in dyspeptic patients, this descriptive, non-interventional study was carried out at the Endoscopy sub-unit of the Surgery Department from April 2011 to February 2012. Three hundred nineteen dyspeptic patients (Male 161 and female 152) with a mean age of 20.12 years were examined for the presence of H. pylori infection by histology staining. The prevalence of H. pylori infection was 50.47%. The most common endoscopic fi ndings was gastritis (47.6%) followed by normal fi ndings 57 (17.87%). A total of 8.47% of gastric Ulcer, oesophagitis 5.64% and gastric cancer 0.94% were detected. All three cases of gastric cancer were positive for H. pylori infection. Among gastritis, H. pylori was observed in 67 (44%) cases and 18 out of 57 (5.6 %) of normal gastric mucosa showed H. pylori. The endoscopic fi ndings such as gastritis, gastric ulcer, duodenal ulcer and gastro duodenal refl ux are signifi cantly associated with H pyloric infection. The prevalence of H. pylori infection is still high in peptic diseases. H pylori infection is signifi cantly correlated with peptic ulcer diseases than with non-ulcer dyspepsia. Further studies are required to establish the H. pylori positive cases with that of other tests such as serological detection of anti H. pylori antibody by ELISA/ICT and culture to establish a diagnosis quickly without any invasive method and institute proper management thus reducing morbidity

    Improving population-level refractive error monitoring via mixture distributions

    Introduction: Sampling and describing the distribution of refractive error in populations is critical to understanding eye care needs, refractive differences between groups and factors affecting refractive development. We investigated the ability of mixture models to describe refractive error distributions. Methods: We used key informants to identify raw refractive error datasets and a systematic search strategy to identify published binned datasets of community-representative refractive error. Mixture models combine various component distributions via weighting to describe an observed distribution. We modelled raw refractive error data with a single-Gaussian (normal) distribution, mixtures of two to six Gaussian distributions and an additive model of an exponential and Gaussian (ex-Gaussian) distribution. We tested the relative fitting accuracy of each method via Bayesian Information Criterion (BIC) and then compared the ability of selected models to predict the observed prevalence of refractive error across a range of cut-points for both the raw and binned refractive data. Results: We obtained large raw refractive error datasets from the United States and Korea. The ability of our models to fit the data improved significantly from a single-Gaussian to a two-Gaussian-component additive model and then remained stable with ≥3-Gaussian-component mixture models. Means and standard deviations for BIC relative to 1 for the single-Gaussian model, where lower is better, were 0.89 ± 0.05, 0.88 ± 0.06, 0.89 ± 0.06, 0.89 ± 0.06 and 0.90 ± 0.06 for two-, three-, four-, five- and six-Gaussian-component models, respectively, tested across US and Korean raw data grouped by age decade. Means and standard deviations for the difference between observed and model-based estimates of refractive error prevalence across a range of cut-points for the raw data were −3.0% ± 6.3, 0.5% ± 1.9, 0.6% ± 1.5 and −1.8% ± 4.0 for one-, two- and three-Gaussian-component and ex-Gaussian models, respectively. Conclusions: Mixture models appear able to describe the population distribution of refractive error accurately, offering significant advantages over commonly quoted simple summary statistics such as mean, standard deviation and prevalence

    X-Ray Magnetic Circular Dichroism of Pulsed Laser Deposited Co2MnSn and Co2MnSb Thin Films Grown on GaAs (001)

    We present the structural and element specific magnetic properties of Co2MnSn and Co2MnSb thin films grown on GaAs (100) substrates using pulsed laser deposition. X-ray magnetic circular dichroism (XMCD) spectra were measured for 400 A thick films at the L2,3 edges of Co and Mn. Element specific moments for Co and Mn in Co2MnSn were calculated from the x-ray absorption and XMCD spectra using the XMCD sum rules. The ratios of orbital to spin magnetic moments for Co and Mn were calculated for Co2MnSn and Co2MnSb
