    The Relationship Between Fall Rates and Fall Risk Inducing Drugs in Older Adult Patients in the Hospital Setting

    The aim of this study was to analyze the risk of falls relative to the use of fall risk inducing drugs (FRIDs) among older adult patients admitted to the hospital setting. Quality improvement of nursing fall risk assessment was of special interest. To analyze this a retroactive review of fall incident reports was conducted in collaboration with the study hospital’s risk manager. Medications were correlated with injuries sustained to identify high risk FRIDs. The selected study population consisted of 166 patients over the age of 65 who fell on the acute care campus of a hospital in the mid-south region of the United States. The results demonstrated no correlation between drug classes and injury sustained from a fall. Anti-hypertensive and cardiovascular medications were found to be involved in the highest number of falls. The study was limited however by a small sample size of nurses who adequately reported fall incidents. Further studies are needed to identify relationships between medications and fall risk.     It began in ponds and rivers : charting the beginnings of the ecology of fish cognition

    But fish cognitive ecology did not begin in rivers and streams. Rather, one of the starting points for work on fish cognitive ecology was work done on the use of visual cues by homing pigeons. Prior to working with fish, Victoria Braithwaite helped to establish that homing pigeons rely not just on magnetic and olfactory cues but also on visual cues for successful return to their home loft. Simple, elegant experiments on homing established Victoria's ability to develop experimental manipulations to examine the role of visual cues in navigation by fish in familiar areas. This work formed the basis of a rich seam of work whereby a fish's ecology was used to propose hypotheses and predictions as to preferred cue use, and then cognitive abilities in a variety of fish species, from model systems (Atlantic salmon and sticklebacks) to the Panamanian Brachyraphis episcopi. Cognitive ecology in fish led to substantial work on fish pain and welfare, but was never left behind, with some of Victoria's last work addressed to determining the neural instantiation of cognitive variation.Publisher PDFPeer reviewe

    How does Information Spread? An Exploratory Study of True and Fake News

    The intentional and non-intentional use of social media platforms resulting in digital wildfires of misinformation has increased significantly over the last few years. However, the factors that influence this rapid spread in the online space remain largely unknown. We study how believability and intention to share information are influenced by multiple factors, in addition to confirmation bias. We conducted an experiment where a mix of true and false articles was evaluated by study participants. Using hierarchical linear modelling to analyze our data, we found that, in addition to confirmation bias, believability is influenced by source endorser credibility and argument quality, both of which are moderated by the type of information – true or false. Source likeability had a positive main effect on believability. After controlling for belief and confirmation bias, intention to share information was affected by source endorser credibility and information source likeability

    Toward Youth Friendly Services: A Survey of Young People in Primary Care

    Background: The World Health Organization encourages the development of youth friendly services, yet little is known on how youth currently present in general practice. Objective: To describe the perspectives, expectations, and service receipt of young people presenting to family doctors to inform the development of youth friendly services. Design: Cross-sectional survey. Participants and measurements: Consecutive young people attending 26 randomly selected practices were recruited in the waiting rooms. Standardized instruments were used to interview them before their consultation. Results: Of 501 young people who were approached, 450 participated (91% participation rate). Most had respiratory (26%) or dermatological complaints (18%). When asked to assess their health status, 59% perceived they had neither a physical nor a mental illness. However, 43% stated they had fears about their health problem and 1 in 5 feared it could be life-threatening. Although only 10% presented with psychological complaints, 24% perceived they currently had a mental illness. The most common expectations were treatment (50%) and good communication (42%). Most youth were prescribed medication (60%), but 40% of those who received a prescription had not expected to receive a treatment. A follow-up appointment was offered to 57% of participants. Conclusions: This study identifies a gap between young people's perception of illness and their presentations to family doctors. It also highlights unexpected fears, and a mismatch between expectations and service receipt. These findings have implications for family medicine training and for clinical practice. They should inform the development of youth friendly service

    Factors That Predict Involvement in Online Instruction; A Comparison of Full-Time and Part-Time Community College Faculty

    Community college faculty represent more than one-third of all faculty within postsecondary education institutions and educate nearly half of all first-time college students (Cohen & Brawer, 2008; Gahn & Twombly, 2001; Huber, 1998; Jaeger & Eagan, 2009). Increasingly, two-year institutions are employing more part-time faculty. From 1988 to 1993, the percentage of part-time faculty at public two-year institutions rose from 52% to 62% (U.S. Department of Education, 1997). According to Outcalt (2002), part-time faculty now account for nearly 65% of all community college faculty. Although the use of part-time faculty is increasing in two-year institutions, part-time community college faculty are not treated in the same ways as their full-time counterparts

    Using an Online Learning Module in a Nursing Program to Improve Knowledge and Skills to Prevent Falls in the Community Dwelling Older Adult Population

    Nationally, 28.4% of older adults fall each year. Falls and resulting injury result in decreased mobility, functional impairment, loss of independence, and increased mortality. Utilization of evidence based protocols by providers to identify older adults at risk of falling is limited and rates of participation by older adults in prevention activities is low. Because of nursing’s increasing role in caring for older adults, development of fall prevention education for nursing students would result in increased awareness of the need for fall prevention in community dwelling older adults and increased access of older adults to falls risk assessment. There is a need to extend research to inform teaching and learning strategies for fall prevention. After pretesting, a convenience sample of 52 BSN students and 20 graduate nursing students completed an online education program and performed a falls risk assessment on an older adult before completing a posttest and self-efficacy survey. Data were analyzed using multivariate statistical tests. A qualitative approach was used to investigate a subset of student’s views about relationships between acquiring knowledge, self-efficacy, and skill mastery and their perceptions of barriers and facilitators to incorporating falls risk assessment into practice. Data were analyzed using the constant comparative method to review, code, and categorize data. Results revealed a rise in knowledge and student self-reporting of self-efficacy with falls risk assessment skills. Themes that emerged from semi-structured interviews included nurses are too busy to perform fall risk assessments, the older adult is a barrier, the importance of increasing awareness of falls prevention among nurses, opportunities to address health beliefs of older adults, and need for policies or guidelines. In conclusion, an online program enhanced with opportunity for hands on practice provides an effective environment for learning to use falls risk tools and should be incorporated into nursing education in order to increase older adults’ access to fall risk assessment

    Text message communication in primary care research: a randomized controlled trial

    Background. Text message communication is increasingly used in clinical practice but rarely in research. Particularly in young people, this method of participation in primary care research appears both feasible and acceptable. However, previous experience shows that text messaging for research may lead to lower response rates. Aim. To test the hypothesis that text message communication in primary care research does not lead to lower response rates compared to a paper-based method. Methods. This randomized controlled trial took place in 26 randomly selected practices in Victoria, Australia. Consecutive patients aged 16-24 years attending general practice appointments were recruited as part of a larger study on patients' perspectives. Patients owning a mobile phone were randomized to receive a question about satisfaction with the consultation either by text message or on a card completed before leaving the practice. Logistic regression was used to estimate the effect on the response rate of using text message rather than the card method, adjusting for clustering within practices and for differences in baseline characteristics among participants. Results. In total, 402 of 409 eligible young people agreed to participate and were randomized to either receive a text message (n = 193) or a card enquiry (n = 209). The response rate was 80.2% [95% confidence interval (CI): 73.3-87.1%] with text message and 85.6% (95% CI: 79.6-91.7%) with the card. The adjusted odds of responding (odds ratio: 0.62; 95% CI: 0.30-1.27) were not significantly lower in the group using text messaging compared to the group using the card response method. Conclusion.     Little research has been published about the varied role of the independent dancer. The aim of this study was to provide insight into the work independent dancers undertake and how their careers change over time. Semi-structured interviews were conducted with 14 independent dancers. Content analysis revealed that the dancers had multifaceted careers that relied on both formal and informal activities, and varied according to three distinct stages (early, middle, late). The experiences reported by the dancers indicated that the realities of the independent dancer's role are not sufficiently recognised or supported within the industry

    Cascading effects of a highly specialized beech-aphid–fungus interaction on forest regeneration

    Specialist herbivores are thought to often enhance or maintain plant diversity within ecosystems, because they prevent their host species from becoming competitively dominant. In contrast, specialist herbivores are not generally expected to have negative impacts on non-hosts. However, we describe a cascade of indirect interactions whereby a specialist sooty mold (Scorias spongiosa) colonizes the honeydew from a specialist beech aphid (Grylloprociphilus imbricator), ultimately decreasing the survival of seedlings beneath American beech trees (Fagus grandifolia). A common garden experiment indicated that this mortality resulted from moldy honeydew impairing leaf function rather than from chemical or microbial changes to the soil. In addition, aphids consistently and repeatedly colonized the same large beech trees, suggesting that seedling-depauperate islands may form beneath these trees. Thus this highly specialized three-way beech-aphid–fungus interaction has the potential to negatively impact local forest regeneration via a cascade of indirect effects

    A direct comparison of the consequences of plant genotypic and species diversity on communities and ecosystem function

    Biodiversity loss is proceeding at an unprecedented rate, yet we lack a thorough understanding of the consequences of losing diversity at different scales. While species diversity is known to impact community and ecosystem processes, genotypic diversity is assumed to have relatively smaller effects. Nonetheless, a few recent studies suggest that genotypic diversity may have quantitatively similar ecological consequences compared to species diversity. Here we show that increasing either genotypic diversity of common evening primrose (Oenothera biennis) or species diversity of old?field plant species resulted in nearly equivalent increases (?17%) in aboveground primary production. The predominant mechanism explaining this effect, niche complementarity, was similar for each type of diversity. Arthropod species richness also increased with both types of plant diversity, but the mechanisms leading to this effect differed: abundance?driven accumulation of arthropod species was important in plant genotypic polycultures, whereas resource specialization was important in plant species polycultures. Thus, similar increases in primary productivity differentially impacted higher trophic levels in response to each type of plant diversity. These results highlight important ecological similarities and differences between genotypic and species diversity and suggest that genotypic diversity may play a larger role in community and ecosystem processes than previously realized.The NSF (DEB- 0950231 to A.
