The Kent-Collins Protocol for Symptom Management in Post-Radiotherapy Eustachian Tube Dysfunction: A Case Report
Introduction: Eustachian tube dysfunction is a common side effect of radiotherapy for head and neck cancers or brain tumors, although few effective treatments exist. The Kent-Collins Protocol represents a novel therapeutic option for post-radiotherapy Eustachian tube dysfunction.
Case Description: The Kent-Collins Protocol was employed in one patient with Eustachian tube dysfunction following fractionated stereotactic radiotherapy for cerebral metastases from HER-2 positive breast cancer. The Kent-Collins Protocol provided relief of symptoms (conductive hearing loss, aural fullness, and otalgia) for a range of 5 to 45 minutes. The protocol could be repeated when symptoms returned to provide additional relief. The patient also reported psychological benefit from this protocol, knowing the hearing loss was reversible and not likely to be permanent.
Discussion: This case demonstrates the potential of The Kent-Collins Protocol for the treatment of post-radiotherapy Eustachian tube dysfunction. The protocol is effective, inexpensive, easy to teach, and safe, offering an alternative to invasive therapies currently in existence
Leader autonomy support in the workplace: A meta-analytic review
Leader autonomy support (LAS) refers to a cluster of supervisory behaviors that are theorized to facilitate self-determined motivation in employees, potentially enabling well-being and performance. We report the results of a meta-analysis of perceived LAS in work settings, drawing from a database of 754 correlations across 72 studies (83 unique samples, Nβ=β32,870). Results showed LAS correlated strongly and positively with autonomous work motivation, and was unrelated to controlled work motivation. Correlations became increasingly positive with the more internalized forms of work motivation described by self-determination theory. LAS was positively associated with basic needs, well-being, and positive work behaviors, and was negatively associated with distress. Correlations were not moderated by the source of LAS, country of the sample, publication status, or the operationalization of autonomy support. In addition, a meta-analytic path analysis supported motivational processes that underlie LAS and its consequences in workplaces. Overall, our findings lend support for autonomy support as a leadership approach that is consistent with self-determination and optimal functioning in work settings
Severe postpartum disruption of the pelvic ring: report of two cases and review of the literature
Pelvic dislocations are rare during labor, and the treatment is controversial. We report two cases of young women who sustained postpartum disruption of the pelvic ring: one case is an 8.8 cm wide separation of the pubic symphysis with sacroiliac joint disruption underwent surgical stabilization and the second case with 4.0 cm disruption being treated non-operatively. These cases illustrated of importance of accurate diagnosis, careful physical exam, fully informed consent and specific treatment for this condition
Update on Apollo Data Restoration by the NSSDC and the PDS Lunar Data Node
The Lunar Data Node (LDN) , under the auspices of the Geosciences Node of the Planetary Data System (PDS) and the National Space Science Data Center (NSSDC), is continuing its efforts to recover and restore Apollo science data. The data being restored are in large part archived with NSSDC on older media, but unarchived data are also being recovered from other sources. They are typically on 7- or 9-track magnetic tapes, often in obsolete formats, or held on microfilm, microfiche, or paper documents. The goal of the LDN is to restore these data from their current form, which is difficult for most researchers to access, into common digital formats with all necessary supporting data (metadata) and archive the data sets with PDS. Restoration involves reading the data from the original media, deciphering the data formats to produce readable digital data and converting the data into usable tabular formats. Each set of values in the table must then be understood in terms of the quantity measured and the units used. Information on instrument properties, operational history, and calibrations is gathered and added to the data set, along with pertinent references, contacts, and other ancillary documentation. The data set then undergoes a peer review and the final validated product is archived with PDS. Although much of this effort has concentrated on data archived at NSSDC in the 1970's, we have also recovered data and information that were never sent to NSSDC. These data, retrieved from various outside sources, include raw and reduced Gamma-Ray Spectrometer data from Apollos 15 and 16, information on the Apollo 17 Lunar Ejecta And Meteorites experiment, Dust Detector data from Apollos 11, 12, 14, and I5, raw telemetry tapes from the Apollo ALSEPs, and Weekly Status Reports for all the Apollo missions. These data are currently being read or organized, and supporting data is being gathered. We are still looking for the calibrated heat flow data from Apollos 15 and 17 for the period 1975-1977, any assistance or information on these data would be welcome. NSSDC has recently been tasked to release its hard-copy archive, comprising photography, microfilm, and microfiche. The details are still being discussed, but we are concentrating on recovering the valuable lunar data from these materials while they are still readily accessible. We have identified the most critical of these data and written a LASER proposal to fund their restoration. Included in this effort are data from the Apollo 15 and 16 Mass Spectrometers and the Apollo 17 Par-UV Spectrometer and ancillary information on the Apollo 17 Surface Electrical Properties Experiment
Contrast nephropathy in patients with impaired renal function: High versus low osmolar media
Contrast nephropathy in patients with impaired renal function: High versus low osmolar media. Prescription of low osmolar contrast to prevent nephrotoxicity in subjects with pre-existing renal impairment is costly and has not been clearly shown to be effective. We entered 249 subjects with a pre-contrast serum creatinine greater than 120 /xmol/liter (1.35 mg/dl) having cardiac catheterization or intravenous contrast into a randomized controlled trial comparing high and low osmolar contrast. The outcome assessed was a rise in serum creatinine repeated 48 to 72 hours after contrast. A further 117 patients entered the non-randomized prospective arm of the study. In the randomized study the serum creatinine rose by at least 25% after contrast in 8 of 117 (6.8%) given high and in 5 of 132 (3.8%) given low osmolar contrast (P > 0.05, one-tailed 95% confidence interval for the difference 3 to 7.8%). More severe renal failure (greater than 50% increase in serum creatinine) after contrast was uncommon (3.4% with high and 1.5% with low osmolar contrast). A rise in serum creatinine after contrast was significantly associated with the severity of the pre-contrast renal impairment and the presence of diabetes mellitus, but not with type of contrast. Diabetics with a serum creatinine greater than 200 Β΅l/liter (2.25 mg/dl) pre-contrast had a highest risk of deterioration in renal function after contrast. We conclude that in patients with pre-existing renal impairment the incidence of contrast nephropathy was not significantly different comparing high osmolar and nonionic contrast. The potential benefit of nonionic contrast in moderate renal impairment is likely to be small, but trials in diabetics with severe renal impairment should be undertaken urgently
Improved AlGaInP-based red (670β690 nm) surface-emitting lasers with novel C-doped short-cavity epitaxial design
Includes bibliographical references (page 331).A modified epitaxial design leads to straightforward implementation of short (1Ξ») optical cavities and the use of C as the sole p-type dopant in AlGaInP/AlGaAs red vertical-cavity surface-emitting lasers (VCSELs). Red VCSELs fabricated into simple etched air posts operate continuous wave at room temperature at wavelengths between 670 and 690 nm, with a peak output power as high as 2.4 mW at 690 nm, threshold voltage of 2.2 V, and peak wallplug efficiency of 9%. These values are all significant improvements over previous results achieved in the same geometry with an extended optical cavity epitaxial design. The improved performance is due primarily to reduced optical losses and improved current constriction and dopant stability.This work was supported by the U.S. Department of Energy under Contract No. DE-AC04-94AL85000
Restoration of Apollo Data by the Lunar Data Project/PDS Lunar Data Node: An Update
The Apollo 11, 12, and 14 through 17 missions orbited and landed on the Moon, carrying scientific instruments that returned data from all phases of the missions, included long-lived Apollo Lunar Surface Experiments Packages (ALSEPs) deployed by the astronauts on the lunar surface. Much of these data were never archived, and some of the archived data were on media and in formats that are outmoded, or were deposited with little or no useful documentation to aid outside users. This is particularly true of the ALSEP data returned autonomously for many years after the Apollo missions ended. The purpose of the Lunar Data Project and the Planetary Data System (PDS) Lunar Data Node is to take data collections already archived at the NASA Space Science Data Coordinated Archive (NSSDCA) and prepare them for archiving through PDS, and to locate lunar data that were never archived, bring them into NSSDCA, and then archive them through PDS. Preparing these data for archiving involves reading the data from the original media, be it magnetic tape, microfilm, microfiche, or hard-copy document, converting the outmoded, often binary, formats when necessary, putting them into a standard digital form accepted by PDS, collecting the necessary ancillary data and documentation (metadata) to ensure that the data are usable and well-described, summarizing the metadata in documentation to be included in the data set, adding other information such as references, mission and instrument descriptions, contact information, and related documentation, and packaging the results in a PDS-compliant data set. The data set is then validated and reviewed by a group of external scientists as part of the PDS final archive process. We present a status report on some of the data sets that we are processing
SWI/SNF-like chromatin remodeling factor Fun30 supports point centromere function in S. cerevisiae
Budding yeast centromeres are sequence-defined point centromeres and are, unlike in many other organisms, not
embedded in heterochromatin. Here we show that Fun30, a poorly understood SWI/SNF-like chromatin remodeling factor
conserved in humans, promotes point centromere function through the formation of correct chromatin architecture at
centromeres. Our determination of the genome-wide binding and nucleosome positioning properties of Fun30 shows that
this enzyme is consistently enriched over centromeres and that a majority of CENs show Fun30-dependent changes in
flanking nucleosome position and/or CEN core micrococcal nuclease accessibility. Fun30 deletion leads to defects in histone
variant Htz1 occupancy genome-wide, including at and around most centromeres. FUN30 genetically interacts with CSE4,
coding for the centromere-specific variant of histone H3, and counteracts the detrimental effect of transcription through centromeres on chromosome segregation and suppresses transcriptional noise over centromere CEN3. Previous work has shown a requirement for fission yeast and mammalian homologs of Fun30 in heterochromatin assembly. As centromeres in budding yeast are not embedded in heterochromatin, our findings indicate a direct role of Fun30 in centromere chromatin by promoting correct chromatin architecture
The GstLAL Search Analysis Methods for Compact Binary Mergers in Advanced LIGO's Second and Advanced Virgo's First Observing Runs
After their successful first observing run (September 12, 2015 - January 12,
2016), the Advanced LIGO detectors were upgraded to increase their sensitivity
for the second observing run (November 30, 2016 - August 26, 2017). The
Advanced Virgo detector joined the second observing run on August 1, 2017. We
discuss the updates that happened during this period in the GstLAL-based
inspiral pipeline, which is used to detect gravitational waves from the
coalescence of compact binaries both in low latency and an offline
configuration. These updates include deployment of a zero-latency whitening
filter to reduce the over-all latency of the pipeline by up to 32 seconds,
incorporation of the Virgo data stream in the analysis, introduction of a
single-detector search to analyze data from the periods when only one of the
detectors is running, addition of new parameters to the likelihood ratio
ranking statistic, increase in the parameter space of the search, and

The Linear Theory Power Spectrum from the Lyman-alpha Forest in the Sloan Digital Sky Survey
We analyze the SDSS Ly-alpha forest P_F(k,z) measurement to determine the
linear theory power spectrum. Our analysis is based on fully hydrodynamic
simulations, extended using hydro-PM simulations. We account for the effect of
absorbers with damping wings, which leads to an increase in the slope of the
linear power spectrum. We break the degeneracy between the mean level of
absorption and the linear power spectrum without significant use of external
constraints. We infer linear theory power spectrum amplitude
Delta^2_L(k_p=0.009s/km,z_p=3.0)=0.452_{-0.057-0.116}^{+0.069+0.141} and slope
n_eff=-2.321_{-0.047-0.102}^{+0.055+0.131} (possible systematic errors are
included through nuisance parameters in the fit - a factor >~5 smaller errors
would be obtained on both parameters if we ignored modeling uncertainties). The
errors are correlated and not perfectly Gaussian, so we provide a chi^2 table
to accurately describe the results. The result corresponds to sigma_8=0.85,
n=0.94, for a LCDM model with Omega_m=0.3, Omega_b=0.04, and h=0.7, but is most
useful in a combined fit with the CMB. The inferred curvature of the linear
power spectrum and the evolution of its amplitude and slope with redshift are
consistent with expectations for LCDM models, with the evolution of the slope,
in particular, being tightly constrained. We use this information to constrain
systematic contamination, e.g., fluctuations in the UV background. This paper
should serve as a starting point for more work to refine the analysis,
including technical improvements such as increasing the size and number of the
hydrodynamic simulations, and improvements in the treatment of the various
forms of feedback from galaxies and quasars.Comment: Improved presentation, including fit results for (z). Simple code
