    Analysis of ISER 2008-2009 Survey Data

    In September 2008, the superintendent of the Anchorage School District and the mayor of Anchorage sent a letter to the governor of Alaska, reporting what they thought might be an influx of students into Anchorage from rural communities. Enrollment in the school district was higher than expected, and it coincided with the largest-ever Alaska Permanent Fund dividend and with a one-time payment of $1,200 the state made per person, to help offset high energy costs. Researchers at the Institute of Social and Economic Research (ISER) at UAA have a longstanding interest in migration patterns in Alaska and the Arctic, and they saw the increased enrollment in Anchorage schools as a potential opportunity to better understand: ‱ If rural Alaskans are moving to Anchorage ‱ Where they are coming from ‱ Why they are moving So with the cooperation of the Anchorage School District, ISER conducted a survey of the parents or guardians of students who had enrolled in Anchorage in the 2007-2008 or 2008-2009 school years and who had transferred in from other Alaska school districts. Besides finding out where students were coming from—and why—another purpose of the study was to provide the Anchorage School District and the Municipality of Anchorage with information about what they could do to help students and families who are new to the city.     Volumes that Speak: The Architectural Books of the Drayton Library Catalog and the Design of Drayton Hall

    Drayton Hall, an early eighteenth-century plantation house on the Ashley River in Charleston, South Carolina, is widely considered to be the first Palladian house in the United States. Now owned and operated by the National Trust for Historic Preservation, Drayton Hall is something of a laboratory for the study of archaeology, landscape architecture, material culture, social history, and historic preservation. Though new discoveries are made almost every day at Drayton Hall, the origins of the house’s design remain unknown. In 2009, the Drayton Library Catalog was discovered within the Drayton manuscript collection containing references to seven popular eighteenth-century architectural books. By comparing measured drawings of Drayton Hall to designs found in these seven books, this project examines the origin of the house’s design in the context of published sources that would have been available at the time of its construction. While some of the books discussed have been previously identified as sources of inspiration, this project led to the discovery of a correlation between a pattern book plate and executed design from one of the seven books in the Drayton Library Catalog. The scope of this project was limited to the seven books in the Catalog and the previously identified sources. For that reason, measured drawings of the interior are included so that this document may be a tool for future studies outside the parameters of this project

    Nurse-Related Clinical Non-licensed Personnel in U.S. Hospitals and their Relationship with Nurse Staffing Levels

    The use of the minimally trained, low wage clinical non-licensed personnel (CNLP), who perform clinical tasks under the supervision of registered nurses (RNs) or other licensed clinical providers, has been a primary strategy for hospitals both to manage professional shortages and reduce costs (Huston 1996; Zimmerman 2000; Orne et al. 1998; Keenan 2003). This study examined the nurse-related clinical non-licensed personnel (CNLP) staffing in U.S. hospitals between 2010 and 2014, in terms of their job categories, staffing trends, and relationship with registered nurse (RN) and licensed practical nurse (LPN) staffing. Key Questions: How were CNLPs used in hospitals in 2014 in comparison to registered nurse (RN) and licensed practice nurse (LPN) staffing levels? How did the variations in staffing for these CNLP groups correlate with RN and LPN staffing, as well as other factors such as patient mix? How have these relationships changed over time

    Hyperreactive Malarial Splenomegaly Syndrome (HMSS) was described and defined before sensitive tests for malaria were available. We present a series of seven individuals who were referred to our clinics with possible HMSS. Chronic malaria was demonstrated in those successfully treated but not in those who failed to respond to therapy. This observation suggests that the newer molecular malaria assays have a role to play in the identification of individuals who are likely to respond to treatment for HMSS in non-endemic regions

    Clinical Features of Imported Loiasis: A Case Series from the Hospital for Tropical Diseases, London.

    We retrospectively analyzed the background, clinical features, and treatment response of 50 cases of imported loiasis who presented between 2000 and 2014 to the Hospital for Tropical Diseases (HTD), London, United Kingdom. Of them, 29 were migrants from, and 21 were visitors to, countries where the disease is endemic. Clinical features differed between these groups. Migrants experienced fewer Calabar swellings (odds ratio [OR] = 0.12), more eye worm (OR = 3.4), more microfilaremia (OR = 3.5), lower filarial antibody levels, and lower eosinophil counts (P 3,000 microfilariae/mL and all those with an elevated C-reactive protein (CRP) (≄ 5 mg/L) before treatment had reactions (P = 0.10 and P = 0.01, respectively). These data suggest that monotherapy with DEC may not be the optimal treatment for patients with loiasis, particularly for those with a high microfilarial load

    Application of an enzymatic extract from Aspergillus niger as coagulant for cheddar cheese manufacture

    Q2 segĂșn Scopus a la fecha de publicaciĂłnThe coagulation of milk by a serin protease from Aspergillus niger NRRL3 was studied by rheology. Cheddar-type cheese was manufactured using 3.5% (v/v) of fungal enzymatic extract and fermentation-produced chymosin was used as control coagulant. Full composition and ripening of both kinds of Cheddar cheese were studied. Differences in the proteolysis of caseins, not only during cheese manufacture but also during ripening, affected cheese composition, texture and peptide profile.     Clinical indicators of bacterial meningitis among neonates and young infants in rural Kenya

    <p>Abstract</p> <p>Background</p> <p>Meningitis is notoriously difficult to diagnose in infancy because its clinical features are non-specific. World Health Organization (WHO) guidelines suggest several indicative signs, based on limited data. We aimed to identify indicators of bacterial meningitis in young infants in Kenya, and compared their performance to the WHO guidelines. We also examined the feasibility of developing a scoring system for meningitis.</p> <p>Methods</p> <p>We studied all admissions aged < 60 days to Kilifi District Hospital, 2001 through 2005. We evaluated clinical indicators against microbiological findings using likelihood ratios. We prospectively validated our findings 2006 through 2007.</p> <p>Results</p> <p>We studied 2,411 and 1,512 young infants during the derivation and validation periods respectively. During derivation, 31/1,031 (3.0%) neonates aged < 7 days and 67/1,380 (4.8%) young infants aged 7-59 days (p < 0.001) had meningitis. 90% of cases could be diagnosed macroscopically (turbidity) or by microscopic leukocyte counting. Independent indicators of meningitis were: fever, convulsions, irritability, bulging fontanel and temperature ≄ 39°C. Areas under the receiver operating characteristic curve in the validation period were 0.62 [95%CI: 0.49-0.75] age < 7 days and 0.76 [95%CI: 0.68-0.85] thereafter (P = 0.07), and using the WHO signs, 0.50 [95%CI 0.35-0.65] age < 7 days and 0.82 [95%CI: 0.75-0.89] thereafter (P = 0.0001). The number needed to LP to identify one case was 21 [95%CI: 15-35] for our signs, and 28 [95%CI: 18-61] for WHO signs. With a scoring system, a cut-off of ≄ 1 sign offered the best compromise on sensitivity and specificity.</p> <p>Conclusion</p> <p>Simple clinical signs at admission identify two thirds of meningitis cases in neonates and young infants. Lumbar puncture is essential to diagnosis and avoidance of unnecessary treatment, and is worthwhile without CSF biochemistry or bacterial culture. The signs of Meningitis suggested by the WHO perform poorly in the first week of life. A scoring system for meningitis in this age group is not helpful.</p

    Oligomerization engineering of the fluorinase enzyme leads to an active trimer that supports synthesis of fluorometabolites in vitro

The fluorinase is known to operate in a hexameric form, but the consequence(s) of the oligomerization status on the enzyme activity and its catalytic properties remain largely unknown. In this work, this aspect was explored by rationally engineering trimeric fluorinase variants that retained the same catalytic rate as the wild-type enzyme. These results ruled out hexamerization as a requisite for the fluorination activity. The Michaelis constant (KM) for S-adenosyl-l-methionine, one of the substrates of the fluorinase, increased by two orders of magnitude upon hexamer disruption. Such a shift in S-adenosyl-l-methionine affinity points to a long-range effect of hexamerization on substrate binding – likely decreasing substrate dissociation and release from the active site. A practical application of trimeric fluorinase is illustrated by establishing in vitro fluorometabolite synthesis in a bacterial cell-free system.Publisher PDFPeer reviewe
