    A survey of Trace Metals Determination in Hospital Waste Incinerator in Lucknow City, India

    Information on the elemental content of incinerator burning of human organ, animal and medical waste is scanty in India Nineteen trace elements were analyzed in the incinerator ash from four major hospitals, one municipal waste incinerator and two R & D laboratories engaged in animal experiment in Lucknow city. Concentrations of Zinc and Lead were found to be very high in comparison to other metals due to burning of plastic products. The source of Ca, P and K are mainly bone, teeth and other animal organs. A wide variation in trace concentration of several toxic elements have been seen due to variation in initial waste composition, design of the incinerator and operating conditions

    ANTI-INFLAMMATORY ACTIVITY OF NEURACANTHUS SPHAEROSTACHYUS DALZ. LEAVES

    Objective: Neuracanthus sphaerostachyus has been traditionally used to treat skin diseases, cough, and asthma. Lack of sufficient scientific evidence indicating the utility of this plant in the treatment of inflammation prompted us to investigate the anti-inflammatory activity of the plant in different experimental screening methods. Methods: In vitro and in vivo anti-inflammatory activity of the methanolic and aqueous extracts of N. sphaerostachyus (MENS and AENS) leaves at doses of 125, 250, and 500 mg/kg was evaluated with albumin denaturation and carrageenan-induced paw edema in rats and acetic acid-induced increased vascular permeability in mice. Results: Methanolic and aqueous extract significantly inhibited protein denaturation as well as edema induced by carrageenan and vascular permeability in mice dose dependently. Aspirin (0.1 mg/ml), indomethacin (10 mg/kg), and dexamethasone (5 mg/kg) were used as a standard control. Conclusion: It is concluded that MENS and AENS leaves exhibited significant anti-inflammatory activity

    Development of Reverse-Phase HPLC Method for Simultaneous Analysis of Metoprolol Succinate and Hydrochlorothiazide in a Tablet Formulation

    Purpose: To develop a simple, sensitive and rapid reverse phase HPLC method for the simultaneous analysis of metoprolol succinate and hydrochlorothiazide in a solid dosage form. Methods: The drugs were analysed by a reverse phase C-18 column using 50mM di-sodium hydrogen phosphate:methanol:acetonitrile in a ratio of 525:225:250 as mobile phase. The flow rate was 1 ml/min and the compounds were detected by a UV-detector at 222 nm at a column temperature of 24 ± 2 ºC. The method was statistically validated for linearity and accuracy. Results: The retention time and drug content of metoprolol succinate and  hydrochlorothiazide were 5.38 min, 96.05 % and 3.04 min., 97.64 %, respectively. Conclusion: The study shows that the developed method is simple and accurate and that it would be suitable for the simultaneous determination of metoprolol succinate and hydrochlorothiazide in pharmaceutical formulations.Keywords: Metoprolol succinate; Hydrochlorothiazide; Simultaneous analysis; High performance liquid chromatography; Tablet formulatio

    Role of skeletal scintigraphy in soft tissue sarcoma: Improving the diagnostic yield

    Background: The presence of skeletal metastases significantly influences the therapeutic strategy adopted for soft tissue sarcoma. However, literature on the prevalence of skeletal metastases in soft tissue sarcoma is limited and none of the available data is based on the Indian patient population. Aim: To determine the prevalence of skeletal metastases at presentation in patients of soft tissue sarcoma and to rationalise the use of preoperative skeletal scintigraphy in such patients. Methods and Material: Preoperative bone scans were evaluated in 122 patients with soft tissue sarcoma (median age, 34 years; range, 4-83). The scans were classified into 3 grades: Grade 1: metastases very likely; Grade 2: equivocal; Grade 3: normal or benign lesion. In all the patients studied, the ability of the patient to localize the site or sites of pain was recorded and that was correlated with the site of metastases in scintigraphy. Result: Seventeen (13.9%) patients had Grade 1 scan; 16 of them had bony pain that was not readily explainable by trauma or other local factors. Ten ( 8.1%) patients had Grade 2 scan, five of them had bony pain which was not readily explainable by trauma or other local factors. Ninety-five patients (77.8%) had Grade 3 scan. Of these, 9 had localised bone pain which could be definitely associated with trauma or joint degeneration. Conclusion: The prevalence of skeletal metastases at presentation in patients with soft tissue sarcoma is low (13.9%). The low rates of skeletal metastases in bone pain-free patients (0.9%) versus the high rate in symptomatic patients (76.1%) supports the use of bone scanning in symptomatic patients only

    Generation of fusion protein EGFRvIII-HBcAg and its anti-tumor effect in vivo

    The epidermal growth factor receptor variant III (EGFRvIII) is the most common variation of EGFR. Because it shows a high frequency in several different types of tumor and has not been detected in normal tissues, it is an ideal target for tumor specific therapy. In this study, we prepared EGFRvIII-HBcAg fusion protein. After immunization with fusion protein, HBcAg or PBS, the titers of antibody in BALB/c mice immunized with fusion protein reached 2.75 × 105. Western blot analysis demonstrated that the fusion protein had specific antigenicity against anti-EGFRvIII antibody. Further observation showed fusion protein induced a high frequency of IFN-γ-secreting lymphocytes. CD4+T cells rather than CD8+T cells were associated with the production of IFN-γ. Using Renca-vIII(+) cell as specific stimulator, we observed remarkable cytotoxic activity in splenocytes from mice immunized with fusion protein. Mice were challenged with Renca-vIII(+) cells after five times immunization. In fusion protein group, three of ten mice failed to develop tumor and all survived at the end of the research. The weight of tumors in fusion protein were obviously lighter than that in other two groups (t = 4.73, P = 0.044;t = 6.89, P = 0.040). These findings demonstrated that EGFRvIII-HBcAg fusion protein triggered protective responses against tumor expressing EGFRvIII

    Risk factors for hospital admission with RSV bronchiolitis in England: a population-based birth cohort study.

    OBJECTIVE: To examine the timing and duration of RSV bronchiolitis hospital admission among term and preterm infants in England and to identify risk factors for bronchiolitis admission. DESIGN: A population-based birth cohort with follow-up to age 1 year, using the Hospital Episode Statistics database. SETTING: 71 hospitals across England. PARTICIPANTS: We identified 296618 individual birth records from 2007/08 and linked to subsequent hospital admission records during the first year of life. RESULTS: In our cohort there were 7189 hospital admissions with a diagnosis of bronchiolitis, 24.2 admissions per 1000 infants under 1 year (95%CI 23.7-24.8), of which 15% (1050/7189) were born preterm (47.3 bronchiolitis admissions per 1000 preterm infants (95% CI 44.4-50.2)). The peak age group for bronchiolitis admissions was infants aged 1 month and the median was age 120 days (IQR = 61-209 days). The median length of stay was 1 day (IQR = 0-3). The relative risk (RR) of a bronchiolitis admission was higher among infants with known risk factors for severe RSV infection, including those born preterm (RR = 1.9, 95% CI 1.8-2.0) compared with infants born at term. Other conditions also significantly increased risk of bronchiolitis admission, including Down's syndrome (RR = 2.5, 95% CI 1.7-3.7) and cerebral palsy (RR = 2.4, 95% CI 1.5-4.0). CONCLUSIONS: Most (85%) of the infants who are admitted to hospital with bronchiolitis in England are born at term, with no known predisposing risk factors for severe RSV infection, although risk of admission is higher in known risk groups. The early age of bronchiolitis admissions has important implications for the potential impact and timing of future active and passive immunisations. More research is needed to explain why babies born with Down's syndrome and cerebral palsy are also at higher risk of hospital admission with RSV bronchiolitis

Many small organizations, such as Operation Rainbow, were thrust into the unfamiliar environment of relief surgery, whereas they normally provide short elective reconstruction missions in developing countries. Materials Because of the chaotic nature of relief work, it was assumed that the organization’s efforts would be less cost-effective than their usual elective work. To evaluate this conclusion, the present study was designed to compare the cost-effectiveness of the organization’s usual elective missions with the emergency relief provided in the wake of the Haiti earthquake. Results and conclusions The assumption that emergency costs would be higher was proven wrong, with estimates of 362perdisability−adjustedlife−year(DALY)avertedintheelectivegroup,and362 per disability-adjusted life-year (DALY) averted in the elective group, and 343 per DALY averted in the relief group

    Stimulating cognition in schizophrenia: A controlled pilot study of the effects of prefrontal transcranial direct current stimulation upon memory and learning

    BACKGROUND: Schizophrenia is characterized by prominent cognitive deficits, impacting on memory and learning; these are strongly associated with the prefrontal cortex. OBJECTIVE/HYPOTHESIS: To combine two interventions, transcranial direct current stimulation (tDCS) over the prefrontal cortex and cognitive training, to examine change in cognitive performance in patients with schizophrenia. METHODS: A double blind, sham-controlled pilot study of 49 patients with schizophrenia, randomized into real or sham tDCS stimulation groups. Subjects participated in 4 days of cognitive training (days 1, 2, 14, 56) with tDCS applied at day-1 and day-14. The primary outcome measure was change in accuracy on working memory and implicit learning tasks from baseline. The secondary outcome measure was the generalization of learning to non-trained task, indexed by the CogState neuropsychological battery. Data analysis was conducted using multilevel modelling and multiple regressions. RESULTS: 24 participants were randomized to real tDCS and 25 to sham. The working memory task demonstrated a significant mean difference in performance in the tDCS treatment group: at day-2 (b = 0.68, CI 0.14-1.21; p = 0.044) and at day-56 (b = 0.71, 0.16-1.26; p = 0.044). There were no significant effects of tDCS on implicit learning. Trend evidence of generalization onto untrained tasks of attention and vigilance task (b = 0.40, 0.43-0.77; p = 0.058) was found. CONCLUSIONS: This is the first study to show a significant longer-term effect of tDCS on working memory in schizophrenia. Given the current lack of effective therapies for cognitive deficits, tDCS may offer an important novel approach to modulating brain networks to ameliorate cognitive deficits in schizophrenia
