    Pharyngeal airway dimensions: a cephalometric, growth-study-based analysis of physiological variations in children aged 6-17

    SummaryOBJECTIVE: The aim was to assess pharyngeal airway dimensions and physiological changes based on lateral cephalometric radiographs from healthy untreated children aged 6-17 years. MATERIALS/METHODS: The sample consisted of 880 lateral cephalograms (412 females and 468 males) of the Zurich Craniofacial Growth Study. Statistical analyses on cephalometric measurements of airway dimensions (distances ‘p': shortest distance between soft palate and posterior pharyngeal wall and ‘t': shortest distance between tongue and posterior pharyngeal wall) and craniofacial parameters were performed. To disclose differences between different age groups, a Kruskal-Wallis test was applied. The influence of gender on ‘p' and ‘t' was analysed by a Mann-Whitney U-test for each age group separately. The Spearman correlation was computed in order to investigate associations between craniofacial parameters. Variables associated with ‘p' and ‘t' were chosen for multiple regression model investigation. RESULTS: The results demonstrated high interindividual variations. A slight influence of age on ‘p' (P = 0.034) could be attested (+1.03mm) but not on ‘t' (P = 0.208). With the exception of the 9-year age group, no significant differences between the genders were found. Correlation analysis revealed several statistically significant correlations between ‘t' or ‘p' and antero-posterior cephalometric variables. All correlation coefficients were, however, very low and the adjusted coefficient of determination also revealed the regression model to be very weak. CONCLUSIONS: The high interindividual variations of ‘p' and ‘t' render the use of reference values problematic. Contrary to other craniofacial structures, neither age-related changes nor sexual dimorphism were found for ‘p' and ‘t'. Any associations to antero-posterior cephalometric characteristics seem lo

    Craniofacial growth and SITAR growth curve analysis

    BACKGROUND: SITAR (SuperImposition by Translation And Rotation) is a shape invariant growth curve model that effectively summarizes somatic growth in puberty. AIM: To apply the SITAR model to longitudinal mandibular growth data to clarify its suitability to facial growth analysis. SUBJECTS AND METHODS: 2D-cephalometric data on two mandibular measurements (AP: articulare–pogonion; CP: condylion–pogonion) were selected from the Denver Growth Study, consisting of longitudinal records (age range: 7.9–19.0 years) of females (sample size N: 21; number of radiographs n: 154) and males (N: 18; n: 137). The SITAR mixed effects model estimated, for each measurement and gender separately, a mean growth curve versus chronological age, along with mean age at peak velocity (APV) and peak velocity (PV), plus subject-specific random effects for PV and mean size. The models were also fitted versus Greulich–Pyle bone age. RESULTS: In males, mean APV occurred at 14.6 years (AP) and 14.4 years (CP), with mean PV 3.1 mm/year (AP) and 3.3 mm/year (CP). In females, APV occurred at 11.6 years (AP and CP), with mean PV 2.3 mm/year (AP) and 2.4 mm/year (CP). The models explained 95–96 per cent of the cross-sectional variance for males and 92–93 per cent for females. The random effects demonstrated standard deviations (SDs) in size of 5.6 mm for males and 3.9 mm for females, and SDs for PV between 0.3 and 0.5 mm/year. The bone age results were similar. CONCLUSION: The SITAR model is a useful tool to analyse epidemiological craniofacial growth based on cephalometric data and provides an array of information on pubertal mandibular growth and its variance in a concise manner

    Motivation for orthognathic treatment and anticipated satisfaction levels - a two-centre cross-national audit

    PURPOSE: This audit investigated factors which motivate patients to seek orthognathic treatment, assessed how confident patients were that they would be satisfied with the outcome of treatment, and explored possible influencing factors. MATERIALS AND METHODS: Questionnaires were distributed to pre-surgical patients at two centres (United Kingdom and Switzerland); questions asked what patients wished to gain from orthognathic treatment and how confident they were that they would be satisfied with treatment outcome. Gender, age and location were recorded as demographic variables, and type of malocclusion was also recorded. RESULTS: A total of 202 questionnaires were returned (UK, n=149; Switzerland, n=53). Reported motivating factors focused on improvements in aesthetics (specified and unspecified) (UK vs. Switzerland: 91.3% vs. 83.0%), function (72.5% vs. 66.0%), psychosocial health (51.7% vs. 20.8%), speech (4.0% vs. 7.5%), alleviation of pain (5.4% vs. 17%) and normalization of breathing (1.3% vs. 7.5%). No significant relationships were observed relative to patient age, gender or malocclusion. The anticipated satisfaction levels were generally high (86.5% vs. 89.9%). CONCLUSION: Although the distribution of motivational factors varied between the two sites, it did not affect the anticipated satisfaction level. Patients were generally confident that they would be satisfied with their treatment outcome and that their reasons for seeking treatment would be addressed

    MRI of the temporo-mandibular joint: which sequence is best suited to assess the cortical bone of the mandibular condyle? A cadaveric study using micro-CT as the standard of reference

    OBJECTIVE: To determine the best suited sagittal MRI sequence out of a standard temporo-mandibular joint (TMJ) imaging protocol for the assessment of the cortical bone of the mandibular condyles of cadaveric specimens using micro-CT as the standard of reference. METHODS: Sixteen TMJs in 8 human cadaveric heads (mean age, 81 years) were examined by MRI. Upon all sagittal sequences, two observers measured the cortical bone thickness (CBT) of the anterior, superior and posterior portions of the mandibular condyles (i.e. objective analysis), and assessed for the presence of cortical bone thinning, erosions or surface irregularities as well as subcortical bone cysts and anterior osteophytes (i.e. subjective analysis). Micro-CT of the condyles was performed to serve as the standard of reference for statistical analysis. RESULTS: Inter-observer agreements for objective (r = 0.83-0.99, P < 0.01) and subjective (κ = 0.67-0.88) analyses were very good. Mean CBT measurements were most accurate, and cortical bone thinning, erosions, surface irregularities and subcortical bone cysts were best depicted on the 3D fast spoiled gradient echo recalled sequence (3D FSPGR). CONCLUSION: The most reliable MRI sequence to assess the cortical bone of the mandibular condyles on sagittal imaging planes is the 3D FSPGR sequence. KEY POINTS: MRI may be used to assess the cortical bone of the TMJ. • Depiction of cortical bone is best on 3D FSPGR sequences. • MRI can assess treatment response in patients with TMJ abnormalities

    Surfing the spectrum - what is on the horizon?

    Diagnostic imaging techniques have evolved with technological advancements - but how far? The objective of this article was to explore the electromagnetic spectrum to find imaging techniques which may deliver diagnostic information of equal, or improved, standing to conventional radiographs and to explore any developments within radiography which may yield improved diagnostic data. A comprehensive literature search was performed using Medline, Web of Knowledge, Science Direct and PubMed Databases. Boolean Operators were used and key-terms included (not exclusively): terahertz, X-ray, ultraviolet, visible, infra-red, magnetic resonance, dental, diagnostic, caries and periodontal. Radiographic techniques are primarily used for diagnostic imaging in dentistry, and continued developments in X-ray imaging include: phase contrast, darkfield and spectral imaging. Other modalities have potential application, for example, terahertz, laser doppler and optical techniques, but require further development. In particular, infra-red imaging has regenerated interest with caries detection in vitro, due to improved quality and accessibility of cameras. Non-ionising imaging techniques, for example, infra-red, are becoming more commensurate with traditional radiographic techniques for caries detection. Nevertheless, X-rays continue to be the leading diagnostic image for dentists, with improved diagnostic potential for lower radiation dose becoming a reality

    Effect of different incisor movements on the soft tissue profile measured in reference to a rough-surfaced palatal implant

    INTRODUCTION The aim of this study was to evaluate soft tissue profile changes after a wide range of incisor movements in the anterior and posterior directions in nongrowing patients. Identifying baseline values more prone to substantial soft tissue profile changes was of high interest. METHODS For this retrospective study, 47 pairs of lateral cephalograms of nongrowing white patients were superimposed. The cephalograms were taken with the same palatal implant in situ before and after treatment. To increase the accuracy of the measurements, the palatal implants were used as stable reference structures in close relation to the incisors. RESULTS Horizontal changes of the most anterior point of the maxillary incisor showed a significant correlation to horizontal changes of the upper and lower lips (P <0.001). For every millimeter of horizontal change of the most anterior point of the maxillary central incisor, a change of 0.59 mm at labrale superior can be expected. Also, the angulations of the upper and lower lips were significantly correlated to the most anterior point of the maxillary incisor. Lip retraction was less pronounced in patients with initially thicker lips than in those with thinner lips. CONCLUSIONS The major contributing factors for predicting the soft tissue profile change during orthodontic treatment are the amount of horizontal movement of the most anterior point of the maxillary incisor, the amount of bite opening, and the initial lip thickness. Although there are significant correlations between dental movements and soft tissue changes in larger samples, predictions for individuals may be inconsistent
