    Análisis espacio-temporal de las condiciones biofísicas y ecológicas de Triatoma dimidiata (Hemiptera: Reduviidae: Triatominae) en la region nororiental de los Andes de Colombia

    Introduction: Due to the lack of information regarding biophysical and spatio-temporal conditions (hydrometheorologic and vegetal coverage density) in areas with Triatoma dimidiata in the Colombian departments of Santander and Boyacá, there is a need to elucidate the association patterns of these variables to determine the distribution and control of this species.Objective: To make a spatio-temporal analysis of biophysical variables related to the distribution of T. dimidiate observed in the northeast region of Colombia.Materials and methods: We used the Intergovernmental Panel on Climate Change Special Report on Emissions Scenarios (IPCC SRES) data bases registering vector presence and hydrometheorologic data. We studied the variables of environmental temperature, relative humidity, rainfall and vegetal coverage density at regional and local levels, and we conducted spatial geostatistic, descriptive statistical and Fourier temporal series analyses.Results: Temperatures two meters above the ground and on covered surface ranged from 14,5°C to 18,8°C in the areas with the higher density of T. dimidiata. The environmental temperature fluctuated between 30 and 32°C. Vegetal coverage density and rainfall showed patterns of annual and biannual peaks. Relative humidity values fluctuated from 66,8 to 85,1%.Conclusions: Surface temperature and soil coverage were the variables that better explained the life cycle of T. dimidiata in the area. High relative humidity promoted the seek of shelters and an increase of the geographic distribution in the annual and biannual peaks of regional rainfall. The ecologic and anthropic conditions suggest that T. dimidiata is a highly resilient species.Introducción. Ante el desconocimiento del contexto espacio-temporal de las condiciones biofísicas (hidrometereológicas y de densidad de la cobertura vegetal) de las zonas con presencia deTriatoma dimidiata en Santander y Boyacá, es necesario dilucidar los patrones asociados con estas variables para determinar su distribución y control.Objetivo. Hacer el análisis espacio-temporal de las variables biofísicas relacionadas con la distribución de Triatoma dimidiata de los departamentos de Santander y Boyacá en la región nororiental de Colombia.Materiales y métodos. Se utilizaron las bases de datos de los registros de presencia deT. dimidiata y de factores hidrometereológicos del Special Report on Emissions Scenarios (SRES) del Intergovernmental Panel on Climate Change, IPCC. Se estudiaron las variables de temperatura, humedad relativa, pluviosidad y densidad de la cobertura vegetal a nivel regional y local, se hizo el análisis espacial y el geoestadístico, así como el análisis estadístico descriptivo y temporal de las series de Fourier.Resultados. En las áreas con mayor presencia de T. dimidiata, las temperaturas a dos metros del suelo y en suelo cubierto oscilaron entre 14,5 y 18,8 °C, y la temperatura ambiente fue de 30 a 32 °C. La densidad de la cobertura vegetal y la pluviosidad en las áreas de mayor presencia de T. dimidiata exhibieron patrones de picos anuales y bienales. Los valores de la humedad relativa fluctuaron entre 66,8 y 85,1 %.Conclusiones. Las temperaturas a nivel de superficie y a dos metros del suelo fueron las variables determinantes de la distribución espacio-temporal de T. dimidiata. La elevada humedad relativa incentivó la búsqueda de refugios e incrementó la distribución geográfica en los picos anual y bienal de pluviosidad a nivel regional. Las condiciones ecológicas y antrópicas sugieren que T. dimidiata es una especie de gran resiliencia

    Incidencia y subregistro de casos de leptospirosis diagnosticados con tres métodos diferentes en Urabá, Colombia

    Introduction: Leptospirosis represents a public health problem and is a significant cause of morbidity and mortality in the region of Urabá. However, its notification reveals diagnostic limitations.Objective: To establish the incidence of leptospirosis in the municipalities of the so-called eje bananero in the Urabá region, to describe the magnitude of underreporting, and to propose guidelines for laboratory diagnosis by the public health network. Materials and methods: Two leptospirosis information sources were used: The national official surveillance system and a cross-sectional study of 479 acute-phase patients from April, 2010, to May, 2012. The diagnosis was made using three different tests: Indirect immunofluorescence, microagglutination test, and blood cultures. The exhaustiveness percentage of each information source was calculatedusing thecapture and recapture test. Results: From the total number of cases, 58% (278/479) were positive for leptospirosis at least by a test and 10.43% (29/278) of cases were positive by all three methods. The inclusion of a native strain in the microagglutination test panel increased the percentage of positivity by 15%. The cumulative incidence rate was 66.5/100,000 inhabitants and the case fatality ratio was 2.15%. The underreporting rates of leptospirosis in the Urabá region were 27.8% in morbidity and 66.6% in mortality.Conclusion: Under-registration of leptospirosis in the region highlights the necessity to use more than one diagnostic test to identify Leptospira in patients from endemic areas. Underregistration could be a common situation throughout the country.Introducción. La leptospirosis representa un problema de salud pública y es una causa importante de morbimortalidad en la región de Urabá, cuya notificación se ve afectada por las deficiencias en el diagnóstico.Objetivo. Establecer la incidencia de la leptospirosis en los municipios del llamado ‘eje bananero’ de la región de Urabá, documentar la magnitud del subregistro y proponer orientaciones para el diagnóstico por laboratorio por parte de la red de salud pública. Materiales y métodos. Se compararon dos fuentes de información sobre la leptospirosis: el sistema oficial nacional de vigilancia y un estudio transversal de 479 pacientes febriles, llevado a cabo entre abril de 2010 y mayo de 2012. El diagnóstico se hizo con base en tres pruebas: inmunofluorescencia indirecta, microaglutinación y hemocultivo. La exhaustividad de cada fuente de información se estimó mediante el método de captura y recaptura. Resultados. El 58 % (278/479) de los pacientes fueron positivos para leptospirosis, por lo menos, en una de las pruebas y, el 10,43 % (29/278), en las tres. La inclusión de una cepa nativa en el panel de la prueba de microaglutinación aumentó el porcentaje de positividad en 15 %. La tasa acumulada de incidencia fue de 66,5 por 100.000 habitantes y la proporción de letalidad fue de 2,15 %. El subregistro de la morbilidad por leptospirosis en la región de Urabá, fue de 27,8 % y, el de la mortalidad, de 66,6 %.Conclusión. El subregistro de leptospirosis en la región reitera la necesidad de usar más de una prueba diagnóstica para identificar Leptospira spp. en pacientes de zonas endémicas. Este subregistro podría ser una situación común en todo el país

    Los sistemas de información geográfica y los sensores remotos son poderosas herramientas para estudiar la distribución actual y predecir áreas de riesgo de presencia de insectos vectores de enfermedades; así mismo, se constituyen en excelentes aliados para la focalización de acciones de prevención y control. Aunque el uso de estas herramientas en investigación de enfermedades transmitidas por vectores se ha incrementado en los últimos años, su aplicación en los programas oficiales de control ha sido limitada. En esta revisión se aborda esta temática de trabajo, se discuten investigaciones con aplicaciones de ambos sistemas en diferentes especies vectores de enfermedades y los retos a futuro de su aplicación potencial en los programas oficiales de control de dichas enfermedades en Colombia. Finalmente, se enfatiza que para avanzar adecuadamente en los programas de control de las enfermedades, es recomendable el entrenamiento de investigadores y tomadores de decisiones en estas herramientas, incrementar la colaboración interinstitucional, estandarizar la colecta de datos referentes a la distribución de las especies de vectores y el desarrollo de una plataforma común de datos.Geographic Information Systems (GIS) and Remote Sensing (RS) are powerful tools for the study of actual distribution and to predict risk areas for insect vectors of diseases. Both have been using increasingly to target prevention and control actions. The use of these techniques in research about vector-borne tropical diseases is growing in the last years; however its application in official control programs has been limited. This paper briefly reviews the use of spatial analysis, Geographic Information Systems and Remote Sensing in different vector species and deal with the future challenges for the potential application of these techniques in official control programs in Colombia. Finally, to advance in vectorborne control programs, training of researchers and decision makers in these tools are recommended. The increase of collaboration between institutions and the collection of information about the distribution of vector species in a standardized way and the development of a common data platform is recommended too

    Experiencias en el control de un foco de leishmaniasis cutánea en San Carlos, Antioquia

    Debido al incremento en el registro de casos de leishmaniasis cutánea (LC) en los empleados y contratistas de la Central Hidroeléctrica de ISAGEN ubicada en el municipio de San Carlos, Antioquia se realizó un estudio para identificar las especies del género Lutzomyia presentes en la zona y seleccionar las posibles vectores de Le. Se realizaron 14 capturas nocturnas de los flebótomos en los apartamentos donde residen los empleados y en tres bases militares ubicadas en los alrededores de la central hidroeléctrica. Se utilizaron trampas CDC y Shannon, además de papeles impregnados con aceite de ricino. Se identificaron cuatro especies y dos géneros de flebotomineos. Lutzomyia panamensis (31.7%) predominó en todos los sitios, se reconoce corno vector demostrado de Leishmania panamensis en Panamá y sospechoso en Venezuela y Honduras. Lutzomyia gomezi (2 1 .1%) se encontró principalmente en el intradomicilio, es reconocida corno vector comprobado en Panamá y es altamente antropofílica, Lutzomyia bifoliata (24.6%) también fue hallada principalmente en el intradomicilio y Warileya rotundipennis no tiene importancia en Salud Pública. Basados en la abun~ dancia de flebótomos encontrada en los sitios de muestreo, su comportamiento antropofílico y antecedentes vectoriales, se considera que L. panamensis y L. gomezi pueden ser las especies responsables de la transmisión de LC en la central hidroeléctrica de Isagen~San Carlos. Se intervino para evitar el contacto hombre~vector. mediante fumigaciones residuales en intra y peridomicilio y postura de mallas en puertas y ventanas. Un año después se reporta una reducción del 100% en el número de casos entre los empleados. Summary Due an increase of cutaneous leishmaniasis cases in employees of the ISAGEN central hydroelectric, located in San Carlos, Antioquia, a study for the identification of the Lutzomyia species in the area was done. 14 phlebotomines catches using Shannon and CDC traps and impregnates papers was done. Four Lutzomyia species was identificated. L. panamensis (31.7%) prevail in all sites, L. gomezi and L. bifoliata prevail at intradomiciliary catches. Acording to the plebotomines abundance, behaviour and vectorial antecedents L. panamensis and L. gomezi are incriminated as vectors of cutaneous leishmaniasis in the area.     A transmission electron-microscope study has been performed on larval and juvenile skin of the Central American arrow-frog Dendrobates auratus to investigate early secretory processes and maturational changes in the serous (poison) glands. Poison biosynthesis involves the endoplasmic reticulum (both smooth and rough types), as well as Golgi stacks which release early serous product as secretory vesicles (or pre-granules). These vesicles contain fine-grained material, along with single electron-opaque bodies, spheroidal in shape, that accompany the grained product throughout its post-Gogian, maturational change. The first steps of this process involve condensation and lead to the formation of secretory granules with a glomerular-like substructure, resulting from a thick, random aggregation of rods (secretory granule subunits). Advanced maturational activity causes the loss of peculiar granule substructure: the dense bodies split into fragments, whereas the thick glomerular arrangement becomes looser, until the secretory product changes into a dispersed material. This ultrastructural study revealed biosynthesis and maturation processes in close sequence, suggesting the poison of D. auratus contains proteins and/or peptides as well as lipophilic compounds. Molecules of both these classes are known to perform several roles relevant to survival strategies in extant anurans. Furthermore, the ephemeral granules with a glomerular-like substructure detected in tadpoles and froglets exhibit the complex patterns of mature poisons in adult specimens of other anurans: Hylidae and related families.     ABSTRACT Myasis are parasitic infestations of animals and humans tissues and is caused by fly larvae. This kind of infestation has Public Health importance. In the Colombian biomedical literature the reports about myiasis in humans are scarce. In this paper, we report two cases of patients with gastrointestinal myiasis where the etiologic agents involved were Ornidia obesa and Ornidia sp (Diptera: Syrphidae). The taxonomic identification of the larvae was done at the Colombian Institute of Tropical Medicine and taxonomic confirmation was done at the laboratory of medicine veterinary and Zoology of Sao Pablo University. These two cases of myiasis are of first report in Colombia.RESUMEN Las miasis son infestaciones de tejidos de animales y humanos, causadas por larvas de moscas, las cuales tienen importancia manifiesta en Salud Publica. En la literatura biomédica Colombiana, son escasos los reportes referentes a los diferentes tipos de miasis que se presentan en los humanos. En este trabajo se reportan dos casos de pacientes con miasis gastrointestinal, donde los agentes etiológicos implicados fueron Ornidia obesa y Ornidia sp (Diptera: Syrphidae). La identificación taxonómica de las larvas halladas en ambos pacientes se realizó en el Instituto Colombiano de Medicina Tropical CES y fue confirmada en el Laboratorio del Departamento de Medicina Veterinaria Preventiva en Salud Animal de la Facultdad de Medicina Veterinaria y Zootecnia de la Universidad de Sao Paulo. Estos dos casos de miasis se constituyen en el primer reporte en Colombia
