Territorial Sustainability and Multifunctional Agriculture: A Case Study☆
Abstract The multifunctional agriculture can be defined as the mechanism of the integrated development and a tool for giving value to the territorial potential in the agricultural activities, which are determinant for the rural sustainability. Under the light of these considerations and given the next 2014-2020 programming periods, it becomes interesting thinking on the effect of the multifunctional agriculture in the realization of sustainable territorial development. Taking into account all of this, the present work, after having underlined short reflections on the relationships between the concept of sustainability and multifunctionality, proposes a case study on a territorial rural system finalized to evaluating the coherence between the policies' orientation of RDP and the evolution paths of the firms
Marketing Strategies of Large Agricultural Holdings and Social Commerce: the Case of Terra Orti PO
Abstract. Generally, the advantages of short food supply chains are associated only to small agricultural holdings and/or farms with a limited production output, but not to larger agricultural holdings. These considerations are limited. For larger producers these forms of marketing could transform from being a mere instrument of commercial diversification into a promotion instrument that makes it possible to translate the acknowledged or desired values in products in the short supply chain to the entire commercial output. The paper, following analysis of the characteristics of e-commerce in agribusiness, proposes the experiences of the Terra Orti Producers Organisation in implementing social commerce for promoting fruit and vegetable products from the "Piana del Sele" territory
Update on Long-Acting Anticholinergics in Children and Adolescents With Difficult and Severe Asthma
Tiotropium bromide is the only long-acting muscarinic antagonist (LAMA) approved for treatment of patients aged ≥6 years old who have symptoms of uncontrolled asthma. Results from several clinical trials have found that once-daily inhaled tiotropium bromide is safe and efficacious in 6- to 17-year-olds with symptomatic asthma despite treatment with inhaled corticosteroids, with or without other medications. There are still few available studies investigating the impact of tiotropium bromide treatment in preschool children with suboptimal control. In this narrative review, we summarize the pharmacological effects of the LAMA tiotropium bromide, provide an overview about current asthma studies at different pediatric ages, and describe future research needs
Human chorionic gonadotropin isoforms in the diagnosis of ectopic pregnancy
This paper has set the scene for re-defining clinical chemistry data for the diagnosis of ectopic pregnancy. Indeed it has proved some assumptions on hCG levels to be false. Professor Iles was/is the principal investigator on these studies
Rapid Evolution of an Aortic Endocarditis
Cardiac surgery is necessary in almost 50% of patients with endocarditis. Early surgery, i.e., the surgery performed during the first hospitalization, is required in the following cases: heart failure secondary to valve regurgitation; S. aureus, fungal organism, or other highly resistant organism infection; heart block, annular or aortic abscess, or destructive penetrating lesions; evidence of persistent infection as manifested by persistent bacteremia or fevers lasting >5 days after onset of appropriate antimicrobial therapy. A 62-year-old man developed a fever (38 °C) 3 days after a transaortic electrophysiological study; blood cultures were positive for S. aureus, and were sensitive to vancomycin and ceftaroline. Antibiotic therapy was started, controlling the fever and the patient's infective and inflammatory profiles well; however, 3 days later, acute aortic regurgitation developed. At transesophageal echocardiography (TEE), a rare condition was revealed-vegetation was attached to the aortic wall, impeding correct aortic valve closure. Cardiac operation was carried out and the time for surgery was discussed; based on the patient's clinically stable condition, and on the infection, which was controlled well by antibiotics therapy, surgery was not performed in emergency circumstance (within 24-48 h)-rather, it was programmed during the hospitalization. A TEE surveillance was initiated, and after 7 days, TEE revealed a new picture, with images of an aortic abscess with small perforation in the right atrium, requiring emergency surgery, carried out 20 h later. In our case, the rapid evolution of the vegetation attached to the aortic wall suggested the following: (1) that the time for the surgery cannot be guided only by clinical procedure but must also be guided by imaging pictures; (2) that strictly TEE surveillance is mandatory in patients with aortic endocarditis not initially referred for emergency surgery
Polymorphism analysis of Ch1 and Ch2 genes in the Siberian cat
Cats are usually spreaders of allergens that are critical for sensitive people; the Siberian cat is a breed supposed to be low level allergenic, according to some breeders’ statements. The sequence of the two genes, namely Ch1 and Ch2, that code for the allergen Fel d 1, the major allergen responsible for outbreaks of allergy symptoms, is not yet known in the Siberian cat, and finding this was the aim of our investigation. Notably, our work is the first survey of the genetic structure of these genes in Siberian cats. The comparison of the sequences of Siberian cats, non-Siberian cats, and sequences present in the National Center for Biotechnology Information database revealed a considerable number of mutations; some of those detected in the Siberian cat, due to their position in exon regions, could affect the Fel d 1 allergenic properties. Therefore, further investigations are recommended to assess if the identified mutations can be responsible for a reduced-allergen synthesis and can be used as markers for selection of low level allergenic cats
