À cet effet de masse global, il faut ajouter une autre évolution, liée à la fois aux innovations financières et aux modifications des conditions de rémunération sur les marchés. Severe corrosion after malpositioning of a metallic head over the Morse taper of a cementless hip arthroplasty. A case report

SummaryMorse tapers are frequently used in total hip replacement to achieve precise adjustment of lengths and femoral offset. Mechanically, they do not raise any specific problems so long as strict positioning requirements are observed and elements from different manufacturers are not mixed together. We report a case in which the implant induced unexplained pain at 2years, in relation to a defective fit between the metallic head and the Morse taper. Asymmetric partial fit of the head onto the taper was detected on control X-ray and was implicated as causing metallosis due to excessive release of metal debris from the Morse taper. Revision required femoral stem exchange because of the damage to the Morse taper as well as replacing the cup with new metal-metal bearings. Evolution was favorable at 3years’ follow-up. Most hip replacements include a Morse taper; the present clinical case is a reminder that strict positioning rules are to be respected, without which corrosion and wear may lead to mechanical failure

La ejecutividad de los acuerdos de mediación en la Union Europea. Algunas reflexiones

The legal system in Spain is undergoing a process of convergence with our neighboring countries in the European Union. Directive 52/2008 on mediation in civil and commercial matters has been recently transposed by the Law 5/2012, of July 6th. This is the first rule about civil and commercial mediation in the state level and it happens when mediation begin to be considered as an alternative to prosecution or via arbitration, as it involves a system of conflict resolution, that can help to get the right to judicial protection, keeping the Courts as a last resort andturning the citizen into active protagonist of the solution of their own conflict.This paper aims to analyze the questions about the mediation agreement in European Union, their enforceability and binding effects, the procedure to be followed or the effectiveness of the agreements reached therein.The gradual consolidation of mediation between legal operators involves reflection on the model adopted in our system, because this method has meant a great improvement in all Member States legal system. It is necessary to promote the culture of mediation to bring this institution to all sectors of society involved.El sistema legal en España está experimentando un proceso de convergencia con los países vecinos de la Unión Europea. La Directiva 52/2008 sobre la mediación en asuntos civiles y mercantiles ha sido transpuesta recientemente por la Ley 5 /2012, de 6 de julio. Esta es la primera norma general sobre mediación civil y mercantil a nivel estatal, y se promulga cuando la mediación empiezan a ser considerada como una alternativa al procedimiento judicial o arbitraje, ya que implica un sistema de resolución de conflictos que puede ayudar a obtener la tutela judicial efectiva, dejando a los órganos jurisdiccionales como último recurso y convirtiendo al ciudadano en protagonista activo de la solución de su propio conflicto.Este trabajo tiene como objetivo analizar las cuestiones relativas al acuerdo de mediación en la Unión Europea, su ejecutividad y efectos vinculantes, el procedimiento a seguir o la eficacia de los acuerdos alcanzados en el mismo.La progresiva consolidación de la mediación entre los operadores jurídicos implica la reflexión sobre el modelo adoptado en nuestro sistema, ya que este método ha supuesto una gran mejora en todos los sistemas procesales de Estados miembros. The remarriage: The role of the stepmother and her relationship with her stepchildren

Ao ingressar em um relacionamento em que o companheiro já possui filhos, é frequente que a madrasta tenha dificuldades para estabelecer a função que irá assumir nessa nova configuração familiar. Diante disso, este estudo qualitativo teve como objetivo investigar qual é o lugar ocupado pela madrasta na família recasada e como é sua relação com os enteados. Para tanto, integraram, neste estudo, seis mulheres que vivenciavam um relacionamento estável com um homem que possuía filhos de um relacionamento anterior, sendo que esses não residiam com o casal. Os dados foram coletados por meio de uma ficha de dados de identificação e de uma entrevista semidirigida com questões abertas, que foi avaliada por meio da análise de conteúdo temática. Os resultados apontaram para o envolvimento das participantes e para a atenção, especialmente em relação ao cuidado dos enteados, evitando interferir nos aspectos disciplinares. Além disso, alguns pais parecem ter assumido uma postura mais passiva na busca por uma aproximação com os filhos, tendo a madrasta exercido uma influência positiva para a manutenção da parentalidade. As considerações finais destacam a importância da flexibilização dos diferentes membros da família como estratégia para facilitar a acomodação dos papéis a serem desempenhados.Palavras-chave: recasamento, madrasta, relações familiares, enteado.When entering in a relationship in which the partner already has kids, it is common for the stepmother to express difficulties in establishing the role that she will play in this new family configuration. Thus, this qualitative research aimed to investigate about the role played by the stepmother in a remarried family and about her relation with her stepchildren. Therefore, six women that lived a stable relationship with a man that already had kids, who did not live with the couple, integrated this study. The data were collected through an identification data sheet and a semi-directed interview with open-ended questions, which were analyzed by a thematic content analysis. The results pointed out to the involvement and the attention of the participants, especially regarding to the care of the stepchildren, avoiding to interfere in disciplinary aspects. Besides, in the participants’ point of view, the fathers seemed to have taken on a passive attitude in the search for getting closer to their kids – stepmothers performed a positive influence to maintenance of parenthood. The final considerations mark the importance of flexibility in different family members as a strategy to facilitate the adaptation to the roles that will be developed. A experiência da maternidade pela primeira vez: as mudanças vivenciadas no tornar-se mãe

Este estudo buscou conhecer as mudanças percebidas pelas mães primíparas em si, em seus relacionamentos e na rede de apoio a partir da vivência da maternidade. Integraram o estudo seis mães primíparas, cujos bebês tinham entre sete e 12 meses de idade, participantes do Programa da Criança de uma Unidade Básica de Saúde de uma cidade do interior do Rio Grande do Sul (Brasil). A pesquisa teve caráter qualitativo, utilizando-se de uma entrevista semiestruturada como instrumento de coleta de dados. Os resultados obtidos pela análise de conteúdo apontaram o predomínio de sentimentos de insatisfação diante das mudanças percebidas no corpo e a presença de mudanças positivas no modo de ser da mulher após a maternidade. Além disso, destaca-se a presença de uma rede de apoio e o papel desempenhado pelo pai do bebê como suporte tanto instrumental como emocional à mãe.Palavras-chave: Relações mãe-criança.
