    The wider value of rural rail provision

    In the context of recent plans for public sector expenditure, the value for money provided by rural public transport is an important issue in Britain and elsewhere, and one aspect of this is the option and non-use value placed on public transport by residents. Whilst there are a small number of studies which have estimated option and non-use values, they rest largely on contingent valuation methods which are subject to dangers of bias, and concentrate on commuter services into cities rather than truly rural services. This paper seeks to overcome these problems by conducting a Stated Preference (SP) experiment in rural communities, which values the provision of rail services and compares this against Post Office provision. We believe that using this approach, and allowing respondents to compare willingness to pay for rail services with that for another important rural service subject to threatened cuts, should produce more reliable results; moreover we achieve this using self completion questionnaires rather than much more expensive interviews. Our results show much lower values for rail than previous studies, though this is to be expected in truly rural areas where the likelihood of commuting by rail is much lower. Other non-use values are greater than option values in this context

    Are Social Impact Bonds an innovation in finance or do they help finance social innovation?

    Outcomes Based Commissioning (OBC) – for example, Pay for Success (in the US) or Payment by Results (in the UK) – has been suggested as a way to provide ‘more’ social services for ‘less’ public resources. Such commissioning is often linked with an innovative financing tool called a Social Impact Bond (SIB). Using data from the Social Finance UK Database and focusing on SIBs in the US and UK, we evaluate whether the SIB approach aligns with the theoretical predictions of social innovation. The results provide limited evidence that SIBs facilitate capital injections from the private sector into the production of social goods as well as facilitate parts of the process of social innovation – namely, piloting and scaling. We conclude that there is significant variation, both between the US and UK and within the US, in social innovation ecosystems and the role played by SIBs

    Can a focus on co-created, strengths-based services facilitate early-stage innovation within social impact bonds?

    While many commentators recognize the potential for Social Impact Bonds (SIBs) to encourage innovation, empirical evidence is less clear cut. We argue that for SIBs to realize their full potential as incubators of innovation they needed to incorporate a stronger element of co-creation and strengths-based working, and suggest some accompanying characteristics of such SIBs. We analyze four UK SIBs as case studies that exhibit these characteristics. We find that within the cases strengths-based service delivery models were successfully implemented through SIBs; alongside this we found extensive evidence of co-production but limited evidence of co-creation. Strengths-based working, including co-production, helped the SIBs to catalyze early stage innovation. We identify several elements of SIB design which were instrumental in supporting strengths-based practices and could also support co-creation: greater autonomy for service providers; shifting risk to investors; use of a rate card; and long-term, flexible funding. Our findings challenge those who are skeptical about the potential for SIBs to provide a setting in which people who use services and frontline service providers are empowered to re-engineer public services and contest current systems

    Carbon exchange between a shelf sea and the ocean: The Hebrides Shelf, west of Scotland

    Global mass balance calculations indicate the majority of particulate organic carbon (POC) exported from shelf seas is transferred via downslope exchange processes. Here we demonstrate the downslope flux of POC from the Hebrides Shelf is approximately 3-to-5-fold larger per unit length/area than the global mean. To reach this conclusion we quantified the offshore transport of particulate and dissolved carbon fractions via the “Ekman Drain”, a strong downwelling feature of the NW European Shelf circulation, and subsequently compared these fluxes to simultaneous regional air-sea CO2 fluxes and on-shore wind-driven Ekman fluxes to constrain the carbon dynamics of this shelf. Along the shelf break we estimate a mean offshelf total carbon (dissolved + particulate) flux of 4.2 tonnes C m−1 d−1 compared to an onshelf flux of 4.5 tonnes C m−1 d−1. Organic carbon represented 3.3% of the onshelf carbon flux but 6.4% of the offshelf flux indicating net organic carbon export. Dissolved organic carbon represented 95% and POC 5% of the exported organic carbon pool. When scaled along the shelf break the total offshelf POC flux (0.007 Tg C d−1) was found to be three times larger than the regional air-sea CO2 ingassing flux (0.0021 Tg C d−1), an order of magnitude larger than the particulate inorganic carbon flux (0.0003 Tg C d−1) but far smaller than the DIC (2.03 Tg C d−1) or DOC (0.13 Tg C d−1) fluxes. Significant spatial heterogeneity in the Ekman drain transport confirms that offshelf carbon fluxes via this mechanism are also spatially heterogeneous. This article is protected by copyright.     A model-based study to estimate the health and economic impact of health technology assessment in Thailand

    Objectives: Health technology assessment (HTA) plays a central role in the coverage and reimbursement decision-making process for public health expenditure in many countries, including Thailand. However, there have been few attempts to quantitatively understand the benefits of using HTA to inform resource allocation decisions. The objective of this research was to simulate the expected net monetary benefit (NMB) from using HTA-based decision criteria compared to a first-come, first-served approach using data from Thailand. Methods: A previously published simulation model was adapted to the Thai context which aimed to simulate the impact of using different decision-making criteria to adopt or reject health technologies for public reimbursement. Specifically, the simulation model provides a quantitative comparison between an HTA-based funding rule and a counterfactual (first-come, first-served) funding rule to make decisions on which health technologies should be funded. The primary output of the model was the NMB of using HTA-based decision criteria compared to the counterfactual approach. The HTA-based decision rule in the model involved measuring incremental cost-effectiveness ratios against a cost-effectiveness threshold. The counterfactual decision rule was a first-come, first-served (random) selection of health technologies. Results: The HTA-based decision rule was associated with a greater NMB compared to the counterfactual. In the investigated analyses, the NMB ranged from THB24,238 million (USD725 million) to THB759,328 million (USD22,719 million). HTA-based decisions led to fewer costs, superior health outcomes (more quality-adjusted life years). Conclusions: The results support the hypothesis that HTA can provide health and economic benefits by improving the efficiency of resource allocation decision-making

    The importance of considering both depth of penetration and crater volume in forwards-ballistic penetrative experiments

    The most common method of analysing armour performance is the Depth of Penetration (DoP). However, this one-dimensional measurement does not provide insight into the method of penetration or energy absorbed by the target; the crater could be particularly narrow or very wide and yield the same DoP. Analysis of the crater through Crater Volume (CV) provides a more detailed metric to be used alongside DoP to visualise the crater, indicating whether energy was dispersed over a large area. CV provides a wider insight into how a material resisted penetration events, giving evidence of potential defeat mechanisms. Digital reconstruction of the craters using X-ray radiographs or Computed Tomography (CT) scanning can also provide a useful tool for computational models to be compared against. The simple calculation of CV through X-ray radiography and image processing has been demonstrated to be accurate to within ±6% of the CT scanned CV. Success in utilising this analytical tool was demonstrated through comparison of three armour configurations. A consistent difference in the ratio of DoP:CV was seen between steel targets, ceramic-steel targets and ceramic-air-steel targets, indicating variation in the defeat mechanism between the three target configurations

    ORALLY ACTIVE INHIBITORS OF HUMAN LEUKOCYTE ELASTASE. II. DISPOSITION OF L-694,458 IN RATS AND RHESUS MONKEYS

    ABSTRACT: The disposition of L-694,458, a potent monocyclic ␤-lactam inhibitor of human leukocyte elastase, was studied in male SpragueDawley rats and rhesus monkeys. After iv dosing, L-694,458 exhibited similar pharmacokinetic parameters in rats and rhesus monkeys. The mean values for its plasma clearance, terminal halflife, and volume of distribution at steady state were 27 ml/min/kg, 1.8 hr, and 4.0 liters/kg in rats and 34 ml/min/kg, 2.3 hr, and 5 liters/kg in rhesus monkeys. The bioavailability of a 10 mg/kg oral dose was higher in rats (65%) than in rhesus monkeys (39%). In both species, concentrations of L-694,458 in plasma increased more than proportionally when the oral dose was increased from 10 mg/kg to 40 mg/kg. In monkeys a protracted plasma concentration-time profile was observed at 40 mg/kg, characterized by a delayed T max (8-24 hr) and a long terminal half-life (6 hr). [ 3 H]L-694,458 was well absorbed after oral dosing to rats at 10 mg/kg, as indicated by the high recovery of radioactivity in bile (83%) and urine (6%) of bile duct-cannulated rats. Only ϳ5% or less of the radioactivity in bile, urine, and feces was a result of intact L-694,458, indicating that the compound was being eliminated by metabolism, followed by excretion of the metabolites in feces, via bile. Demethylenation of the methylenedioxyphenyl group resulting in the catechol was the primary metabolic pathway in human and rhesus monkey liver microsomes. In rat liver microsomes, the major metabolite was the N-oxide of the methyl-substituted piperazine nitrogen. In rats dosed iv and orally with [ 3 H]L-694,458, concentrations of radioactivity were highest in the lung (the primary target tissue), adrenals, and liver. L-694,458 was unstable in rat blood and plasma, degrading via a pathway believed to be catalyzed by B-esterases and to involve cleavage of the ␤-lactam ring and loss of the methylpiperazine phenoxy group. In vitro studies indicated that in human liver, L-694,458 was metabolized by CYP3A and 2C isozymes, and in both monkey and human liver microsomes the compound acted as an inhibitor of testosterone 6␤-hydroxylation. Leukocyte elastase is a serine protease capable of proteolytic degradation of a variety of substrates, including elastin and collagen, which are components of connective tissue. Specific inhibitors of leukocyte elastase are being explored as potential therapeutic agents for the treatment of inflammatory diseases, such as cystic fibrosis and rheumatoid arthritis where high amounts of extracellular elastase, either free or bound to its natural inhibitors, ␣ 1 -proteinase inhibitor and secretory leukocyte proteinase inhibitor, have been detected extracellularly (1-3).
