Prisoners as hospice volunteers in Poland

Creating programmes combining penitentiary area with hospice-palliative care and voluntary work in different countries is an interesting direction of implementing the ideals of hospice. The first part of this publication refers to the description of major initiatives for the reintegration of prisoners through hospice voluntary work. In 2009 Poland was awarded ‘The Crystal Scales of Justice’ thanks to a programme called ‘WHAT’ (hospice voluntary work as a tool of acceptance and tolerance for people leaving penal institutions). The project was aimed at social reintegration of prisoners through volunteering for a hospice located outside prison. The results of several years of satisfactory cooperation between the hospice and prison in Gdańsk were passed over to penitentiary counsellors and hospice teams in ten cities in Poland. The results of the initial research show that involving prisoners in a hospice team, where they face suffering and dying, improves their psychosocial functioning and the quality of team care. Thanks to the correctional programmes prisoners are currently working in 29 hospices and more than 70 nursing homes, helping patients in need of instant care. Moreover, the convicted from semi-open and open penal institutions are directed to volunteer for hospitals or children’s homes. In February 2012 a survey among the sentenced was conducted. The purpose of the study: Its main objective was to characterize the meaning of life among prisoners doing voluntary work for a hospice, and convicts not involved in volunteering. Method: In the study a short personal questionnaire together with the PIL (Purpose in Life) questionnaire by Crumbaugh and Maholic were used (Polish adaptation by Płużek) to examine the understanding of the meaning of life. Results: Based on the results obtained it was found that most respondents have a high level of meaning of life. A qualitative analysis of the data suggests that experiences of hospice volunteering may affect the nature of understanding the meaning of life among the convicts. Conclusions: Prisoners not involved in hospice volunteering are more focused on their current situation, leaving prison, having a family and money. They use conventional phrases to describe life and themselves. Prisoners who volunteer for hospices have further time perspectives, are realistic about their abilities, express more goals in life and use a wider variety of language to talk about life, suffering and themselves

Skuteczne zastosowanie nilotynibu u pacjentki z przewlekłą białaczką szpikową po niepowodzeniu leczenia dwoma inhibitorami kinaz tyrozynowych z powodu nietolerancji i oporności

Treatment with tyrosine kinase inhibitors (TKI) produces good results in patients with chronicmyelogenous leukemia (CML). However, some patients need to change the therapy due to intoleranceor resistance to TKI applied. In these situations, the possibility of 2nd generation TKI administrationallows to achieve deep molecular response (DMR) and long survival of patients. In thispaper, a case of adult patient with CML who experienced hepatic toxicity of imatinib and failureof dasatinib treatment due to resistance is presented. Subsequently, the treatment with nilotinibresulted in a long-term DMR.Leczenie za pomocą inhibitorów kinaz tyrozynowych (TKI) przynosi bardzo dobre efekty u pacjentów z przewlekłą białaczką szpikową (CML). Jednak u części chorych zachodzi potrzeba zmiany leczenia z powodu niepowodzenia terapii uwarunkowanego nietolerancją bądź opornością na stosowany TKI. W takich sytuacjach możliwość zastosowania TKI II generacji pozwala na uzyskanie głębokiej odpowiedzi molekularnej (DMR) i długiego przeżycia pacjentów. W niniejszym artykule opisano przypadek pacjentki z rozpoznaniem CML, u której w przebiegu leczenia imatynibem wystąpiła toksyczność wątrobowa, następnie doszło do niepowodzenia leczenia dazatynibem z powodu oporności. Prawa do ochrony zdrowia i godności osobistej są jednymi z podstawowych praw człowieka. Uregulowanie w Polsce praw pacjenta w jednej ustawie ma olbrzymie znaczenie społeczne. Przepisy te są dowodem swobody obywatelskiej i poczucia wolności. Dotyczy to zarówno sfery tworzenia, jak i stosowania prawa, kształtujące świadomość ludzi. Mobilization of human hematopoietic stem/progenitor-enriched CD34+ cells into peripheral blood during stress related to ischemic stroke.

The bone marrow-derived stem/progenitor cells were demonstrated to play an important role in a regeneration of damaged tissue. Based on these observations we asked whether the stroke-related stress triggers mobilization of stem/progenitor cells from the bone marrow into the peripheral blood, which subsequently could contribute to regeneration of damaged organs. To address this issue, the peripheral blood samples were harvested from patients with ischemic stroke during the first 24 hrs as well as after the 48 (2nd day) and 144 hrs (6th day) since the manifestation of symptoms. In these patients we evaluated the percentage of hematopoietic stem/progenitor-enriched CD34+ cells by employing flow cytometry and the number of hematopoietic progenitor cells for the granulocyto-monocytic (CFU-GM) and erythroid (BFU-E)-lineages circulating in peripheral blood. We concluded that stress related to ischemic stroke triggers the mobilization of hematopoietic stem/progenitor cells from the bone marrow into peripheral blood. These circulating stem/progenitor cells may play an important role in the process of regeneration of the ischemic tissue
