    Introducción De acuerdo con las estadísticas suministradas en el 2014 por la Federación Internacional de Diabetes, la prevalencia de pacientes con diabetes está en aumento a nivel mundial, actualmente alrededor de 382 millones de personas sufren de esta patología. En relación con la diabetes tipo 1 se estima que corresponde al 5% de todos los casos diagnosticados de diabetes en adultos. Para el caso de Colombia, la incidencia de diabetes tipo 1 es de 3-4 por 100.000 niños menores de 15 años y la prevalencia se estima en 0,07%. Para el municipio de Santiago de Cali, según datos de la oficina de estadística de la Secretaría de Salud Pública, la diabetes (tipo 1 y 2) se encuentra entre las diez primeras causas de muerte en la ciudad, ocupando el octavo lugar en el grupo de hombres y el séptimo entre las mujeres. Cabe resaltar que la prevalencia de diabetes para Cali es del 6,76%, seguida del 5,4% para el departamento del Valle del Cauca, cifra mucho más alta cuando se compara con la prevalencia nacional que es de 2%. Específicamente, cuando nos referimos a la diabetes tipo 1, es una patología autoinmune crónica, que se presenta con una predisposición genética específica y en un 0,4% aparece sin antecedentes familiares. Su presentación se hace de forma inesperada, a causa de la destrucción rápidamente progresiva de las células beta pancreáticas, por esta razón requiere la suplencia total de insulina exógena de forma crónica. Lo anterior ocasiona que la enfermedad esté ligada a una carga económica alta para los pacientes, sus familias y el sistema de salud. A nivel global se estima que más del 10% de la inversión en salud se destina a gastos relacionados directamente con diabetes. En cuanto al tratamiento médico de la diabetes tipo 1, la recomendación actual está basada en la terapia intensiva con insulina exógena, sin embargo, este tratamiento se caracteriza por ser de difícil adherencia, costoso, y por conllevar a un incremento considerable de los eventos de hipoglicemia severa. Estos eventos aumentan en alto grado el riesgo de sufrir deterioro cardiovascular y cognitivo. Por lo tanto, existe una gran necesidad de encontrar mejores alternativas de tratamiento para estos pacientes, que impliquen menores riesgos biológicos y ofrezcan mejor calidad de vida

    Enteric methane mitigation strategies for ruminant livestock systems in the Latin America and Caribbean region: a meta-analysis.

    Latin America and Caribbean (LAC) is a developing region characterized for its importance for global food security, producing 23 and 11% of the global beef and milk production, respectively. The region?s ruminant livestock sector however, is under scrutiny on environmental grounds due to its large contribution to enteric methane (CH4) emissions and influence on global climate change. Thus, the identification of effective CH4 mitigation strategies which do not compromise animal performance is urgently needed, especially in context of the Sustainable Development Goals (SDG) defined in the Paris Agreement of the United Nations. Therefore, the objectives of the current study were to: 1) collate a database of individual sheep, beef and dairy cattle records from enteric CH4 emission studies conducted in the LAC region, and 2) perform a meta-analysis to identify feasible enteric CH4 mitigation strategies, which do not compromise animal performance. After outlier?s removal, 2745 animal records (65% of the original data) from 103 studies were retained (from 2011 to 2021) in the LAC database. Potential mitigation strategies were classified into three main categories (i.e., animal breeding, dietary, and rumen manipulation) and up to three subcategories, totaling 34 evaluated strategies. A random effects model weighted by inverse variance was used (Comprehensive Meta-Analysis V3.3.070). Six strategies decreased at least one enteric CH4 metric and simultaneously increased milk yield (MY; dairy cattle) or average daily gain (ADG; beef cattle and sheep). The breed composition F1 Holstein × Gyr decreased CH4 emission per MY (CH4IMilk) while increasing MY by 99%. Adequate strategies of grazing management under continuous and rotational stocking decreased CH4 emission per ADG (CH4IGain) by 22 and 35%, while increasing ADG by 22 and 71%, respectively. Increased dietary protein concentration, and increased concentrate level through cottonseed meal inclusion, decreased CH4IMilk and CH4IGain by 10 and 20% and increased MY and ADG by 12 and 31%, respectively. Lastly, increased feeding level decreased CH4IGain by 37%, while increasing ADG by 171%. The identified effective mitigation strategies can be adopted by livestock producers according to their specific needs and aid LAC countries in achieving SDG as defined in the Paris Agreement

    Design and baseline characteristics of the finerenone in reducing cardiovascular mortality and morbidity in diabetic kidney disease trial

    Background: Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials. Patients and Methods: The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate >= 25 mL/min/1.73 m(2) and albuminuria (urinary albumin-to-creatinine ratio >= 30 to <= 5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level alpha = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure. Conclusions: FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen.     Refined identification of Vibrio bacterial flora from Acanthasther planci based on biochemical profiling and analysis of housekeeping genes

    We used a polyphasic approach for precise identification of bacterial flora (Vibrio­naceae) isolated from crown-of-thorns starfish (COTS) from Lizard Island (Great Barrier Reef, Australia) and Guam (USA, Western Pacific Ocean). Previous 16S rRNA gene phylogenetic analysis was useful to allocate and identify isolates within the Photobacterium, Splendidus and Harveyi clades but failed in the identification of Vibrio harveyi-like isolates. Species of the V. harveyi group have almost indistinguishable phenotypes and genotypes, and thus, identification by standard biochemical tests and 16S rRNA gene analysis is commonly inaccurate. Biochemical profiling and sequence analysis of additional topA and mreB housekeeping genes were carried out for definitive identification of 19 bacterial isolates recovered from sick and wild COTS. For 8 isolates, biochemical profiles and topA and mreB gene sequence alignments with the closest relatives (GenBank) confirmed previous 16S rRNA-based identification: V. fortis and Photobacterium eurosenbergii species (from wild COTS), and V. natriegens (from diseased COTS). Further phylogenetic analysis based on topA and mreB concatenated sequences served to identify the remaining 11 V. harveyi-like isolates: V. owensii and V. rotiferianus (from wild COTS), and V. owensii, V. rotiferianus, and V. harveyi (from diseased COTS). This study further confirms the reliability of topA−mreB gene sequence analysis for identification of these close species, and it reveals a wider distribution range of the potentially pathogenic V. harveyi group
