NGC 6153: a super-metal-rich planetary nebula?
We have obtained deep optical spectra of the planetary nebula NGC 6153, both along its minor axis and by uniformly scanning a long slit across the whole nebula. The scanned spectra, when combined with the nebular total Hβ flux, yield integrated fluxes for all the lines (∼400) in our spectra, which are rich in strong recombination lines from C, N, O and Ne ions A weak O VI λ 3811 emission line from the central star has been detected, suggesting that the nucleus of NGC 6153 has a hydrogen-deficient surface. The optical data, together with the ISO LWS 43-197 μm spectrum and the archival IUE and IRAS LRS spectra, are used to study the thermal and density structure and to derive the heavy-element abundances from lines produced by different excitation mechanisms. In all cases, the C2+/H+ N2+/H+, O2+/H+ and Ne2+/H+ abundances derived from multiple optical recombination lines (ORLs) are consistently higher, by about a factor of 10, than the corresponding values deduced from optical, UV or infrared (IR) collisionally excited lines (CELs). regardless of the excitation energies or critical densities of the latter. The agreement between the temperature-sensitive optical forbidden lines and the temperature-insensitive IR fine-structure lines rules out temperature fluctuations as the cause of the large difference between the ORL and CEL abundances. We present the results of a new calculation of recombination coefficients for [O II] which lead to good agreement between the observed and predicted [O II] λλ7320,7330 forbidden line intensities if these lines are solely excited by recombination at the Balmer jump temperature. Recombination excitation is also found to be important in exciting the [N II] λ5754 line, which, if unaccounted for, would lead to an overestimated [N II] temperature from the observed (λ6548 +λ6584)/λ5754 ratio. Analysis of a number of C II lines arising from levels as high as 7g in the recombination ladder reveals excellent agreement between their reddening-corrected relative intensities and those predicted by recombination theory. Spatial analysis of the long-slit spectra taken along the nebular minor axis yields a varying [O III] temperature, whereas the hydrogen Balmer jump temperature of 6000 K is approximately constant across the nebula, and is 2000-3000 K lower than the [O III] temperature The observed high-n Balmer line decrement indicates that the hydrogen lines arise from material having an electron density of 2000+2000-1000cm-3, consistent with the optical and IR forbidden-line density diagnostics, which yield average line-of-sight electron densities along the minor axis varying between 2000 and 4000 cm-3. While the He/H ratio mapped by He I and He II recombination lines is constant within 5 per cent across the nebula, the C2+/H+ and O2+/H+ recombination-line abundances decrease by a factor of 2-3 over a radius of 15 arcsec from the centre, pointing to the presence of abundance gradients. We consider a variety of hypotheses to account for the observed behaviour of the various thermal, density and abundance diagnostics. Empirical nebular models containing two components with differing densities and temperatures are able to account for many of the observed patterns, but only if one of the components is significantly hydrogen-deficient. One such model, which gives a good fit to the observed line intensities and patterns, has 500-K H-depleted material, presumed to be evaporating from dense neutral inclusions, embedded in 9500-K material with 'normal' abundances. An alternative model, which appears more physically plausible on a number of grounds, has high-density (2 × 106 cm-3), fully ionized, H-deficient knots embedded in the 'normal' component, although this model fails to account adequately for the observed low (6000 K) hydrogen Balmer jump temperature. However, the observed fact that the ORLs and CELs yield heavy-element abundance ratios that are identical within the uncertainties finds no obvious explanation in the context of H-deficient knot models
Aspergillus mastoiditis, presenting with unexplained progressive otalgia, in an immunocompetent (older) patient
Aspergillus mastoidits and skull-base osteomyelitis are extremely rare, even in immunocompromised patients. We report a case of an 81-year-old immunocompetent man, who underwent a mastoidectomy because of unexplained, progressive otalgia in spite of a noninflamed and air-containing middle-ear space. Histopathology yielded Aspergillus fumigatus. When confronted with otitis with an unexpected clinical course a high index of suspicion is required to facilitate early diagnosis and appropriate therapy of a potential lethal Aspergillus infection, even in immunocompetent patients. This seems to be more so in older patients with an open middle-ear cavity and/or when there is facial nerve involvement
The Dynamics of Nestedness Predicts the Evolution of Industrial Ecosystems
In economic systems, the mix of products that countries make or export has
been shown to be a strong leading indicator of economic growth. Hence, methods
to characterize and predict the structure of the network connecting countries
to the products that they export are relevant for understanding the dynamics of
economic development. Here we study the presence and absence of industries at
the global and national levels and show that these networks are significantly
nested. This means that the less filled rows and columns of these networks'
adjacency matrices tend to be subsets of the fuller rows and columns. Moreover,
we show that nestedness remains relatively stable as the matrices become more
filled over time and that this occurs because of a bias for industries that
deviate from the networks' nestedness to disappear, and a bias for the missing
industries that reduce nestedness to appear. This makes the appearance and
disappearance of individual industries in each location predictable. We
interpret the high level of nestedness observed in these networks in the
context of the neutral model of development introduced by Hidalgo and Hausmann
(2009). We show that, for the observed fills, the model can reproduce the high
level of nestedness observed in these networks only when we assume a high level
of heterogeneity in the distribution of capabilities available in countries and
required by products. In the context of the neutral model, this implies that
the high level of nestedness observed in these economic networks emerges as a
combination of both, the complementarity of inputs and heterogeneity in the
number of capabilities available in countries and required by products. The
stability of nestedness in industrial ecosystems, and the predictability
implied by it, demonstrates the importance of the study of network properties
in the evolution of economic networks.Comment: 26 page
Strategies to reduce clinical inertia in hypertensive kidney transplant recipients
<p>Abstract</p> <p>Background</p> <p>Many kidney transplant recipients have hypertension. Elevated systolic blood pressures are associated with lower patient and kidney allograft survival.</p> <p>Methods</p> <p>This retrospective analysis examined the prevalence of clinical inertia (failure to initiate or increase therapy) in the treatment of hypertension before and after the introduction of an automated device (BpTRU) in the kidney transplant clinic.</p> <p>Results</p> <p>Historically only 36% (49/134) of patients were prescribed a change in therapy despite a systolic blood pressure ≥ 130 mmHg. After the introduction of BpTRU, 56% (62/110) of the patients had a change in therapy. In a multivariate logistic regression analysis of the entire cohort (n = 244) therapeutic changes were associated with higher blood pressures (OR 1.08 per mmHg, 95% CI 1.04–1.12) and use of the BpTRU (OR 2.12, 95% CI 1.72–3.83). In addition patients on more medications were also more likely to have a change in therapy.</p> <p>Conclusion</p> <p>Blood pressure measurement with automated devices may help reduce clinical inertia in the kidney transplant clinic.</p
Feasibility and safety of setting up a donor breastmilk bank in a neonatal prem unit in a resource limited setting: An observational, longitudinal cohort study
<p>Abstract</p> <p>Background</p> <p>The beneficial effects of human milk on decreasing rates of paediatric infections such as necrotizing enterocolitis (NEC) and sepsis have been clearly demonstrated. Donor breastmilk has been encouraged as the milk of choice when a mother's own breastmilk is not available. The objectives of this study were to assess feasibility of providing donor breastmilk to infants in a resource limited Neonatal Prem Unit (NPU). In addition we sought to determine whether donor breastmilk could be safely pasteurized and administered to infants without any adverse events.</p> <p>Methods</p> <p>Low birth weight infants < 1800 g and under 32 weeks gestational age were followed up in the NPU over a 3 week period; feeding data and morbidity data was collected in order to determine if there were any adverse events associated with donor breastmilk. Samples of pasteurized breastmilk were cultured to check for any bacterial contamination.</p> <p>Results</p> <p>191 infants met the inclusion criteria of whom 96 received their mother's own breastmilk. Of the 95 infants who were potentially eligible to receive donor milk, only 40 did in fact receive donor milk. There was no evidence of bacterial contamination in the samples analyzed, and no evidence of adverse events from feeding with donor breastmilk.</p> <p>Conclusion</p> <p>It is feasible to supply donor breastmilk to infants in an NPU in a resource limited setting, however staff needs to be sensitized to the importance of donor breastmilk to improve uptake rates. Secondly we showed that it is possible to supply donor breastmilk according to established guidelines with no adverse events therefore making it possible to prevent NEC and other side effects often associated with formula feeding of premature infants.</p
Molecular mechanisms of extracellular adenine nucleotides-mediated inhibition of human Cd4+ T lymphocytes activation
We have previously reported that ATPγS, a slowly hydrolyzed analog of ATP, inhibits the activation of human CD4+ T lymphocytes by anti-CD3 and anti-CD28 mAb. In this report we have partially characterized the signaling mechanisms involved in this immunosuppressive effect. ATPγS had no inhibitory effect on CD4+ T-cell activation induced by PMA and anti-CD28, indicating that it acts proximally to the TCR. It had no effect on the calcium rise induced by CD3/CD28 stimulation, but inhibited the phosphorylation of three kinases, ERK2, p38 MAPK and PKB, that play a key role in the activation of T cells. Infant feeding counselling in Uganda in a changing environment with focus on the general population and HIV-positive mothers - a mixed method approach
<p>Abstract</p> <p>Background</p> <p>Health workers' counselling practices are essential to improve infant feeding practices. This paper will assess how infant feeding counselling was done and experienced by counsellors and mothers in Eastern Uganda in the context of previous guidelines. This has implications for implementation of the new infant feeding guidelines from 2009.</p> <p>Methods</p> <p>This paper combines qualitative and quantitative data from Mbale District in Eastern Uganda. Data was collected from 2003 to 2005 in a mixed methods approach. This includes: key-informant interviews among eighteen health workers in the public hospital, health clinics and non-governmental organisations working with people living with HIV, fifteen focus group discussions in the general population and among clients from an HIV clinic, two cross-sectional surveys including 727 mothers from the general population and 235 HIV-positive mothers.</p> <p>Results</p> <p>The counselling sessions were often improvised. Health workers frequently had pragmatic approaches to infant feeding as many clients struggled with poverty, stigma and non-disclosure of HIV. The feasibility of the infant feeding recommendations was perceived as challenging among health workers, both for HIV-positive mothers and in the general population. Group counselling with large groups was common in the public health service. Some extra infant feeding teaching capacities were mobilised for care-takers of undernourished children. A tendency to simplify messages giving one-sided information was seen. Different health workers presented contradicting simplified perspectives in some cases. Outdated training was a common concern with many health workers not being given courses or seminars on infant feeding since professional graduation. Other problems were minimal staffing, lack of resources, and programs being started and subsequently stopped abruptly. Many of the HIV-counsellors in the non-governmental organisations got extended training in counselling which seemed to be beneficial.</p> <p>Conclusions</p> <p>Health workers were faced with challenges related to workload, resources, scientific updating, and also a need to adjust to frequent changes in programs, recommendations and guidelines. The clients were faced with difficult choices, poverty, lack of education and stigma. Feasibility of the recommendations was a major concern. Systematic approaches to update health workers should be a priority.</p
Helminth and Intestinal Protozoa Infections, Multiparasitism and Risk Factors in Champasack Province, Lao People's Democratic Republic
Multiparsitism is a general public health concern in tropical countries, and is of particular importance in the Mekong River basin of Southeast Asia. Here, we report results obtained from an in-depth study of hepato-biliary and intestinal multiparasitism and associated risk factors in three settings of the most southern province of Lao People's Democratic Republic. Multiple species intestinal parasite infections were very common: more than 80% of the study participants harbored at least two and up to seven different intestinal parasites concurrently. Of particular concerns are the high prevalence of the liver fluke Opisthorchis viverrini (64.1%) and the moderate prevalence of the blood fluke Schistosoma mekongi (24.2%), as these fluke infections are responsible for severe hepato-biliary morbidity, including the bile duct cancer cholangiocarcinoma. Hookworm was the most common nematode infection (76.8%). We conclude that given the very high prevalence rates of parasite infections and the extent of multiparasitism, regular deworming is warranted and that this intervention should be coupled with health education and improved assess to clean water and adequate sanitation to consolidate morbidity control and ensure long-term sustainability
IL-33 Is Produced by Mast Cells and Regulates IgE-Dependent Inflammation
Background: IL-33 is a recently characterized IL-1 family cytokine and found to be expressed in inflammatory diseases, including severe asthma and inflammatory bowl disease. Recombinant IL-33 has been shown to enhance Th2-associated immune responses and potently increase mast cell proliferation and cytokine production. While IL-33 is constitutively expressed in endothelial and epithelial cells, where it may function as a transcriptional regulator, cellular sources of IL-33 and its role in inflammation remain unclear. Methodology/Principal Findings: Here, we identify mast cells as IL-33 producing cells. IgE/antigen activation of bone marrow-derived mast cells or a murine mast cell line (MC/9) significantly enhanced IL-33. Conversely, recombinant IL-33 directly activated mast cells to produce several cytokines including IL-4, IL-5 and IL-6 but not IL-33. We show that expression of IL-33 in response to IgE-activation required calcium and that ionomycin was sufficient to induce IL-33. In vivo, peritoneal mast cells expressed IL-33 and IL-33 levels were significantly lower within the skin of mast cell deficient mice, compared to littermate controls. Local activation of mast cells promotes edema, followed by the recruitment of inflammatory cells. We demonstrate using passive cutaneous anaphylaxis, a mast cell-dependent model, that deficiency in ST2 or antibody blockage of ST2 or IL-33 ablated the late phase inflammatory response but that the immediate phase response was unaffected.
