    Evaluation of mutagenic/antimutagenic activity of conjugated linoleic acid in mice by micronucleus test

    Conjugated linoleic acids (CLAs) are positional and geometrical isomers of linoleic acid and some researchers have shown biological activities including modulation of lipid metabolism, atherogenesis, diabetes, and immune functions. In this study, the animals were supplemented with 2% of the average feed consumption with CLA (G1 = positive control) and safflower oil (G2 = negative control) and the test groups were supplemented with CLA at the concentration of 2 and 4% of the average feed consumption. To assess the CLA potential protective effect, two groups were used; G3 = CLA 2% + cyclophosphamide (CP) and G5 = CLA 4% + CP. To assess the mutagenic effects of CLA, two groups were used; G4 = CLA 2% + NaCl 0.9% and G6 = CLA 4% + NaCl 0.9%. In order to investigate the mutagenic/antimutagenic effects of CLA, micronucleus test was used. The results showed variation of feed consumption in the groups that received 4% of CLA, when compared to the control group (G1 and G2) and CLA groups (G3 and G4) (p<0.05), during the period studied. It was observed that CLA did not show mutagenic effect at the concentrations tested (2 and 4%). Also, CLA showed antimutagenic effect at the same concentrations. However, the animals that received 4% of CLA, presented clinical signs of malnutrition.Key words: Conjugated linoleic acid, antimutagenicity, cyclophosphamide

    Physical activity level in people with high blood pressure

    This study aimed to analyze the level of physical activity for people with high blood pressure cared for in a health center. This cross-sectional study was conducted with 310 individuals between 18 and 69 years of age through the International Physical Activity Questionnaire (IPAQ). The majority (80%) of participants presented low or moderate levels of physical activity. The analysis of association between sociodemographic variables and physical activity level revealed that older men from the interior of the state with higher educational levels have a greater chance of exhibiting lower levels of physical activity. The logistic regression model revealed that origin, schooling and diabetes are predictors of low physical activity level. The conclusion is that part of the sample does not match the desirable profile to practice regular physical activity.El objetivo de este estudio fue analizar el nivel de actividad física de personas portadoras de hipertensión arterial, acompañadas en un centro de atención ambulatorial. Se trata de un estudio transversal desarrollado con 310 individuos de 18 a 69 años. Se utilizó como base el International Physical Activity Questionnaire (IPAQ). La mayor parte del grupo (80%) fue encuadrada en los niveles de baja y moderada actividad física. En el análisis de asociación entre las variables sociodemográficas y el nivel de actividad física, se verificó que personas del sexo masculino, procedentes del interior del Estado, con más edad y con mayor nivel de escolaridad son más propensas a desarrollar un nivel de actividad física bajo. En el modelo de regresión logística, la procedencia, tiempo de escolaridad y de diabetes fueron indicadores obtenidos como factores de predicción significativos para el nivel bajo de actividad física. Se concluye que parte de la muestra no posee un perfil deseable en relación a la práctica regular de actividad física.O objetivo desse estudo foi analisar o nível de atividade física de pessoas portadoras de hipertensão arterial, acompanhadas em centro de atendimento ambulatorial.     Scaling-Up of Dental Pulp Stem Cells Isolated from Multiple Niches

    Dental pulp (DP) can be extracted from child’s primary teeth (deciduous), whose loss occurs spontaneously by about 5 to 12 years. Thus, DP presents an easy accessible source of stem cells without ethical concerns. Substantial quantities of stem cells of an excellent quality and at early (2–5) passages are necessary for clinical use, which currently is a problem for use of adult stem cells. Herein, DPs were cultured generating stem cells at least during six months through multiple mechanical transfers into a new culture dish every 3–4 days. We compared stem cells isolated from the same DP before (early population, EP) and six months after several mechanical transfers (late population, LP). No changes, in both EP and LP, were observed in morphology, expression of stem cells markers (nestin, vimentin, fibronectin, SH2, SH3 and Oct3/4), chondrogenic and myogenic differentiation potential, even after cryopreservation. Six hours after DP extraction and in vitro plating, rare 5-bromo-2′-deoxyuridine (BrdU) positive cells were observed in pulp central part. After 72 hours, BrdU positive cells increased in number and were found in DP periphery, thus originating a multicellular population of stem cells of high purity. Multiple stem cell niches were identified in different zones of DP, because abundant expression of nestin, vimentin and Oct3/4 proteins was observed, while STRO-1 protein localization was restricted to perivascular niche. Our finding is of importance for the future of stem cell therapies, providing scaling-up of stem cells at early passages with minimum risk of losing their “stemness”

    Mortality from gastrointestinal congenital anomalies at 264 hospitals in 74 low-income, middle-income, and high-income countries: a multicentre, international, prospective cohort study

    Background: Congenital anomalies are the fifth leading cause of mortality in children younger than 5 years globally. Many gastrointestinal congenital anomalies are fatal without timely access to neonatal surgical care, but few studies have been done on these conditions in low-income and middle-income countries (LMICs). We compared outcomes of the seven most common gastrointestinal congenital anomalies in low-income, middle-income, and high-income countries globally, and identified factors associated with mortality. // Methods: We did a multicentre, international prospective cohort study of patients younger than 16 years, presenting to hospital for the first time with oesophageal atresia, congenital diaphragmatic hernia, intestinal atresia, gastroschisis, exomphalos, anorectal malformation, and Hirschsprung's disease. Recruitment was of consecutive patients for a minimum of 1 month between October, 2018, and April, 2019. We collected data on patient demographics, clinical status, interventions, and outcomes using the REDCap platform. Patients were followed up for 30 days after primary intervention, or 30 days after admission if they did not receive an intervention. The primary outcome was all-cause, in-hospital mortality for all conditions combined and each condition individually, stratified by country income status. We did a complete case analysis. // Findings: We included 3849 patients with 3975 study conditions (560 with oesophageal atresia, 448 with congenital diaphragmatic hernia, 681 with intestinal atresia, 453 with gastroschisis, 325 with exomphalos, 991 with anorectal malformation, and 517 with Hirschsprung's disease) from 264 hospitals (89 in high-income countries, 166 in middle-income countries, and nine in low-income countries) in 74 countries. Of the 3849 patients, 2231 (58·0%) were male. Median gestational age at birth was 38 weeks (IQR 36–39) and median bodyweight at presentation was 2·8 kg (2·3–3·3). Mortality among all patients was 37 (39·8%) of 93 in low-income countries, 583 (20·4%) of 2860 in middle-income countries, and 50 (5·6%) of 896 in high-income countries (p<0·0001 between all country income groups). Gastroschisis had the greatest difference in mortality between country income strata (nine [90·0%] of ten in low-income countries, 97 [31·9%] of 304 in middle-income countries, and two [1·4%] of 139 in high-income countries; p≤0·0001 between all country income groups). Factors significantly associated with higher mortality for all patients combined included country income status (low-income vs high-income countries, risk ratio 2·78 [95% CI 1·88–4·11], p<0·0001; middle-income vs high-income countries, 2·11 [1·59–2·79], p<0·0001), sepsis at presentation (1·20 [1·04–1·40], p=0·016), higher American Society of Anesthesiologists (ASA) score at primary intervention (ASA 4–5 vs ASA 1–2, 1·82 [1·40–2·35], p<0·0001; ASA 3 vs ASA 1–2, 1·58, [1·30–1·92], p<0·0001]), surgical safety checklist not used (1·39 [1·02–1·90], p=0·035), and ventilation or parenteral nutrition unavailable when needed (ventilation 1·96, [1·41–2·71], p=0·0001; parenteral nutrition 1·35, [1·05–1·74], p=0·018). Administration of parenteral nutrition (0·61, [0·47–0·79], p=0·0002) and use of a peripherally inserted central catheter (0·65 [0·50–0·86], p=0·0024) or percutaneous central line (0·69 [0·48–1·00], p=0·049) were associated with lower mortality. // Interpretation: Unacceptable differences in mortality exist for gastrointestinal congenital anomalies between low-income, middle-income, and high-income countries. Improving access to quality neonatal surgical care in LMICs will be vital to achieve Sustainable Development Goal 3.2 of ending preventable deaths in neonates and children younger than 5 years by 2030
