Drug utilisation study in surgery outpatient department at Ashwini Rural Medical College, Kumbhari, Solapur
Background: Drug utilization studies should be regularly conducted to increase therapeutic efficacy, decrease adverse effects and provide feedback to prescribers to promote the rational use of drugs. Systematic audit of prescriptions in Surgery Outpatient Department are very few in India. This study was conducted to study drug prescription pattern in Surgery Outpatient Department (OPD) in Ashwini Rural Medical College, Kumbhari.Methods: Six hundred prescriptions were screened and analyzed as per the study parameters at Surgery OPD of Ashwini Rural Medical College, Kumbhari. Study parameters like demographic profile of the patient like age, sex and diagnosis were recorded. Also groups of drugs commonly prescribed, number of drugs per patient, drug profile and drawbacks of prescription if any were recorded and analyzed. The percentage of the drugs prescribed by generic names and from the essential drug list was calculated.Results: Most common group of drugs prescribed by physicians were drugs for peptic ulcer (27.26%), followed by antimicrobials (20.89%). The average number of drugs prescribed per patient was 2.98. Drug prescription by generic name was low (8.82%). The incidence of polypharmacy was common occurrence and some prescriptions had small drawbacks like absence of diagnosis, absence of doctor’s signature, absence of patient age etc.Conclusions: Prescribing by generic name and from the essential drug lists should be encouraged. Training sessions on the correct method of writing prescriptions are needed

This study was conducted to study drug prescription pattern in Medicine Outpatient Department (OPD) in Ashwini Rural Medical College, Kumbhari. Methods: Six hundred prescriptions were screened & analyzed as per the study parameters at Medicine OPD of Ashwini Rural Medical College, Kumbhari. Study parameters like demographic profile of the patient like age, sex and diagnosis were recorded. Doctor details and patient details like age, sex and address were analyzed. Also, groups of drugs commonly prescribed, number of drugs per patient, drug profile and drawbacks of prescription if any were recorded and analyzed.Results: Most common group of drugs prescribed by physicians were Analgesics (21.84%), followed by Drugs for Peptic ulcer (17.92%) and Multivitamins/multimineral (14.70%). The average number of drugs prescribed per patient was 2.38. The incidence of Polypharmacy was common occurrence and some prescriptions had small drawbacks like absence of diagnosis, absence of doctor’s signature, etc.Conclusions: Authors conclude that most of the prescriptions which were analyzed at Ashwini Rural Medical College Hospital, Kumbhari, were according to the standard norms of WHO prescriptions and also most of the drugs prescribed were from the list of essential drug list. But still there is scope for improvement in prescription pattern
Impact of Covid-19 on Tourism Sector in Nepal.
Tourism is an important source of foreign exchange earnings for the government and contributes to the livelihood of millions in developing countries. The purpose of this paper is to explore and illuminate the preliminary impacts of COVID-19 in tourism sector in Nepal and further the paper puts forward policy recommendations for government to avert the worst effects and facilitate recovery. In Nepal, the travel and tourism sector contributes to 8 percent of GDP, 6.7 percent of total employment, and it generates 6 percent of the total foreign exchange earnings. Nepal Tourism Board estimates that loss of 85.2 billion USD monthly from tourism sector only and three in five employees lost their jobs due to COVID-19 in Nepal. The “Visit Nepal 2020” campaign had cancelled which aimed to attract 2 million tourists in the country this year. Tourism sector has already suffered a huge loss, and it is going to take quite to restore. The government should form special task force to create economic response package that will support Nepalese, their job, their businesses from the global impact of COVID-19, and to ready the economy to recover.

The comparative study shows that the classification accuracy can be enhanced by using a combination of the learning techniques and proposes an ensemble technique on publicly available datasets
Multi-Communication System for Physically Disabled PeopleUsing Raspberry Pi
Today to lead our life we needed to keep running with our quick materialistic world. We express our considerations, by speaking with various individuals in various languages and in various ways Therefore there effectively. In any case, it is troublesome for physically incapacitated individuals, for example, for example, dump, hard of hearing, visually impaired and deadened to express their considerations and thoughts. Is a need to develop such a concrete solution for physically disabled people which will results as a better communication media for them. The designed system resolves the said problems. (1) Dump people can express their thoughts by pressing keyboard buttons (2) When blind/deaf person speaks, Raspberry pi based device converts it into text and displays on screen. (3) The paralyzed people by wearing flex sensor glove can express their thoughts through feasible figure movement. All these activities are possible with a unique embedded system using raspberry pi and disabled people can lead their life peacefully and independently through smooth communication of their ideas and thoughts with their family members, friends and society
The Discovery of Modern Anaesthesia – Contributions of Davy, Clarke, Long, Wells and Morton
While many may argue as to who deserves the most credit for the discovery of modern anaesthesia, events in the late 18 th and early 19 th centuries led to the introduction and development of modern anaesthetic techniques. English physicist and chemist Humphry Davy [1778-1829] first became aware of the sedative and analgesic properties of nitrous oxide in 1795. Although he never experimented with the drug during a surgical procedure, he was the first to suggest that it would be beneficial in relieving pain during surgical procedures. The mind-altering properties of nitrous oxide and ether were often abused for recreational purposes, and the term \u27ether frolics\u27 was coined to describe such use. While physician William Crawford Williamson Long [1815-1878] first used ether during general surgery, medical student William Edward Clarke [1819-1898] was the first to use ether for dental extraction in 1842. Dr. Long neglected to publicize his findings until 1849, thereby denying himself much of the credit he deserved. Dentist Horace Wells [1815-1848] successfully used nitrous oxide for dental procedures, but a public demonstration which he held in January 1845 turned out to be a fiasco. Medical student William Thomas Green Morton [1819-1868] was the first to publicly demonstrate the effectiveness of ether for general surgery on October 16, 1846. This article seeks to give rightful credit to each of these individuals for their unique contributions to the discovery of modern anaesthesia
Estudios efectivos de potencial para algunas nuevas moléculas híbridas para su actividad contra el cáncer de próstata
Objective: The present work aimed at developing novel hybrid molecules for targeting the prostate cancer. It is observed that two human shock proteins Hsp70 and Hsp90 are over-expressed in prostate cancer making them one of the important drug targets. We have designed and developed twelve new hybrid molecules 6a-j for targeting these proteins. Methods: The designed molecules were prepared following a four step reaction protocol and characterized on the basis of proton NMR and Mass spectrometry. These were subjected to in vitro studies by means of Oncotest and CCK-8 assays with two cell lines DU145 and 22Rv1. The selected molecules 6b and 6i were subjected to molecular docking and then for SPR based affinity assay. Results: Compounds 6b and 6i were found to be highly active anticancer compounds comparable to standard drug enzalutamide. They have significant IC50 and high dock score for the Hsp70 and Hsp90. These compounds are selective and have good binding affinity for the Hsp70 due to high Kd. Conclusion: Compound 6b and 6i can serve as lead molecules for the development of antiprostate cancer drugs with Hsp70 as target.Objetivo: El presente trabajo tuvo como objetivo desarrollar nuevas moléculas híbridas para atacar el cáncer de próstata. Se observa que dos proteínas de choque humano, Hsp70 y Hsp90, se sobreexpresan en el cáncer de próstata, lo que las convierte en uno de los objetivos farmacológicos importantes. Hemos diseñado y desarrollado doce nuevas moléculas híbridas 6a-j para dirigir estas proteínas. Métodos: Las moléculas diseñadas se prepararon siguiendo un protocolo de reacción de cuatro etapas y se caracterizaron sobre la base de RMN de protón y espectrometría de masas. Estos se sometieron a estudios in vitro por medio de ensayos Oncotest y CCK-8 con dos líneas celulares DU145 y 22Rv1. Las moléculas seleccionadas 6b y 6i se sometieron a acoplamiento molecular y luego a ensayo de afinidad basado en SPR. Resultados: Se descubrió que los Compuestos 6b y 6i son compuestos anticancerígenos muy activos comparables al fármaco estándar enzalutamida. Tienen un IC50 significativo y una puntuación alta para el muelle de Hsp70 y Hsp90. Estos compuestos son selectivos y tienen una buena afinidad de unión por la Hsp70 debido a la alta Kd. Conclusión: Los compuestos 6b y 6i pueden servir como moléculas principales para el desarrollo de fármacos antiprostáticos contra el cáncer con Hsp70 como objetivo
Study of early perinatal outcome in lower segment caesarean section in severe foetal distress at tertiary care centre
Background: For asphyxia, the fetus reacts with a series of responses. First there is redistribution of blood flow to vital centres to limit the deleterious effects of oxygen limitation in the brain, heart and adrenal glands. A further compensatory response is that overall fetal oxygen consumption declines to values as low as 50% of the control.Methods: This was a prospective study. Early perinatal outcome of newborns delivered through Caesarean section due to clinical foetal distress in labour was compared with a group of newborns similarly delivered via Caesarean section without a diagnosis of clinical foetal distress. Data collected and analysed using appropriate standard statistical methods i.e. Chi-square (X2) test and ‘Z’ - test.Results: Most common indication for NICU admission in study group was MAS (Meconium Aspiration Syndrome 14%) followed by MAS with perinatal asphyxia (5.33%), MAS with Hypoxic ischaemic encephalopathy (HIE) - stage III (3.33%) paerintal asphyxia (2.66%), severe PNA with HIE - III (0.66%) and TTN (0.66%) meconium gastritis (0.66%) respectively.Conclusions: Clinical foetal distress (study group) was found to be significantly associated with low 1 min and 5 min. Apgar score. There was no significant difference in immediate NICU admission whether D-D (i.e. detection fetal detection to delivery interval) time interval 30 minutes. But rate of mortality was high when D-D (i.e. detection fetal detection to delivery interval) was >30 minutes
Functional outcome of locking anatomical plate in extra articular fractures of the distal humerus
Background: Extra articular distal humerus fractures are difficult to treat with conventional implants like intra medullary nail, 4.5 DCP and dual plate. The present study aims to study the functional outcome of the extra articular distal humerus plate (EADHP).Methods: 48 patients with displaced extra articular distal humerus fractures were included in the study. Inclusion criteria were age more than 18 years, closed fractures with or without radial nerve palsy and less than 3 weeks old trauma. Patients aged less than 18 years, those having open fractures, fractures more than 3 weeks old, non – unions and pathological fractures were excluded from the study. All patients were operated with EADHP. Clinically, the outcome was assessed by the disability of arm, shoulder and hand (DASH) score and elbow range of motion radiologically, union was evaluated on anteroposterior and lateral radiographs.Results: Out of 48 patients, 12 had AO type 12 A1 fracture, 26 patients had type B1 fractures and remaining 10 had type C1 fractures. Mean DASH score at final follow up was 18.1; range being 12.6 to 35.7 points. The mean elbow range of motion was 0 to 130 degrees (range: 120 to 140 degrees). The mean duration for complete radiological fracture union was 14 weeks, range being 12 to 18 weeks.Conclusions: The extraarticular distal humerus plate is an ideal implant for the fixation of distal humerus fractures since it provides good stability of fracture and enables early return to function
