The study of correlation between forward head posture and neck pain in Iranian office workers
Objectives: Factors such as prolonged sitting at work or improper posture of head during work may have a great role in neck pain occurrence among office employees, particularly among those who work with computers. Although some studies claim a significant difference in head posture between patients and pain-free participants, in literature the forward head posture (FHP) has not always been associated with neck pain. Since head, cervical and thoracic postures and their relation with neck pain has not been studied in Iranian office employees, the purpose of this study was to investigate the relationship between some work-related and individual factors, such as poor posture, with neck pain in the office employees. Material and Methods: It was a cross-sectional correlation study carried out to explore the relationship between neck pain and sagittal postures of cervical and thoracic spine among office employees in forward looking position and also in a working position. Forty-six subjects without neck pain and 55 with neck pain were examined using a photographic method. Thoracic and cervical postures were measured using the high thoracic (HT) and craniovertebral (CV) angles, respectively. Results: High thoracic and CV angles were positively correlated with the presence of neck pain only in working position (p 0.05). Conclusions: Our findings have revealed that office employees had a defective posture while working and that the improper posture was more severe in the office employees who suffered from the neck pain
The effect of exercise therapy on knee osteoarthritis: A randomized clinical trial
Background: Knee osteoarthritis (OA) is the most common musculoskeletal disease among old individuals which affects ability for sitting on the chair, standing, walking and climbing stairs. Our objective was to investigate the short and long-term effects of the most simple and the least expensive exercise protocols in combination to conventional conservative therapy for knee OA. Methods: It was a single blind RCT study with a 12-months follow-up. Totally, 56 patients with knee OA were assigned into 2 random groups. The patients in exercise group received exercise for knee muscles in combination with non-steroid anti-inflammatory drugs (NSAIDs) and 10 sessions acupuncture and physiotherapy modalities. Non-exercise group received similar treatments except exercise program. The changes in patients' pain and functional status were evaluated by visual analog scale (VAS), knee and osteoarthritis outcome score (KOOS) questionnaire and functional tests (4 steps, 5 sit up, and 6 min walk test) before and after treatment (1 and 3 months after intervention), and 1 year later at the follow-up. Results: The results showed that the patients with knee OA in exercise group had significant improvement in pain, disability, walking, stair climbing, and sit up speed after treatment at first and second follow-up when compared with their initial status and when compared with non-exercise group. At third follow up (1 year later) there was significant difference between groups in VAS and in three items of KOOS questionnaire in functional status. Conclusion: Non aerobic exercises for muscles around knee can augment the effect of other therapeutic interventions like medical therapy, acupuncture, and modalities for knee OA
Buprenorphine added on brief cognitive behavioral therapy for treatment of methamphetamine use disorder
Background: Methamphetamine (MA) use remains a major public health concern around the world. Recent findings suggest that buprenorphine may be helpful for cocaine use reduction. Moreover, animal studies described reduced dopamine peak effect following MA use, due to the administration of low dose buprenorphine.
Objectives: This study examined the effectiveness of buprenorphine with brief cognitive behavioral therapy on MA use disorder.
Methods: The study was conducted in an outpatient substance abuse treatment center in Qazvin, Iran. Nineteen MA users received buprenorphine for 24 weeks combined with brief cognitive behavioral therapy in an outpatient substance abuse treatment program, three times per week, as a before and after non - randomization study. Clinical outcomes included treatment retention, MA use, degree of MA dependency and craving, quality of life, cognitive abilities questionnaire, addiction severity and also adverse events. Data was analyzed by performing repeated measures analysis and the Friedman test for nonparametric variables.
Results: Fifteen participants completed the study during six months and frequency of MA use was significantly decreased at 24 weeks (P < 0.001). There were also significant reductions in craving (P < 0.001), degree of MA dependence (P < 0.001), and improvements in quality of life, cognitive ability, and some subscales of addiction severity.
Conclusions: The results of this preliminary clinical study demonstrated that buprenorphine could potentially attenuate MA craving and alternate rewarding effects of MA and had promising effects on cognitive impairment. Furthermore, buprenorphine can be considered as a harm reduction intervention in some communities, in which the people, as a result of cultural beliefs, do not accept a therapy, which only consists of counseling and no medications
The impact of provider payment reforms and associated care delivery models on cost and quality in cancer care: A systematic literature review
Objectives To investigate the impact of provider payment reforms and associated care delivery models on cost and quality in cancer care. Methods Data sources/study setting: Review of English-language literature published in PubMed, Embase and Cochrane library (2007-2019). Study design: We performed a systematic literature review (SLR) to identify the impact of cancer care reforms. Primary endpoints were resource use, cost, quality of care, and clinical outcomes. Data collection/extraction methods: For each study, we extracted and categorized comparative data on the impact of policy reforms. Given the heterogeneity in patients, interventions and outcome measures, we did a qualitative synthesis rather than a metaanalysis. Results Of the 26 included studies, seven evaluations were in fact qualified as quasi experimental designs in retrospect. Alternative payment models were significantly associated with reduction in resource use and cost in cancer care. Across the seventeen studies reporting data on the implicit payment reforms through care coordination, the adoption of clinical pathways was found effective in reduction of unnecessary use of low value services and associated costs. The estimates of all measures in ACO models varied considerably across participating providers, and our review found a rather mixed impact on cancer care outcomes. Conclusion The findings suggest promising improvement in resource utilization and cost control after transition to prospective payment models, but, further primary research is needed to apply robust measures of performance and quality to better ensure that providers are delivering high-value care to their patients, while reducing the cost of care. © 2019 Nejati et al. Effectiveness of exercise therapy and manipulation on sacroiliac joint dysfunction: A randomized controlled trial
Background: The sacroiliac joint dysfunction (SIJD) has been found to be the primary culprit for lower back pain (LBP), but it is still overlooked and treated as LBP. There are no guidelines or appropriate therapeutic protocols for SIJD. Thus, there is a need for an effective treatment strategy for SIJD. Objective: To compare exercise therapy (ET), manipulation therapy (MT), and a combination of the 2 (EMT) in terms of their effectiveness in treating SIJD. Study Design: A comparative, prospective, single-blind randomized controlled trial. Setting: Sports Medicine Department of Rasoul Akram Hospital. Methods: A total of 51 patients with lower back or buttock pain resulting from SIJD were randomly assigned to 1 of 3 study groups: ET, MT, or EMT. The ET group received posterior innominate self-mobilization, sacroiliac joint stretching, and spinal stabilization exercises. The MT group underwent posterior innominate mobilization and SIJ manipulation. Lastly, the EMT group received manipulation maneuvers followed by exercise therapy. Pain and disability were assessed at 6, 12, and 24 weeks after the interventions. Results: All 3 groups demonstrated significant improvement in pain and disability scores compared to the baseline (P < 0.05). The difference among these therapeutic protocols was found to be a function of time. At week 6, MT showed notable results, but at week 12, the effect of ET was remarkable. Finally, at week 24, no significant difference was observed among the study groups. Limitations: A major limitation of the present study is lack of a control group receiving a type of intervention other than the experimental protocols. Another limitation is the short duration of follow-ups. Conclusions: Exercise and manipulation therapy appear to be effective in reducing pain and disability in patients with SIJD. However, the combination of these 2 therapies does not seem to bring about significantly better therapeutic results than either approach implemented separately. © 2019, American Society of Interventional Pain Physicians. The association of emotional intelligence with sport injuries and receiving penalty cards among Iranian professional soccer players
Background: High emotional intelligence (EI) seems to be preventive for unconventional sports behavior within competitions leading prevention of sport injuries and also minimization of giving penalty cards. Objectives: The present study aimed to examine this relationship among Iranian Premier League footballers. Methods: This study was performed on Iranian professional soccer players participating in Premier League in 2014-2015 season. To assess emotional intelligence among athletes, the Schutte Self-Report Emotional Intelligence test (SSEIT) was employed. Sport-related injuries were recorded by the physician of each team. Also, the reports of the number of yellow and red cards for each athlete as well as for all teams in two phases (middle and end of each season) was recorded by the Football League Organization were reviewed and recorded. The chi-square test and t-test were used for comparing the variables. The Pearson�s correlation test and the multivariable regression model were also used for discovering association and relationship, respectively. P values of 0.05 or less were considered statistically significant. Results: Among different subscales of EI, only regulation of emotions was significantly different between injured and non-injured athletes (P = 0.04). Lower ability to regulate emotions was associated with higher risk for sport injuries (OR = 0.88, 95 CI: 0.79-0.98, P = 0.02). None of the subscales of EI was related to receiving yellow card, but utilizing emotions was adversely related to receiving red card. The association between utilizing emotions and receiving red card changed to insignificant after using the multivariable regression modeling. Conclusions: By regulating emotions, sport-related injuries can be preventable in soccer players. However, EI may not be helpful in reducing sport fines. © 2019, Author(s)
Service-Oriented Architecture Modeling: Bridging the Gap between Structure and Behavior
International audienceModel-driven development of large-scale software systems is highly likely to produce models that describe the systems from many diverse perspectives using a variety of modeling languages. Checking and maintaining consistency of information captured in such multi-modeling environments is known to be challenging. In this paper we describe an approach to systematically synchronize multi-models. The approach specifically addresses the problem of synchronizing business processes and domain models in a Service-oriented Architecture development environment. In the approach, the human effort required to synchronize independently developed models is supplemented with significant automated support. This process is used to identify concept divergences, that is, a concept in one model which cannot be matched with concepts in the other model. We automate the propagation of divergence resolution decisions across the conflicting models. We illustrate the approach using models developed for a Car Crash Crisis Management System (CCCMS), a case study problem used to assess Aspect-oriented Modeling approaches
International genomic evaluation methods for dairy cattle
<p>Abstract</p> <p>Background</p> <p>Genomic evaluations are rapidly replacing traditional evaluation systems used for dairy cattle selection. Higher reliabilities from larger genotype files promote cooperation across country borders. Genomic information can be exchanged across countries using simple conversion equations, by modifying multi-trait across-country evaluation (MACE) to account for correlated residuals originating from the use of foreign evaluations, or by multi-trait analysis of genotypes for countries that use the same reference animals.</p> <p>Methods</p> <p>Traditional MACE assumes independent residuals because each daughter is measured in only one country. Genomic MACE could account for residual correlations using daughter equivalents from genomic data as a fraction of the total in each country and proportions of bulls shared. MACE methods developed to combine separate within-country genomic evaluations were compared to direct, multi-country analysis of combined genotypes using simulated genomic and phenotypic data for 8,193 bulls in nine countries.</p> <p>Results</p> <p>Reliabilities for young bulls were much higher for across-country than within-country genomic evaluations as measured by squared correlations of estimated with true breeding values. Gains in reliability from genomic MACE were similar to those of multi-trait evaluation of genotypes but required less computation. Sharing of reference genotypes among countries created large residual correlations, especially for young bulls, that are accounted for in genomic MACE.</p> <p>Conclusions</p> <p>International genomic evaluations can be computed either by modifying MACE to account for residual correlations across countries or by multi-trait evaluation of combined genotype files. The gains in reliability justify the increased computation but require more cooperation than in previous breeding programs.</p
Diagnosing and measuring incompatibilities between pairs of services
International audienceThis text presents a tool, from its design to its implementation, which detects all behavioural incompatibilities between two service interfaces. Unlike prior work, the proposed solution does not simply check whether two services are incompatible or not, it rather provides detailed diagnosis, including the incompatibilities and for each one the location in the service interfaces where these incompatibilities occur. A measure of similarity between interfaces which considers outputs from the detection algorithm is proposed too. A visual report of the comparison analysis is also provided which pinpoints a set of incompatibilities that cause a behavioural interface not to simulate another one

Modeling and analysis of interactions among services is a crucial issue in
