    Immunocytochemical and electrophysiological characterization of GABA receptors in the frog and turtle retina

    AbstractThe expression of GABA receptors (GABARs) was studied in frog and turtle retinae. Using immunocytochemical methods, GABAARs and GABACRs were preferentially localized to the inner plexiform layer (IPL). Label in the IPL was punctate indicating a synaptic clustering of GABARs. Distinct, but weaker label was also present in the outer plexiform layer. GABAAR and GABACR mediated effects were studied by recording electroretinograms (ERGs) and by the application of specific antagonists. Bicuculline, the GABAAR antagonist, produced a significant increase of the ERG. Picrotoxin, when co-applied with saturating doses of bicuculline, caused a further increase of the ERG due to blocking of GABACRs. The putative GABACR antagonist Imidazole-4-acidic acid (I4AA) failed to antagonize GABACR mediated inhibition and, in contrast, appeared rather as an agonist of GABARs

    Awareness and use of intertrochanteric osteotomies in current clinical practice. An international survey

    Current literature shows that intertrochanteric osteotomies can produce excellent results in selected hip disorders in specific groups of patients. However, it appears that this surgical option is considered an historical one that has no role to play in modern practice. In order to examine current awareness of and views on intertrochanteric osteotomies among international hip surgeons, an online survey was carried out. The survey consisted of a set of questions regarding current clinical practice and awareness of osteotomies. The second part of the survey consisted of five clinical cases and sought to elicit views on preoperative radiological investigations and preferred (surgical) treatments. The results of our survey showed that most of these experts believe that intertrochanteric osteotomies should still be performed in selected cases. Only 56% perform intertrochanteric osteotomies themselves and of those, only 11% perform more than five per year. The responses to the cases show that about 30–40% recommend intertrochanteric osteotomies in young symptomatic patients. This survey shows that the role of intertrochanteric osteotomies is declining in clinical practice

    The Harris hip score: Do ceiling effects limit its usefulness in orthopedics?: A systematic review

    The Harris hip score (HHS), a disease-specific health status scale that is frequently used to measure the outcome of total hip arthroplasty, has never been validated properly. A questionnaire is suitable only when all 5 psychometric properties are of sufficient quality. We questioned the usefulness of the HHS by investigating its content validity. We performed a systematic review based on a literature search in PubMed, Embase, and the Cochrane Library for descriptive studies published in 2007. 54 studies (59 patient groups) met our criteria and were included in the data analysis. To determine the content validity, we calculated the ceiling effect (percentage) for each separate study and we pooled data to measure the weighted mean. A subanalysis of indications for THA was performed to differentiate the populations for which the HHS would be suitable and for which it would not. A ceiling effect of 15% or less was considered to be acceptable. Over half the studies (31/59) revealed unacceptable ceiling effects. Pooled data across the studies included (n = 6,667 patients) suggested ceiling effects of 20% (95%CI: 18-22). Ceiling effects were greater (32%, 95%CI:12-52) in those patients undergoing hip resurfacing arthroplasty.     Poorer outcome of elderly patients treated with extended-field radiotherapy compared with involved-field radiotherapy after chemotherapy for Hodgkin's lymphoma: an analysis from the German Hodgkin Study Group

    Background: The optimal treatment of elderly patients with Hodgkin's lymphoma (HL) is still a matter of debate. Since many of these patients receive combined modality treatment, we evaluated the impact of different radiation field sizes, that is extended-field (EF) or involved-field (IF) technique when given after four cycles of chemotherapy. Patients and methods: In the multicenter HD8 study of the German Hodgkin Study Group, 1204 patients with early-stage unfavorable HL were randomized to receive four cycles of chemotherapy followed by either radiotherapy (RT) of 30 Gy EF + 10 Gy to bulky disease (arm A) or 30 Gy IF + 10 Gy to bulky disease (arm B). A total of 1064 patients were assessable for the analysis. Of these, 89 patients (8.4%) were 60 years or older. Results: Elderly patients had a poorer risk profile. Acute toxicity from RT was more pronounced in elderly patients receiving EF-RT compared with IF-RT [World Health Organization (WHO) grade 3/4: 26.5% versus 8.6%)]. Freedom from treatment failure (FFTF, 64% versus 87%) and overall survival (OS, 70% versus 94%) after 5 years was lower in elderly patients compared with younger patients. Importantly, elderly patients had poorer outcome when treated with EF-RT compared with IF-RT in terms of FFTF (58% versus 70%; P = 0.034) and OS (59% versus 81%; P = 0.008). Conclusion: Elderly patients with early-stage unfavorable HL generally have a poorer risk profile and outcome when compared with younger patients.     Retinal horizontal cells use different synaptic sites for global feedforward and local feedback signaling

    In the outer plexiform layer (OPL) of the mouse retina, two types of cone photoreceptors (cones) provide input to more than a dozen types of cone bipolar cells (CBCs). This transmission is modulated by a single horizontal cell (HC) type, the only interneuron in the outer retina. Horizontal cells form feedback synapses with cones and feedforward synapses with CBCs. However, the exact computational role of HCs is still debated. Along with performing global signaling within their laterally coupled network, HCs also provide local, cone-specific feedback. Specifically, it has not been clear which synaptic structures HCs use to provide local feedback to cones and global forward signaling to CBCs. Here, we reconstructed in a serial block-face electron microscopy volume the dendritic trees of five HCs as well as cone axon terminals and CBC dendrites to quantitatively analyze their connectivity. In addition to the fine HC dendritic tips invaginating cone axon terminals, we also identified “bulbs”, short segments of increased dendritic diameter on the primary dendrites of HCs. These bulbs are located well below the cone axon terminal base and make contact to other cells mostly identified as other HCs or CBCs. Using immunolabeling we show that HC bulbs express vesicular gamma-aminobutyric acid transporters and co-localize with GABA receptor γ2 subunits. Together, this suggests the existence of two synaptic strata in the mouse OPL, spatially separating cone-specific feedback and feedforward signaling to CBCs.     Linking Unserved Energy to Weather Regimes

    The integration of renewable energy sources into power systems is expected to increase significantly in the coming decades. This can result in critical situations related to the strong variability in space and time of weather patterns. During these critical situations the power system experiences a structural shortage of energy across multiple time steps and regions, leading to Energy Not Served (ENS) events. Our research explores the relationship between six weather regimes that describe the large scale atmospheric flow and ENS events in Europe by simulating future power systems. Our results indicate that most regions have a specific weather regime that leads to the highest number of ENS events. However, ENS events can still occur during any weather regime, but with a lower probability. In particular, our findings show that ENS events in western and central European countries often coincide with either the positive Scandinavian Blocking (SB+), characterised by cold air penetrating Europe under calm weather conditions from north-eastern regions, or North Atlantic Oscillation (NAO+) weather regime, characterised by westerly flow and cold air in the southern half of Europe. Additionally, we found that the relative impact of one of these regimes reaches a peak 10 days before ENS events in these countries. In Scandinavian and Baltic countries, on the other hand, our results indicate that the relative prevalence of the negative Atlantic Ridge (AR-) weather regime is higher during and leading up to the ENS event.Comment: Rogier H.     Dynamics of Wetting Fronts in Porous Media

    We propose a new phenomenological approach for describing the dynamics of wetting front propagation in porous media. Unlike traditional models, the proposed approach is based on dynamic nature of the relation between capillary pressure and medium saturation. We choose a modified phase-field model of solidification as a particular case of such dynamic relation. We show that in the traveling wave regime the results obtained from our approach reproduce those derived from the standard model of flow in porous media.
