Challenges and Prospects for Integrating the Assessment of Health Impacts in the Licensing Process of Large Capital Project in Brazil
Brazil was one of the first countries in Latin America to institutionalize a National Environmental Policy in 1981, including the environmental impact assessment (EIA) process of economic activities with anticipated impacts on the environment. Today, EIA practice in Brazil comes with a number of limitations: it is constrained by its environmental advocacy role; application is strongly oriented towards large capital projects; and social responsibility considerations are only partially included. Consequently, EIA studies mainly address issues connected to localised and direct environmental impacts, largely ignoring any socio-economic and health impacts. This perspective paper highlights limitations of current EIA practice in Brazil with a focus on health considerations in impact assessment. While recognizing the positive impact to municipalities where large capital projects are being developed and operated, adverse impacts on health are a reality with measurable evidence in Brazil. Therefore, we argue that specificities on how to systematically assess and monitor potential health impacts cannot remain invisible in the Brazilian legislation, as currently seen in the reformulation of the licensing process in the country. Estimación de la concentración media diaria de material particulado fino en la región del Complejo Industrial y Portuario de Pecém, Ceará, Brasil
A exposição ao material particulado fino (MP2,5) está associada a inúmeros
desfechos à saúde. Desta forma, monitoramento da concentração ambiental
do MP2,5 é importante, especialmente em áreas amplamente industrializadas,
pois abrigam potenciais emissores do MP2,5 e de substâncias com potencial de
aumentar a toxicidade de partículas já suspensas. O objetivo desta pesquisa é estimar a concentração diária do MP2,5 em três áreas de influência do
Complexo Industrial e Portuário do Pecém (CIPP), Ceará, Brasil. Foi aplicado
um modelo de regressão não linear para a estimativa do MP2,5, por meio de
dados de profundidade óptica monitorados por satélite. As estimativas foram
realizadas em três áreas de influência (Ai) do CIPP (São Gonçalo do Amarante – Ai I, Paracuru e Paraipaba – Ai II e Caucaia – Ai III, no período de
2006 a 2017. As médias anuais das concentrações estimadas foram inferiores
ao estabelecido pela legislação nacional em todas as Ai (8µg m-3). Em todas as
Ai, os meses referentes ao período de seca (setembro a fevereiro) apresentaram
as maiores concentrações e uma predominância de ventos leste para oeste. Os
meses que compreendem o período de chuva (março a agosto) apresentaram as
menores concentrações e ventos menos definidos. As condições meteorológicas
podem exercer um papel importante nos processos de remoção, dispersão ou
manutenção das concentrações do material particulado na região. Mesmo com
baixas concentrações estimadas, é importante avaliar a constituição das partículas finas dessa região, bem como sua possível associação a efeitos adversos à
saúde da população local.Exposure to fine particulate matter (PM2.5) is associated with numerous negative health outcomes.
Thus, monitoring the environmental concentration of PM2.5 is important, especially in heavily
industrialized areas, since they harbor potential
emitters of PM2.5 and substances with the potential
to increase the toxicity of already suspended particles. This study aims to estimate daily concentrations of PM2.5 in three areas under the influence of
the Industrial and Port Complex of Pecém (CIPP),
Ceará State, Brazil. A nonlinear regression model
was applied to estimate PM2.5, using satellitemonitored optical depth data. Estimates were
performed in three areas of influence (Ai) of the
CIPP (São Gonçalo do Amarante – AiI, Paracuru
and Paraipaba – AiII, and Caucaia – AiIII), from
2006 to 2017. Estimated mean annual concentrations were lower than established by Brazil’s national legislation in all three Ai (8µg m-³). In all
the Ai, the months of the dry season (September to
February) showed the highest concentrations and
a predominance of east winds, while the months
of the rainy season (March to August) showed
the lowest concentrations and less defined winds
Weather conditions can play an important role in
the removal, dispersal, or maintenance of concentrations of particulate matter in the region. Even
at low estimated concentrations, it is important
to assess the composition of fine participles in this
region and their possible association with adverse

Por ello, la supervisión de la concentración ambiental del MP2,5 es importante, especialmente en
áreas ampliamente industrializadas, puesto que
albergan potenciales emisores de MP2,5 y de sustancias con potencial de aumentar la toxicidad
de partículas ya suspendidas. El objetivo de esta
investigación es estimar la concentración diaria
del MP2,5 en tres áreas de influencia del Complejo Industrial y Portuario de Pecém (CIPP), Ceará,
Brasil. Se aplicó un modelo de regresión no lineal
para la estimación del MP2,5, mediante datos de
profundidad óptica supervisados por satélite. Las
estimaciones fueron realizadas en tres áreas de influencia (Ai) del CIPP (São Gonçalo do Amarante
– Ai I, Paracuru y Paraipaba – Ai II y Caucaia
– Ai III en el período de 2006 a 2017. Las medias
anuales de las concentraciones estimadas fueron
inferiores a lo establecido por la legislación nacional en todas las Ai (8µg m-³). En todas las Ai, los
meses referentes al período de sequía (de setiembre
a febrero) presentaron las mayores concentraciones y una predominancia de vientos este a oeste,
los meses que comprenden el período de lluvia
(marzo a agosto) presentaron las menores concentraciones y vientos menos definidos. Las condiciones meteorológicas pueden ejercer un papel importante en los procesos de eliminación, dispersión o
mantenimiento de las concentraciones del material
particulado en la región. Incluso con bajas concentraciones estimadas es importante que se evalúe la
constitución de las partículas finas de esta región,
así como su posible asociación con efectos adversos
para la salud de la población local

The relevance of social network analysis on the epidemiology and prevention of sexually transmitted diseases
Historically, the epidemiology of sexually transmitted diseases (STD) has been based on individual attributes and behavior. However, STD constitute a good example of diseases that depend on personal contacts for dissemination. Social network analysis is a relatively new technique that studies the interactions among people. Since 1985 when it was first used for STD, some studies have been done using the technique, especially in the last 10 years. The two network-based designs, sociocentric or complete networks and egocentric or personal networks, are currently recognized as important tools for a better understanding of STD's dynamic. Here an overview is presented of social network analysis: the technique, its use, and its limitations. Ethical considerations regarding social network analyses are also briefly discussed
High HIV infection prevalence in a group of men who have sex with men
Brazil is characterized by a concentrated AIDS epidemic, it has a prevalence of less than 1% in the general population. However, there are higher rates in specific populations, especially in men who have sex with men. The study's aim was to analyze the association between sociodemographic characteristics, sexual practices, sexual behaviors and the HIV infection in a group of men who have sex with men. Secondary data was collected between June 2014 and September 2015 in a research of cross-sectional design in the city of Rio de Janeiro, Brazil. Volunteers answered an online computerized questionnaire and took HIV test. Chi-squared distribution and multiple logistic regression was used. There were 341 participants. Most of them were racially mixed, single, average age of 30.6 years and with a higher education level. The HIV prevalence was 13.9%. Two logistic models were fit (insertive or receptive anal intercourse). Both models showed an association with HIV among those who had a HIV positive sexual partner (Odds Ratio ≈ 2.5) and a high self-perception of acquiring HIV (Model 1: Odds Ratio ≈ 7/Model 2: Odds Ratio ≈ 10). Low condom usage in receptive anal intercourse with casual partners had a direct association with HIV seropositivity, whereas insertive anal intercourse with casual partners with or without condoms were inversely related. The study identified a high prevalence of HIV infections among a group of men who sex with men with a high self-perception risk of acquiring HIV. The findings also showed a relation with sociodemographic and sexual behavior variables. CANINE VISCERAL LEISHMANIASIS CASE INVESTIGATION IN THE JACARE REGION OF NITEROI, RIO DE JANEIRO, BRAZIL
A leishmaniose visceral americana é zoonose em expansão no Brasil com transmissão vetorial, onde o cão é o principal reservatório urbano. Partindo de um caso canino da doença no Jacaré, Niterói/RJ, realizouse inquérito epidemiológico canino e levantamento entomológico para avaliar a extensão da enfermidade na localidade. As amostras foram coletadas ao redor do caso e os cães identificados por testes sorológicos (teste rápido de imunocromatografia em dupla plataforma, ensaio imunoenzimático/ELISA, reação de imunofluorescência indireta/RIFI). O diagnóstico parasitológico foi realizado nos animais reagentes em ao menos um desses exames. O levantamento entomológico foi feito com armadilhas luminosas e coleta manual. As associações entre variáveis caninas coletadas e o desfecho (ELISA e RIFI reagentes) foram avaliadas pelo teste qui-quadrado e ajustadas pela regressão logística multivariada para variáveis com p< 0,1 na análise bivariada. Foram detectados 17 casos de LVA entre os 110 cães avaliados (prevalência de 15,5%). Presença de ectoparasitas (OR 6,5; 95% IC 1,1-37,4), animais com sinais clínicos (OR 9,5; 95% IC 1,2-76,6) e casos prévios de LVA canina na residência (OR 17,9; 95% IC 2,2-147,1) foram associados ao desfecho. Não foi detectada Lutzomyia longipalpis. Nossos resultados indicam situação de transmissão contínua na área.American visceral leishmaniasis is a vector-borne zoonosis in expansion in Brazil. Dogs are the main urban reservoir. Departing from a case of canine visceral leishmaniasis (CVL) in Jacaré, Niterói, Rio de Janeiro State, an epidemiological canine and entomological study was performed to assess the extension of the disease at the location. Sample was collected around the case and the dogs identified by serological tests (rapid double platform immunochromatographic exams, immunoenzymatic assay/ELISA, indirect immunofluorescence/IFAT). The parasitological diagnosis was performed in animals positive in at least one of these tests. The entomological study was carried out by using light traps and manual collection. The associations between canine variables and outcome (ELISA and IFAT reagents) were assessed by the chi-square test and adjusted by multivariate logistic regression for those associations with p < 0.1 in the bivariate analysis. Seventeen cases of CVL were detected among 110 evaluated dogs (prevalence of 15.5%). Presence of ectoparasites (OR 6.5; 95% CI 1.1-37.4), animals with clinical signs (OR 9.5; 95% CI 1.2-76.6), and previous cases of CVL in the same house (OR 17.9; 95% CI 2.2-147.1) were associated with the outcome.
