Complex-linear invariants of biochemical networks
The nonlinearities found in molecular networks usually prevent mathematical
analysis of network behaviour, which has largely been studied by numerical
simulation. This can lead to difficult problems of parameter determination.
However, molecular networks give rise, through mass-action kinetics, to
polynomial dynamical systems, whose steady states are zeros of a set of
polynomial equations. These equations may be analysed by algebraic methods, in
which parameters are treated as symbolic expressions whose numerical values do
not have to be known in advance. For instance, an "invariant" of a network is a
polynomial expression on selected state variables that vanishes in any steady
state. Invariants have been found that encode key network properties and that
discriminate between different network structures. Although invariants may be
calculated by computational algebraic methods, such as Gr\"obner bases, these
become computationally infeasible for biologically realistic networks. Here, we
exploit Chemical Reaction Network Theory (CRNT) to develop an efficient
procedure for calculating invariants that are linear combinations of
"complexes", or the monomials coming from mass action. We show how this
procedure can be used in proving earlier results of Horn and Jackson and of
Shinar and Feinberg for networks of deficiency at most one. We then apply our
method to enzyme bifunctionality, including the bacterial EnvZ/OmpR osmolarity
regulator and the mammalian
6-phosphofructo-2-kinase/fructose-2,6-bisphosphatase glycolytic regulator,
whose networks have deficiencies up to four. We show that bifunctionality leads
to different forms of concentration control that are robust to changes in
initial conditions or total amounts. Finally, we outline a systematic procedure
for using complex-linear invariants to analyse molecular networks of any

nes de la columna y describimos un sistema de última generación que permite montajes híbridos (uñas, tornillos
pediculares y cables de tracción). Se han intervenido un total de 200 escoliosis idiopáticas neuropáticas y congé

nitas desde 2004 hasta 2010 con el sistema Transpine
®
mediante montaje híbrido. Todos los pacientes han sido
intervenidos por el mismo cirujano. Se analizó el porcentaje de corrección posoperatoria obtenida. Se observa
un cambio 70% y 30% del eje coronal y sagital respectivamente (P<0.001). Las pérdidas angular fueron <10º. El
sistema Transpine
®
permite la posibilidad de realizar montajes híbridos sin complicaciones para el tratamiento de
las deformidades. Los resultados obtenidos son comparables con los publicados en la bibliografía.Correction systems of the latest generation for the treatment of spinal deformities have aroused
great controversy. We conducted a historical review of the surgical treatment and we describe a new system that
allows the use of hybrid assemblies (claws, pedicle screws and pull cable wires). A total of 200 patients from
2004 to 2010 with idiopathic scoliosis, congenital neuropathic were operated with the Transpine
®
system using
hybrid assembly. The correction percentage of postoperative obtained was analyzed. Change 70% and 30% res

pectively of the coronal and sagittal axis was observed (P <0.001). The angular losses were <10º. The Transpine
®
system let surgeons to perform uncomplicated hybrid safely assemblies for treatment of deformities. The results
are comparable with those published in the literature
Cirugía de revisión de la columna lumbar
Objetivo: Analizar las causas del fracaso de la cirugía en el raquis lumbar y los resultados obtenidos en la cirugía de revisión.
Pacientes: Entre 1995 y 1998 un total de 39 pacientes fueron remitidos a nuestra Unidad de Columna para revisión de la cirugía de su raquis lumbar. Los diagnósticos más frecuentes en la revisión fueron la estenosis de canal y la fibrosis perirradicular. El tratamiento empleado en la mayoría de los casos fue una descompresión radicular, y una osteosístesis y artrodesis vertebral. El seguimiento mínimo fue de 12 meses.
Resultados: Realizamos una valoración económico-funcional con la escala modificada de Prolo, observando un aumento de 0.5 puntos en la vertiente económica, y de 1.5 puntos en la funcional, lo que se interpretó como una capacidad de los pacientes de reincorporarse a su entorno laboral y poder realizar la práctica totalidad de sus actividades cotidianas excepto las deportivas. El 72% de los pacientes estaban satisfechos o muy satisfechos con los resultados.
Conclusiones: El éxito de la cirugía de revisión radica en realizar un correcto diagnóstico del fracaso de la cirugía inicial. Los peores resultados de la cirugía inicial. Los peores resultados de la cirugía de revisión han sido aquellos en los que el motivo de la misma fue una fibrosis perirradicular, mientras que los mejores resultados se dieron en las recidivas heriarias.Objective: To analyse the causes of failure in lumbar surgery and the results with revision surgery.
Patients: Between 1995-1998, 39 patients were seen in our Spine Unit for a revision of their lumbar spine surgery. Most common diagnosis of the revision were spinal stenosis and peridural fibrosis. The principal treatment employed was nerve decompression and instrumentation with vertebral fusion. Minimum follow up: 12 months.
Results: We did an economic-functional evaluation with the modified scale of Prolo, and noticed an increase of 0.5 points at the economic side and of 1.5 points at the functional side; which meant that the patients could return to their usual work and do all the diary activities except sports. 72% of the patients were satisfied or very satisfied with the results.
Conclusions: The success of the revision surgery is based on an accurate analysis of the failure of the first surgery. The worst result of the revision surgery was caused by treatment of peridural fibrosis, while the best results were found in reoccurrence of discal herniation
Fracturas de columna toraco-lumbar: Complicaciones de la vía anterior
Objetivo: Analizar las complicaciones de las fracturas torazo-lumbares tratadas mediante abordaje anterior, derivadas de la misma vía de abordaje, de la técnica de descompresión o de la estabilización propiamente dicha.
Pacientes: Durante un periodo de 10 años (1983-1993), se realizaron 39 abordajes anteriores para el tratamiento de las fracturas del tránsito torazo-lumbar. Los pacientes presentaban clínica neurológica parcial. Todos los casos tuvieron un seguimiento mínimo de 5 años.
Resultados: Siete de los 39 pacientes intervenidos mostraron algún tipo de complicación, siendo 9 las complicaciones totales encontradas, lo que supone una tasa del 23%. A corto plazo encontramos 3 complicaciones derivadas de la vía de abordaje y 1 infección profunda de partes blandas. A largo plazo apareció una infección profunda, 2 roturas de material y 2 pseudoartrosis. Las complicaciones de la vía del abordaje se resolvieron con medidas conservadoras, mientras que los pacientes con infección profunda precisaron reintervención y retirada de la osteosíntesis. Los casos de pseudoartrosis se resolvieron con fijación posterior de apoyo, y las roturas de material no originaron ningún gesto quirúrgico añadido para su resolución.
Conclusiones: El porcentaje de complicaciones que muestra nuestra serie es similar a los publicados en la literatura. La osteosíntesis y artrodesis anterior con empleo de injertos estructurales permite reconstruir la altura perdida, recurriendo a la artrodesis posterior de apoyo sólo en aquellos casos con marcada osteoporosis o defecto técnico constatado en la cirugía.Objective: Analyse the complications of the thoraco-lumbar fractures treated with anterior approach, due to the approach, decompression technique, or the stabilisation by itself.
Patients: Between 1983-1993, we review 39 anterior approaches for treatment of the thoraco-lumbar fractures. Patients had incomplete neurological clinic. All of them were followed for a minim of 5 years.
Results: Seven of the 39 patients had any kind of complication. There were 9 complications in overall (23%). In a short time there were 3 complications due to the approach and 1 deep infection. In a long period we found 1 deep infection, 2 failures of material and 2 pseudoarthrosis. Complications of the approach were resolved without surgery, but patients with deep infection had a new surgery for treatment. The pseudoarthrosis were treated by posterior approach, and failures of material did not need another surgery.
Conclusions: We have a percentage of complications similar to the other authors. The anterior instrumentation and fusion with use of structural graft allows to reconstruct the lost height, using the posterior artrodesis only in cases with severe osteoporosis or defect technical observed during the surgery
La laminoartrectomía en el tratamiento de la estenosis de canal lumbar
Los autores presentan una revisión de 42 pacientes, 33 varones y 9 mujeres,
con estenosis del canal lumbar, tratados quirúrgicamente y con un seguimiento medio de 3
años y 8 meses. El tratamiento quirúrgico empleado ha sido la laminoartrectomía a uno o varios
niveles, combinada o no con artrodesis. Los mejores resultados se obtuvieron en los pacientes
tratados con laminoartrectomía, artrodesis e instrumentación vertebral.The authors present a review of 42 patients, 33 male and 9 female, with lumbar
spinal canal stenosis, treated surgically and with a mean follow-up of 3 years-8 months.
The surgical treatment was wide laminoarthrectomy involving one o more levels, combined
in some cases with spinal fusion. The best results were obtained in patients treated with laminoarthrectomy,
arthrodesis and vertebral instrumentation
Deformidad vertebral y mielomeningocele: actitud terapéutica y resultados
Presentamos nuestra experiencia en el tratamiento quirúrgico de la deformidad
vertebral relacionada con el mielomeningocele entre los años 1972 y 1995. En ese
período un total de 48 pacientes fueron intervenidos. Se han evaluado las condiciones generales
previas, el estado prequirúrgico, las diversas modalidades de instrumentación y
el resultado de las mismas, así como el tipo y número de complicaciones. Durante el período
posquirúrgico inmediato hemos encontrado un porcentaje de infecciones del 68%.
Sin embargo, el resultado final, tanto del equilibrio pélvico como del vertebral conseguido,
no estuvo influenciado significativamente por esta complicación. El tipo de patología,
las complicaciones generales derivadas de la misma, el tiempo quirúrgico empleado y las
pérdidas sanguíneas, son condiciones estrechamente relacionadas con el número de infecciones
encontradas.We present our experience on the surgical treatment of myelomeningocelelinked
vertebral deformity between 1972 and 1995. We evaluated the previous general
condition, the presurgical status, the instrument modalities and their outcome, as well as
the type and number of complications in a series of 48 patients. During the immediate
postoperative period, we found a 68% infection rate. However, the end result for pelvic as
well as vertebral balance was not influenced significantly by this complication. The number
of infections found is closely linked to the specific pathology, the complications, duration
of operation, and blood loss
El corsé de Milwaukee en la escoliosis idiopática: resultados en el plano sagital
Se presentan los resultados obtenidos en el tratamiento de la escoliosis
idiopática con el cors é de Milwauke e en el plano sagital. Han sido estudiado s 51 paciente
s (44 mujere s y 7 varones) con una edad medi a al inicio del tratamient o de 13
años. El tipo de curva según la clasificación de King y Moe corresponde a 18 del tipo I,
16 del tipo II, 12 del III, 1 del IV, 2 del V y dos curvas lumbares. El seguimiento medio
de la serie fue de 5 años. El 88% de los pacientes (45 casos) mostró una disminución del
valor angular medio de la cifosis torácica. El promedio de disminución fue de 10° que
representa el 32% del valor angular inicial. El 86% (44 pacientes) mostró aplanamiento
de la lordosis lumbar , pasando de un valor inicial medi o de 59°, a un valor final del
tratamiento de 49°. Esta disminución representa un 16% del valor inicial. La edad, el tipo
y la elasticidad de la curva, así como el Risser no mostraron relación con la evolución
de las curvas en el plano sagital.The results obtained in the sagittal plane in patients with idiopathic scoliosis
treated by Milwauke e brace are presented. A series of 51 patients (44 females, 7
males) with average of 13 years, wer e included in the study. Using the King and More's
classification, 18 patients had type I curves,16 type II, 12 type III, 1 type IV and 2 patients
showed tipe V curves. Two had lumbar curves. The mean follow-up was 5 years.
In 45 patients (88%) the thoracic Kyphosis showed a decrease of 10° in average angular
value (32% of initial value). 44 patients (86%) showed loss of lumbar lordosis, passing
from an initial average value of 59° to 49° of final average value (16%). Age, type of the
curve, Risser and bending test wer e not related with the evolution of curvatures in the
sagittal plane
All hormone-producing cell types of the pituitary intermediate and anterior lobes derive from prop1-expressing progenitors
Mutations in PROP1, the most common known cause of combined pituitary hormone deficiency in humans, can result in the progressive loss of all hormones of the pituitary anterior lobe. In mice, Prop1 mutations result in the failure to initiate transcription of Pou1f1 (also known as Pit1) and lack somatotropins, lactotropins, and thyrotropins. The basis for this species difference is unknown. We hypothesized that Prop1 is expressed in a progenitor cell that can develop into all anterior lobe cell types, and not just the somatotropes, thyrotropes, and lactotropes, which are collectively known as the PIT1 lineage. To test this idea, we produced a transgenic Prop1-cre mouse line and conducted lineage-tracing experiments of Prop1-expressing cells. The results reveal that all hormone-secreting cell types of both the anterior and intermediate lobes are descended from Prop1-expressing progenitors. The Prop1-cre mice also provide a valuable genetic reagent with a unique spatial and temporal expression for generating tissue-specific gene rearrangements early in pituitary gland development. We also determined that the minimal essential sequences for reliable Prop1 expression lie within 10 kilobases of the mouse gene and demonstrated that human PROP1 can substitute functionally for mouse Prop1. Tratamiento quirúrgico de la escoliosis en el síndrome de Rett: sistema de instrumentación TRANSPINE
El Síndrome de Rett es una afectación neurológica que en los estadios últimos presenta deformidades evolutivas que suelen precisar cirugía. Se han revisado en este estudio 11 pacientes con Síndrome de Rett y deformidad en columna que precisaron cirugía. Se analizaron las magnitudes de las curvas con ángulo de Cobb, alteraciones del plano sagital (cifosis y lordosis), oblicuidad pélvica pre y postoperatoria, niveles de fusión y complicaciones postoperatorias. La corrección media fue del 65%. Se realizó una fusión posterior en todos los pacientes sin incluir sacro pelvis. Los montajes se realizaron principalmente con sistema híbrido de tornillos, ganchos, pinzas y cables laminares TRANSPINE®. Las escoliosis neuromusculares, como el síndrome de Rett, que precisan corrección, pueden controlarse de forma segura y efectiva mediante artrodesis posterior con instrumentación híbrida.Rett Syndrome is a neurological impairment with a typically long C scoliosis curve that often require surgery. In this study it has been reviewed 11 patients with Rett syndrome and spine deformity. We analyzed the magnitudes of the curves with Cobb angle, sagittal plane (kyphosis and lordosis), preoperative and postoperative pelvic obliquity, fusion levels and postoperative complications. The average correction was 65%. Posterior fusion was performed in all patients excluding sacrum pelvis. The assemblies are made mainly with the hybrid system of screws, hooks, clips and cables TRANSPINE®. Neuromuscular scoliosis, such as Rett syndrome, which needs correction, can be controlled safely and effectively by posterior fusion with hybrid constructs
Tratamiento de la escoliosis idiopática mediante artrodesis anterior e instrumentación de Zielke
—Hemos revisado 16 pacientes afectos de escoliosis toracolumbar y lumbar tratados
mediante artrodesis anterior e instrumentación de Zielke. La corrección de la curva en el
plano anteroposterior ha sido del 68%. La escoliosis torácica asociada se ha corregido de forma
espontánea un 30%. La lordosis global final ha sido de 40°, sin embargo la instrumentación
provoca un aumento de la cifosis regional de 10°. Las complicaciones de la serie han sido
3 roturas de barra, una progresión de la curva torácica y un derrame pleural.The authors showed the clinical outcome of 16 patients with thoracolumbar
and lumbar curvatures treated by Zielke instrumentation and anterior fusion. The correction
obtained in the primary curvature was 68%. The average correction in the compensatory thoracic
curvature was 30%. The average lordosis correction at follow was 40°. However the Zielke's
system lead to an increase of 10° of kyphotic at the instrumented levels. Complications
