    A thermally powered electrical generating system for use in a space vehicle is disclosed. The rate of storage in a thermal energy storage medium is controlled by varying the rate of generation and dissipation of electrical energy in a thermally powered electrical generating system which is powered from heat stored in the thermal energy storage medium without exceeding a maximum quantity of heat. A control system (10) varies the rate at which electrical energy is generated by the electrical generating system and the rate at which electrical energy is consumed by a variable parasitic electrical load to cause storage of an amount of thermal energy in the thermal energy storage system at the end of a period of insolation which is sufficient to satisfy the scheduled demand for electrical power to be generated during the next period of eclipse.     Cardiovascular responses during light-intensity aerobic exercise with varying levels of limb occlusion pressures.

The study aimed to assess cardiovascular responses to low-intensity aerobic exercise with varying levels of limb occlusion pressures (LOP) in a healthy population of men and women 30 to 60 years. The study was a single-session repeated measures design. Thirty individuals completed the study. All subjects participated in a single bout of low-intensity cycling (30-39% HRR) with bilateral lower extremity (LE) BFR for four 5-minute stages [0% (No BFR), 40%, 60%, and 80% LOP] with a 2-minute active rest between stages (BFR pressure released). The subjects’ systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), heart rate (HR), oxygen saturation (SpO2), and ratings of perceived exertion (RPE) were measured at rest, peak, immediately post, post-15 minutes, and post-30 minutes. Peak SBP (no BFR 160.7 ±19.1 mmHg; 40% LOP 173.6 ± 18.7 mmHg; 60 % LOP; 182.5 ± 21.1 mmHg; 80% LOP 193.5± 23.3 mmHg ; p\u3c0.001; =.747), DBP (no BFR 74.9 ± 8.5 mmHg; 40% LOP (83.0 ± 9.0 mmHg;60 % LOP 90.4 ± 8.7 mmHg; 80% LOP 97.7 ± 9.5 mmHg ;p\u3c0.001; =.924), MAP (no BFR 103.5 ± 10.1 mmHg; 40% LOP 113.2 ± 10.5 mmHg; 60% LOP 121.1 ± 11.7 mmHg; 80% LOP 129.7 ± 12.9 mmHg; p\u3c0.001; =.960), and RPE (No BFR 10.0 ± 2.0; 40 % LOP 11.5 ± 2.3; 60% LOP 13.2 ± 2.6; 80% LOP 14.5 ± 3.; p\u3c0.001; =.826) were significantly higher with each progressing stage. The results indicate that low-intensity cycling with bilateral LE BFR for each LOP stage resulted in elevated SBP, DBP, MAP, and RPE despite maintaining a fixed HR

    Seven-month developmental outcomes of very low birth weight infants enrolled in a randomized controlled trial of delayed versus immediate cord clamping

    Objective: The results from our previous trial revealed that infants with delayed cord clamping (DCC) had significantly lesser intraventricular hemorrhage (IVH) and late-onset sepsis (LOS) than infants with immediate cord clamping (ICC). A priori, we hypothesized that infants with DCC would have better motor function by 7 months corrected age. Study Design: Infants between 24 and 31 weeks were randomized to ICC or DCC and follow-up evaluation was completed at 7 months corrected age. Result: We found no differences in the Bayley Scales of Infant Development (BSID) scores between the DCC and ICC groups. However, a regression model of effects of DCC on motor scores controlling for gestational age, IVH, bronchopulmonary dysplasia, sepsis and male gender suggested higher motor scores of male infants with DCC. Conclusion: DCC at birth seems to be protective of very low birth weight male infants against motor disability at 7 months corrected age

    Effects of Placental Transfusion on Neonatal and 18 Month Outcomes in Preterm Infants: A Randomized Controlled Trial

    Objective: To assess the effect of delayed cord clamping (DCC) vs immediate cord clamping (ICC) on intraventricular hemorrhage (IVH), late onset sepsis (LOS), and 18-month motor outcomes in preterm infants. Study Design: Women (n = 208) in labor with singleton fetuses (\u3c32 weeks gestation) were randomized to either DCC (30-45 seconds) or ICC (\u3c10 seconds). The primary outcomes were IVH, LOS, and motor outcomes at 18-22 months corrected age. Intention-to-treat was used for primary analyses. Results: Cord clamping time was 32 ± 16 (DCC) vs 6.6 ± 6 (ICC) seconds. Infants in the DCC and ICC groups weighed 1203 ± 352 and 1136 ± 350 g and mean gestational age was 28.3 ± 2 and 28.4 ± 2 weeks, respectively. There were no differences in rates of IVH or LOS between groups. At 18-22 months, DCC was protective against motor scores below 85 on the Bayley Scales of Infant Development, Third Edition (OR 0.32, 95% CI 0.10-0.90, P = .03). There were more women with preeclampsia in the ICC group (37% vs 22%, P = .02) and more women in the DCC group with premature rupture of membranes/preterm labor (54% vs 75%, P = .002). Preeclampsia halved the risk of IVH (OR 0.50, 95% CI 0.2-1.0) and premature rupture of membranes/preterm labor doubled the risk of IVH (OR 2.0, 95% CI 1.2-4.3). Conclusions: Although DCC did not alter the incidence of IVH or LOS in preterm infants, it improved motor function at 18-22 months corrected age

    High-fat diets and seizure control in myoclonic-astatic epilepsy: A single center's experience

    AbstractPurposeTo determine the efficacy of the Modified Atkins Diet (MAD) and Ketogenic Diet (KD) in seizure control within a population of myoclonic-astatic epilepsy (MAE) patients.MethodsThis was a retrospective, single center study evaluating the seizure control by high fat diets. Seizure diaries kept by the parents performed seizure counts. All patients met the clinical criteria for MAE.ResultsNine patients met the clinical criteria. We found that both the MAD and KD were efficacious in complete seizure control and allowed other medications to be stopped in seven patients. Two patients had greater than 90% seizure control without medications, one on the KD and the other on the MAD. Seizure freedom has ranged from 13 to 36 months, and during this time four patients have been fully weaned off of diet management. One patient was found to have a mutation in SLC2A1.ConclusionOur results suggest that strictly defined MAE patients respond to the MAD with prolonged seizure control. Some patients may require the KD for seizure freedom, suggesting a common pathway of increased requirement for fats. Once controlled, those fully responsive to the Diet(s) could be weaned off traditional seizure medications and in many, subsequently off the MAD or KD

    Mapping MOS-HIV to HUI3 and EQ-5D-3L in Patients With HIV

    Objectives: The Medical Outcomes Study HIV Health Survey (MOS-HIV) is frequently used in HIV clinical trials; however, scores generated from the MOS-HIV are not suited for cost-effectiveness analyses as they do not assign utility values to health states. Our objective was to estimate and externally validate several mapping algorithms to predict Health Utilities Index Mark 3 (HUI3) and EQ-5D-3L utility values from the MOS-HIV. Methods: We developed and validated mapping algorithms using data from two HIV clinical trials. Data from the first trial (n = 367) formed the estimation data set for the HUI3 (4,610 observations) and EQ-5D-3L (4,662 observations) mapping algorithms; data from the second trial (n = 168) formed the HUI3 (1,135 observations) and EQ-5D-3L (1,152 observations) external validation data set. We compared ordinary least squares (OLS) models of increasing complexity with the more flexible two-part, beta regression, and finite mixture models. We assessed model performance using mean absolute error (MAE) and mean squared error (MSE). Results: The OLS model that used MOS-HIV dimension scores along with squared terms gave the best HUI3 predictions (mean observed 0.84; mean predicted 0.80; MAE 0.0961); the finite mixture model gave the best EQ-5D-3L predictions (mean observed 0.90; mean predicted 0.88; MAE 0.0567). All models produced higher prediction errors at the lower end of the HUI3 and EQ-5D-3L score ranges

    DESCRIPTION: Update of the US Preventive Services Task Force (USPSTF) recommendation on screening for impaired visual acuity in older adults. METHODS: The USPSTF reviewed the evidence on screening for visual acuity impairment associated with uncorrected refractive error, cataracts, and age-related macular degeneration among adults 65 years or older in the primary care setting; the benefits and harms of screening; the accuracy of screening; and the benefits and harms of treatment of early vision impairment due to uncorrected refractive error, cataracts, and age-related macular degeneration. POPULATION: This recommendation applies to asymptomatic adults 65 years or older who do not present to their primary care clinician with vision problems. RECOMMENDATION: The USPSTF concludes that the current evidence is insufficient to assess the balance of benefits and harms of screening for impaired visual acuity in older adults. (I statement)

    Recurrence of chronic venous ulcers on the basis of clinical, etiologic, anatomic, and pathophysiologic criteria and air plethysmography

    AbstractIntroduction: Leg ulcers associated with chronic venous insufficiency (CVI) frequently recur after healing. The risk of recurrence has not been well defined for patients in different anatomic and hemodynamic groups. We reviewed the risk of ulcer recurrence on the basis of clinical, etiologic, anatomic, and pathophysiologic criteria and hemodynamic characteristics of the affected limb as assessed with air plethysmography (APG). Methods: Ninety-nine limbs with class 6 CVI were assessed clinically and with standing duplex ultrasound scanning and APG for the definition of clinical, etiologic, anatomic, and pathophysiologic criteria. Leg ulcers were treated with high-pressure compression protocols. Surgical correction of venous abnormalities was offered to patients with appropriate conditions. After ulcer healing, the limbs were placed in compressive garments and followed at 6-month intervals for ulcer recurrence. Results: The mean patient age was 54.3 years, and 46% of the patients were female. Corrective venous surgery was performed in 37 limbs. The mean follow-up time for all 99 limbs was 28 months. The ulcer recurrence rate with life table was 37% ± 6% at 3 years and 48% ± 10% at 5 years. The patients who underwent venous surgery had a significantly lower recurrence rate (27% ± 9% at 48 months) than did those patients who had not undergone surgery (67% ± 8% at 48 months; P =.005). The patients with deep venous insufficiency (DVI; n=51) had significantly higher recurrence rates (66% ± 8% at 48 months) than did the patients without DVI (n = 48; 29% ± 9% at 48 months; P =.006). This difference was significant even after accounting for the effects of surgery (P =.03).The hazard ratio of ulcer recurrence increases by 14% for every unit increase in the venous filling index (VFI; P =.001). This remains significant even after accounting for the effects of surgery (P =.001). The combination of DVI and a VFI of more than 4 mL/s yields a risk of ulcer recurrence of 43% ± 9% at 1 year and 60% ± 10% at 2 years. Conclusion: Leg ulcers associated with CVI have a high rate of recurrence. Ulcer recurrence is significantly increased in patients with DVI and in patients who do not have venous abnormalities corrected surgically. The VFI obtained from APG is useful in the prediction of increased risk for recurrence, particularly in association with anatomic data. (J Vasc Surg 2002;35:723-8.
