Focal Retrograde Amnesia: Voxel-Based Morphometry Findings in a Case without MRI Lesions
Focal retrograde amnesia (FRA) is a rare neurocognitive disorder presenting with an isolated loss of retrograde memory. In the absence of detectable brain lesions, a differentiation of FRA from psychogenic causes is difficult. Here we report a case study of persisting FRA after an epileptic seizure. A thorough neuropsychological assessment confirmed severe retrograde memory deficits while anterograde memory abilities were completely normal. Neurological and psychiatric examination were unremarkable and high-resolution MRI showed no neuroradiologically apparent lesion. However, voxel-based morphometry (VBM)-comparing the MRI to an education-, age-and sex-matched control group (n = 20) disclosed distinct gray matter decreases in left temporopolar cortex and a region between right posterior parahippocampal and lingual cortex. Although the results of VBM-based comparisons between a single case and a healthy control group are generally susceptible to differences unrelated to the specific symptoms of the case, we believe that our data suggest a causal role of the cortical areas detected since the retrograde memory deficit is the preeminent neuropsychological difference between patient and controls. This was paralleled by grey matter differences in central nodes of the retrograde memory network. We therefore suggest that these subtle alterations represent structural correlates of the focal retrograde amnesia in our patient. Beyond the implications for the diagnosis and etiology of FRA, our results advocate the use of VBM in conditions that do not show abnormalities in clinical radiological assessment, but show distinct neuropsychological deficits
Vergleichbarkeit von Qualitätsuntersuchungen mit den Bildschaffenden Methoden (Kupferchlorid-Kristallisation)
In der vorliegenden Arbeit wird der wissenschaftliche Ansatz, die Methode zu validieren beschrieben und der Einfluss der Kammerbedingungen auf die Kristallisation systematisch untersucht und statistisch ausgewertet. Die Klimakonditionen haben einen signifikanten Einfluss auf die Streuung der Werte, sowohl innerhalb eines Kammerdurchgangs, als auch im Vergleich zwischen zwei Kammerdurchgängen. Bei der Einstellung der Startfeuchte auf dem Ring variieren die Feuchtenwerte die dafür erreicht werden müssen von Kammer zu Kammer; weitere Untersuchungen werden zeigen, ob dies auf die unterschiedlichen Abstände beim Aufbau der Kammern (bis zu 2 Jahren Differenz) an den einzelnen Orten zurückzuführen ist (Adaption des Werkstoffes). Eine Vergleichbarkeit von Qualitätsuntersuchungen mit den Bildschaffenden Methoden ist aus unserer Sicht möglich, wenn die experimentellen Randbedingungen bekannt und kontrolliert oder gemessen werden können. Die Festlegung der Prozessschritte in normativen Dokumenten und die Verfolgung der gesamten Probensequenz ist möglich und eine weitere Voraussetzung, um Qualitätsuntersuchungen mit den Bildschaffenden Methoden vergleichen zu können. Die hier vorgelegten Untersuchungen zur Methodenrobustheit im Prozessschritt Kristallisation erlauben die Ermittlung der Wiederhol- und Laborpräzision und sind damit Voraussetzung, die Vergleichspräzision mit der Durchführung von Ringversuchen ermitteln zu können. Das Problem einer fehlenden Bezugsnormalen (und damit der Überprüfung der Richtigkeit) bleibt davon unberührt.

Leitlinienbasierte Standards zur Struktur- und Prozessqualität neuropsychologischer Diagnostik und Therapie
Die vorliegende Arbeit untersucht die aktuelle wissenschaftliche Evidenz zur Diagnostik und Therapie neuropsychologischer
Störungen nach Hirnschädigung, wie sie bis 2020 in Leitlinien publiziert wurde. Deren Umsetzung ist nur möglich, wenn die institutionellen
Rahmenbedingungen dies erlauben. Unter Einbezug der klinischen Erfahrung wurden daher auf Basis der Leitlinien Standards fĂĽr eine
wissenschaftlich fundierte neuropsychologische Diagnostik und Therapie erarbeitet. Es entstanden Best-Practice-Empfehlungen zu Strukturund
Prozessqualität, insbesondere zu Intensität und Häufigkeit der Interventionen. Diese werden für die wichtigsten neuropsychologischen
Funktionsbereiche vorgestellt. Sowohl die Deutsche Gesellschaft fĂĽr Neuropsychologie e. V. (GNP) als auch die Deutsche Gesellschaft fĂĽr Neurologie
e. V. (DGN) unterstützen diese Empfehlungen. Sie richten sich an Neuropsycholog_innen sowie an Einrichtungsleitende und Sozialversicherungsträger
und definieren die Rahmenbedingungen fĂĽr eine auf den individuellen Fall angepasste leitliniengerechte neuropsychologische
Behandlung.Recent years have seen the establishment of evidence-based guidelines for neuropsychological diagnostics and therapy; however,
implementing these guidelines depends on structures and processes necessary to enable essential aspects like therapy frequency and intensity.
The present work examines the current scientific evidence for the neuropsychological treatment of traumatic and nontraumatic brain injury,
as published in guidelines up to 2020. Standards for evidence-based neuropsychological diagnostics and therapy were developed on this
basis, including clinical experience and additional literature research. Best-practice recommendations on both general and specific structural
and process quality emerged, especially on the intensity and frequency of interventions. These are presented for the most important neuropsychological
functional areas. The German Neuropsychological Society (GNP) and the German Society for Neurology (DGN) support these recommendations.
They are aimed at neuropsychologists as well as facility managers and social insurance providers, and they define the framework
for guideline-based neuropsychological treatment adapted to individual cases
Anorexia nervosa during COVID-19: loss of personal control and alexithymia as important contributors to symptomatology in adolescent girls
Background
In the course of the COVID-19 pandemic, a steady increase in adolescent anorexia nervosa admissions has been observed. Contributing factors may have been uncontrollable changes in school attendance due to lockdowns and social restrictions. However, patients’ reports on the impact of these factors have not been assessed in detail as of yet. Furthermore, alexithymia, the difficulty to identify and describe one’s own emotions, has increased during the pandemic and is known to be heightened in eating disorders. Thus, it may have contributed to symptom severity in anorexia nervosa during the pandemic.
Methods
The present study examined pandemic-related changes in social media use, body satisfaction, and perceived loss of control and their impact on depressive, anxious, and eating disorder symptomatology in a sample of adolescent girls with anorexia nervosa (n = 29) and healthy controls (n = 23). Additionally, the influence of current alexithymia as a cross-diagnostic risk factor was assessed. Adolescents answered questionnaires once shortly after admission to inpatient, outpatient, or daycare treatment.
Results
An increase in perceived loss of control during the pandemic and heightened alexithymia explained a significant portion of variance in present depressive symptomatology, which in turn contributed to eating disorder symptomatology.
Conclusions
These relationships emphasize alexithymia and perceived loss of control as valuable constructs for early screenings and interventions
