Self-esteem, stress and self-rated health in family planning clinic patients
BACKGROUND: The independent effects of stress on the health of primary care patients might be different for different types of clinic populations. This study examines these relationships in a low-income female population of patients attending a family planning clinic. METHODS: This study investigated the relevance of different sources of personal stress and social support to self-rated health, adjusting for mental health, health behavior and demographic characteristics of subjects. Five hundred women who attended family planning clinics were surveyed and 345 completed the form for a response rate of 72 percent. RESULTS: Multiple logistic regression analysis revealed that liking oneself was related to good self-rated health (Odds ratio = 7.11), but stress or support from children, parents, friends, churches or spouses were not significant. White non-Hispanic and non-white non-Hispanic respondents had lower odds of reporting good self-rated health than Hispanic respondents (odds ratios were 2.87 and 2.81, respectively). Exercising five or more days per week also was related to good self-rated health. Smoking 20 or more cigarettes per day, and obese III were negatively related to good self-rated health (odds ratios were .19 and .22, respectively with corresponding p-values equal to .0043 and .0332). CONCLUSIONS: Among younger low-income women, addressing low self-esteem might improve health status
Uso combinado de modelos de estresse no trabalho e a saúde auto-referida na enfermagem
OBJETIVO: Identificar combinações de dois modelos do estresse psicossocial do trabalho em equipes de enfermagem e sua associação com a saúde auto-referida. MÉTODOS: Estudo transversal com trabalhadoras de três hospitais públicos do Município do Rio de Janeiro, RJ (N=1307). Foi aplicado questionário multidimensional que incluiu duas escalas de estresse no trabalho (modelo demanda-controle e desequilíbrio esforço-recompensa) em 2006. Foram considerados o modelo demanda e controle parcial e completo (inclui apoio social no trabalho), assim como o esforço e recompensa parcial e completo (inclui excesso de comprometimento com o trabalho). Modelos de regressão múltipla foram utilizados para estimar razões de chances ajustadas e seus respectivos intervalos com 95% de confiança. RESULTADOS: As dimensões de ambos os modelos estiveram independentemente associadas à situação de saúde, com odds ratios entre 1,70 e 3,37. O modelo parcial demanda-controle mostrou-se menos associado à saúde (OR = 1,79; IC95% 1,26;2,53) quando comparado ao de desequilíbrio esforço-recompensa (OR=2,27; IC95% 1,57;3,30). A incorporação do apoio social e do excesso de comprometimento com o trabalho aumentou a força de associação dos modelos demanda-controle e desequilíbrio esforço-recompensa, respectivamente. Foi observado aumento na força de associação quando os dois modelos parciais foram combinados. CONCLUSÕES: Os resultados indicam melhor desempenho do modelo desequilíbrio esforço-recompensa para este grupo específico e para o desfecho avaliado e vantagem do uso de modelos completos ou do uso combinado em modelos parciais.OBJETIVO: Identificar combinaciones de dos modelos de estrés psicossocial del trabajo en equipos de enfermería y su asociación con la salud auto referida. Se consideraron el modelo demando y control parcial y completo (incluye apoyo social en el trabajo), así como el esfuerzo y recompensa parcial y completo (incluye exceso de compromiso con el trabajo). Se utilizaron modelos estadísticos múltiples para estimar razones de probabilidades ajustadas y sus respectivos intervalos con 95% de confianza. RESULTADOS: Las dimensiones de ambos modelos estuvieron independientemente asociadas con la salud autoreferida, con odds ratios entre 1,70 y 3,37. El modelo parcial demanda-control se mostró menos asociado a la salud (OR=1,79; IC 95% 1,26;2,53) al compararlo con el desequilibrio esfuerzo-recompensa (OR=2,27; IC 95% 1,57;3,30). La incorporación del apoyo social y del exceso de compromiso con el trabajo aumentó la fuerza de asociación de los modelos demanda-control y desequilibrio esfuerzo-recompensa, respectivamente. Se observó aumento en la fuerza de asociación al combinarse los dos modelos parciales. CONCLUSIONES: Los resultados indican mejor desempeño del modelo desequilibrio esfuerzo-recompensa para este grupo específico y para el resultado evaluado y ventaja en el uso de modelos completos o del uso combinado en modelos parciales.OBJECTIVE: To identify combinations of two models of psychosocial stress at work among nursing teams and their associations with self-rated health. METHODS: This was a cross-sectional study among workers at three public hospitals in the municipality of Rio de Janeiro, Southeastern Brazil (N = 1307). In 2006, a multidimensional questionnaire including two scales for measuring stress at work (demand-control and effort-reward imbalance models) was administered. Partial and complete (including social support at work) demand-control models were considered, along with partial and complete (including excessive commitment to work) effort-reward models. Multiple logistic regression models were used to estimate adjusted odds ratios and their respective 95% confidence intervals. RESULTS: The dimensions of both models were independently associated with self-rated health, with odds ratios between 1.70 and 3.37. The partial demand-control model was less associated with health (OR = 1.79; 95%CI 1.26;2.53) than was the partial effort-reward imbalance model (OR = 2.27; 95%CI 1.57;3.30). Incorporation of social support and excessive commitment to work increased the strength of the demand-control and effort-reward imbalance models, respectively. Increased strength of association was observed when the two partial models were combined. CONCLUSIONS: The results indicate that the effort-reward imbalance model performed better for this specific group and for the outcome evaluated, and that there was an advantage in using complete models or combinations of partial models

Effect of Body Mass Index on work related musculoskeletal discomfort and occupational stress of computer workers in a developed ergonomic setup
<p>Abstract</p> <p>Background</p> <p>Work urgency, accuracy and demands compel the computer professionals to spend longer hours before computers without giving importance to their health, especially body weight. Increase of body weight leads to improper Body Mass Index (BMI) may aggravate work related musculoskeletal discomfort and occupational-psychosocial stress. The objective of the study was to find out the effect of BMI on work related musculoskeletal discomforts and occupational stress of computer workers in a developed ergonomic setup.</p> <p>Methods</p> <p>A descriptive inferential study has been taken to analyze the effect of BMI on work related musculoskeletal discomfort and occupational-psychosocial stress. A total of 100 computer workers, aged 25-35 years randomly selected on convenience from software and BPO companies in Bangalore city, India for the participation in this study. BMI was calculated by taking the ratio of the subject's height (in meter) and weight (in kilogram). Work related musculoskeletal discomfort and occupational stress of the subjects was assessed by Cornell University's musculoskeletal discomfort questionnaire (CMDQ) and occupational stress index (OSI) respectively as well as a relationship was checked with their BMI.</p> <p>Results</p> <p>A significant association (p < 0.001) was seen among high BMI subjects with their increase scores of musculoskeletal discomfort and occupational stress.</p> <p>Conclusion</p> <p>From this study, it has been concluded that, there is a significant effect of BMI in increasing of work related musculoskeletal discomfort and occupational-psychosocial stress among computer workers in a developed ergonomic setup.</p
The gap in injury mortality rates between urban and rural residents of Hubei province, China
<p>Abstract</p> <p>Background</p> <p>Injury is a growing public health concern in China. Injury death rates are often higher in rural areas than in urban areas in general. The objective of this study is to compare the injury mortality rates in urban and rural residents in Hubei Province in central China by age, sex and mechanism of injury.</p> <p>Methods</p> <p>Using data from the Disease Surveillance Points (DSP) system maintained by the Hubei Province Centers for Disease Control and Prevention (CDC) from 2006 to 2008, injury deaths were classified according to the International Classification of Disease-10<sup>th </sup>Revision (ICD-10). Crude and age-adjusted annual mortality rates were calculated for rural and urban residents of Hubei Province.</p> <p>Results</p> <p>The crude and age-adjusted injury death rates were significantly higher for rural residents than for urban residents (crude rate ratio 1.9, 95% confidence interval 1.8-2.0; adjusted rate ratio 2.4, 95% confidence interval 2.3-2.4). The age-adjusted injury death rate for males was 81.6/100,000 in rural areas compared with 37.0/100 000 in urban areas; for females, the respective rates were 57.9/100,000 and 22.4/100 000. Death rates for suicide (32.4 per 100 000 vs 3.9 per 100 000), traffic-related injuries (15.8 per 100 000 vs 9.5 per 100 000), drowning (6.9 per 100 000 vs 2.3 per 100 000) and crushing injuries (2.0 per 100 000 vs 0.7 per 100 000) were significantly higher in rural areas. Overall injury death rates were much higher in persons over 65 years, with significantly higher rates in rural residents compared with urban residents for suicide (279.8 per 100 000 vs 10.7 per 100 000), traffic-related injuries, and drownings in this age group. Death rates for falls, poisoning, and suffocation were similar in the two geographic groups.</p> <p>Conclusions</p> <p>Rates of suicide, traffic-related injury deaths and drownings are demonstrably higher in rural compared with urban locations and should be targeted for injury prevention activity. There is a need for injury prevention policies targeted at elderly residents, especially with regard to suicide prevention in rural areas in Central China.</p
Night shift work and modifiable lifestyle factors
Objectives: Night shift work has been linked to some chronic diseases. Modification of lifestyle by night work may partially contribute to the development of these diseases, nevertheless, so far epidemiological evidence is limited. The aim of the study was to explore association between night shift work and lifestyle factors using data from a cross-sectional study among blue-collar workers employed in industrial plants in Łódź, Poland. Material and Methods: The anonymous questionnaire was self-administered among 605 employees (236 women and 369 men, aged 35 or more) - 434 individuals currently working night shifts. Distribution of the selected lifestyle related factors such as smoking, alcohol drinking, physical activity, body mass index (BMI), number of main meals and the hour of the last meal was compared between current, former, and never night shift workers. Adjusted ORs or predicted means were calculated, as a measure of the associations between night shift work and lifestyle factors, with age, marital status and education included in the models as covariates. Results: Recreational inactivity (defined here as less than one hour per week of recreational physical activity) was associated with current night shift work when compared to never night shift workers (OR = 2.43, 95% CI: 1.13-5.22) among men. Alcohol abstinence and later time of the last meal was associated with night shift work among women. Statistically significant positive relationship between night shift work duration and BMI was observed among men (p = 0.029). Conclusions: This study confirms previous studies reporting lower exercising among night shift workers and tendency to increase body weight. This finding provides important public health implication for the prevention of chronic diseases among night shift workers. Initiatives promoting physical activity addressed in particular to the night shift workers are recommended
