Tumors of the oral cavity : CO2 laser management
Cancer of the oral cavity combined with oropharyngeal cancer is the sixth leading cause of death for cancer worldwide. Surgery remains the standard treatment for this disease in early clinical and locally advanced stages. Numerous studies have shown that laser management is useful for premalignant lesions in the oral cavity; however, there is no conclusive evidence that its use is appropriate in cancer of the oral cavity and that results are comparable with traditional surgery. The objective of this study is to determine cancer control after wide local resection with CO2 laser for oral malignant neoplasms. Retrospective study in patients with tumors of the oral cavity who were considered for surgical resection with CO2 laser from January 2006-December 2015. Demographic data, treatment modalities, histopathological diagnosis and clinical stage variables were obtained. All resections were done with the use of the microspot. Patients with cancer of the tongue were not included because a specific protocol for these patients does exist in our institution. There were twenty patients, 10 male and 10 female with a average age of 58 years (range: 20-92 years). Mean age was 53.5 years for females and 63 years for males. Twelve (60%) patients are alive and disease free and four (20%) were lost free of disease. CO2 laser is an acceptable surgical method for the management of small lesions in the oral cavity. We cannot rule out that small lesions of the oral cavity with positive neck could be managed in this manner, adding treatment to the neck, producing an adequate local regional control. However, this hypothesis requires additional studies
Nuevos datos cronoestratigráficos de la Fm. Marismas (Bajo Guadalquivir)
Los gasterópodos terrestres del Cuaternario Superior de Gran Canaria (Islas Canarias)- Land gastropods of the Upper Quaternary in Gran Canaria (Canary Islands)
- A stratigraphic, chronological and paleontological study of eight sedimentary deposits (seven palaeodunes and a colluvial slide) in the Upper Pleistocene of Gran Canaria (Canary Islands) has been undertaken. The palaeodunes are characterized by dune-palaeosoil alternation, from an only cycle in Playa de Jinámar to eleven in Lazareto (Gando). The chronology of these deposits, obtained by amino acid racemisation and 14C datations in Theba genus samples, displays an age between 44,4 Ka and 22,2 Ka. The aminostratigraphy study appoints to these sediments belong to four amino zones (AM2 to AM5) reported by Ortiz et al. (2006). The land snail fossil assemblages are represented by 20 species belonging to eight families, with an endemic degree around 95%. The detected fossil fauna changes with regard to present fauna reveal an Upper Pleistocene species extinction at about 25% and geographic extension decreasing of some species as Theba arinagae and T. aff. grasset
Large bowel obstruction due to sesame seed bezoar: a case report
peer-reviewedWe report a case of a 79 year old man with a known benign anastomotic stricture presenting with
large bowel obstruction. At laparotomy the obstruction was found to be caused by a large sesame
seed bezoar. Seed bezoars are well known to cause impaction in the rectum but have never been
previously reported to cause large bowel obstruction. We recommend that patients with known
large bowel strictures should be advised not to eat seeds as this could ultimately lead to
obstruction, ischaemia or perforationPUBLISHEDpeer-reviewe

Ancient DNA reveals differences in behaviour and sociality between brown bears and extinct cave bears
Ancient DNA studies have revolutionized the study of extinct species and populations, providing insights on phylogeny, phylogeography, admixture and demographic history. However, inferences on behaviour and sociality have been far less frequent. Here, we investigate the complete mitochondrial genomes of extinct Late Pleistocene cave bears and middle Holocene brown bears that each inhabited multiple geographically proximate caves in northern Spain. In cave bears, we find that, although most caves were occupied simultaneously, each cave almost exclusively contains a unique lineage of closely related haplotypes. This remarkable pattern suggests extreme fidelity to their birth site in cave bears, best described as homing behaviour, and that cave bears formed stable maternal social groups at least for hibernation. In contrast, brown bears do not show any strong association of mitochondrial lineage and cave, suggesting that these two closely related species differed in aspects of their behaviour and sociality. This difference is likely to have contributed to cave bear extinction, which occurred at a time in which competition for caves between bears and humans was likely intense and the ability to rapidly colonize new hibernation sites would have been crucial for the survival of a species so dependent on caves for hibernation as cave bears. Our study demonstrates the potential of ancient DNA to uncover patterns of behaviour and sociality in ancient species and populations, even those that went extinct many tens of thousands of years ago
Metallothionein in human oesophagus, Barrett's epithelium and adenocarcinoma
The potential of the metal-binding protein, metallothionein, in assessing the progression of normal oesophagus through Barrett's to adenocarcinoma was investigated. Metallothionein was quantitatively determined in resected tissues from patients undergoing oesophagectomy for high grade dysplasia/adenocarcinoma and in biopsies from patients with Barrett's syndrome. In 10 cancer patients, metallothionein concentrations in adenocarcinoma were not significantly different from normal oesophagus, although six had elevated metallothionein concentrations in the metaplastic tissue bordering the adenocarcinoma. In 17 out of 20 non-cancer patients with Barrett's epithelium, metallothionein was significantly increased by 108% (P<0.004). There was no association between the metallothionein levels in Barrett's epithelium and the presence of inflammatory cells, metaplasia or dysplasia. Metallothionein is a marker of progression from normal to Barrett's epithelium but is not increased in oesophageal adenocarcinoma
