    Clinical and diagnostic importance of proteinuria: A review

    A continual function of the kidney is essential to good health playing active roles in urine formation. Under normal physiological state urine is expected to be protein free. The production of protein freeurine is exclusively carried out by the kidney nephrons. Nephrons are structured to perform an important role of filtration and reabsorption. Therefore, a defective or diseased kidney is associatedwith defective reabsorption mechanisms and an indication of injured nephrons. The appearance of proteins in urine (proteinuria) is a strong indicator of kidney disease. The assessment of proteinuria isclinically and diagnostically an important index in renal function generally and particularly that of nephrons. However, pathological proteinuria may be due to various factors or diseases. It is has beenshown that diabetis mellitus, cardiovascular disease and hypertension could provoke secondary kidney problems. Proteinuria is also significant in some non-pathological cases such as pregnancyand static proteinuria found among military men. Various methods are used for the screening and detection of pathological and non-pathological proteinuria; the methods have their individual merit and demerits. Researchers had made significant contributions in improving the traditional techniques used in detecting and estimating urine protein. Current research in proteomics and genomics science with the development of new techniques in chromatography. (2D-gel chromatography) had produced interesting and promising results in discovery of other classes of urine proteins that could be used asbiomarkers of renal disease. This review examines the clinical and diagnostic importance of proteinuria and available techniques. It is hoped that this paper would throw more light into this important but taken-for-granted diagnostic tool in laboratory medicine

    Economic Analysis of Locust Beans Processing and Marketing in Ilorin, Kwara State, Nigeria

    This study was designed to estimate the economic analysis of locust bean processing and marketing in Ilorin, Kwara State, Nigeria. Primary data was used and purposive sampling technique was adopted to select the respondents used for the study. A total number of 60 respondents were interviewed. The data collected were analyzed using inferential statistical tool such as regression analysis. Budgetary analysis technique was also used to analyze the profitability of locust bean processing and marketing in the study area. Majority of the processors and marketers are making profits; 68.3% operate above breakeven point while 26.7% operate at breakeven point and the rest 5% was below the breakeven point, this indicates that they neither profit nor lost. The regression analysis result shows that quantity processed, family size and years of experience in processing are significant at 1%, 5% and 10% respectively while education level and stall rent is negative and significant at 1% and 5% respectively. F- Test also explained that independent variables are jointly significant at 1% probability level with an adjusted R2 of 78.9%. The overall rate of return on investment indicates that average rate of return is 0.5 (50%), which is positive. It is therefore concluded that profit made by the processors and marketers can be improved on by increasing the quantity of locust bean being processed through adoption of newly discovered method of processing and improved method of preservation, packaging and marketing of the product to international standard by reducing the odour of the product without the loss of essential nutrients and palability in order to generate foreign exchange.     Risk factors associated with mental illness in Oyo State, Nigeria: A Community based study

    BACKGROUND: The main objective of this study was to determine the prevalence and factors associated with mental illness in Oyo State at community level using the general health questionnaire as a screening tool. METHOD: This cross-sectional, community- based survey was carried out among adults in three randomly selected LGAs using multi-stage sampling technique. RESULTS: A total of 1105 respondents were assessed in all. The overall prevalence of psychiatric morbidity in Oyo state Nigeria was found to be 21.9%, (18.4% in the urban areas and 28.4% in the rural areas, p = 0.005). Young age ≤ 19 yrs (X(2 )= 20.41, p = 0.00013), Unemployment (X(2 )= 11.86 p = 0.0005), living condition below average (X(2 )= 12.21, p = 0.00047), physical health (X(2 )= 6.07, p = 0.014), and large family size (X(2 )= 14.09 p = 0.00017) were associated with increase risk for psychiatric morbidity. Following logistic regression analysis, Unemployment (C.I = 1.18–3.70, OR -2.1) and living conditions perceived to be above average (C.I = 1.99–5.50, OR-3.3) were significant predictors of mental illness while family size less than 6 (C.I = 0.86–0.97, OR-0.91) was protective. CONCLUSION: The teenagers and the rural populations are in greater need of mental health promotional services. Family planning should be made freely available in order to reduce the family size and hence incidence of mental illness in the African population

    Reactive Oxygen Species (ROS) could be a causative factor for perfumeinduced testicular toxicity in male rats

    Fragrance materials have been implicated in male infertility. These ingredients are unsteady and tend to oxidize easily. The dearth of information regarding the mechanism of action in which it induces testicular toxicity prompted us to evaluate its possible mechanism pathway in animal model. Sixty adult male wistar rats were randomly divided into six groups of ten rats each. Group A and B rats (Controls) rats were exposed (6hrs day-1) to 5 ml kg-1 body weight of normal saline for 56 days and 112 days via whole body inhalation respectively, Group C and D rats were exposed (6hrs day-1) to 5 ml kg-1 body weight of one of the perfume designated as F1 for a period of 56 days and 112 days via whole body inhalation respectively while Group E and Group F rats were exposed (6hrs day-1) to 5 ml kg-1 body weight of one of the perfume designated as F2 for a period of 56 days and 112 days via whole body inhalation respectively. Result obtained showed a significant decrease in the activity level of SOD, CAT, GPx and GSH and a significant elevation in the level of MDA when compared to values from the control groups of rat. The decrease in level of these testicular enzymatic and testicular non-enzymatic antioxidants (SOD, CAT, GPx and GSH and MDA respectively) in group exposed for 112 days (D and F) were more significant (P<0.005) when compared to groups of rats exposed for 56 days (C and E). However, the GSH content in group E rats was not significantly (P>0.05) different from that of the control groups of rats. From our study, we can only conclude that oxidative stress in testicular tissue might be responsible, at least in part, for perfume induced toxicity in animal model

    Case report: Actinomycosis in a West African dwarf goat in Nigeria

    Actinomycosis, also called Lumpy jaw is a chronic, progressive, indurated, granulomatous, suppurative abscess that most frequently involves the mandible, the maxillae or other bony tissues in the head. It is a sporadic but common disease in cattle, occasional in pigs and horses and rarely in goats (Radostits et al., 2007). Members of the genus Actinomyces are Gram positive, non-acid fast, non-spore forming rods (Songer and Post, 2005) that form a mycelium of branching filaments that fragment into irregular-sized rods (Blood et al., 2007). The species that commonly cause disease in domestic animals include A. bovis, A. hordeovulneris, A. hyovaginalis, A. israelii, A. naeslundii, A. suis, A. viscosus and Arcanobacterium pyogenes (Songer and Post, 2005). Actinomyces bovis is a common inhabitant of the bovine mouth and infection is presumed to occur through wounds to the buccal mucosa caused by sharp pieces of feed or foreign material. Infection may also occur through dental alveoli, and may account for the more common occurrence of the disease in young cattle when the teeth are erupting (Radostits et al., 2007).Actinomyces viscosus causes periodontal disease and subgingival plaques in hamsters fed a high carbohydrate diet, and also abscessation in dogs (Timoney et al., 1988) in which it is an opportunistic infection (Blood et al., 2007). The present report describes a case of actinomycosis due to A. viscosus involving a doe in a herd of 42 West African Dwarf (WAD) goats kept intensively at the Teaching and Research Farm, University of Agriculture, Abeokuta, Nigeria

    Clinical effects of Garcinia kola in knee osteoarthritis

    <p>Abstract</p> <p>Objectives</p> <p>Over the past years, there has been a growing number of knee osteoarthritis (KOA) patients who are not willing to comply with long-term non-steroidal anti-inflammatory drugs (NSAID) treatment and wish to use herbal anti- rheumatic medicine. This study assessed the clinical effects of <it>Garcinia kola </it>(GK) in KOA patients.</p> <p>Patients and methods</p> <p>Prospective randomized, placebo controlled, double blind, clinical trial approved by the institutional medical ethics review board and written informed consent obtained from each patient. All KOA patients presenting at the Obafemi Awolowo University Teaching Hospital complex were recruited into the study. The patients were grouped into four (A = Placebo, B = Naproxen, C = <it>Garcinia kola</it>, D = Celebrex). The drugs and placebo were given twice a day per oral route. Each dose consisted of 200 mg of <it>G. kola</it>, Naproxen (500 mg), Celebrex (200 mg) and Ascorbic acid (100 mg). The primary outcome measure over six weeks study period was the change in mean WOMAC pain visual analogue scales (VAS). Secondary outcome measures included the mean change in joint stiffness and physical function (mobility/walking).</p> <p>Results</p> <p>143 patients were recruited, 84 (58.7%, males – 24, females – 60) satisfied the selection criteria and completed the study. The effect of knee osteoarthritis bilateralism among the subjects was not significant on their outcome (p > 0.05). The change in the mean WOMAC pain VAS after six weeks of <it>G. kola </it>was significantly reduced compared to the placebo (p < 0.001). Multiple comparisons of the mean VAS pain change of <it>G. kola </it>group was not lowered significantly against the naproxen and celebrex groups (p > 0.05). The onset of <it>G. kola </it>symptomatic pain relief was faster than the placebo (p < 0.001). However, it was slower than the active comparators (p > 0.05). The duration of therapeutic effect of <it>Garcinia kola </it>was longer than the placebo (p > 0.001). <it>G. kola </it>period of effect was less than naproxen and celebrex (p < 0.001). <it>G. kola </it>subjects had improved mean change mobility/walking after six weeks better than the control group(p < 0.001). The mean change in mobility of the <it>G. kola </it>group when compared to the active comparators was not significantly better (p < 0.05). The mean change of knee joint stiffness (p < 0.001) and the change of mean WOMAC score (p < 0.001) were improved on <it>Garcinia kola </it>as compared to the placebo. The mid term outcome of eleven <it>Garcinia kola </it>subjects after cessation of use had a mean pain relief period of 17.27 +/- 5.15 days (range: 9–26 days). There was no significant cardiovascular, renal or drug induced adverse reaction to <it>Garcinia kola</it>.</p> <p>Conclusion</p> <p><it>Garcinia kola </it>appeared to have clinically significant analgesic/anti-inflammatory effects in knee osteoarthritis patients. <it>Garcinia kola </it>is a potential osteoarthritis disease activity modifier with good mid term outcome. Further studies are required for standardization of dosages and to determine long-term effects.</p

    Trigger finger: etiology, evaluation, and treatment

    Trigger finger is a common finger aliment, thought to be caused by inflammation and subsequent narrowing of the A1 pulley, which causes pain, clicking, catching, and loss of motion of the affected finger. Although it can occur in anyone, it is seen more frequently in the diabetic population and in women, typically in the fifth to sixth decade of life. The diagnosis is usually fairly straightforward, as most patients complain of clicking or locking of the finger, but other pathological processes such as fracture, tumor, or other traumatic soft tissue injuries must be excluded. Treatment modalities, including splinting, corticosteroid injection, or surgical release, are very effective and are tailored to the severity and duration of symptoms

    Bone turnover markers for early detection of fracture healing disturbances: A review of the scientific literature

    Imaging techniques are the standard method for assessment of fracture healing processes. However, these methods are perhaps not entirely reliable for early detection of complications, the most frequent of these being delayed union and non-union. A prompt diagnosis of such disorders could prevent prolonged patient distress and disability. Efforts should be directed towards the development of new technologies for improving accuracy in diagnosing complications following bone fractures. The variation in the levels of bone turnover markers (BTMs) have been assessed with regard to there ability to predict impaired fracture healing at an early stage, nevertheless the conclusions of some studies are not consensual. In this article the authors have revised the potential of BTMs as early predictors of prognosis in adult patients presenting traumatic bone fractures but who did not suffer from osteopenia or postmenopausal osteoporosis. The available information from the different studies performed in this field was systematized in order to highlight the most promising BTMs for the assessment of fracture healing outcome.As técnicas imagiológicas são o método convencional para a avaliação dos processos de cicatrização das fraturas. No entanto, estes métodos não são talvez totalmente confiáveis para a deteção precoce de complicações, as mais frequentes destas sendo o atraso da união e a não-união. Um diagnóstico eficaz destas desordens poderia prevenir a dor e a incapacidade prolongada do paciente. Esforços devem ser dirigidos no sentido do desenvolvimento de novas tecnologias para melhorar a exatidão no diagnóstico de complicações após fraturas ósseas. A variação nos níveis dos marcadores do turnover ósseo (BTMs) têm sido avaliados com vista à sua capacidade para prever o comprometimento da cicatrização das fraturas numa fase inicial, no entanto, as conclusões de alguns estudos não são consensuais. Neste artigo os autores fizeram uma revisão do potencial dos BTMs como fatores de previsibilidade precoce do prognóstico em doentes adultos que apresentavam fraturas ósseas traumáticas mas que não sofriam de osteopenia ou osteoporose pós-menopausa. A informação disponível nos diferentes estudos realizados neste campo foi sistematizada com vista a evidenciar-se os BTMs mais promissores para a avaliação da evolução da cicatrização das fraturas.SFRH/BD/45018/200

    Self-medication with antibiotics for the treatment of menstrual symptoms in southwest Nigeria: a cross-sectional study

    Background: Self-medication with antibiotics is an important factor contributing to the development of bacterial antibiotic resistance. The purpose of this study was to evaluate the prevalence of self-medication with antibiotics for the treatment of menstrual symptoms among university women in Southwest Nigeria. Methods: A cross-sectional survey was administered to female undergraduate and graduate students (n = 706) at four universities in Southwest Nigeria in 2008. The universities were selected by convenience and the study samples within each university were randomly selected cluster samples. The survey was self-administered and included questions pertaining to menstrual symptoms, analgesic and antibiotic use patterns, and demographics. Data were analyzed using descriptive statistics and logistic regression. Results: The response rate was 95.4%. Eighty-six percent (95% CI: 83-88%) of participants experienced menstrual symptoms, and 39% (95% CI: 36-43%) reported using analgesics to treat them. Overall, 24% (95% CI: 21-27%) of participants reported self-medicated use of antibiotics to treat the following menstrual symptoms: cramps, bloating, heavy bleeding, headaches, pimples/acne, moodiness, tender breasts, backache, joint and muscle pain. Factors associated with this usage were: lower levels of education (Odds Ratio (OR): 2.8, 95% CI: 1.1-7.1, p-value: 0.03); nonscience major (OR: 1.58, 95% CI: 1.03-2.50, p-value: 0.04); usage of analgesics (OR: 3.17, 95% CI: 2.07-4.86, p-value: <0.001); and mild to extreme heavy bleeding (OR: 1.64, 95% CI: 1.01-2.67, p-value: 0.05) and pimples/acne (OR: 1.57, 95% CI: 0.98-2.54, p-value: 0.06). Ampicillin, tetracycline, ciprofloxacin and metronidazole were used to treat the most symptoms. Doctors or nurses (6%, 95% CI: 4-7%), friends (6%, 95% CI: 4-7%) and family members (7%, 95% CI: 5-8%) were most likely to recommend the use of antibiotics for menstrual symptoms, while these drugs were most often obtained from local chemists or pharmacists (10.2%, 95% CI: 8-12%). Conclusions: This is the first formal study to report that approximately 1 out of 4 university women surveyed in Southwest Nigeria self-medicate with antibiotics to treat menstrual symptoms. This practice could provide monthly, low-dose exposures to antibiotics among users. Further studies are necessary to evaluate the impacts of selfmedication on student health
