    Pumice as Precursor in Geopolymer Paste and Mortar

    Natural rocks of magmatic origin are alternative precursors in alkali-activated materials and provide opportunities in the search for more environmentally friendly binders compared to portland cement. The pumice is one of these rocks and its amorphous structure and chemical composition make it one of the candidates as a precursor in producing geopolymer binder when finely ground. Since the majority of the pumice reserves are located in Turkey increases its potential utilization in this area, even more. This paper evaluates the physical, mechanical, and microstructural properties of geopolymer pastes and mortars manufactured with pumice powder (PP) and ground granulated blast furnace slag (BFS) with the activating agents sodium hydroxide (NaOH), potassium hydroxide (KOH), and sodium silicate (SS) solution. The experimental results showed that the compressive strength of the geopolymer pastes was mainly affected by the activator concentration and the PP ratio, rather than the activator type, for single activator mixes. However, the incorporation of SS changed this trend as the KOH and SS combination resulted in higher compressive strength compared to the NaOH and SS. The gradual increase of the PP ratio in the mix design decreased the density and thermal conductivity, on the other hand, increased the water absorption values of the geopolymer mortars.     Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P < 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction.     Volatility Modelling In Crude Oil and Natural Gas Prices

    This study analysis the return volatility of spot market prices of crude oil (WTI) and natural gas (Henry Hub) for two different terms which cover 02.01.2009 -28.04.2014 and 04.01.2010-28.04.2014 with different version of the GARCH class models such as GARCH, IGARCH, GJRGARCH, EGARCH, FIGARCH, FIAPARCH. In particular, the main idea of employing various GARCH models is to determine which one of these linear and nonlinear asymmetric models perform more accurate in terms of ingroups and intergroups activities. Therefore, the main purpose of the paper is to determine a model which ensures to get a maximum return with response to the minimum loss for returns of the investments held by individual investors and fund managers, private sector budget planning decision makers, and state agencies forecasting about macroeconomic indicators. To do this, the ten-days out-of-sample volatility forecasts of Loss Functions to capture the forecasting performance of GARCH class models and to prevent forecasting errors with efficiency hedge ratio in energy market are being considered. For two periods, asymmetric and integrated GARCH models give relatively more accurate performance than other available models. Respectively, for the first period, minimum loss model is FIGARCH-BBM (SST) and for the second period, is EGARCH(GED) for WTI crude oil series in consideration of MSE and MAE criterion. Similarly, for the first period minimum loss model is FIGARCH-BBM (SST) and for the second period, is EGARCH(GED) for Henry Hub natural gas series in consideration of MSE and MAE criterion. This study has potential recommendations for investors from developed and developing countries, which differs it from the current studies. (C) 2016 Published by Elsevier B.V

    Approximate analytic solutions to non-symmetric stance trajectories of the passive Spring-Loaded Inverted Pendulum with damping

    This paper introduces an accurate yet analytically simple approximation to the stance dynamics of the Spring-Loaded Inverted Pendulum (SLIP) model in the presence of non-negligible damping and non-symmetric stance trajectories. Since the SLIP model has long been established as an accurate descriptive model for running behaviors, its careful analysis is instrumental in the design of successful locomotion controllers. Unfortunately, none of the existing analytic methods in the literature explicitly take damping into account, resulting in degraded predictive accuracy when they are used for dissipative runners. We show that the methods we propose not only yield average predictive errors below 2% in the presence of significant damping, but also outperform existing alternatives to approximate the trajectories of a lossless model. Finally, we exploit both the predictive performance and analytic simplicity of our approximations in the design of a gait-level running controller, demonstrating their practical utility and performance benefits

    Comparison of Two Different Enteral Nutrition Protocol in Critically Ill Patients

    WOS: 000388699700011PubMed ID: 27909608Objective: In this study, two enteral nutrition protocols with different gastric residual volumes (GRVs) and different monitoring intervals were compared with respect to gastrointestinal intolerance findings in intensive care unit (ICU) patients. Methods: The study was carried out prospectively in 60 patients in the anaesthesiology and reanimation ICU under mechanical ventilation support, who were scheduled to take enteral feeding. Patients were sequentially divided into two groups: Group 1, GRV threshold of 100 mL, and monitoring interval of 4 hours, and Group 2, GRV threshold of 200 mL, monitoring interval of 8 hours. To test the significant difference between the groups, Student's t test, chi-square text and Fisher exact test were used. Results: In Group 1, 3.3% vomiting, 6.6% diarrhoea was observed; in Group 2, 16.6% vomiting, 10% diarrhoea. In terms of total intolerance (vomiting and/or diarrhoea) of the two groups, the incidence was significantly higher in Group 2 (33.3%) than in Group 1 (10%) (p=0.02). Conclusion: According to the results of the study, a lower gastrointestinal intolerance rate was detected in the GRV threshold 100 mL, monitoring interval for 4 hours protocol (Group 1) than in GRV threshold 200 mL, monitoring interval for 8 hours protocol (Group 2); Group 1 may be preferred renovation

    Clinical Features and Etiology of Adult Patients with Fever and Rash

    Background: Patients with fever and rash often pose an urgent diagnostic and therapeutic dilemma for the clinician. The nonspecificity of many fever and rash syndromes mandates a systemic approach to diagnosis. Objective: We aimed to determine the etiology of fever and rash in 100 adult patients followed-up as in- or outpatients prospectively. Methods: All the patients, who presented with rash and fever, were followed-up prospectively and their clinical and laboratory studies were evaluated. Results: The median age was 35 years (14 similar to 79 years); 45 were female and 55 were male. Patients were divided into 3 groups according to the etiology: infectious (50%), noninfectious (40%) and undiagnosed (10%). The most common type of rash was maculopapular, and the most common 5 causes were measles, cutaneous drug reactions, varicella, adult-onset Still's disease (ASD) and rickettsial disease. Viral diseases among infectious causes and cutaneous drug reactions, among the noninfectious causes, were determined as the main diseases. The mortality rate was 5% and the reasons of mortality were as follows: toxic epidermal necrolysis (2 patients), ASD (1), staphylococcal toxic shock syndrome (1) and graft-versus-host disease (1). Conclusion: Adult patients with fever and rash had a wide differential diagnosis. The most common type of rash was determined as maculopapular, and the most frequent five diseases were measles, drug reactions, chickenpox, ASD and rickettsial infection.     DECREASED FUNCTIONING AND CONTENTS OF WORRIES ASSOCIATED WITH ANXIETY OR DEPRESSION SYMPTOM LEVELS WITHIN PATIENTS HOSPITALIZED WITH COVID-19 INFECTION

    Background: High levels of anxiety and depression symptoms have been reported in patients with COVID-19 compared to the general population. These symptoms were related to variables such as gender, age, and education level with anxiety/depression levels. We aimed to determine the relationship between anxiety and depression symptoms and epidemic-related decreased functioning, worry, and quality of life (QoL). Subjects and methods: The study included 238 hospitalized participants due to COVID-19 and 168 participants who were hospitalized for reasons other than COVID-19. The Hospital Anxiety and Depression Scale (HADS), Short Form 36 (SF-36) QoL Scale, and questionnaires prepared by the researchers were applied. The effects of current worries, impairment in QoL, and decreased functioning during quarantine on levels of anxiety and depressive symptoms were investigated by implementing multiple linear regression analyzes. Results: Our study results suggested the anxiety and depression levels of patients with COVID-19 were not higher than those in the internal medicine inpatient unit at the same time. Worries about transmission to others, uncertainty, social media news, and health anxiety increased the psychiatric symptoms of participants with COVID-19. Disruptions in social relationships and health also have an effect on anxiety/depression symptom levels. Conversely, results indicated losses and worries in occupation and finance did not significantly affect mental symptoms. Conclusion: Worries about transmission to others, uncertainty and health anxiety are closely related to anxiety and depression among patients with COVID-19. There is a need for research in the mental health field for the later stages of the pandemic in different cultures
