    Calcitonin gene-related peptide receptor antagonist BIBN 4096 BS for the acute treatment of migraine

    Background: Calcitonin gene–related peptide (CGRP) may have a causative role in migraine. We therefore hypothesized that a CGRP-receptor antagonist might be effective in the treatment of migraine attacks. Methods: In an international, multicenter, double-blind, randomized clinical trial of BIBN 4096 BS, a highly specific and potent nonpeptide CGRP-receptor antagonist, 126 patients with migraine received one of the following: placebo or 0.25, 0.5, 1, 2.5, 5, or 10 mg of BIBN 4096 BS intravenously over a period of 10 minutes. A group-sequential adaptive treatment-assignment design was used to minimize the number of patients exposed. Results: The 2.5-mg dose was selected, with a response rate of 66 percent, as compared with 27 percent for placebo (P=0.001). The BIBN 4096 BS group as a whole had a response rate of 60 percent. Significant superiority over placebo was also observed with respect to most secondary end points: the pain-free rate at 2 hours; the rate of sustained response over a period of 24 hours; the rate of recurrence of headache; improvement in nausea, photophobia, phonophobia, and functional capacity; and the time to meaningful relief. An effect was apparent after 30 minutes and increased over the next few hours. The overall rate of adverse events was 25 percent after the 2.5-mg dose of the drug and 20 percent for the BIBN 4096 BS group as a whole, as compared with 12 percent for placebo. The most frequent side effect was paresthesia. There were no serious adverse events. Conclusions: The CGRP antagonist BIBN 4096 BS was effective in treating acute attacks of migraine

    Relationship quality and levels of depression and anxiety in a large population-based survey

    PURPOSE: There is substantial literature suggesting that the mental health benefits of marriage (compared to being single) are greater for those in 'good quality' relationships in comparison to those in 'poor quality' relationships. However, little of this research utilises large population-based surveys. Large surveys in psychiatric epidemiology have focused almost exclusively on the association between marital status and mental health. The current study explores some of the reasons for this gap in the literature, and adopts a large, representative community-based sample to investigate whether associations between relationship status and levels of depression and anxiety are moderated by relationship quality. METHODS: Participants were from Wave 3 of the PATH Survey, a longitudinal community survey assessing the health and well-being of residents of the Canberra region, Australia (n=3820). Relationship quality was measured using the 7 item Dyadic Adjustment Scale (DAS-7), and levels of depression and anxiety were measured using the Goldberg Scales. RESULTS: Both cross-sectional and prospective analyses showed that associations between relationship status and mental health were moderated by relationship quality for both men and women, such that only good quality relationships bestowed mental health benefits over remaining single. For women, being in a poor quality relationship was associated with greater levels of anxiety than being single. CONCLUSIONS: Epidemiological studies need to measure relationship quality to qualify the effect of relationship status on mental health.NHMRC (National Health and Medical Research Council of Australia

    Intravenous antibiotics given for 2 weeks do not eradicate persistent Staphylococcus aureus clones in cystic fibrosis patients

    AbstractStaphylococcus aureus is the most commonly isolated pathogen in respiratory tract secretions from young patients with cystic fibrosis (CF), and several treatment strategies are used to control the infection. However, it is not known whether intensified treatment with antimicrobial agents causes eradication of S. aureus clones. We retrospectively determined the impact of intravenous (IV) antimicrobial agents on the suppression and eradication of S. aureus clones. One thousand and sixty-one S. aureus isolates cultured from 2526 samples from 130 CF patients during a 2-year study period were subjected to spa typing. Intervals between positive samples and the occurrence of clone replacements were calculated in relation to courses of IV antimicrobial agents. Of 65 patients chronically infected with S. aureus, 37 received 139 courses of IV antimicrobial agents with activity against S. aureus (mean duration, 15 days; range, 6–31 days). Administration of IV antibiotics increased the time to the next sample with growth of S. aureus: the mean interval between two positive samples was 68 days if IV treatment had been administered, in contrast to 49 days if no IV treatment had been administered (p 0.003). When S. aureus recurred in sputum after IV treatment, the isolate belonged to a different clone in 33 of 114 (29%) intervals, in comparison with 68 of 232 (29%) intervals where IV treatment had not been prescribed (OR 0.98, 95% CI 0.60–1.61). In conclusion, we show that 2 weeks of IV antimicrobial treatment can significantly suppress chronic staphylococcal infection in CF, but is not associated with the eradication of persistent bacterial clones

    Mental health affects future employment as job loss affects mental health: findings from a longitudinal population study

    BACKGROUND: Workforce participation is a key feature of public mental health and social inclusion policies across the globe, and often a therapeutic goal in treatment settings. Understanding the reciprocal relationship between participation and mental health has been limited by inadequate research methods. This is the first study to simultaneously examine and contrast the relative effects of unemployment on mental health and mental health on employment status in a single general population sample. METHOD: Data were from working-age respondents (20 to 55 years at baseline) who completed nine waves of the Household, Income and Labour Dynamics in Australia (HILDA) Survey (N=7176). Cross-lagged path analyses were used to test the lagged and concurrent associations between unemployment and mental health over time, adjusting for sociodemographic characteristics. RESULTS: Mental health was shown to be both a consequence of and risk factor for unemployment. Thus, the poorer mental health observed amongst people who are not working is attributable to both the impact of unemployment and existing mental health problems. While the strength of these two effects was similar for women, the results for men suggested that the effect of unemployment on subsequent mental health was weaker than the effect of mental health on subsequent risk of unemployment. CONCLUSION: Disentangling the reciprocal links between mental health and workforce participation is central to the development and success of clinical goals and health and social policies that aim to promote either aspect.     Tensile testing of cellulose based natural fibers for structural composite applications

    A series of tensile tests were conducted on a Lloyd LRX tensile testing machine for numerous natural fibers deemed potential candidates for development in composite applications. The tensile tests were conducted on the fibers jute, kenaf, flax, abaca, sisal, hemp, and coir for samples exposed to moisture conditions of (1) room temperature and humidity, (2) 65% moisture content, (3) 90% moisture content, and (4) soaked fiber. These seven fibers were then tested for the four conditions and the mechanical properties of tensile strength, tensile strain to failure, and Young's modulus were calculated for the results. These results were then compared and verified with those from the literature, with some of the fibers showing distinctly promising potential. Additionally, a study on the effect of alkalization using 3% NaOH solution was carried out on flax, kenaf, abaca, and sisal to observe impact that this common fiber pre-treatment process has on fiber mechanical properties. The result of the investigation indicated that over treatment of natural fibers using NaOH could have a negative effect on the base fiber properties. It is consequently apparent that a treatment time of less than 10 min is sufficient to remove hemicelluloses and to give the optimum effect

    Hadron Correlators and the Structure of the Quark Propagator

    The structure of the quark propagator of QCDQCD in a confining background is not known. We make an Ansatz for it, as hinted by a particular mechanism for confinement, and analyze its implications in the meson and baryon correlators. We connect the various terms in the K\"allen-Lehmann representation of the quark propagator with appropriate combinations of hadron correlators, which may ultimately be calculated in lattice QCDQCD. Furthermore, using the positivity of the path integral measure for vector like theories, we reanalyze some mass inequalities in our formalism. A curiosity of the analysis is that, the exotic components of the propagator (axial and tensor), produce terms in the hadron correlators which, if not vanishing in the gauge field integration, lead to violations of fundamental symmetries. The non observation of these violations implies restrictions in the space-time structure of the contributing gauge field configurations. In this way, lattice QCDQCD can help us analyze the microscopic structure of the mechanisms for confinement.Comment: 12 pp in LaTeX, preprint Univ. of Valencia, FTUV/94-16, IFIC/94-15.     Nucleation of quark matter bubbles in neutron stars

    The thermal nucleation of quark matter bubbles inside neutron stars is examined for various temperatures which the star may realistically encounter during its lifetime. It is found that for a bag constant less than a critical value, a very large part of the star will be converted into the quark phase within a fraction of a second.     Naupliar and Metanaupliar development of Thysanoessa raschii (Malacostraca, Euphausiacea) from GodthĂĄbsfjord, Greenland, with a reinstatement of the ancestral status of the free-living Nauplius in Malacostracan evolution

    The presence of a characteristic crustacean larval type, the nauplius, in many crustacean taxa has often been considered one of the few uniting characters of the Crustacea. Within Malacostraca, the largest crustacean group, nauplii are only present in two taxa, Euphauciacea (krill) and Decapoda Dendrobranchiata. The presence of nauplii in these two taxa has traditionally been considered a retained primitive characteristic, but free-living nauplii have also been suggested to have reappeared a couple of times from direct developing ancestors during malacostracan evolution. Based on a re-study of Thysanoessa raschii (Euphausiacea) using preserved material collected in Greenland, we readdress this important controversy in crustacean evolution, and, in the process, redescribe the naupliar and metanaupliar development of T. raschii. In contrast to most previous studies of euphausiid development, we recognize three (not two) naupliar (= ortho-naupliar) stages (N1-N3) followed by a metanauplius (MN). While there are many morphological changes between nauplius 1 and 2 (e.g., appearance of long caudal setae), the changes between nauplius 2 and 3 are few but distinct. They involve the size of some caudal spines (largest in N3) and the setation of the antennal endopod (an extra seta in N3). A wider comparison between free-living nauplii of both Malacostraca and non-Malacostraca revealed similarities between nauplii in many taxa both at the general level (e.g., the gradual development and number of appendages) and at the more detailed level (e.g., unclear segmentation of naupliar appendages, caudal setation, presence of frontal filaments). We recognize these similarities as homologies and therefore suggest that free-living nauplii were part of the ancestral malacostracan type of development. The derived morphology (e.g., lack of feeding structures, no fully formed gut, high content of yolk) of both euphausiid and dendrobranchiate nauplii is evidently related to their non-feeding (lecithotrophic) status

    Correlator of Fundamental and Anti-symmetric Wilson Loops in AdS/CFT Correspondence

    We study the two circular Wilson loop correlator in which one is of anti-symmetric representation, while the other is of fundamental representation in 4-dimensional N=4{\cal N}=4 super Yang-Mills theory. This correlator has a good AdS dual, which is a system of a D5-brane and a fundamental string. We calculated the on-shell action of the string, and clarified the Gross-Ooguri transition in this correlator.
