    Annual Performance Reviews Of, For and By Faculty: A Qualitative Analysis of One Department's Experiences

    Purpose: Although annual performance reviews and feedback are recommended for faculty development, best practices and faculty perceptions have not been documented. The authors sought to evaluate the process in one medical school department that established and has sustained an innovative review tradition for 25 years. Method: Content analysis of faculty reports and immersion/crystallization to analyze interviews. Results: Faculty reports described satisfaction and dissatisfaction; facilitators and barriers to goals; and requests for feedback, with community, collaboration and mentorship integral to all three. Interviewees emphasized practical challenges, the role of the mentor and the power of the review to establish community norms. Conclusion: Respondents generally found reviews constructive and supportive. The process informs departmental expectations and culture

    Evaluation of a metal fuselage frame selectively reinforced with filamentary composites for space shuttle application

    The development of metal structures reinforced with filamentary composites as a weight saving feature of the space shuttle components is discussed. A frame was selected for study that was representative of the type of construction used in the bulk frames of the orbiter vehicle. Theoretical and experimental investigations were conducted. Component tests were performed to evaluate the critical details used in the designs and to provide credibility to the weight saving results. A model frame was constructed of the reinforced metal material to provide a final evaluation of the construction under realistic load conditions

    A Systems Approach to Stress, Stressors and Resilience in Humans

    The paper focuses on the biology of stress and resilience and their biomarkers in humans from the system science perspective. A stressor pushes the physiological system away from its baseline state towards a lower utility state. The physiological system may return towards the original state in one attractor basin but may be shifted to a state in another, lower utility attractor basin. While some physiological changes induced by stressors may benefit health, there is often a chronic wear and tear cost due to implementing changes to enable the return of the system to its baseline state and maintain itself in the high utility baseline attractor basin following repeated perturbations. This cost, also called allostatic load, is the utility reduction associated with both a change in state and with alterations in the attractor basin that affect system responses following future perturbations. This added cost can increase the time course of the return to baseline or the likelihood of moving into a different attractor basin following a perturbation. Opposite to this is the system’s resilience which influences its ability to return to the high utility attractor basin following a perturbation by increasing the likelihood and/or speed of returning to the baseline state following a stressor. This review paper is a qualitative systematic review; it covers areas most relevant for moving the stress and resilience field forward from a more quantitative and neuroscientific perspective

    A Systems Approach to Stress and Resilience in Humans: Mindfulness Meditation, Aging, and Cognitive Function

    Psychological stress is common and contributes to many physical and mental health problems. Its effects are mediated by a complex neurobiological system centering in the brain with effectors including autonomic nervous system, hypothalamic-pituitary-adrenal axis, inflammatory system, and gene expression. A stressor pushes the human physiological system away from its baseline state towards a lower utility state. The physiological system may return towards the original state but may be shifted to a lower utility state. While some physiological changes induced by stressors may benefit health, chronic stressors usually have negative effects on health. In contrast to this stressor effect is the system\u27s resilience which influences its ability to return to the high utility attractor basin following a perturbation by increasing the likelihood and/or speed of returning to the baseline state following a stressor. Age-related cognitive decline is a major public health issue with few preventative options. Stress contributes to this cognitive decline, and mindfulness meditation (MM) is a behavioral intervention that reduces stress and stress reactivity in many health conditions. A randomized clinical trial was performed to determine if MM in older adults would improve measures of cognitive function, as well as psychology and physiology, and to determine what factors might predict who would improve. 134 at least mildly stressed 50-85 year olds were randomized to a MM intervention or a wait-list control. Outcome measures included a broad cognitive function battery with emphasis on attention and executive function, self-rated psychological measures of affect and stress, and physiological measures of stress. Self-rated measures related to negative affect and stress were all significantly improved as a result of the MM intervention compared to wait-list control. There were no changes in cognition, salivary cortisol, and heart rate variability. Potential explanations for the discrepancy between the beneficial mental health outcomes and lack of impact on cognitive and physiological outcomes are discussed. To determine which factors predict MM responsiveness, a responder was defined by determining if there was a minimum clinically important improvement in mental health. Predictors included demographic information and selected self-rated baseline measures related to stress and affect. Classification was performed using decision tree analysis. There were 61 responders and 60 non-responders. Univariate statistical analysis of the baseline measures demonstrated significant differences between the responder and non-responders in several self-rated mental health measures. However, decision tree was unable to achieve a reliable classification rate better than 65%. A number of future research directions were suggested by this study, including to optimize the MM intervention itself, to better select participants who would benefit from MM, and to improve the outcome measures perhaps by focusing on decreased reactivity to stressful events. Finally, a less well-defined but always present future research direction is the development of better models and better quantitative analysis approaches to the multivariate but dynamically limited human empirical data that can be practically collected

    Street-view greenspace exposure and objective sleep characteristics among children

    Greenspace may benefit sleep by enhancing physical activity, reducing stress or air pollution exposure. Studies on greenspace and children's sleep are limited, and most use satellite-derived measures that do not capture ground-level exposures that may be important for sleep. We examined associations of street view imagery (SVI)-based greenspace with sleep in Project Viva, a Massachusetts pre-birth cohort. We used deep learning algorithms to derive novel metrics of greenspace (e.g., %trees, %grass) from SVI within 250m of participant residential addresses during 2007–2010 (mid-childhood, mean age 7.9 years) and 2012–2016 (early adolescence, 13.2y) (N = 533). In early adolescence, participants completed >5 days of wrist actigraphy. Sleep duration, efficiency, and time awake after sleep onset (WASO) were derived from actigraph data. We used linear regression to examine cross-sectional and prospective associations of mid-childhood and early adolescence greenspace exposure with early adolescence sleep, adjusting for confounders. We compared associations with satellite-based greenspace (Normalized Difference Vegetation Index, NDVI). In unadjusted models, mid-childhood SVI-based total greenspace and %trees (per interquartile range) were associated with longer sleep duration at early adolescence (9.4 min/day; 95%CI:3.2,15.7; 8.1; 95%CI:1.7,14.6 respectively). However, in fully adjusted models, only the association between %grass at mid-childhood and WASO was observed (4.1; 95%CI:0.2,7.9). No associations were observed between greenspace and sleep efficiency, nor in cross-sectional early adolescence models. The association between greenspace and sleep differed by racial and socioeconomic subgroups. For example, among Black participants, higher NDVI was associated with better sleep, in neighborhoods with low socio-economic status (SES), higher %grass was associated with worse sleep, and in neighborhoods with high SES, higher total greenspace and %grass were associated with better sleep time. SVI metrics may have the potential to identify specific features of greenspace that affect sleep

    Early childhood growth trajectory and later cognitive ability:Evidence from a large prospective birth cohort of healthy term-born children

    BACKGROUND: Most studies of associations between child growth and cognitive ability were based on size at one or two ages and a single measure of cognition. We aimed to characterize different aspects of early growth and their associations with cognitive outcomes in childhood through adolescence. METHODS: In a sample of 12 368 Belarusian children born at term, we examined associations of length/height and weight trajectories over the first 6.5 years of life with cognitive ability at 6.5 and 16 years and its change over time. We estimated growth trajectories using two random-effects models—the SuperImposition by Translation and Rotation to model overall patterns of growth and the Jenss-Bayley to distinguish growth in infancy from post infancy. Cognitive ability was measured using the Wechsler Abbreviated Scales of Intelligence at 6.5 years and the computerized NeuroTrax test at 16 years. RESULTS: Higher length/height between birth and 6.5 years was associated with higher cognitive scores at 6.5 and 16 years {2.7 points [95% confidence interval (CI): 2.1, 3.2] and 2.5 points [95% CI: 1.9, 3.0], respectively, per standard deviation [SD] increase}. A 1-SD delay in the childhood height-growth spurt was negatively associated with cognitive scores [–2.4 (95% CI: –3.0, –1.8) at age 6.5; –2.2 (95% CI: –2.7, –1.6) at 16 years]. Birth size and post-infancy growth velocity were positively associated with cognitive scores at both ages. Height trajectories were not associated with the change in cognitive score. Similar results were observed for weight trajectories. CONCLUSIONS: Among term infants, the overall size, timing of the childhood growth spurt, size at birth and post-infancy growth velocity were all associated with cognitive ability at early-school age and adolescence

    Maternal corticotropin-releasing hormone is associated with LEP DNA methylation at birth and in childhood: an epigenome-wide study in Project Viva

    BackgroundCorticotropin-releasing hormone (CRH) plays a central role in regulating the secretion of cortisol which controls a wide range of biological processes. Fetuses overexposed to cortisol have increased risks of disease in later life. DNA methylation may be the underlying association between prenatal cortisol exposure and health effects. We investigated associations between maternal CRH levels and epigenome-wide DNA methylation of cord blood in offsprings and evaluated whether these associations persisted into mid-childhood.MethodsWe investigated mother-child pairs enrolled in the prospective Project Viva pre-birth cohort. We measured DNA methylation in 257 umbilical cord blood samples using the HumanMethylation450 Bead Chip. We tested associations of maternal CRH concentration with cord blood cells DNA methylation, adjusting the model for maternal age at enrollment, education, maternal race/ethnicity, maternal smoking status, pre-pregnancy body mass index, parity, gestational age at delivery, child sex, and cell-type composition in cord blood. We further examined the persistence of associations between maternal CRH levels and DNA methylation in children's blood cells collected at mid-childhood (n = 239, age: 6.7-10.3 years) additionally adjusting for the children's age at blood drawn.ResultsMaternal CRH levels are associated with DNA methylation variability in cord blood cells at 96 individual CpG sites (False Discovery Rate <0.05). Among the 96 CpG sites, we identified 3 CpGs located near the LEP gene. Regional analyses confirmed the association between maternal CRH and DNA methylation near LEP. Moreover, higher maternal CRH levels were associated with higher blood-cell DNA methylation of the promoter region of LEP in mid-childhood (P < 0.05, β = 0.64, SE = 0.30).ConclusionIn our cohort, maternal CRH was associated with DNA methylation levels in newborns at multiple loci, notably in the LEP gene promoter. The association between maternal CRH and LEP DNA methylation levels persisted into mid-childhood
