    Factors associated with fatigue in hip and/or knee osteoarthritis: a systematic review and best evidence synthesis

    Objective: The aim was systematically to identify and evaluate factors related to fatigue in individuals with hip and/or knee OA. Methods: A systematic literature search was conducted using AMED, CINAHL, MEDLINE, ProQuest and Web of Science Core Collections databases. Inclusion criteria comprised cross-sectional, case-control or longitudinal studies on patients with a diagnosis of hip and/or knee OA that included self-reported fatigue measures. Study quality was assessed using the National Heart, Lung and Blood Institute quality appraisal tool, and factors were synthesized within a bio-behavioural framework. Study designs and quality were combined to determine current evidence levels using best evidence synthesis grading. The full review protocol is available from PROSPERO (PROSPERO 2019: CRD42019138571). Results: Twenty-four studies were included, of which 19 were high, 4 moderate and 1 low quality. There was strong evidence of an association between poor self-reported physical function and high depressive symptoms with higher fatigue. Moderate evidence of an association was found between severe pain, high numbers of co-morbidities and low physical activity levels with higher fatigue. There was moderate or limited evidence of no association between most sociodemographic factors and radiographic OA severity with fatigue. Conclusion: Targets for fatigue management might include improving physical function, reducing depressive symptoms, pain and co-morbidities, and increasing physical activity levels. There is a need for more rigorous longitudinal studies to understand the causal effect of fatigue determinants within the hip and knee OA populations

    Prevalence and pattern of work-related musculoskeletal disorders among Nigerian bricklayers

    BACKGROUND: Typically, bricklayers in developing countries’ contexts manually lay bricks, concrete blocks and other similar materials to construct walls and buildings which make them susceptible to work-related musculoskeletal disorders (WRMSDs). The burden of WRMSDs among this high-risk group seems has not been well documented. OBJECTIVES: This study examined the prevalence of WRMSDs among bricklayers in Nigeria. METHODS: A cross-sectional survey of 118 consenting bricklayers from a Nigerian setting was carried out. The standardized Nordic musculoskeletal disorder questionnaire and a proforma were used to profile the prevalence of WRMSDs and socio-demographic information of the respondents. Data was analyzed using descriptive and inferential statistics. Alpha level was set at p <  0.05. RESULTS: The 12-months and 7-days prevalence of WRMSDs were 87.3% and 67.4% . Shoulder (61.0%) and the low-back (59.3%) were the two most affected anatomical sites based on 12-month prevalence. WRMSDs affecting the knees (6.8%) was the most disabling in carrying normal work routines. Working for less than 10 years was significantly associated with high prevalence of WRMSDs in the shoulder (odd ratio (OR) = 0.27, 95% confidence interval (CI) = 0.09 to 0.87) and wrist region (OR = 0.24, 95% CI 0.08 to 0.73). Having neck pain led to higher odds (OR = 0.29, 95% CI 0.13 to 0.68) of taking a break from work among the bricklayers. CONCLUSIONS: WRMSDs were high among Nigerian bricklayers. Years of work experience was associated with high prevalence of WRMSDs in the shoulder and wrist. In addition, taking work breaks was associated with neck pain

    Physical activity during covid-19 lockdown: Relationship with sedentary behaviour, health-related quality of life, loneliness, and sleep quality among a sample of Nigerian adults

    COVID-19 lockdown, targeted at preventing the spread of coronavirus, had deleterious effects on physical and psychosocial health. This study examined the association between physical activity (PA), sedentary behaviour, health-related quality of life (HRQoL), loneliness and quality of sleep of 507 Nigerian adults during the COVID-19 lockdown. Five hundred and seven respondents (aged 18-67 years) from 12 States completed an online survey on RedCap. Questionnaires comprising the Stages of Change scale, Pittsburgh Sleep Quality Index, International Sedentary Assessment Tool, UCLA Loneliness Questionnaire, Short Form-12 Health Survey, and International Physical Activity Questionnaire were used. Respondents were categorised based on exercise behaviour as non-exercisers, non-regular exercisers, or regular exercisers; and based on age into 18-24, 25-34, 35-44 and > 44 years categories. Descriptive and inferential statistics were used to analyse the data. About 33.1% of respondents did not meet moderate-to-vigorous PA levels. The physical component of HRQoL was positively associated with total PA (p = 0.04). Among the non- exercisers, the odds of being regular exercisers during pre-COVID-19 lockdown were significantly higher for the 35-44 (odds ratio [OR] = 3.49; 95% CI = 1.44, 8.48, p = 0.01) and > 44 years age groups (OR = 2.98; 95% CI: 1.16, 7.62, p = 0.02) relative to 18-24 years age category. During COVID-19 lockdown, > 44 (OR = 3.65; 95% CI: 1.47, 9.07, p = 0.005), 35-44 (OR = 6.42; 95% CI 2.75, 14.96, p = 0.001) and 25-34 (OR = 2.35; 95% CI: 1.15, 4.80, p = 0.02) years age categories had significant higher odds of being regular exercisers compared to the 18-24 years age group. There was a high rate of physical inactivity among Nigerian adults during the COVID-19 lockdown, which was directly influenced by the physical components of HRQoL. Older age was an independent predictor of exercise behaviour before and during the COVID-19 lockdown among Nigerian adults

    Patterns of Work-related Musculoskeletal Disorders among Sonographers in selected Health Facilities in Nigeria

    Abstract Work-related Musculoskeletal Disorders (WMSDs) are the most common work-related health problems in the World, affecting millions of workers. Health professionals are also vulnerable to WMSDs during the course of routine work especially those who are in direct contact with patients. The literature on WMSDs is scarce in Nigeria, especially with regards to sonographers. This study therefore set out to determine the prevalence and patterns of WMSDs among sonographers in Nigeria. A total of 55 self-administered structured questionnaires were distributed to sonographers. Items covered included types of examination performed, time spent daily/ weekly, duration of examination, average number of examination done daily, etc as related to the types of symptoms experienced by the sonographers and the measures taken to alleviate the symptoms. Descriptive statistics of means, standard deviations and percentages were obtained and strengths of correlations of measured parameters were tested using the Pearson's Product moment correlation coefficient statistics. Alpha level was set at 0.05significant level. The total of fifty-one respondents which included thirty-five (69.6%) males and sixteen (31.4%) females ultrasonographers were studied. The ages ranged between 25-60 years. The mean weight and height of all the respondents were 74.3± 11.7kg and 1.66± 0.25m respectively. All 51 respondents reported varying degrees of WMSDs which they associated with tasks of scanning. There was a high preponderance of back pain [upper and lower] (41%) and wrist joint (27%) respectively among other areas of the body. Symptoms manifested ranged from weakness, movement restriction (stiffness) to tingling sensation. The study recorded high prevalence of WMSDs among Nigerian sonographers with back region and the wrist joints respectively worse affected by symptoms.     Influence of body and hand anthropometric characteristics on handgrip strength in young Nigerian women

    Abstract Background and aim Women are well known for having lower handgrip strength (HGS) compared to men. However, factors contributing to lower HGS in women remain unclear. This study investigated the influence of body and hand anthropometric characteristics (HAC) on HGS among young Nigerian undergraduate women. Methods Apparently healthy 500 young female adults from a Nigerian University were recruited conveniently. Age and physical characteristics were recorded. Right and left (R&L) HGS were measured using an electronic dynamometer. HAC including R&L arm girth (ArG), forearm girth (FaG), wrist girth (WrG), finger span (FSp), finger breath (FBr), finger length (FLg) and palm length (PLg) were measured. Data were analysed using descriptive and inferential statistics. Alpha level was set at p < 0.05. Results The mean age of the participants was 22.46 ± 3.72 years. Age and body mass index (BMI) contributed about 20.0 and 12.0% to the prediction of HGS, respectively. Similarly, R&L HAC: ArG, FaG, WrG, FSp, FBr, FLg and PLg contributed about 22.8 and 14.8% to the prediction of HGS, respectively. Significant correlations occurred between HGS and each of BMI, R&L HAC (p < .05). Furthermore, significant correlations were also found between age and each of R&L HAC (p < .001). Conclusions Age, BMI and HAC have significant influence on the HGS of young Nigerian undergraduate women. Hence, physical factors such as weight, height, BMI, and hand anthropometric measurements are recommended as part of routine assessment for effective rehabilitation plans in the care of women with hand disability or poor hand function

    Physiotherapists’ Knowledge, Attitude and Willingness to Care for Covid-19 Patients: a Nationwide Survey in Nigeria

    Introduction: COVID-19 requires an all-inclusive task sharing/shifting approach by clinical professions in order to stem the tide of the pandemic. Physiotherapists have, therefore, potential for front line duties to COVID-19 patients as part of their traditional role in acute care for patients with respiratory complications. This study assessed the knowledge, attitude, and willingness of physiotherapists to care for patients with COVID-19 at the onset of the pandemic. Method: An electronic (WhatsApp) survey of 210 licenced physiotherapists was conducted from March 2020 to July 2020 using an adapted questionnaire from a previous validated tool. Data was analysed using descriptive and inferential statistics. The level of statistical significance was set at p < 0.05. Results: The physiotherapists displayed a high level of awareness (99.0%) about COVID-19, and the social media (96.2%), especially WhatsApp (74.8%), was the main source of information on COVID-19. 28.6% of the respondents had a sufficient level of knowledge of COVID-19. The knowledge about COVID-19 was significantly influenced by educational qualifications (p = 0.00). Also, 77.1%, 74.8% and 87.1% of the respondents werewilling to provide care for patients with COVID-19, be part of front-line care team, and care for COVID-19 survivors, respectively. Conclusion: Nigerian physiotherapists displayed an appreciable level of knowledge of COVID-19 at the onset of the pandemic.     Assessment of Nigerian physiotherapy students’ knowledge, attitudes and awareness of physiotherapy roles in the prevention and management of Covid-19: a nationwide online survey

    This cross-sectional study assessed Nigerian Physiotherapy students’ knowledge, attitudes and awareness of the roles of physiotherapy in the prevention and management of COVID-19.  Four hundred (400) Nigerian physiotherapy students responded to the survey via an electronic (WhatsApp) questionnaire. The survey revealed a high degree of COVID-19 awareness (98.3%) with the social media (83.3%), especially WhatsApp (70.8%), indicated as the. main source of information. 52% of the respondents exhibited a good level of knowledge of the virus, as well as anti-COVID-19 precautions and hygiene. The level of knowledge correlated with the participants’ ages; the final year (class v) had the highest knowledge score (40). While Nigerian physiotherapy students demonstrate a high level of awareness with regard to the virus and its transmission, the level of knowledge relating to physiotherapy roles in COVID-19 appears to be low

    Assessment of functional capacity and sleep quality of patients with chronic heart failure

    Background: Adequate sleep improves physical and mental alertness. However, there is a dearth of empirical data on functional capacity (FC) and sleep quality (SpQ) in patients with chronic heart failure (CHF). Objective: This study investigated the relationship between FC and SpQ of patients with CHF and apparently healthy controls (HCs). Methods: This case-control study recruited 50 patients with CHF whose left ventricular ejection fraction (LVEF) was <40%, attending cardiac clinics of selected government hospitals in Osun State. Furthermore, 50 age- and sex-matched healthy individuals were recruited as controls. Socio-demographic characteristics and cardiovascular parameters were assessed. The FC (VO2 max) and SpQ were assessed using the 6-minute walk test (6-MWT) and Pittsburgh Sleep Quality Index (PSQI), respectively. Data were analysed using descriptive and inferential statistics. Alpha level was set at p < 0.05. Results: Patients had a significantly lower FC and poorer SpQ than HCs, 4.6 ± 0.5 versus 11.3 ± 1.6 mL/kg/min (t = −3.452; p = 0.001) and 8.74 ± 1.6 versus 3.8 ± 1.3 (t = −5.371; p = 0.001), respectively. HCs were about five times more likely to walk longer distance [odds ratio (OR), 4.8; confidence interval (CI), 2.0–11.1] and had a better heart rate (OR, 2.8; CI, 1.4–5.3) than patients. SpQ had a significant negative correlation with FC of patients (r = −0.362; p = 0.001) but a significant positive correlation with HCs (r = 0.481; p = 0.041). Furthermore, there were significant correlations between FC and body mass index in both groups (CHF: r = 0.247, p = 0.022; HCs: r = 0.321, p = 0.040). Conclusion: Patients with heart failure demonstrated lower functional capacity and poorer sleep quality

    Adaptation of the Wound Healing Questionnaire universal-reporter outcome measure for use in global surgery trials (TALON-1 study): mixed-methods study and Rasch analysis

    BackgroundThe Bluebelle Wound Healing Questionnaire (WHQ) is a universal-reporter outcome measure developed in the UK for remote detection of surgical-site infection after abdominal surgery. This study aimed to explore cross-cultural equivalence, acceptability, and content validity of the WHQ for use across low- and middle-income countries, and to make recommendations for its adaptation.MethodsThis was a mixed-methods study within a trial (SWAT) embedded in an international randomized trial, conducted according to best practice guidelines, and co-produced with community and patient partners (TALON-1). Structured interviews and focus groups were used to gather data regarding cross-cultural, cross-contextual equivalence of the individual items and scale, and conduct a translatability assessment. Translation was completed into five languages in accordance with Mapi recommendations. Next, data from a prospective cohort (SWAT) were interpreted using Rasch analysis to explore scaling and measurement properties of the WHQ. Finally, qualitative and quantitative data were triangulated using a modified, exploratory, instrumental design model.ResultsIn the qualitative phase, 10 structured interviews and six focus groups took place with a total of 47 investigators across six countries. Themes related to comprehension, response mapping, retrieval, and judgement were identified with rich cross-cultural insights. In the quantitative phase, an exploratory Rasch model was fitted to data from 537 patients (369 excluding extremes). Owing to the number of extreme (floor) values, the overall level of power was low. The single WHQ scale satisfied tests of unidimensionality, indicating validity of the ordinal total WHQ score. There was significant overall model misfit of five items (5, 9, 14, 15, 16) and local dependency in 11 item pairs. The person separation index was estimated as 0.48 suggesting weak discrimination between classes, whereas Cronbach's α was high at 0.86. Triangulation of qualitative data with the Rasch analysis supported recommendations for cross-cultural adaptation of the WHQ items 1 (redness), 3 (clear fluid), 7 (deep wound opening), 10 (pain), 11 (fever), 15 (antibiotics), 16 (debridement), 18 (drainage), and 19 (reoperation). Changes to three item response categories (1, not at all; 2, a little; 3, a lot) were adopted for symptom items 1 to 10, and two categories (0, no; 1, yes) for item 11 (fever).ConclusionThis study made recommendations for cross-cultural adaptation of the WHQ for use in global surgical research and practice, using co-produced mixed-methods data from three continents. Translations are now available for implementation into remote wound assessment pathways
