    Polyp prevalence at colonoscopy among Nigerians: A prospective observational study

    Background: This study was aimed at identifying the prevalence, distribution, and clinicopathologic characteristic of colonic polyps among Nigerians undergoing colonoscopy at the Obafemi Awolowo University Teaching Hospitals complex, Ile‑Ife, Nigeria. We also determined the polyp detection rate (PDR), polyps per colonoscopy (PPC) and adenoma detection rate (ADR).Materials and Methods: This is a prospective study of all colonoscopy examinations performed at the endoscopy unit of our hospital from January, 2007 to December 2013. The patient demographics, indications for colonoscopy, colonoscopic findings, number of the polyps, their sizes, possible risk factors in the individual case histories, and histopathological characteristics of the polyps.Results: During the study period, a total of 415 patients met the inclusion criteria and only 67 out of these had colonic polyps. The overall PDR was 16.1%. The age ranged was 2‑87 years with a median of 57 years. Forty‑three (64.2%) patients were 50 years or above and there were 40 (59.7%) males. Thirty‑three (49.3%) patients were referred as a result of lower gastrointestinal bleeding, 14 (20.9%) for colorectal cancer (CRC) and 13 (19.4%) for routine screening. Thirty‑nine (58.2%) patients had the polyps at the rectosigmoid region of the colon, 17 (25.4%) had the polyps located proximal to sigmoid colon and 11 (16.4%) patients had multiple polyps involving both segments. Adenomatous polyps was the most common (28 [47.5%]) histopathological finding of which two patients had adenomatous polyposis. Other findings include inflammatory polyps in 17 (18.8%) patients, 5 (8.5%) patients each had hyperplastic and malignant polyps, while 4 (6.8%) patients had juvenile polyps. The ADR was 6.8 and the PPC was 0.2. Statistically, patients 50 years and older were more likely to have adenomatous and hyperplastic polyps than those younger than this age (P = 0.010).Conclusion: We conclude that polyps are probably not as rare among black Africans especially when they are above 50 years. Our histopathological finding of adenomatous change in a good proportion of the detected polyps show that they are likely to be associated with CRCs in our compatriots and as such we would recommend a routine screening colonoscopy for Nigerians aged 50 and above.Key words: Adenoma, hyperplastic, inflammatory, Nigeria, polyps, prevalenc

    Evaluation of Rapid Methods in Malaria Diagnosis from Persons attending Primary Health Facilities, Ogun State, Nigeria

    Malaria in Nigeria is treated blindly in most cases and where diagnostic measures exist, efficacies of several are still in doubt. This study evaluated performance of microscopy and rapid (RDT) methods in confirming prevalence of malaria parasites (MP),parasite species and percent parasitized red blood cells in 384 persons systematically selected from seven primary healthcare facilities in Ogun State, Nigeria from October to December 2012. Giemsa thin and thick film techniques and RDT (SD Bioline Ag P.f/Pan kit) were employed on the blood samples. Data were analysed using SPSS version 16. Demographic results showed that females were 61% and males 39%,but of varying age groups. Seventy-one percent of the blood samples were positive for MP by microscopic blood films, and only 24% samples were positive by RDT. Paired sample T-test showed a significant difference (P=0.000)between microscopy and RDT methods. Of the 273 positive blood samples,95.6%were P. falciparum,3.3% were P. malariae, 0.7% was P. ovale and P. vivax (0.4%). For RDT, P. falciparum was 74% while mixed infections were 26%. Prevalence in risk groups 0–10 and 11–20 age groups was observed to be higher with significant differences (p=0.011, p=0.023) for both microscopy and RDT respectively. RDT sensitivity and specificity were 23.4% and 74.7% compared to microscopy. Positive Predictive Value and Negative Predictive Value were 69.6% and 28.4%. The efficiency of the RDT was 38.3%. The low sensitivity of RDT observed means that malaria diagnosis by microscopy method remains the gold standar

    Relationship Between Gestation Length and Birth Weight in Nigerian Sheep and Their Crosses

    Effect of genotype and birth weight on gestation length was evaluated using Nigerian breeds of sheep and their crosses. The study was carried out at the Sheep Project Unit of Small Ruminant Research Programme (SRRP) of National Animal Production Research Institute (NAPRI), Ahmadu Bello University, Shika-Zaria. The sheep breeds used were Balami, Uda and Yankasa. Heat (estrus) detection was carried out twice daily; in the morning (07:00-08:00 hours); and in the evening (16:00-17:00 hours) using apron fitted rams to pick does on heat. A total of 56 lambs were used for this study. The data obtained were subjected to analysis of variance using General Linear Model and Correlation Procedure of SAS. All the genotypes had similar gestation length with values between 150.3±0.61 days and 153.3±0.60 days, except for Balami pure breed (BAL X BAL) lambs that recorded a shorter gestation length (137.1±0.81 days). There was a wide variation in the birth weight of lambs with the crosses between Balami rams and Yankasa ewes recording a distinct birth weight of 3.5±0.08 kg while the crosses obtained using Yankasa rams on Uda and Balami ewes gave the lowest birth weights (1.7±0.19 and 1.4±0.18, respectively). Sex and litter type have no significant effect (P>0.05) on gestation length. Birth weight was however affected by sex of lambs and their litter type. Litter type was negatively correlated with birth weight (-0.372). Gestation length had a low and non-significant relationship with birth weight; litter type and lamb genotype. Lamb genotype does not have a significant relationship with litter type. Genetic improvement of Nigeria sheep breed is possible if the resources of within and between breed is exploited.     Perceptions of dietary intake amongst Black, Asian and other minoritized ethnic groups in high-income countries: a systematic review of qualitative literature

    Background Minority ethnic groups are a fast-growing population in many high-income countries, partly due to the increasing population of immigrants and second-generation migrants. The dietary practices of some of these minority ethnic groups might make them to be disproportionately affected by obesity and increase their risks of developing non-communicable diseases. Population-specific interventions and strategies are vital to addressing poor nutritional practices among this population. Thus, this study systematically reviewed the perceptions of dietary intake amongst Black, Asian and other minority ethnic groups in high-income countries. Methods This systematic review was conducted in line with the guidelines of the Joanna Briggs Institute (JBI) methodology for systematic reviews, using a meta-aggregative design. This systematic review identified and synthesised qualitative literature on the perceptions of dietary intake amongst BlackAsian and other minority ethnic groups in high-income countries. An extensive and comprehensive database search was conducted between January 2000 – May 2022 and included twenty (20) studies that met the eligibility criteria from six countries. The included studies were assessed for quality using the JBI qualitative assessment and review instrument. The JBI data extraction tools were used to retrieve relevant data from included articles, and the data were thematically analysed. Results We identified eight major themes across this database: (1) “Social and Cultural Factors,” (2) “Availability and Accessibility,” (3) “Family and Community Influences,” (4) “Food Preferences”, (5) “Home Country Food Versus Host Country Food” (6) “Dietary Acculturation” (7) “Health and Healthy Eating” (8) “Perception of Nutritional Information.” Conclusion Overall, Black, Asian, and other minority ethnic groups individuals were found to be aware of the effects of unhealthy eating on their health, and some of them have nutritional knowledge, but social and cultural factors, including structural factors, were deterrents to their healthy eating behaviours. An important finding from this review is that some participants believed that nutritional information, based on bio-medical science, was intended for only White population groups and that it was antagonistic to their cultural and community well-being

    Self-medication with antibiotics for the treatment of menstrual symptoms in southwest Nigeria: a cross-sectional study

    Background: Self-medication with antibiotics is an important factor contributing to the development of bacterial antibiotic resistance. The purpose of this study was to evaluate the prevalence of self-medication with antibiotics for the treatment of menstrual symptoms among university women in Southwest Nigeria. Methods: A cross-sectional survey was administered to female undergraduate and graduate students (n = 706) at four universities in Southwest Nigeria in 2008. The universities were selected by convenience and the study samples within each university were randomly selected cluster samples. The survey was self-administered and included questions pertaining to menstrual symptoms, analgesic and antibiotic use patterns, and demographics. Data were analyzed using descriptive statistics and logistic regression. Results: The response rate was 95.4%. Eighty-six percent (95% CI: 83-88%) of participants experienced menstrual symptoms, and 39% (95% CI: 36-43%) reported using analgesics to treat them. Overall, 24% (95% CI: 21-27%) of participants reported self-medicated use of antibiotics to treat the following menstrual symptoms: cramps, bloating, heavy bleeding, headaches, pimples/acne, moodiness, tender breasts, backache, joint and muscle pain. Factors associated with this usage were: lower levels of education (Odds Ratio (OR): 2.8, 95% CI: 1.1-7.1, p-value: 0.03); nonscience major (OR: 1.58, 95% CI: 1.03-2.50, p-value: 0.04); usage of analgesics (OR: 3.17, 95% CI: 2.07-4.86, p-value: <0.001); and mild to extreme heavy bleeding (OR: 1.64, 95% CI: 1.01-2.67, p-value: 0.05) and pimples/acne (OR: 1.57, 95% CI: 0.98-2.54, p-value: 0.06). Ampicillin, tetracycline, ciprofloxacin and metronidazole were used to treat the most symptoms. Doctors or nurses (6%, 95% CI: 4-7%), friends (6%, 95% CI: 4-7%) and family members (7%, 95% CI: 5-8%) were most likely to recommend the use of antibiotics for menstrual symptoms, while these drugs were most often obtained from local chemists or pharmacists (10.2%, 95% CI: 8-12%). Conclusions: This is the first formal study to report that approximately 1 out of 4 university women surveyed in Southwest Nigeria self-medicate with antibiotics to treat menstrual symptoms. This practice could provide monthly, low-dose exposures to antibiotics among users. Further studies are necessary to evaluate the impacts of selfmedication on student health

    Exposure to anti-malarial drugs and monitoring of adverse drug reactions using toll-free mobile phone calls in private retail sector in Sagamu, Nigeria: implications for pharmacovigilance

    <p>Abstract</p> <p>Background</p> <p>Adverse drug reactions (ADRs) contribute to ill-health or life-threatening outcomes of therapy during management of infectious diseases. The exposure to anti-malarial and use of mobile phone technology to report ADRs following drug exposures were investigated in Sagamu - a peri-urban community in Southwest Nigeria.</p> <p>Methods</p> <p>Purchase of medicines was actively monitored for 28 days in three Community Pharmacies (CP) and four Patent and Proprietary Medicine Stores (PPMS) in the community. Information on experience of ADRs was obtained by telephone from 100 volunteers who purchased anti-malarials during the 28-day period.</p> <p>Results and Discussion</p> <p>A total of 12,093 purchases were recorded during the period. Antibiotics, analgesics, vitamins and anti-malarials were the most frequently purchased medicines. A total of 1,500 complete courses of anti-malarials were purchased (12.4% of total purchases); of this number, purchases of sulphadoxine-pyrimethamine (SP) and chloroquine (CQ) were highest (39.3 and 25.2% respectiuvely). Other anti-malarials purchased were artesunate monotherapy (AS) - 16.1%, artemether-lumefantrine (AL) 10.0%, amodiaquine (AQ) - 6.6%, quinine (QNN) - 1.9%, halofantrine (HF) - 0.2% and proguanil (PR) - 0.2%. CQ was the cheapest (USD 0.3) and halofantrine the most expensive (USD 7.7). AL was 15.6 times ($4.68) more expensive than CQ. The response to mobile phone monitoring of ADRs was 57% in the first 24 hours (day 1) after purchase and decreased to 33% by day 4. Participants in this monitoring exercise were mostly with low level of education (54%).</p> <p>Conclusion</p> <p>The findings from this study indicate that ineffective anti-malaria medicines including monotherapies remain widely available and are frequently purchased in the study area. Cost may be a factor in the continued use of ineffective monotherapies. Availability of a toll-free telephone line may facilitate pharmacovigilance and follow up of response to medicines in a resource-poor setting.</p
