    The role of Porphyromonas gingivalis in the regulation of epithelial-mesenchymal transition markers in migration.

    A relationship between periodontal disease and oral squamous cell carcinoma (OSCC) has been demonstrated in recent studies. Porphyromonas gingivalis’ antigen have been found in oral cancer cells. The embryonic program “epithelial-mesenchymal transition” (EMT) is thought to trigger cancer invasion by allowing tumor cell propagation. The objective of this study was to examine the role of P. gingivalis in the expression of ZEB1, vimentin, and repression of miRNA-200 family as EMT prognostic markers in cancer cell metastasis. Human telomerase immortalized gingival keratinocytes (TIGK) cells were challenged with P. gingivalis strain ATCC 33277 in vitro for 24h at MOI 50 and 100. Expression of ZEB1, and vimentin mRNA levels was determined using qRT-PCR. The expression of ZEB1 and vimentin was up egulated 2.5-fold, and 3-fold respectively, while that of miRNA-200 family was repressed in P. gingivalis infected TIGK cells compared to controls. P. gingivalis infected cells invaded Matrigel-coated Transwell membrane compared to non-infected cells. Knockdown of ZEB1 reduced the invasiveness of P. gingivalis infected TIGK cells. Thus, the presence of P. gingivalis in the periodontium of cancer patients may contribute significantly to the overexpression of ZEB1, and vimentin as crucial promoters of oral squamous cell cancer invasion

    Exploring volunteer tourism as a panacea for sustainable poverty alleviation in Nigeria

    One of the most intractable menaces that humanity has been battling with several decades ago is poverty. There have been a general consensus about the pervasiveness of poverty and its grave consequences on the well-being of citizens of developing (Nigeria) and least developed nations. Notwithstanding the enormity of human, material and natural resources that nature has endowed in the country which should have been effectively and prudently channeled to alleviate the poverty situation of its citizenry. The said resources have over the years not been properly harnessed and or prudently utilized for societal development. One of the ways to address this cankerworm is economic diversification from crude oil reliance to new areas such as volunteer tourism or voluntourism. This paper examine efforts at poverty alleviation strategies by the government of Nigeria and realized that the efforts has not yielded positive results because the citizenry are still wallowing in abject poverty, inadequate social infrastructure, insecurity, unemployment amongst others as an offspring of poverty. This assertion prompted this study and recommendations among other the utilization of voluntourism that are characterized with financial assistance, knowledge transfer, international connection, integrated national endowment via restoration of environment through skill acquisitions, education, cultural immersion, wealth creation and equitable distribution and economic development. We then recommend that volunteer tourism may be adopted as the main antidote for sustainable poverty alleviation if the opportunity is thoroughly harnessed

    Some Adverse Effects of Used Engine Oil (Common Waste Pollutant) On Reproduction of Male Sprague Dawley Rats

    AIM: Used oil is contaminated not only with heavy metals but also with polycyclic aromatic hydrocarbons (PAHs) that are insignificant in the unused oil. In our study we determined possible reproductive effects of used engine oil on male rats.MATERIAL AND METHODS: Twenty eight male Wistar rats were used for the study. The rats had average weight of 181.5 ± 10 g, animal feeds and portable water was provided ad-libitum. The rats were assigned to 4 groups (n = 7) including control. The treated groups orally received 0.1 ml/rat, 0.2 ml/rat and 0.4 ml/rat of the used engine oil every other day for 28 days using oral canulla. The spermatozoa were collected from epididymis for sperm analysis and testes were removed and preserved in Bouin’s fluid for routine histological analysis.RESULTS: Our results showed that there was progressive weight increase among the control group of rats that received distilled water. Meanwhile, rats that received 0.4 ml/rat of the used engine oil showed significant (P < 0.05) weight loss in second and third week of administration while rats that received 0.2 ml/rat and 0.1 ml/rat of the used engine oil showed non-significant (P > 0.05) weight reduction. The spermatozoa number was decreased with significance (P < 0.05) at 0.2 ml/rat (2.38 ± 0.29) and 0.4 ml/rat (1.98 ± 0.08) when compared with the control (5.00 ± 0.89). However, the percentage of motile sperms was reduced significantly (P <0.05) at 0.2 ml/rat (52.86 ± 3.59) and 0.4 ml/rat (45.71 ± 2.94) except at 0.1 ml/rat where the reduction (64.00 ± 7.5) was not significant (P> 0.05). The percentage of head deformity been 41.43 ± 2.61 and 42.00 ± 3.74 at 0.2 ml/rat and 0.4 ml/rat respectively, also significant increase of middle piece deformity was observed only at 0.1 ml/rat (45.71 ± 2.02) while tail deformity significantly decreased (15.71 ± 2.02, 20.00 ± 4.36 and 20.00 ± 4.47) when compared with the control (30.00 ± 1.29). The testicular seminiferous tubules were slightly degenerated with absence of Lumen. The germinal cell layer consisting of necrosis of spermatogonia and interstitial (Leydig) cells with affected Sertoli cells at different maturation stages.CONCLUSION: Hence, it can be said that there is a negative relation between used engine oil and male reproductive parameters. And it can be concluded that used engine oil should be  prevented from leaking, spilling or improperly discarded as through medium it may enter storm water runoff and eventually affect the environmental health receiving water bodies

    Porphyromonas gingivalis initiates a mesenchymal-like transition through ZEB1 in gingival epithelial cells

    The oral anaerobe Porphyromonas gingivalis is associated with the development of cancers including oral squamous cell carcinoma (OSCC). Here we show that infection of gingival epithelial cells with P. gingivalis induces expression and nuclear localization of the ZEB1 transcription factor which controls epithelial-mesenchymal transition (EMT). P. gingivalis also caused an increase in ZEB1 expression as a dual species community with Fusobacterium nucleatum or Streptococcus gordonii. Increased ZEB1 expression was associated with elevated ZEB1 promoter activity and did not require suppression of the miR-200 family of micro RNAs. P. gingivalis strains lacking the FimA fimbrial protein were attenuated in their ability to induce ZEB1 expression. ZEB1 levels correlated with an increase in expression of mesenchymal markers, including vimentin and MMP-9, and with enhanced migration of epithelial cells into matrigel. Knockdown of ZEB1 with siRNA prevented the P. gingivalis-induced increase in mesenchymal markers and epithelial cell migration. Oral infection of mice by P. gingivalis increased ZEB1 levels in gingival tissues, and intracellular P. gingivalis were detected by antibody staining in biopsy samples from OSCC. These findings indicate that FimA-driven ZEB1 expression could provide a mechanistic basis for a P. gingivalis contribution to OSCC

    The impact of systemic hypertension on outcomes in hospitalized COVID-19 patients – a systematic review

    Background: Several observational reports from different parts of the world have shown that systemic hypertension (hypertension) was the single commonest comorbid condition in hospitalized COVID-19 patients. Hypertension is also the most prevalent comorbidity reported among patients who developed severe disease, were admitted to Intensive Care Unit, needed mechanical ventilatory support, or who died on admission. The objective of this systematic review is to study the association between hypertension and specific clinical outcomes of COVID-19 disease which are- development of severe COVID-19 disease, need foradmission in the intensive care unit (ICU) or critical care unit (CCU), need for mechanical ventilation or death Methods: We searched the PubMed, SCOPUS, and Google Scholar databases up till June 28, 2020 for original research articles that documented the risk factors of mortality in patients with COVID-19 using the PRISMA guideline. Results: One hundred and eighty-two articles were identified using pre-specified search criteria, of which 33 met the study inclusion criteria. Only three were prospective studies. Most studies documented hypertension as the most prevalent comorbidity. The association of hypertension with development of severe COVID-19 disease was not conclusive, majority of studies however found an associated with mortality. Conclusion: Hypertension affects the clinical course and outcome of COVID-19 disease in many cohorts. Prospective studies are needed to further understand this relationship.     APOE E4 is associated with impaired self-declared cognition but not disease risk or age of onset in Nigerians with Parkinson's disease

    The relationship between APOE polymorphisms and Parkinson's disease (PD) in black Africans has not been previously investigated. We evaluated the association between APOE polymorphic variability and self-declared cognition in 1100 Nigerians with PD and 1097 age-matched healthy controls. Cognition in PD was assessed using the single item cognition question (item 1.1) of the MDS-UPDRS. APOE genotype and allele frequencies did not differ between PD and controls (p > 0.05). No allelic or genotypic association was observed between APOE and age at onset of PD. In PD, APOE ε4/ε4 conferred a two-fold risk of cognitive impairment compared to one or no ε4 (HR: 2.09 (95% CI: 1.13-3.89; p = 0.02)), while APOE ε2 was associated with modest protection against cognitive impairment (HR: 0.41 (95% CI 0.19-0.99, p = 0.02)). Of 773 PD with motor phenotype and APOE characterized, tremor-dominant (TD) phenotype predominated significantly in ε2 carriers (87/135, 64.4%) compared to 22.2% in persons with postural instability/gait difficulty (PIGD) (30/135) and 13.3% in indeterminate (ID) (18/135, 13.3%) (p = 0.037). Although the frequency of the TD phenotype was highest in homozygous ε2 carriers (85.7%), the distribution of motor phenotypes across the six genotypes did not differ significantly (p = 0.18). Altogether, our findings support previous studies in other ethnicities, implying a role for APOE ε4 and ε2 as risk and protective factors, respectively, for cognitive impairment in PD

    MasakhaNEWS: News Topic Classification for African languages

    African languages are severely under-represented in NLP research due to lack of datasets covering several NLP tasks. While there are individual language specific datasets that are being expanded to different tasks, only a handful of NLP tasks (e.g. named entity recognition and machine translation) have standardized benchmark datasets covering several geographical and typologically-diverse African languages. In this paper, we develop MasakhaNEWS -- a new benchmark dataset for news topic classification covering 16 languages widely spoken in Africa. We provide an evaluation of baseline models by training classical machine learning models and fine-tuning several language models. Furthermore, we explore several alternatives to full fine-tuning of language models that are better suited for zero-shot and few-shot learning such as cross-lingual parameter-efficient fine-tuning (like MAD-X), pattern exploiting training (PET), prompting language models (like ChatGPT), and prompt-free sentence transformer fine-tuning (SetFit and Cohere Embedding API). Our evaluation in zero-shot setting shows the potential of prompting ChatGPT for news topic classification in low-resource African languages, achieving an average performance of 70 F1 points without leveraging additional supervision like MAD-X. In few-shot setting, we show that with as little as 10 examples per label, we achieved more than 90\% (i.e. 86.0 F1 points) of the performance of full supervised training (92.6 F1 points) leveraging the PET approach.Comment: Accepted to IJCNLP-AACL 2023 (main conference

    Effect of early tranexamic acid administration on mortality, hysterectomy, and other morbidities in women with post-partum haemorrhage (WOMAN): an international, randomised, double-blind, placebo-controlled trial

    Background Post-partum haemorrhage is the leading cause of maternal death worldwide. Early administration of tranexamic acid reduces deaths due to bleeding in trauma patients. We aimed to assess the effects of early administration of tranexamic acid on death, hysterectomy, and other relevant outcomes in women with post-partum haemorrhage. Methods In this randomised, double-blind, placebo-controlled trial, we recruited women aged 16 years and older with a clinical diagnosis of post-partum haemorrhage after a vaginal birth or caesarean section from 193 hospitals in 21 countries. We randomly assigned women to receive either 1 g intravenous tranexamic acid or matching placebo in addition to usual care. If bleeding continued after 30 min, or stopped and restarted within 24 h of the first dose, a second dose of 1 g of tranexamic acid or placebo could be given. Patients were assigned by selection of a numbered treatment pack from a box containing eight numbered packs that were identical apart from the pack number. Participants, care givers, and those assessing outcomes were masked to allocation. We originally planned to enrol 15 000 women with a composite primary endpoint of death from all-causes or hysterectomy within 42 days of giving birth. However, during the trial it became apparent that the decision to conduct a hysterectomy was often made at the same time as randomisation. Although tranexamic acid could influence the risk of death in these cases, it could not affect the risk of hysterectomy. We therefore increased the sample size from 15 000 to 20 000 women in order to estimate the effect of tranexamic acid on the risk of death from post-partum haemorrhage. All analyses were done on an intention-to-treat basis. This trial is registered with ISRCTN76912190 (Dec 8, 2008); ClinicalTrials.gov, number NCT00872469; and PACTR201007000192283. Findings Between March, 2010, and April, 2016, 20 060 women were enrolled and randomly assigned to receive tranexamic acid (n=10 051) or placebo (n=10 009), of whom 10 036 and 9985, respectively, were included in the analysis. Death due to bleeding was significantly reduced in women given tranexamic acid (155 [1·5%] of 10 036 patients vs 191 [1·9%] of 9985 in the placebo group, risk ratio [RR] 0·81, 95% CI 0·65–1·00; p=0·045), especially in women given treatment within 3 h of giving birth (89 [1·2%] in the tranexamic acid group vs 127 [1·7%] in the placebo group, RR 0·69, 95% CI 0·52–0·91; p=0·008). All other causes of death did not differ significantly by group. Hysterectomy was not reduced with tranexamic acid (358 [3·6%] patients in the tranexamic acid group vs 351 [3·5%] in the placebo group, RR 1·02, 95% CI 0·88–1·07; p=0·84). The composite primary endpoint of death from all causes or hysterectomy was not reduced with tranexamic acid (534 [5·3%] deaths or hysterectomies in the tranexamic acid group vs 546 [5·5%] in the placebo group, RR 0·97, 95% CI 0·87-1·09; p=0·65). Adverse events (including thromboembolic events) did not differ significantly in the tranexamic acid versus placebo group. Interpretation Tranexamic acid reduces death due to bleeding in women with post-partum haemorrhage with no adverse effects. When used as a treatment for postpartum haemorrhage, tranexamic acid should be given as soon as possible after bleeding onset.
