    Grupos de pesquisa em enfermagem no Brasil: comparação dos perfis de 2006 e 2016

    RESUMO Objetivos Comparar o perfil dos grupos de pesquisa em Enfermagem cadastrados no Diretório do CNPq em 2006 e 2016. Métodos Estudo descritivo documental. A coleta de dados aconteceu em 2006 e 2016 a partir de consulta parametrizada com o termo Enfermagem no Diretório dos Grupos de Pesquisa, na página online do CNPq, sendo realizada a análise descritiva. Os dados foram organizados em planilha do Excel. Resultados O número de Grupos de Pesquisa aumentou de 251 em 2006 para 617 em 2016, com incremento no número de participantes. Houve redução do número de grupos sem estudantes, embora 22% permaneçam sem participação de alunos de graduação. Conclusões Os grupos de pesquisa em Enfermagem refletem avanços estruturais e políticos na geração de ciência, tecnologia e inovação da área, entretanto ainda deve ser incentivada a participação de alunos de graduação e pesquisadores estrangeiros, bem como a ampliação de recursos tecnológicos e das parcerias interinstitucionais

    Predictors of low cervical cancer screening among immigrant women in Ontario, Canada

    <p>Abstract</p> <p>Background</p> <p>Disparities in cervical cancer screening are known to exist in Ontario, Canada for foreign-born women. The relative importance of various barriers to screening may vary across ethnic groups. This study aimed to determine how predictors of low cervical cancer screening, reflective of sociodemographics, the health care system, and migration, varied by region of origin for Ontario's immigrant women.</p> <p>Methods</p> <p>Using a validated billing code algorithm, we determined the proportion of women who were not screened during the three-year period of 2006-2008 among 455 864 identified immigrant women living in Ontario's urban centres. We created eight identical multivariate Poisson models, stratified by eight regions of origin for immigrant women. In these models, we adjusted for various sociodemographic, health care-related and migration-related variables. We then used the resulting adjusted relative risks to calculate population-attributable fractions for each variable by region of origin.</p> <p>Results</p> <p>Region of origin was not a significant source of effect modification for lack of recent cervical cancer screening. Certain variables were significantly associated with lack of screening across all or nearly all world regions. These consisted of not being in the 35-49 year age group, residence in the lowest-income neighbourhoods, not being in a primary care patient enrolment model, a provider from the same region, and not having a female provider. For all women, the highest population-attributable risk was seen for not having a female provider, with values ranging from 16.8% [95% CI 14.6-19.1%] among women from the Middle East and North Africa to 27.4% [95% CI 26.2-28.6%] for women from East Asia and the Pacific.</p> <p>Conclusions</p> <p>To increase screening rates across immigrant groups, efforts should be made to ensure that women have access to a regular source of primary care, and ideally access to a female health professional. Efforts should also be made to increase the enrolment of immigrant women in new primary care patient enrolment models.</p

    Pattern recognition receptors as potential therapeutic targets in inflammatory rheumatic disease

    The pattern recognition receptors of the innate immune system are part of the first line of defence against pathogens. However, they also have the ability to respond to danger signals that are frequently elevated during tissue damage and at sites of inflammation. Inadvertent activation of pattern recognition receptors has been proposed to contribute to the pathogenesis of many conditions including inflammatory rheumatic diseases. Prolonged inflammation most often results in pain and damage to tissues. In particular, the Toll-like receptors and nucleotide-binding oligomerisation domain-like receptors that form inflammasomes have been postulated as key contributors to the inflammation observed in rheumatoid arthritis, osteoarthritis, gout and systemic lupus erythematosus. As such, there is increasing interest in targeting these receptors for therapeutic treatment in the clinic. Here the role of pattern recognition receptors in the pathogenesis of these diseases is discussed, with an update on the development of interventions to modulate the activity of these potential therapeutic targets

    As constantes transformações no mundo decorrentes de determinantes sociais, políticos, econômicos dentre outros têm provocado mudanças no modo de vida e consequentes condições de saúde das populações. Aspectos que igualmente exigem das áreas da saúde no cenário nacional e internacional a produção de tecnologias específicas visando prover cuidados de saúde efetivos.  Neste cenário a produção deste conhecimento tem sido contínua e rápida, na mesma velocidade em que se dá a necessidade de atenção à saúde da população nos diferentes níveis e contextos, fato que tem orientado o a ser investigado.

    Implementação de práticas assistenciais para prevenção e reparo do trauma perineal no parto

    RESUMO Objetivos Implementar práticas assistenciais para prevenção e reparo do trauma perineal no parto normal. Métodos Estudo quase-experimental, realizado no Hospital da Mulher Mãe-Luzia, Macapá, AP. Realizaram-se 74 entrevistas com enfermeiros e médicos e 70 com puérperas, e analisaram-se dados de prontuários (n=555). O desenvolvimento da pesquisa se deu em três fases: pré-auditoria e auditoria de base (fase 1); intervenção educativa e implementação de boas práticas assistenciais (fase 2); auditoria pós-implementação (fase 3); a análise foi pela comparação das fases 1 e 3. Resultados Após a intervenção educativa, menos profissionais incentivavam puxos dirigidos, realizavam episiotomia e suturavam lacerações de primeiro grau; mais mulheres informaram que o parto foi em posição litotômica; mais registros nos prontuários indicaram o uso de Vicryl® na sutura da mucosa e pele. Conclusões A intervenção educativa melhorou os cuidados e os desfechos perineais, porém há lacunas na implementação das evidências e inadequações no manejo do cuidado perineal

    Interventions to improve cervical cancer screening uptake amongst young women: A systematic review

    <p><b>Objectives.</b> In view of declining screening uptake in young women, this review aims to summarise the available evidence relating to interventions designed to increase cervical screening uptake amongst women aged ≤ 35 years.</p> <p><b>Methods</b>. Electronic databases were searched and further articles located by manual searches. Study designs employing a valid comparison group and including women aged ≤ 35 years published through 2012 were considered. Data was extracted on the uptake from either screening programme statistics or as reported by the study subjects. A narrative synthesis was undertaken for each category of interventions identified.</p> <p><b>Results.</b> Ninety-two records were screened with 36 articles retrieved for further assessment. Four studies met the inclusion criteria, two of which evaluated more than one intervention. One of the studies evaluated the use of a modified invitation letter and reported no significant increase in uptake compared to a standard invitation. Three studies investigated the use of a reminder letter, with two reporting a positive effect on screening uptake in women aged 24–34. Three studies were included which supported the use of physician and telephone reminders. One study on HPV self-sampling reported a positive effect when compared with a reminder letter.</p> <p><b>Conclusions.</b> There is a lack of randomised controlled trials designed to specifically address falling cervical screening uptake in amongst young women. Cervical screening programmes need to look beyond the use of invitation/reminders letters in this group of women to develop interventions which attempt to overcome as many barriers to uptake as possible.</p
