Modified triangular posterior osteosynthesis of unstable sacrum fracture
We report preliminary results for unstable sacral fractures treated with a modified posterior triangular osteosynthesis. Seven patients were admitted to our trauma center with an unstable sacral fracture. The average age was 31years (22-41). There were four vertical shear lesions of the pelvis and three transverse fracture of the upper sacrum. The vertical shear injuries were initially treated with an anterior external fixator inserted at the time of admission. Definitive surgery was performed at a mean time of 9days after trauma. The operation consisted in a posterior fixation combining a vertebropelvic distraction osteosynthesis with pedicle screws and a rod system, whereby the transverse fixation was obtained using a 6mm rod as a cross-link between the two main rods. Late displacement of the posterior pelvis or fracture was measured on X-ray films according to the criteria of Henderson. The patients were followed-up for a minimum time of 12months. Four patients who presented with a pre-operative perineal neurological impairment made a complete recovery. No iatrogenic nerve injury was reported. One case of deep infection was managed successfully with surgical debridement and local antibiotics. All patients complained of symptoms related to the prominence of the iliac screws. The metalwork was removed in all cases after healing of the fracture, at a mean time of 4.3months after surgery. No loss of reduction of fracture was seen at final radiological follow-up. The preliminary results are promising. The fixation is sufficiently stable to allow an immediate progressive weight-bearing, and safe nursing care in polytrauma cases. Anatomic anterior cruciate ligament reconstruction: the two-incision technique
This article describes the anatomical two-incision reconstruction of anterior cruciate ligament (ACL) of knee. The major part of currently single incision tibial endoscopic techniques attempts to reproduce the most isometric anteromedial bundle of ACL. Often a relatively vertical femoral tunnel, respect to the notch, is drilled, which is not really efficacious in providing rotatory stability. The single incision technique was developed to obviate the necessity of the lateral femoral incision and dissection. This technical note describes a two-incision ACL reconstruction using an instrumentation, which avoids a large lateral femoral soft tissue dissection, and discusses the rational use of the two-incision ACL reconstruction technique
The VO-Neural project: recent developments and some applications
VO-Neural is the natural evolution of the Astroneural project which was
started in 1994 with the aim to implement a suite of neural tools for data
mining in astronomical massive data sets. At a difference with its ancestor,
which was implemented under Matlab, VO-Neural is written in C++, object
oriented, and it is specifically tailored to work in distributed computing
architectures. We discuss the current status of implementation of VO-Neural,
present an application to the classification of Active Galactic Nuclei, and

Inhibition of translation initiation complex formation by GE81112 unravels a 16S rRNA structural switch involved in P-site decoding
In prokaryotic systems, the initiation phase of protein synthesis is governed by the presence of initiation factors that guide the transition of the small ribosomal subunit (30S) from an unlocked preinitiation complex (30S preIC) to a locked initiation complex (30SIC) upon the formation of a correct codon-anticodon interaction in the peptidyl (P) site. Biochemical and structural characterization of GE81112, a translational inhibitor specific for the initiation phase, indicates that the main mechanism of action of this antibiotic is to prevent P-site decoding by stabilizing the anticodon stem loop of the initiator tRNA in a distorted conformation. This distortion stalls initiation in the unlocked 30S preIC state characterized by tighter IF3 binding and a reduced association rate for the 50S subunit. At the structural level we observe that in the presence of GE81112 the h44/h45/h24a interface, which is part of the IF3 binding site and forms ribosomal intersubunit bridges, preferentially adopts a disengaged conformation. Accordingly, the findings reveal that the dynamic equilibrium between the disengaged and engaged conformations of the h44/h45/h24a interface regulates the progression of protein synthesis, acting as a molecular switch that senses and couples the 30S P-site decoding step of translation initiation to the transition from an unlocked preIC to a locked 30SIC state

QSOs. A method for the determination of photometric
redshifts of QSOs based on multiwavelength
photometry and on a combination of data
mining techniques will be discussed. This procedure,
specifically suited for accompanying the candidate
selection method discussed in (D’Abrusco
et al. 2008), makes use of specific tools developed
under the EuroVO and NVO frameworks for data
gathering, pre-processing and mining, while relying
on the scaling capabilities of the computing
grid. This method allowed us to obtain photometric
redshifts with an increased accuracy (up to 30%)
with respect to the literature
The DAME/VO-Neural Infrastructure: an Integrated Data Mining System Support for the Science Community
Astronomical data are gathered through a very large number of heterogeneous
techniques and stored in very diversified and often incompatible data
repositories. Moreover in the e-science environment, it is needed to integrate
services across distributed, heterogeneous, dynamic "virtual organizations"
formed by different resources within a single enterprise and/or external
resource sharing and service provider relationships. The DAME/VONeural project,
run jointly by the University Federico II, INAF (National Institute of
Astrophysics) Astronomical Observatories of Napoli and the California Institute
of Technology, aims at creating a single, sustainable, distributed
e-infrastructure for data mining and exploration in massive data sets, to be
offered to the astronomical (but not only) community as a web application. The
framework makes use of distributed computing environments (e.g. S.Co.P.E.) and
matches the international IVOA standards and requirements. The integration
process is technically challenging due to the need of achieving a specific
quality of service when running on top of different native platforms. In these
terms, the result of the DAME/VO-Neural project effort will be a
service-oriented architecture, obtained by using appropriate standards and
incorporating Grid paradigms and restful Web services frameworks where needed,
that will have as main target the integration of interdisciplinary distributed

Pediatric nurses in pediatricians' offices: a survey for primary care pediatricians
Background: The role played by nurses in caring for children in pediatricians' officies in the community is crucial to ensure integrated care. In Italy, pediatricians are responsible for the health of children aged 0-14 years living in the community. This study aimed to describe Italian primary care pediatricians' opinions about the usefulness of several nursing activities that pediatric nurses could perform in pediatricians' offices.
Methods: An online survey with pediatricians working in primary care in Italy was conducted between April-December 2018. A 40-item questionnaire was used to assess four types of nursing activities: clinical care, healthcare education, disease prevention, and organizational activities. The answers ranged from 1 (not useful at all) to 6 (very useful). Moreover, three open-ended questions completed the questionnaire.
Results: Overall, 707 pediatricians completed the online survey. Participants were mainly female (63%), with a mean age of 57.74 (SD = 6.42). The presence of a pediatric nurse within the pediatrician's office was considered very useful, especially for healthcare education (Mean 4.90; SD 1.12) and disease prevention (Mean 4.82; SD 1.11). Multivariate analysis confirmed that pediatricians 'with less working experience', 'having their office in a small town', and 'collaborating with a secretary and other workers in the office' rated the nurse's activities significantly more useful.
Conclusions: A pediatric nurse in the pediatrician's office can significantly contribute to many activities for children and their families in the community. These activities include clinical care, healthcare education, disease prevention, and the organizational processes of the office. Synergic professional activity between pediatricians and pediatric nurses could ensure higher health care standards in the primary care setting
