    Testing of Drilled Shafts Socketed Into Limestone

    Construction of the first phase of the South Parking Garage at the Tampa International Airport in Florida was completed in December 1991. A full-scale field load testing program was used in design of the drilled shaft foundations, with specific goals of determining the bond strength and characterizing the load-displacement behavior of the rock sockets. The displacement behavior and bond strength of the test shafts were predicted from elastic solutions and semiempirical methods. Input parameters included the rock uniaxial compressive strength and elastic modulus. The load test results are compared to the predictions herein, and the practical application of these comparisons is demonstrated by a sample evaluation of a drilled shaft supporting the South Parking Garage

    Encroachment of Sills onto State-Owned Bottom: Design Guidelines for Chesapeake Bay

    Recent efforts have sought to expand the use of “Living Shorelines” by waterfront property owners in Virginia and Maryland to combat tidal shoreline erosion. Living shorelines represent a shoreline management option that combines various erosion control methodologies and/or structures while at the same time restoring or preserving natural shoreline vegetation communities. Some regulatory agencies and non-governmental organizations prefer living shorelines over “traditional” shore hardening using bulkheads or stone revetments because these structures create a “barrier” or disconnect between the upland and marine environments. Typically, creation of a living shoreline involves the placement of sand, planting marsh flora, and, if necessary, construction of a rock structure on the shoreline or in the nearshore (Figure 1). When any type of material, sand and/or rock, is placed below/beyond mean high water (MHW) two situations could occur: 1) encroachment onto regulated lands necessitates a permit and 2) one habitat is traded for another -- non-vegetated wetlands and/or nearshore bottom for marsh fringe and rocky substrate. Encroachment beyond mean low water (MLW) in Virginia (MHW in Maryland) is onto state-owned bottom. This latter point is of concern to the Virginia Marine Resources Commission (VMRC) which manages this region. Its concern is the determination of how much encroachment onto public state bottom is necessary for a shore protection project. The Virginia Chesapeake Bay Local Assistance Program (CBLA) regulates the area above MHW and the landward limit of tidal wetlands. They are concerned about how much encroachment landward is required for bank stabilization. The goal of this report is to offer some guidance toward these concerns, most particularly as it pertains to state bottom. Specifically, it is the intent of this report to look at encroachment primarily bayward of MHW/MLW for sill-type systems installed for shore protection

    The weaponry technology associated with Clovis and related Early Paleoindians represents the earliest well-defined evidence of humans in Pleistocene North America. We assess the technological diversity of these fluted stone points found at archaeological sites in the western and eastern halves of North America by employing statistical tools used in the quantification of ecological biodiversity. Our results demonstrate that the earliest hunters in the environmentally heterogeneous East used a more diverse set of points than those in the environmentally homogenous West. This and other evidence shows that environmental heterogeneity in the East promoted the relaxation of selective constraints on social learning and increased experimentation with point designs

     We present the case of a 33 year old female presenting with acute neurological signs including right sided neglect and confusion four days postpartum. The original non-contrast CT brain demonstrated an acute left temporal intraparenchymal hemorrhage. A cerebral catheter angiogram was performed and demonstrated the typical appearances of RCVS (Reversible cerebral vasoconstriction syndrome)

    Statistical Analysis of Paradigmatic Class Richness Supports Greater Paleoindian Projectile-Point Diversity in the Southeast

    Ronald Mason\u27s hypothesis from the 1960s that the southeastern United States possesses greater Paleoindian projectile-point diversity than other regions is regularly cited, and often assumed to be true, but in fact has never been quantitatively tested. Even if valid, however, the evolutionary meaning of this diversity is contested. Point diversity is often linked to Clovis origins, but point diversity could also arise from group fissioning and drift, admixture, adaptation, or multiple founding events, among other possibilities. Before archaeologists can even begin to discuss these scenarios, it is paramount to ensure that what we think we know is representative of reality. To this end, we tested Mason\u27s hypothesis for the first time, using a sample of 1,056 Paleoindian points from eastern North America ami employing paradigmatic classification and rigorous statistical tools used in the quantification of ecological biodiversity. Our first set of analyses, which compared the Southeast to the Northeast, showed that the Southeast did indeed possess significantly greater point-class richness. Although this result was consistent with Mason\u27s hypothesis, our second set of analyses, which compared the Upper Southeast to the Lower Southeast and the Northeast showed that in terms of point-class richness the Upper Southeast \u3e Lower Southeast \u3e Northeast. Given current chronometric evidence, we suggest that this latter result is consistent with the suggestion that the area of the Ohio, Cumberland, and Tennessee River valleys, as well as the mid-Atlantic coastal plain, were possible initial and secondary staging areas for colonizing Paleoindian foragers moving from western to eastern North America

    Scleromyxedema is a systemic disease characterized by lichenoid papules, nodules, and plaques on the skin and often diffuse skin induration resembling the cutaneous involvement of systemic sclerosis. The systemic involvement affects the musculoskeletal, pulmonary, cardiovascular, gastrointestinal, and central nervous systems, and the disorder is commonly associated with a paraproteinemia. Involvement of the kidney is rare and not considered a feature of the disease. Here, we describe an unusual case of scleromyxedema complicated by the development of scleroderma renal crisis-like acute renal failure with a marked intimal deposition of mucin, mucopolysaccharides, and hyaluronic acid in the intrarenal vessels

    Ibrutinib restores immune cell numbers and function in first-line and relapsed/refractory chronic lymphocytic leukemia

    © 2020 The Authors Ibrutinib positively modulates many T-cell subsets in chronic lymphocytic leukemia (CLL). To understand ibrutinib\u27s effects on the broader landscape of immune cell populations, we comprehensively characterized changes in circulating counts of 21 immune blood cell subsets throughout the first year of treatment in patients with relapsed/refractory (R/R) CLL (n = 55, RESONATE) and previously untreated CLL (n = 50, RESONATE-2) compared with untreated age-matched healthy donors (n = 20). Ibrutinib normalized abnormal immune cell counts to levels similar to those of age-matched healthy donors. Ibrutinib significantly decreased pathologically high circulating B cells, regulatory T cells, effector/memory CD4+ and CD8+ T cells (including exhausted and chronically activated T cells), natural killer (NK) T cells, and myeloid-derived suppressor cells; preserved naive T cells and NK cells; and increased circulating classical monocytes. T-cell function was assessed in response to T-cell receptor stimulation in patients with R/R CLL (n = 21) compared with age-matched healthy donors (n = 18). Ibrutinib significantly restored T-cell proliferative ability, degranulation, and cytokine secretion. Over the same period, ofatumumab or chlorambucil did not confer the same spectrum of normalization as ibrutinib in multiple immune subsets. These results establish that ibrutinib has a significant and likely positive impact on circulating malignant and nonmalignant immune cells and restores healthy T-cell function

    New hints towards a precision medicine strategy for IDH wild-type glioblastoma.

    Glioblastoma represents the most common primary malignancy of the central nervous system in adults and remains a largely incurable disease. The elucidation of disease subtypes based on mutational profiling, gene expression and DNA methylation has so far failed to translate into improved clinical outcomes. However, new knowledge emerging from the subtyping effort in the IDH-wild-type setting may provide directions for future precision therapies. Here, we review recent learnings in the field, and further consider how tumour microenvironment differences across subtypes may reveal novel contexts of vulnerability. We discuss recent treatment approaches and ongoing trials in the IDH-wild-type glioblastoma setting, and propose an integrated discovery stratagem incorporating multi-omics, single-cell technologies and computational approaches
