    Genomic homogeneity in fibrolamellar carcinomas

    Background-Fibrolamellar carcinoma (FLC) is a variant of hepatocellular carcinoma (HCC) with distinctive clinical and histological features. To date there have been few studies on the genotypic aspects of FLC and no previous attempts have been made to use the arbitrarily primed-polymerase chain reaction (AF-FCR) technique to detect genetic alterations in this disease.Aim-The aim of this study was to assess the degree of genomic heterogeneity of FEC using the AP-PCR technique. Methods-A fetal of 50 tissue samples of primary and metastatic FLCs from seven patients were microdissected. AP-PCR amplification of each genomic DNA sample was carried out using two arbitrary primers.Results-DNA fingerprints of the primary FLCs and all their metastatic lesions (both synchronous and metachronous disease) were identical in an individual patient. The fingerprints were different between tumours of different patients. No evidence of intratumour heterogeneity was observed.Conclusions-Such genomic homogeneity in FLCs may explain their indolent growth. The absence of clonal evolution, which is present in other tumours (particularly HCCs), may explain the distinct behaviour in this tumour. The tumorigenic pathway and degree of somatic genomic changes in this disease may be less complex than in HCC

    Towards a compact representation of temporal rasters

    Big research efforts have been devoted to efficiently manage spatio-temporal data. However, most works focused on vectorial data, and much less, on raster data. This work presents a new representation for raster data that evolve along time named Temporal k^2 raster. It faces the two main issues that arise when dealing with spatio-temporal data: the space consumption and the query response times. It extends a compact data structure for raster data in order to manage time and thus, it is possible to query it directly in compressed form, instead of the classical approach that requires a complete decompression before any manipulation.     BackgroundCatastrophic fractures are among the most common cause of fatalities in racehorses. Several factors, including genetics, likely contribute to increased risk for fatal injuries. A variant in the procollagen-lysine, 2-oxoglutarate 5-dioxygenase1 gene (PLOD1 c.2032G>A) was shown to cause Warmblood fragile foal syndrome type 1 (WFFS), a fatal recessive defect of the connective tissue. Screening of multiple horse breeds identified the presence of the WFFS allele in the Thoroughbred. PLOD1 is involved in cross-linking of collagen fibrils and thus could potentially increase the risk of catastrophic breakdown.ObjectivesEstimate the frequency of the WFFS allele (PLOD1 c.2032G>A) and determine if it is a risk factor for catastrophic breakdown in the Thoroughbred.Study designCase-control genetic study.MethodsGenomic DNA from hair and/or tissue samples was genotyped for the WFFS allele. Fisher's Exact tests were performed to compare allele and carrier frequencies between the case cohort (catastrophic breakdown, n = 22) and several cohorts with no record of injury (n = 138 raced/trained at same track and season and n = 185 older than 7 years and raced during same season), nonracers (n = 92), and a random sample without consideration for racing history (n = 279).ResultsThe frequency of the PLOD1 c.2032G>A variant in the Thoroughbred breed is low (1.2%). Seventeen of 716 Thoroughbreds tested were carriers (2.4%) and no WFFS homozygotes were detected. Only one catastrophic breakdown case carried the WFFS allele. No statistically significant difference in allele or carrier frequency was identified between case and control cohorts (P>0.05 in all comparisons performed).Main limitationsThis study evaluated cases from one single track.ConclusionsThis study demonstrated that the PLOD1 c.2032G>A associated with WFFS is present at very low frequency in Thoroughbreds and is not a genetic risk factor for catastrophic breakdown

    Application of Failure Mode Effect Analysis (FMEA) to analyze the safety of medication dispensing in a tertiary care hospital

    Background: Failure Mode Effect Analysis (FMEA) is a prospective, team based, structured process used to identify system failures of high risk processes before they occur. Medicines dispensing is a high risk process that should be analyzed for its inherent risks.Objectives: To identify possible failures, their effects and causes of the dispensing process of the study setting using Failure Mode Effect Analysis.Methods: This prospective, cross sectional study was carried out for two months in the Pharmacy Department of a selected teaching hospital, Colombo, Sri Lanka. Thirteen pharmacists had discussions in two independent groups (Group A, n=06; Group B, n=07) to conduct a FMEA. Each group had one in-charge pharmacist, at least one senior pharmacist (>ten years of working experience), and one graduate pharmacist. Each group had five meetings of two hours each, where the dispensing process and sub processes were mapped, and possible failure-modes, their effects, and causes, were identified. A score for potential severity (S), frequency (F) and detectability (D) was assigned for each failure-mode according to specified guidelines. Risk Priority Numbers (RPNs) were calculated (RPN = SxFxD) to prioritize identified failure-modes. Feedbackwas obtained from participants about the usefulness of FMEA.Results: Group A identified 48 failure-modes while Group B identified 43. Among all 91 failure modes, 69 failure-modes were common to both groups. The 22 that were not in-common scored low RPNs. Considering the RPN, Group A prioritized one failure-mode, while Group B prioritized three failure-modes (having identical RPNs). Both groups identified overcrowded dispensing counters as a cause for 57 identified failure-modes. All participants accepted FMEA as an effective method to analyze the safety of the dispensing process.Conclusions: FMEA was successfully utilized to identify and prioritize possible failure-modes of the dispensing process through active involvement of pharmacists.     Harmonising nursing terminologies using a conceptual framework

    The International Classification for Nursing Practice (ICNP®) and the Clinical Care Classification (CCC) System are standardised nursing terminologies that identify discrete elements of nursing practice, including nursing diagnoses, interventions, and outcomes. While CCC uses a conceptual framework or model with 21 Care Components to classify these elements, ICNP, built on a formal Web Ontology Language (OWL) description logic foundation, uses a logical hierarchical framework that is useful for computing and maintenance of ICNP. Since the logical framework of ICNP may not always align with the needs of nursing practice, an informal framework may be a more useful organisational tool to represent nursing content. The purpose of this study was to classify ICNP nursing diagnoses using the 21 Care Components of the CCC as a conceptual framework to facilitate usability and inter-operability of nursing diagnoses in electronic health records. Findings resulted in all 521 ICNP diagnoses being assigned to one of the 21 CCC Care Components.     Hornblendite delineates zones of mass transfer through the lower crust

    Geochemical signatures throughout the layered Earth require significant mass transfer through the lower crust, yet geological pathways are under-recognized. Elongate bodies of basic to ultrabasic rocks are ubiquitous in exposures of the lower crust. Ultrabasic hornblendite bodies hosted within granulite facies gabbroic gneiss of the Pembroke Valley, Fiordland, New Zealand, are typical occurrences usually reported as igneous cumulate hornblendite. Their igneous features contrast with the metamorphic character of their host gabbroic gneiss. Both rock types have a common parent; field relationships are consistent with modification of host gabbroic gneiss into hornblendite. This precludes any interpretation involving cumulate processes in forming the hornblendite; these bodies are imposter cumulates. Instead, replacement of the host gabbroic gneiss formed hornblendite as a result of channeled high melt flux through the lower crust. High melt/rock ratios and disequilibrium between the migrating magma (granodiorite) and its host gabbroic gneiss induced dissolution (grain-scale magmatic assimilation) of gneiss and crystallization of mainly hornblende from the migrating magma. The extent of this reaction-replacement mechanism indicates that such hornblendite bodies delineate significant melt conduits. Accordingly, many of the ubiquitous basic to ultrabasic elongate bodies of the lower crust likely map the ‘missing’ mass transfer zones

    Whole-Page Optimization and Submodular Welfare Maximization with Online Bidders

    In the context of online ad serving, display ads may appear on different types of webpages, where each page includes several ad slots and therefore multiple ads can be shown on each page. The set of ads that can be assigned to ad slots of the same page needs to satisfy various prespecified constraints including exclusion constraints, diversity constraints, and the like. Upon arrival of a user, the ad serving system needs to allocate a set of ads to the current webpage respecting these per-page allocation constraints. Previous slot-based settings ignore the important concept of a page and may lead to highly suboptimal results in general. In this article, motivated by these applications in display advertising and inspired by the submodular welfare maximization problem with online bidders, we study a general class of page-based ad allocation problems, present the first (tight) constant-factor approximation algorithms for these problems, and confirm the performance of our algorithms experimentally on real-world datasets. A key technical ingredient of our results is a novel primal-dual analysis for handling free disposal, which updates dual variables using a “level function” instead of a single level and unifies with previous analyses of related problems. This new analysis method allows us to handle arbitrarily complicated allocation constraints for each page. Our main result is an algorithm that achieves a 1 &minus frac 1 e &minus o(1)-competitive ratio. Moreover, our experiments on real-world datasets show significant improvements of our page-based algorithms compared to the slot-based algorithms. Finally, we observe that our problem is closely related to the submodular welfare maximization (SWM) problem.
