    Burnout among surgeons before and during the SARS-CoV-2 pandemic: an international survey

    Background: SARS-CoV-2 pandemic has had many significant impacts within the surgical realm, and surgeons have been obligated to reconsider almost every aspect of daily clinical practice. Methods: This is a cross-sectional study reported in compliance with the CHERRIES guidelines and conducted through an online platform from June 14th to July 15th, 2020. The primary outcome was the burden of burnout during the pandemic indicated by the validated Shirom-Melamed Burnout Measure. Results: Nine hundred fifty-four surgeons completed the survey. The median length of practice was 10 years; 78.2% included were male with a median age of 37 years old, 39.5% were consultants, 68.9% were general surgeons, and 55.7% were affiliated with an academic institution. Overall, there was a significant increase in the mean burnout score during the pandemic; longer years of practice and older age were significantly associated with less burnout. There were significant reductions in the median number of outpatient visits, operated cases, on-call hours, emergency visits, and research work, so, 48.2% of respondents felt that the training resources were insufficient. The majority (81.3%) of respondents reported that their hospitals were included in the management of COVID-19, 66.5% felt their roles had been minimized; 41% were asked to assist in non-surgical medical practices, and 37.6% of respondents were included in COVID-19 management. Conclusions: There was a significant burnout among trainees. Almost all aspects of clinical and research activities were affected with a significant reduction in the volume of research, outpatient clinic visits, surgical procedures, on-call hours, and emergency cases hindering the training.     Linguistic Analysis of Insta, Twit Posts and LJ Blogs in the Context of Their Functions (Based on the Russian Language)

    The present article is concerned with identifying the linguistic and extralinguistic features of Instagram, Twitter, and Live Journal hypertexts, depending on their functional focus. The relevance of the topic is due to the need for a more detailed study of Internet communication from the point of view of functional and stylistic aspects. The study provides a comparative analysis of Instagram, Twitter, and Live Journal online services based on the Russian language material. The results are correlated with the questionnaire data on the studied problem. The article discusses graphic, lexical, stylistic, syntactic, and spelling features. The authors conducted a comparative analysis of the hypertexts of Instagram, Twitter, and Live Journal online services in the context of their functions; identified linguistic and extralinguistic features of the hypertext of the services under study; established the relationship between the language of the text and the function implemented. It has been established that all three online services perform, first of all, a communicative function, which is expressed through the use of slang, vernacular, online discourse vocabulary, graphic means, markers of increased emotionality, recreation of a specific accent/dialect, spelling and punctuation violations, incomplete, unextended sentences. Along with the socialization function, Instagram performs the function of self-presentation, for the implementation of which the authors of publications resort to linguistic creativity. Unlike Instagram, Twitter, and Live Journal fulfill a more political function, expressed in stylistic devices such as metaphor, comparison, irony, hyperbole, and repetition. The results were verified using a questionnaire survey conducted among Russian students.</span

    Ethno-Lingual Distribution of Anthroponyms in the Name System of Multinational Region

    The purpose of the study is to examine the ethnolinguistic distribution of anthroponymic models in the multinational Republic of Bashkortostan. The authors consider the linguistic features of proper names taking into account the system capabilities of the analysed languages in the areas of language and cultural interaction. Six thousand anthroponymic units of living names of four nationalities of the region under study (Russians, Bashkirs, Germans and Latvians) were analysed. Ethno-linguistic affiliation in the anthroponymic formula of each nationality is revealed through a comprehensive analysis of proper names. Ethnically marked proper names within the framework of the corresponding system of a given language are characterised by increased distinctive ability in various communicative situations. Results show that there is no robust and direct connection between the ethnic characteristics of the first name and the nominator’s ethnic identity. With that, the name as an information source in the study of human identity allows the researchers to draw certain conclusions, as the name is the result of the choice made to reach a certain goal. It enables the actual means used to be correlated with a variety of other possible means, which makes it possible to compare socially important knowledge, shared by everyone, with individual aspects of the general. The results of this research can be useful in the practice of naming children born in exogamous marriages as well as in describing the onomastic situation in countries with a large influx of migrants.&nbsp

    Cause of death and predictors of all-cause mortality in anticoagulated patients with nonvalvular atrial fibrillation: Data from ROCKET AF

    Background-Atrial fibrillation is associated with higher mortality. Identification of causes of death and contemporary risk factors for all-cause mortality may guide interventions. Methods and Results-In the Rivaroxaban Once Daily Oral Direct Factor Xa Inhibition Compared with Vitamin K Antagonism for Prevention of Stroke and Embolism Trial in Atrial Fibrillation (ROCKET AF) study, patients with nonvalvular atrial fibrillation were randomized to rivaroxaban or dose-adjusted warfarin. Cox proportional hazards regression with backward elimination identified factors at randomization that were independently associated with all-cause mortality in the 14 171 participants in the intentionto- treat population. The median age was 73 years, and the mean CHADS2 score was 3.5. Over 1.9 years of median follow-up, 1214 (8.6%) patients died. Kaplan-Meier mortality rates were 4.2% at 1 year and 8.9% at 2 years. The majority of classified deaths (1081) were cardiovascular (72%), whereas only 6% were nonhemorrhagic stroke or systemic embolism. No significant difference in all-cause mortality was observed between the rivaroxaban and warfarin arms (P=0.15). Heart failure (hazard ratio 1.51, 95% CI 1.33-1.70, P&lt;0.0001) and age 6575 years (hazard ratio 1.69, 95% CI 1.51-1.90, P&lt;0.0001) were associated with higher all-cause mortality. Multiple additional characteristics were independently associated with higher mortality, with decreasing creatinine clearance, chronic obstructive pulmonary disease, male sex, peripheral vascular disease, and diabetes being among the most strongly associated (model C-index 0.677). Conclusions-In a large population of patients anticoagulated for nonvalvular atrial fibrillation, 487 in 10 deaths were cardiovascular, whereas &lt;1 in 10 deaths were caused by nonhemorrhagic stroke or systemic embolism. Optimal prevention and treatment of heart failure, renal impairment, chronic obstructive pulmonary disease, and diabetes may improve survival
