    Iterative Near-Maximum-Likelihood Detection in Rank-Deficient Downlink SDMA Systems

    Abstract—In this paper, a precoded and iteratively detected downlink multiuser system is proposed, which is capable of operating in rankdeficient scenarios, when the number of transmitters exceeds the number of receivers. The literature of uplink space division multiple access (SDMA) systems is rich, but at the time of writing there is a paucity of information on the employment of SDMA techniques in the downlink. Hence, we propose a novel precoded downlink SDMA (DL-SDMA) multiuser communication system, which invokes a low-complexity nearmaximum-likelihood sphere decoder and is particularly suitable for the aforementioned rank-deficient scenario. Powerful iterative decoding is carried out by exchanging extrinsic information between the precoder’s decoder and the outer channel decoder. Furthermore, we demonstrate with the aid of extrinsic information transfer charts that our proposed precoded DL-SDMA system has a better convergence behavior than its nonprecoded DL-SDMA counterpart. Quantitatively, the proposed system having a normalized system load of Ls = 1.333, i.e., 1.333 times higher effective throughput facilitated by having 1.333 times more DL-SDMA transmitters than receivers, exhibits a “turbo cliff” at an Eb/N0 of 5 dB and hence results in an infinitesimally low bit error rate (BER). By contrast, at Eb/N0 = 5 dB, the equivalent system dispensing with precoding exhibits a BER in excess of 10%.     Client Service Receipt Inventory as a standardised tool for measurement of socio-economic costs in the rare genetic disease population (CSRI-Ra)

    The measurement of costs is fundamental in healthcare decision-making, but it is often challenging. In particular, standardised methods have not been developed in the rare genetic disease population. A reliable and valid tool is critical for research to be locally meaningful yet internationally comparable. Herein, we sought to develop, contextualise, translate, and validate the Client Service Receipt Inventory for the RAre disease population (CSRI-Ra) to be used in cost-of-illness studies and economic evaluations for healthcare planning. Through expert panel discussions and focus group meetings involving 17 rare disease patients, carers, and healthcare and social care professionals from Hong Kong, we have developed the CSRI-Ra. Rounds of forward and backward translations were performed by bilingual researchers, and face validity and semantic equivalence were achieved through interviews and telephone communications with focus group participants and an additional of 13 healthcare professional and university students. Intra-class correlation coefficient (ICC) was used to assess criterion validity between CSRI-Ra and electronic patient record in a sample of 94 rare disease patients and carers, with overall ICC being 0.69 (95% CI 0.56–0.78), indicating moderate to good agreement. Following rounds of revision in the development, contextualisation, translation, and validation stages, the CSRI-Ra is ready for use in empirical research. The CSRI-Ra provides a sufficiently standardised yet adaptable method for collecting socio-economic data related to rare genetic diseases. This is important for near-term and long-term monitoring of the resource consequences of rare diseases, and it provides a tool for use in economic evaluations in the future, thereby helping to inform planning for efficient and effective healthcare.     Evaluating the health-related quality of life of the rare disease population in Hong Kong using EQ-5D 3-level

    Objectives This study aimed to establish a normative profile of health-related quality of life (HRQOL) of the rare disease (RD) population in Hong Kong (HK) and identify potential predictors. Methods Between March 2020 and October 2020, patients with RD and caregivers were recruited through Rare Disease Hong Kong, the largest RD patient group alliance in HK. HRQOL was derived using the EQ-5D 3-Level with reference to the established HK value set. Utility scores were stratified according to demographics and disease-related information. Multiple linear regression was performed to explore the associations between patient characteristics and HRQOL. Results A total of 286 patients, covering 107 unique RDs, reported a mean utility score of 0.53 (SD 0.36). Thirty patients (10.5%) reported negative utility scores, indicating worse-than-death health states. More problems were recorded in the “usual activities” and “self-care” dimensions. Univariate analyses revealed that neurologic diseases, high out-of-pocket expenditure, home modification, and living in public housing or subdivided flats/units were significantly associated with lower HRQOL. A total of 99 caregivers reported a mean utility score of 0.78 (SD 0.17), which was significantly associated with the utility score of patients they took care of (r = 0.32; P = .001). Conclusions The normative profile of the RD population was established, which revealed lower HRQOL in the RD population than other chronic disease groups and general population in HK. Findings were corroborated by evidence from other cohorts using EQ-5D, combined as part of a meta-analysis. Identifying predictors highlight areas that should be prioritized to improve HRQOL of RD population through clinical and psychosocial dimensions

    Exploring the Relationship Between Morphine Concentration and Oversedation in Children After Cardiac Surgery

    Titrating analgesic and sedative drugs in pediatric intensive care remains a challenge for caregivers due to the lack of pharmacodynamic knowledge in this population. The aim of the current study is to explore the concentration-effect relationship for morphine-associated oversedation after cardiac surgery in children aged 3 months to 3 years. Data on morphine dosing, as well as morphine plasma concentrations, were available from a previous study on the pharmacokinetics of morphine after cardiac surgery in children. Oversedation was defined as scores below 11 on the validated COMFORT-behavioral scale. Population pharmacokinetic-pharmacodynamic modeling was performed in NONMEM 7.3. The probability of oversedation as a function of morphine concentration was best described using a step function in which the EC50 was 46.3 ng/mL. At morphine concentrations below the EC50, the probability of oversedation was 2.9% (0.4& to 18%), whereas above the EC50 percentages were 13% (1.9% to 52%) (median value [95% prediction interval from interindividual variability]).     Global Incidence and mortality of oesophageal cancer and their correlation with socioeconomic indicators temporal patterns and trends in 41 countries

    Oesophageal cancers (adenocarcinomas [AC] and squamous cell carcinomas [SCC]) are characterized by high incidence/mortality in many countries. We aimed to delineate its global incidence and mortality, and studied whether socioeconomic development and its incidence rate were correlated. The age-standardized rates (ASRs) of incidence and mortality of this medical condition in 2012 for 184 nations from the GLOBOCAN database; national databases capturing incidence rates, and the WHO mortality database were examined. Their correlations with two indicators of socioeconomic development were evaluated. Joinpoint regression analysis was used to generate trends. The ratio between the ASR of AC and SCC was strongly correlated with HDI (r = 0.535 [men]; r = 0.661 [women]) and GDP (r = 0.594 [men]; r = 0.550 [women], both p < 0.001). Countries that reported the largest reduction in incidence in male included Poland (Average Annual Percent Change [AAPC] = −7.1, 95%C.I. = −12,−1.9) and Singapore (AAPC = −5.8, 95%C.I. = −9.5,−1.9), whereas for women the greatest decline was seen in Singapore (AAPC = −12.3, 95%C.I. = −17.3,−6.9) and China (AAPC = −5.6, 95%C.I. = −7.6,−3.4). The Philippines (AAPC = 4.3, 95%C.I. = 2,6.6) and Bulgaria (AAPC = 2.8, 95%C.I. = 0.5,5.1) had a significant mortality increase in men; whilst Columbia (AAPC = −6.1, 95%C.I. = −7.5,−4.6) and Slovenia (AAPC = −4.6, 95%C.I. = −7.9,−1.3) reported mortality decline in women.     A Real Time Quality Monitoring System for the Lighting Industry: A Practical and Rapid Approach Using Computer Vision and Image Processing (CVIP) Tools

    In China, the manufacturing of lighting products is very labour intensive. The approach used to check quality and control production relies on operators who test using various types of fixtures. In order to increase the competitiveness of the manufacturer and the efficiency of production, the authors propose an integrated system. This system has two major elements: a computer vision system (CVS) and a real‐time monitoring system (RTMS). This model focuses not only on the rapid and practical application of modern technology to a traditional industry, but also represents a process innovation in the lighting industry. This paper describes the design and development of the prototyped lighting inspection system based on a practical and fast approach using computer vision and imaging processing (CVIP) tools. LabVIEW with IMAQ Vision Builder is the chosen tool for building the CVS. Experimental results show that this system produces a lower error rate than humans produce in the quality checking process. The whole integrated manufacturing strategy, aimed at achieving a better performance, is most suitable for a China and other labour intensive environments such as India.

    OBJECTIVE—Variation in the transcription factor 7-like 2 (TCF7L2) locus is associated with type 2 diabetes across multiple ethnicities. The aim of this study was to elucidate which variant in TCF7L2 confers diabetes susceptibility in African Americans. RESEARCH DESIGN AND METHODS—Through the evalua-tion of tagging single nucleotide polymorphisms (SNPs), type 2 diabetes susceptibility was limited to a 4.3-kb interval, which contains the YRI (African) linkage disequilibrium (LD) block containing rs7903146. To better define the relationship between type 2 diabetes risk and genetic variation we resequenced this 4.3-kb region in 96 African American DNAs. Thirty-three novel and 13 known SNPs were identified: 20 with minor allele frequencies (MAF).0.05 and 12 with MAF.0.10.     ABSTRACT: The main objective of this research work was to model and optimise the production of a locally-developed Infectious Coryza (IC) vaccine. The simulation work was performed using a commercially available batch process simulator SuperPro Designer v5.5. Six debottlenecking schemes were analysed using throughput analysis and cost to benefit ratio (CBR) when the annual production was set to increase by 100%. Based on the economic analysis, the selected debottlenecking scheme has an annual predicted revenue of USD 240 million, with a gross margin of 9.13% and a return on investment (ROI) of 46.12%. In addition, the payback period of the selected scheme is estimated to be within three years.  ABSTRAK: Objektif utama dalam penyelidikan ini adalah untuk memodelkan dan mengoptimumkan hasil pembuatan vaksin tempatan Coryza berjangkit.     Analysis of whole exome sequencing with cardiometabolic traits using family-based linkage and association in the IRAS Family Study

    Family-based methods are a potentially powerful tool to identify trait-defining genetic variants in extended families, particularly when used to complement conventional association analysis. We utilized two-point linkage analysis and single variant association analysis to evaluate whole exome sequencing (WES) data from 1,205 Hispanic Americans (78 families) from the Insulin Resistance Atherosclerosis Family Study. WES identified 211,612 variants above the minor allele frequency threshold of ≥0.005. These variants were tested for linkage and/or association with 50 cardiometabolic traits after quality control checks. Two-point linkage analysis yielded 10,580,600 LOD scores with 1,148 LOD scores ≥3, 183 LOD scores ≥4, and 29 LOD scores ≥5. The maximal novel LOD score was 5.50 for rs2289043:T\u3eC, in UNC5C with subcutaneous adipose tissue volume. Association analysis identified 13 variants attaining genome-wide significance (pT in APOA5, and triglyceride levels (p=3.67×10-10). Overall, there was a 5.2-fold increase in the number of informative variants detected by WES compared to exome chip analysis in this population, nearly 30% of which were novel variants relative to dbSNP build 138. Thus, integration of results from two-point linkage and single-variant association analysis from WES data enabled identification of novel signals potentially contributing to cardiometabolic traits
