A Wii Bit of Fun: A Novel Platform to Deliver Effective Balance Training to Older Adults
BACKGROUND: Falls and fall-related injuries are symptomatic of an aging population. This study aimed to design, develop, and deliver a novel method of balance training, using an interactive game-based system to promote engagement, with the inclusion of older adults at both high and low risk of experiencing a fall.STUDY DESIGN: Eighty-two older adults (65 years of age and older) were recruited from sheltered accommodation and local activity groups. Forty volunteers were randomly selected and received 5 weeks of balance game training (5 males, 35 females; mean, 77.18 ± 6.59 years), whereas the remaining control participants recorded levels of physical activity (20 males, 22 females; mean, 76.62 ± 7.28 years). Child–parent interaction in relation to road safety education : Part 2 – main report
• Children and young people are particularly vulnerable road users. • Child pedestrian injury rates are poor compared with the rest of Europe. • The factors that impact on children’s road safety and their capability in traffic are numerous, multi-faceted and complex. • • The systematic review conducted by Cattan et al. (2008) as the initial phase of this study shows that: • parents see themselves as being responsible for developing their children’s road safety awareness and skills; • holding hands is the most common road-crossing interaction between parents and children; • adults rarely make use of road-crossing events to give oral instructions; • few parents and children are consistent in their road-crossing behaviour; • roadside training by volunteer parents for groups of children can lead to significant improvements in children’s road safety behaviour; • belief in fate seems to influence the likelihood of parents using restraints, such as seat belts or car seats, with their children; and • parents’ understanding of the child’s perspective in carrying out road safety tasks and their motivation to actively involve their child in making decisions at the roadside can be improved through training. • Social Cognitive Theory (Bandura, 1986) suggests that the modelling role of parents can make a significant contribution to children’s learning about road use and their development of traffic competence whether or not parents are aware of this. • The main aim of this study was to explore the way parents influence children and young people aged 0–16 years to be safer road users. • This study included children and young people aged 5–16 and parents of children aged 0–16 years old
Measurement properties of tools used to assess self-harm in autistic and general population adults
Autistic people are at increased risk of experiencing self-harm compared to the general population. However, it is unclear which tools are being used to assess self-harm in autistic people, or whether existing tools need to be adapted for this group. This two-stage systematic review aimed to identify tools used to assess self-harm in autistic and general population adults, evaluate these tools on their measurement properties, and make recommendations for their appropriate use in research and clinical practice. Four databases were systematically searched (PsycINFO, Embase, MEDLINE and Web of Science). Eight frequently used self-harm assessment tools were identified and assessed for risk of bias, criteria for good measurement properties, and quality of evidence using the COSMIN checklist. Of these, two tools had sufficient evidence of internal consistency (ISAS, QNSSI), and one had been frequently used with autistic adults (NSSI-AT). These three tools may have potential for use with autistic adults but require further investigation for content validity and measurement properties in the autistic population. More research and potential adaptations to current self-harm assessment tools are recommended in order to better conceptualise and understand self-harm and its measurement in autism.</p
Child–parent interaction in relation to road safety education : part 1 – A critical literature review
The purpose of this review was: • to identify and provide a critical review of the research and literature concerned with parentchild interaction in relation to road safety education in order to inform the Child–parent interaction in relation to road safety education study completed in 2007; and • to consider the published evidence for the effect of strategies that parents use in training their children to be safer road users and to consider ways of engaging parents in road safety education. The term ‘children’ refers to the age group 0 to 18. The reason for this is that, although most research on childparent interaction tends to focus on the 0 to 14 age group, some studies, mainly those concerned with teen drivers, include young people aged 16 to 18. A concept‐wide association study to identify potential risk factors for nonadherence among prevalent users of antihypertensives
PurposeWe sought to determine whether an association study using information contained in clinical notes could identify known and potentially novel risk factors for nonadherence to antihypertensive medications.MethodsWe conducted a retrospective concept‐wide association study (CWAS) using clinical notes to identify potential risk factors for medication nonadherence, adjusting for age, sex, race, baseline blood pressure, estimated glomerular filtration rate, and a combined comorbidity score. Participants included Medicare beneficiaries 65 years and older receiving care at the Harvard Vanguard Medical Associates network from 2010‐2012 and enrolled in a Medicare Advantage program. Concepts were extracted from clinical notes in the year prior to the index prescription date for each patient. We tested associations with the outcome for 5013 concepts extracted from clinical notes in a derivation cohort (4382 patients) and accounted for multiple hypothesis testing by using a false discovery rate threshold of less than 5% (q < .05). We then confirmed the associations in a validation cohort (3836 patients). Medication nonadherence was defined using a proportion of days covered (PDC) threshold less than 0.8 using pharmacy claims data.ResultsWe found 415 concepts associated with nonadherence, which we organized into 11 clusters using a hierarchical clustering approach. Volume depletion and overload, assessment of needs at the point of discharge, mood disorders, neurological disorders, complex coordination of care, and documentation of noncompliance were some of the factors associated with nonadherence.ConclusionsThis approach was successful in identifying previously described and potentially new risk factors for antihypertensive nonadherence using the clinical narrative.Peer Reviewedhttps://deepblue.lib.umich.edu/bitstream/2027.42/151999/1/pds4850.pdfhttps://deepblue.lib.umich.edu/bitstream/2027.42/151999/2/pds4850_am.pd
Interactive versus Passive Distraction and Parent Psychoeducation as pain management techniques during pediatric venepuncture: A Randomized Controlled Trial
Objectives The aim of this research was twofold: to explore 1) the efficacy of active vs. passive distraction on self-reported pain and distress of children during a venepuncture; and 2) the impact ofparental psychoeducation on child and parent outcomes, parental knowledge of distraction procedures and parental engagement in effective pain management strategies. Methods This cross-sectional study included 213 children scheduled for a venepuncture, and one of their parents, who were randomly allocated to one of four conditions; interactive distraction, passive distraction, interactive distraction with parent psychoeducation and passive distraction with parent psychoeducation. ANCOVA’s were used to investigate the impact of distraction type and the use of parent psychoeducation on child and parent pain related outcome variables. Results Statistical analyses revealed no significant differences between groups for child-reported pain and distress. Parents who received parent psychoeducation had a significantly higher level of knowledge than parents who did not receive psychoeducation, but did not engage in more effective pain management behaviour. Conclusions The results indicated that passive vs. active distraction does not have a significantly different influence on child pain-related outcome variables. In addition, while psychoeducation was demonstrated to be effective in increasing parental knowledge, it was not sufficient to change parental behaviour
“Catch 22”: biosecurity awareness, interpretation and practice amongst poultry catchers
Campylobacter contamination of chicken on sale in the UK remains at high levels and has a substantial public health impact. This has prompted the application of many interventions in the supply chain, including enhanced biosecurity measures on-farm. Catching and thinning are acknowledged as threats to the maintenance of good biosecurity, yet the people employed to undertake this critical work (i.e. ‘catchers’) are a rarely studied group. This study uses a mixed methods approach to investigate catchers’ (n = 53) understanding of the biosecurity threats posed by the catching and thinning, and the barriers to good biosecurity practice. It interrogated the role of training in both the awareness and practice of good biosecurity. Awareness of lapses in biosecurity was assessed using a Watch-&-Click hazard awareness survey (n = 53). Qualitative interviews (n = 49 catchers, 5 farm managers) explored the understanding, experience and practice of catching and biosecurity. All of the catchers who took part in the Watch-&-Click study identified at least one of the biosecurity threats with 40% detecting all of the hazards. Those who had undergone training were significantly more likely to identify specific biosecurity threats and have a higher awareness score overall (48% compared to 9%, p = 0.03). Crucially, the individual and group interviews revealed the tensions between the high levels of biosecurity awareness evident from the survey and the reality of the routine practice of catching and thinning. Time pressures and a lack of equipment rather than a lack of knowledge appear a more fundamental cause of catcher-related biosecurity lapses. Our results reveal that catchers find themselves in a ‘catch-22′ situation in which mutually conflicting circumstances prevent simultaneous completion of their job and compliance with biosecurity standards
