    Head And Neck Reconstruction Using Infrahyoid Myocutaneous Flaps.

    The use of pedicled myocutaneous flaps in head and neck reconstruction is widely accepted. Here we describe our experience with infrahyoid flaps (IHFs) employed to cover surgical defects in the oral cavity and oropharynx in patients with benign and malignant tumors. The aim was to evaluate the success rate for infrahyoid myocutaneous flap procedures performed at a single institution. Retrospective study, at the Head and Neck Surgery Service, Unicamp. Fourteen IHFs were used to reconstruct surgical defects in eleven men (78.5%) and three women (21.5%) with a mean age of 66.4 years. The anterior floor of the mouth was reconstructed in nine patients (64.2%), the base of tongue in three (21.4%), the lateral floor in one (7.1%), and the retromolar area (7.1%) in one. Thirteen patients (92.8%) had squamous cell carcinoma (SCC) and one (7.2%) ameloblastoma. The disease stage was T3 in eight (61.5%) of the SCC cases and T4 in five (38.5%). No patient presented total flap loss or fistula. The most common complication was epidermolysis, which delayed the beginning of oral ingestion. The patients with SCC received postoperative radiotherapy without major consequences to the flap. IHF is a safe and reliable procedure for reconstructing head and neck surgical defects. Due to its thinness and malleability, its use for oral cavity and oropharynx defects provides favorable cosmetic and functional outcomes. Complications, when present, are easy to manage.124271-

    The objective was to report the injuries on liver and kidney promoted by experimental colorectal carcinogenesis induction in rats and evaluate the effect of supplementation with Euterpe edulis M. pulp products on resolution of this injuries. Colorectal carcinogenesis with 1,2-dimethylhydrazine was induced in young male rats, allocated into: C - induced to carcinogenesis; CJ - induced to carcinogenesis and supplemented with juçara fruit pulp; and CE - induced to carcinogenesis and supplemented with juçara fruit lyophilized extract. Nine animals were a negative control. Supplementation occurred three times a week, totaling 54 days of administration with 1 mg of cyanidin-3-glycoside per kilogram live weight. The hepatic and renal histopathological injuries were assessed at 10 and 23 weeks. In liver, at 10-week biliary hyperplasia was more evident in colorectal cancer induced groups compared to N group (p = 0.0230), as well as megalocytosis (p = 0.0269), and juçara fruit-based product do not promote cytoprotection. At 23-week biliary hyperplasia continued present, and liver necrosis was evident in C group and CJ group. Hepatic degeneration was greater in C group, and megalocytosis was evident in the cancer-induced groups, without cytoprotection by juçara fruit-based product. In kidney, at 23-week, renal congestion was more evident in CJ group, and tubular degeneration in C and CE groups. Important hepatic and renal injuries were observed in rats induced to colorectal cancer and the supplementation with juçara fruit-based product, in the dose used, did not interfere in the prevention and resolution of these injuries, mainly with the chronic use

    Os aspectos semiológicos do acidente vascular encefálico: uma abordagem neurológica

    O Acidente Vascular Encefálico (AVC) é um evento neurológico súbito, com um foco de isquemia ou hemorragia. Ambos, qualificados pelo déficit neurológico focal abrupto. Ressaltando, que estes déficits podem ocorrer, sendo a ocorrência espontânea, perduração de 15 minutos, autoresolutiva é denominada como Ataque Isquêmico Transitório (AIT), no entanto, toda insuficiência neural que não melhorar pós esse período deve ser manejado como AVC. O artigo objetivou descrever os principais aspectos clínicos do AVC. O AVC é uma emergência para a saúde pública, em razão de ser um potencial em gerar morbimortalidades para os portadores e prejuízos para os sistemas de saúde. O AVC do tipo isquêmico representa a maioria das ocorrências, o quadro clínico do paciente é correspondente ao tecido neural afetado, inicialmente a tomografia computadorizada sem contraste é o primeiro exame, por ser crucial para descartar a etiologia hemorrágica, a condução terapêutica se baseia em medidas neuroprotetoras através da estabilização da glicemia, temperatura e sódio, adequar os níveis pressóricos, mediante o prazo estipulado impor terapia antitrombótica. A manifestação hemorrágica, pode ocorrer por torção de aneurisma sacular originando o sangramento subaracnóideo ou por hipertensão gerando o sangramento intraparenquimatoso. A partir da análise das informações coletadas, elucida-se que o diagnóstico precoce e o período transcorrido até o manejo terapêutico são cruciais para o desfecho clínico do portador, ou seja, é possível a normalização ou ocorrer sequelas neurais e óbito

Ananindeua, PA, Brasil.In 2018, during the surveillance for West Nile virus (WNV) in horses with neurological clinical signs in the state of Espírito Santo (Brazil), 19 animals were investigated, and 52 biological samples were collected for WNV diagnostic. One brain sample was positive for WNV by RT-qPCR and the virus was isolated in C6/36 cell culture and sequenced. We obtained a nearly complete genome of WNV co-infected with Peruvian horse sickness virus (PHSV) in the cell culture. After confirmation of PHSV by next-generation sequencing, a new PHSV RT-qPCR protocol was developed, which was used to detect another horse positive only for PHSV. This assay provides a simple and direct method for easy identification of PHSV from biological samples from horses and may become a useful tool in the epidemiological surveillance of this virus. It is the first case of PHSV in Brazil, and only the third country overall to report, 23 years after the first confirmed notification in Peru. Moreover, it is the first reported co-infection of PHSV and WNV in a horse with neurological signs, confirmed by RT-qPCR
