    Caractérisation des risques de contamination des agrosystÚmes périurbains de Dakar par les éléments traces métalliques

    L’application de produits rĂ©siduaires organiques (PRO) amĂ©liore la fertilitĂ© des sols, mais induit des effets nĂ©gatifs sur les agrosystĂšmes, en raison de la prĂ©sence de contaminants tels que les Ă©lĂ©ments traces mĂ©talliques (ETM). Cette Ă©tude vise Ă  Ă©valuer l’influence de deux doses (20 et 60 T.ha-1) d’apport de boue de station d’épuration STEP et de fiente de volaille sur la disponibilitĂ© de quatre ETM (Ni, Cd, Cr et Pb) dans un arenosol et un fluvisol. Une incubation de 88 jours en conditions contrĂŽlĂ©es a Ă©tĂ© conduite sur les deux sols mĂ©langĂ©s aux diffĂ©rentes doses de boue et de fiente. La disponibilitĂ© des ETM dans les sols incubĂ©s a Ă©tĂ© Ă©valuĂ©e par la technique DGT (Diffusive Gradient in Thin films). Les boues ont abaissĂ© le pH des deux sols tandis que les fientes ont alcalinisĂ© l’arenosol durant les 14 premiers jours d’incubation. Ces PRO ont augmentĂ© les concentrations de Ni, Cd, Pb et Cr disponibles dans les sols. Les  disponibilitĂ©s du Ni, Cd et Pb sont plus Ă©levĂ©es dans l’arenosol que dans le fluvisol. La boue Ă  60 T.ha-1 a induit les plus fortes disponibilitĂ©s en Cd et Pb tandis que la fiente Ă  60 T.ha-1 a induit les plus fortes disponibilitĂ©s en Ni et Cr dans les deux sols.Mots clĂ©s : ElĂ©ments traces mĂ©talliques, arenosol, fluvisol, boue d’épuration, fiente.  CHARACTERIZATION OF CONTAMINATION RISKS OF THE  SUBURBANAGROECOSYSTEMS OF DAKAR BY TRACE ELEMENTSOrganic wastes (OW) spreading improve soil fertility, but can induce negative effects on agroecosystems, due to the presence of contaminants such as trace elements (TE). This study aims to evaluate the impacts of two doses (20 and 60 T.ha-1) of sewage sludge and poultry manure on the availability of four trace elements (Ni, Cd, Cr and Pb) in arenosol and  fluvisol. Soils mixed with different doses of sewage sludge and poultry manure were incubated for 88 days under controlled conditions. The  availability of trace elements in incubated soils was evaluated by the DGT (Diffusive Gradient in Thin films) technique. Sewage sludge has decreased the pH of both soils while poultry manure induced an alkalinization of arenosol during the first 14 days of incubation. OW increased Ni, Cd, Pb and Cr availability in the soils. Availability of Ni, Cd and Pb were higher in arenosol than in the fluvisol. Sewage sludge at 60 T.ha-1 induced the highest availability of Cd and Pb while the poultry manure at 60 T.ha-1 induced the highest availability of Ni and Cr in both soils.Keywords : Trace elements, arenosol, fluvisol, sewage sludge, poultry manure

    Ectopia cordis about a case at Ourossogui regional hospital center

    We report in this work, an extremely rare and major case of anterior body wall defects included ectopia cordis define by abnormal location of heart outside of the thorax. This case was diagnosed at the maternity of Ourossogui regional hospital center, in Senegal. Any scan was performed during the pregnancy. Newborn died 10 minutes after birth. Ectopia cordis is related to a possible ventral midline developmental abnormality. It’s associated to other midline abnormalities and is a part of pentalogy of Cantrell. An X-linked genetic abnormality

    Maternal and perinatal outcomes in multiple versus singleton pregnancies in Dakar, Senegal: a cross sectional study over 10 years

    Background: The birth of twins is a singular event in most societies, and even more when it comes to multiple births. The objective of this study was to investigate maternal and perinatal outcomes in multiple versus singleton pregnancies.Methods: Cross-sectional study carried out at Philippe Maguilen Senghor health center in Dakar, Senegal from January 1, 2011 to June 30, 2019. Data were extracted from this E-perinatal electronic database and then analyzed in statistical package for social science software (SPSS 24, Mac version).Results: A total 42,870 mothers delivered 44,149 newborns including 1250 twins (2.8%) and 29 triplets. The mean maternal age was 27 years. Mothers with multiple pregnancies had 3 times the odds of poor maternal outcome compared to mothers with single pregnancies (OR 2.42, 95% CI; 1.98-2.94, p <0.001, for high blood pressure; OR, 2.66; 95% CI, 2.11-3.32, p= <0.001, for severe pre-eclampsia; and OR, 3.04; 95% CI, 1.64-5.66, p <0.001, for postpartum hemorrhage). Likewise, women with multiple gestations had significantly higher rates of preterm birth (OR 5.62; 95% CI: 4.91-6.41, p <0.001), breech presentations (OR = 11.02; CI = 9.68-12.53, p <0.001) and neonatal deaths (OR = 2.94; CI = 9.6852-12.5328 p= 0.004) as compared to women with singleton gestations. Furthermore, women with multifetal gestations had increased risk for caesarean section (OR 2.14; 95% CI: 1.91-2.41, p <0.001) compared with their singleton counterparts. The risks for episiotomy and perineal injuries were higher for women with singleton gestations as compared to multiple gestation mothers.Conclusions: This study results are in line with previous findings and contradict others. Particular attention should always be paid to multiple pregnancies’ management. However, the pattern of certain complications traditionally correlated with multiple pregnancies is to be confirmed

    Village Baseline Study: Site Analysis Report for Kaffrine – Kaffrine, Senegal

    The village baseline study of Toune MosquĂ©e village in the CCAFS benchmark site Kaffrine in Senegal took place from June 2 to 4, 2011. Natural resources in the village are in a progressive state of degradation. The ‘protected’ forest has almost disappeared, the soils have low fertility and are degraded, and crop production is not sufficient to meet the food needs of a family throughout the year. Families must buy food to fill the gap in production. For that they harvest and sell forest products, which creates a vicious cycle of resource degradation. The male and female groups identified 35 organisations operating in the village including informal groups, state services, associations, NGOs and Muslim brotherhoods. While 18 of those work on food security issues, only 4 are involved in the management and protection of natural resources. Very few agricultural extension and training opportunities target women despite the women’s significant role in agriculture and livestock production. Women obtain information on livestock feeding techniques from people and organisations such as the horticultural project, women’s associations, and water and forest services. Men get information on soil inputs and soil fertility management from other farmers, organisations, radio and television, and from community leaders, notably the office of the village chief and the mosque on Fridays. The radio is the form of media most commonly used by the women but few women own a radio

    Completeness of information in electronic compared with paper-based patients’ records in a maternity setting in Dakar, Senegal

    Background: Evaluate the consistency of information in paper-based records when registered in parallel with an electronic medical record.Methods: The study was performed at PMSHC in Dakar Senegal. From the end of year 2016, patients’ files were recorded on both paper-based and electronically. Additionally, previous records were electronically registered. To investigate the completeness of records before and after the electronic recording system has been implemented, information about some maternal and fetal/neonatal characteristics were assessed. When the variable was recorded, the system returned 1, unrecorded variables were coded as 0. We then calculated, for each variable, the unrecorded rate before 2017 and after that date. The study period extended from 2011 to June 2019, a nearly ten-year period. Data were extracted from E-perinatal to MS excel 2019 then SPSS 25 software. Frequencies of unrecorded variables were compared with chi-squared test at a level of significance of 5%.Results: A total of 48,270 unique patients’ records were identified during the eight-year period.  Among the study population, data for patients’ age, address and parity were available most of the time before and after 2017 (0.5% missing data versus 0.3% for age and 2.6% versus 1.3% for home address and from 0.3% to 0.0% for parity). However, phone number, maternal weight, maternal height, last menstrual period and blood group were found to be missing up to 96% before 2017. From 2017, these rates experienced a sudden decrease at a significant level: from 82.4% to 27.8% for phone number, from 96% to 56.3% for maternal weight and from 60.1% to 21.3% for blood group. Regarding newborns’ data, it was found that fetal height, head circumference and chest circumference were missing up to just under 25% before 2017. After that date, their completeness improved and flattened under 5%.Conclusions: Structured and computerized files reduce missing data. There is an urgent need the Ministry of health provides hospitals and health care providers with guidelines that describes the standardized information that should be gathered and shared in health and care records

    Hepatocellular carcinoma associated with pregnancy about 2 cases at the gynecological and obstetrical clinic of the Aristide Le Dantec hospital, Dakar, Senegal

    The objective of our study was to report 2 cases of hepatocellular carcinomas associated with pregnancy followed in our structure and to review the literature. Our patients were 30 and 37-year-old multi-gesture females with chronic unattended viral hepatitis B in whom the diagnosis of hepatocellular carcinoma was made in the third trimester of pregnancy at 31 weeks of amenorrhea and 4 days and at 32 weeks of amenorrhea after the incidental finding of tumor hepatomegaly on abdominal-pelvic ultrasound. The main clinical signs were jaundice and hepatomegaly and paraclinical signs were dominated by hepatic cytolysis and anemia in addition to ultrasound images. Follow-up of pregnancies revealed no particularities. A caesarean section was scheduled at 32 weeks of amenorrhea and 32 weeks of amenorrhea and 3 days allowing the birth of two preterm newborns weighing 1210 and 1500 gm with Apgar scores of 8-10/10 and 7-9/10 respectively at the fifth minute. The immediate post-operative follow-up was simple. However, the maternal-fetal prognosis was poor with the death of both patients in a multi-visceral failure table occurring respectively at 6 weeks and 3 weeks after caesarean section. The newborns had died 8 days after birth. Although rare, these two cases challenge any obstetrician to think about liver cancer in pregnant women, especially those with chronic hepatitis B. Ultrasound examination of the liver, or even better, the MRI, which is more efficient, in order to suspect early on a possible liver cancer. Indeed, early diagnosis and a thorough medical approach are essential for the treatment of HCC in pregnant patients

    Aggravation de l’anĂ©mie et polymorphisme de l’haptoglobine au cours de la drĂ©panocytose au SĂ©nĂ©gal

    La drĂ©panocytose homozygote s’accompagne d’une augmentation de l’hĂ©moglobine (Hb) plasmatique, susceptible d’exposer les hĂ©maties Ă  un stress oxydant. L’haptoglobine prĂ©sente trois phĂ©notypes majeurs (Hp1-1, Hp 2-1 et Hp 2-2) susceptibles de fixer l’hĂ©moglobine extracellulaire avec une efficacitĂ© diffĂ©rente. L’objectif de ce travail est de voir si la connaissance du phĂ©notype d’Hp pouvait constituer un Ă©lĂ©ment prĂ©dictifde l’anĂ©mie sĂ©vĂšre. Pour cela, il a Ă©tĂ© recrutĂ© 68 drĂ©panocytaires  homozygotes, ĂągĂ©s de 5 Ă  31 ans. Pour chaque patient, un tĂ©moin de mĂȘme sexe et de mĂȘme Ăąge ± 2 ans a Ă©tĂ© recrutĂ©. Le phĂ©notypage de l’Hp a Ă©tĂ© rĂ©alisĂ© par Ă©lectrophorĂšse sur gel de polyacrylamide. Les rĂ©sultats du dosage de l’Hb font ressortir que les taux d’Hb sont significativement diffĂ©rentes chez les patients comparĂ©es Ă  celles des tĂ©moins (p = 0,001). Lorsque la rĂ©partition a Ă©tĂ© faite en fonction du phĂ©notype d’Hp, une diffĂ©rence statistiquement significative a Ă©tĂ©retrouvĂ©e entre le phĂ©notype Hp1-1 et le phĂ©notype Hp2-2 (p &lt; 0,001) chez les patients et non chez les tĂ©moins. Les rĂ©sultats de cette Ă©tude prĂ©liminaire suggĂ©reraient que la connaissance du phĂ©notype d’Hp seraitun facteur prĂ©dictif de l’anĂ©mie sĂ©vĂšre au cours de la drĂ©panocytose.Mots clĂ©s : DrĂ©panocytose, anĂ©mie, phĂ©notypes d’haptoglobine
