The Influence of Socio-Demographic Factors on Financial Sustainability of Public Services: A Comparative Analysis in Regional Governments and Local Governments
In recent years, financial sustainability (FS) of public policies has become a key concept in
all governmental levels due to the need for ensuring public services delivery for future generations.
Prior research has been focused on the financial sustainability in local governments (LGs) although
its findings suggested the need to analyze this magnitude in other levels of government because
political decisions could be different in each type of public entity. So, FS has also become very
relevant in the Regional Government (RGs) context because their public policies affect not only at
the regional level but also the local one. Therefore, from a comparative approach, this paper seeks
to identify socio-demographic factors which could influence on the financial sustainability, in the
Spanish context for both LGs vs. RGS in order to establish public policies to make sustainable public
goods and services. Findings demonstrate that differences in influential factors between these two
levels of public administration exist: factors such as population size and foreign population could
have an effect on the financial sustainability of both governmental levels while the unemployment
rate, dependent population, and population density affect differently on LGs and RGs
Traumatic pulmonary artery perforation
El drenaje del espacio pleural es una técnica invasiva ampliamente extendida dentro del campo de la medicina intensiva,
y como toda técnica invasiva no se encuentra exenta de riesgos. Llevada a cabo por primera vez por Hipócrates, es a partir
del siglo XX cuando dicha técnica se hace rutinaria para el diagnóstico y tratamiento de aquellas situaciones en las que
se produce acúmulo de aire, sangre, líquido o pus en el espacio pleural. Las causas más frecuentes son el traumatismo
torácico, el neumotórax espontáneo y el derrame pleural (paraneumónico, postoperatorio, etc). Es importante conocer
bien la técnica de inserción y las complicaciones más frecuentes derivadas de ella.The drainage of the pleural space is an invasive technique widely spread in the intensive care medicine, and as any
invasive technique is not free of risks. Carried out for the first time by Hippocrates, it is from the XXth century when
this technique becomes routine for diagnosis and treatment of those situations in which there are accumulations of air,
blood, fluid or pus in the pleural space. The most frequent etiology will be chest trauma, spontaneous pneumothorax
and pleural effusion (parapneumonic, postoperative, etc). It is important to know the technique of insertion and the
most frequent complications derived from it
Analysis of the degree of acquision of competences in Basic Life Support (BLS) by cooperants
Objetivo: Analizar el grado de adquisición de competencias científico / técnicas en soporte vital básico (SVB) por parte de
cooperantesque ejercen su actividad en diferentes organizaciones no gubernamentales (ONGs).
Material y método: Se trata de un estudio observacional, descriptivo y transversal. Se establece como población diana a
un grupo de 95 cooperantes de diversas ONGs sin formación sanitaria previa. La selección fue realizada por parte de UGR
solidaria que a su vez fue la encargada de la organización administrativa del proceso formativo. Se realizó la medición de
un total de 19 variables dicotómicas, 8 relacionadas con la reanimación cardiopulmonarbásica (RCPb), 6 con la obstrucción
de la vía aérea (OVA) y 5 con la desfibrilación externa automatizada (DEA). La formación fue impartida por un grupo de
profesionales sanitarios, todos ellos titulados en soporte vital avanzado (SVA) y / o soporte vital básico (SVB) por parte
del Plan Nacional de RCP (PNRCP). Los parámetros evaluados son los establecidos a nivel internacional por el European
Resuscitation Council (ERC). La evaluación de dichos parámetros fue llevada a cabo por profesionales sanitarios (auditores
independientes) distintos a los que participaron como docentes en la formación de los cooperantes.
Resultados: El grado de adquisición de competencias en SVB por parte de los sujetos participantes en el estudio fue superior al 75 % en todos los casos, adquiriéndose el 100 % de las competencias en 6 de las 19 variables analizadas.
Conclusiones: Según los resultados obtenidos en este estudio, los sujetos participantes en el estudio presentaron un nivel
elevado de adquisición de competencias científico / técnicas en SVB, a pesar de no disponer de formación sanitaria previa
de ningún tipo.Aim: To analyze the degree of acquisition of scientific / technical competences in basic life support (BLS) by cooperants
who work in different non-governmental organizations (NGOs).
Method: It is an observational, descriptive and transversal study. A group of 95 cooperators from different NGOs without
previous health training is established as target population. The selection was made by UGR solidaria that in turn was responsible for the administrative organization of the training process. A total of 19 dichotomous variables were measured,
8 related to basic cardiopulmonary resuscitation (CPRR), 6 to airway obstruction (OVA) and 5 to automated external defibrillation (AED). The training was given by a group of health professionals, all of them qualified in advanced life support
(SVA) and / or basic life support (BLS) by the National Plan of CPR (PNRCP). The parameters evaluated are those established
at the international level by the European Resuscitation Council (ERC). The evaluation of these parameters was carried
out by health professionals (independent auditors) different from those who participated as teachers in the training of
the cooperators.
Results: The degree of acquisition of competencies in BLS by the participant subjects in the study was over 75% in all cases,
with 100% of the competences acquired in 6 of the 19 variables analyzed.
Conclusions: According to the results obtained in this study, the participant subjects in the study had a high level of acquisition of scientific / technical skills in BLS, despite not having any previous health training of any kind
Analysis of the development of the forgetfulness curve within a program of training in Basic Life Support for students of the Degree of Medicine from de University of granada. Pilot study.
Objetivo: El objetivo del estudio es analizar la curva del olvido en el aprendizaje del Soporte Vital Básico en estudiantes
de medicina de la Universidad de Granada.
Material y métodos: Se trata de un estudio observacional, descriptivo transversal, que establece como población
diana a los estudiantes de Grado de Medicina de la Universidad de Granada que cursan cuarto y quinto curso.
Periodo de estudio comprendido entre octubre de 2015 y enero de 2017.Se emplearon los datos obtenidos en una
evaluación inicial que se realizó en la Facultad de Medicina, y los de una reevaluación que se les realizó a una muestra
de cada uno de los cursos. Se incluyeron en el estudio de forma voluntaria todos los alumnos matriculados en
la asignatura bases de la medicina interna II para cada uno de los cursos académicos. El análisis se realizó utilizando
medidas de tendencia central, tablas de frecuencias y posteriormente se realizó un análisis estadístico mediante la
Prueba T de Student.
Resultados: Los alumnos de cuarto consiguieron una media en la primera evaluación de 13,91 sobre 15 puntos,
mientras que en la segunda la media fue de 13,81. Por otro lado, los alumnos de quinto consiguieron una media de
13,80 en la primera evaluación y una de 13,18 en la reevaluación. Estos datos nos hablan de que, a pesar de que
los cursos resultaron muy satisfactorios en ambos cursos, encontramos un mayor impacto de la curva del olvido en
alumnos que llevan más tiempo sin realizar un reciclaje en Soporte Vital Básico.
Conclusiones: A pesar de existir una disminución de los conocimientos y habilidades relacionados con la práctica
del soporte vital al año de recibir la formación específica en ambos grupos, no existen diferencias estadísticamente
significativas para los sujetos de nuestro grupo que obliguen a establecer programas de reciclaje entre seis y nueve
meses como establecen las recomendaciones actuales. A la luz de nuestro estudio, recomendamos el desarrollo
de programas específicos de reciclaje en períodos de tiempo superiores a los doce meses después de recibir la
formación inicial.Aim: The aim of this study was to analyze the oblivion curve after the learning of Basic Life Support (BLS) in medicine
students from the University of Granada.
Method: This was an observational, transversal descriptive study, which established as target population students
from the Faculty of Medicine (University of Granada) who attended fourth and fifth year. The Study period comprised between October 2015 and January 2017. The data which was used was obtained in an initial evaluation that was
done at the Faculty of Medicine and in a re-evaluation which was applied to a selected sample of students from each
year. All the students included voluntarily in the study were enrolled in the subject Internal Medicine II. The analysis
was carried out using measures of central tendency, frequency tables and after that, a statistical analysis using the
Student´s T-test.
Results: Fourth year students obtained an average in the first evaluation of 13,91 over 15 points, while in the second
evaluation it was 13,81. Meanwhile, fifth year students achieved an average of 13,80 in the first evaluation and of
13,18 in the re-evaluation. This data speak of very satisfactory results for students in both years, although the impact
of the oblivion curve is higher in students who have spent more time without taking a recycling course of BLS.
Conclusions:.Even though there is a decrease of knowledge and abilities related to the practice of basic life support
a year after receiving the specific training in both groups, there are not differences statistically significant for the
subjects of our group that would make us establish recycling programs after six or nine months, as advice the actual
recommendations. 2.Based on the results we obtained, we would recommend the development of specific recycling
programs in periods of time bigger than twelve months after receiving the initial training
Prosthetic aortic graft thrombosis by aspergillus infection
Las infecciones de injertos vasculares y protésicos de etiología fúngica son entidades raras, precisan de alta sospecha diagnóstica y conllevan una elevada mortalidad. Presentamos el caso de paciente de 24 años portadora de tubo de dacron supracoronario que presenta trombosis de dicho injerto por Aspergillus flavus.
Aspergillus es un hongo filamentoso cuyo principal medio de transmisión es la inhalación de sus esporas. Presenta predilección por infección de prótesis valvulares izquierdas formando vegetaciones grandes, friables y con alto riesgo embolígeno, entrañando mortalidad de hasta el 80-90%. Como principal complicación destaca la embolización séptica. Difícilmente es detectado en cultivos habituales y tratamiento se basa en terapia antifúngica intensiva y cirugía para desbridamiento.
La ETE se trata de una herramienta de gran rendimiento a pie de cama, aunque difícilmente nos orienta el diagnóstico. Debido al reto que supone su diagnóstico, se necesitan mejorar las herramientas diagnósticas de las que disponemos actualmente.Infection of prosthetic vascular grafts of fungal etiology are rare entities, requiring high diagnostic suspicion and carry a high mortality. We present the case of patient with supracoronary dacron graft with thrombosis by Aspergillus flavus. Aspergillus is a filamentous fungus whose primary mode of transmission is via inhalation of spores. It has a predilection for infection of prosthetic left valves, forming large, friable and high risk of embolism vegetations, with a mortality rate of up to 80-90%. Main complication is septic embolization. It is hardly detected in normal blood cultures. The treatment is based on intensive antifungal therapy and surgery.
In most cases the definitive diagnosis is through analysis of the surgical specimen. The transesophageal echocardiography is an important tool at the bedside, though hardly orients the diagnosis. Due to challenge their diagnosis, it is needed to improve the diagnostic tools we have now
