    Presentation and Symptom Predictors of Coronary Heart Disease in Patients With and Without Diabetes

    The aims of this prospective, observational study were to compare: (1) symptom presentation of coronary heart disease (CHD) between patients with and without diabetes and (2) symptom predictors of CHD in patients with and without diabetes. We directly observed 528 patients with symptoms suggestive of CHD as they presented to the ED of a 900-bed cardiac referral center in the northeastern United States. There were no significant differences in symptom presentation of CHD between patients with and without diabetes, although patients with diabetes were slightly more likely to present with shortness of breath (P =.056). Patients with diabetes reported their symptoms to be more severe compared with those without diabetes (P =.036). Neck/throat pain and arm/shoulder pain were of borderline significance in predicting CHD in patients with diabetes (P =.059 and P =.052, respectively). Classic chest symptoms and diaphoresis were independent predictors of CHD in patients without diabetes (P =.002 and P =.049, respectively). The perceived severity of symptoms was not predictive of CHD in patients with or without diabetes. Symptoms thought to be diagnostic of CHD are not helpful in patients with diabetes. Future research should focus on identifying more useful predictors of CHD in patients with diabetes

    Identification of factors associated with good response to growth hormone therapy in children with short stature: results from the ANSWER Program®

    <p>Abstract</p> <p>Objective</p> <p>To identify factors associated with growth in children on growth hormone (GH) therapy using data from the American Norditropin Studies: Web-enabled Research (ANSWER) Program<sup>® </sup>registry.</p> <p>Methods</p> <p>GH-naïve children with GH deficiency, multiple pituitary hormone deficiency, idiopathic short stature, Turner syndrome, or a history of small for gestational age were eligible (N = 1,002). Using a longitudinal statistical approach, predictive factors were identified in patients with GHD for change from baseline in height standard deviation score (ΔHSDS) following 2 years of treatment.</p> <p>Results</p> <p>Gradual increases in ΔHSDS over time were observed for all diagnostic categories. Significant predictive factors of ΔHSDS, ranked by significance were: height velocity (HV) at 4 months > baseline age > baseline HSDS > baseline body mass index (BMI) SDS > baseline insulin-like growth factor I (IGF-I) SDS; gender was not significant. HV at 4 months and baseline BMI SDS were positively correlated, whereas baseline age, HSDS, and IGF-I SDS were negatively correlated with ΔHSDS.</p> <p>Conclusions</p> <p>These results may help guide GH therapy based on pretreatment characteristics and early growth response.</p

    The present work is to carry out theoretical basis of factors that affect the performance of solar concentration systems, and to present the aplanat as an alternative to solar concentration. A review is made and concepts such as spherical aberration and coma present in most mirrors and lenses. The aplanat, which has historically been used in precision devices such as telescope, and is currently also used as a hub forming free stock photo of spherical aberration and coma then appears. The work is aimed to provide the theoretical basis for the use of aplanatic concentrator and analyze the various configurations of it according to the dimensionless parameters that determine it. The most appropriate settings for the use of aplanático as concentrator are being analysed, and finally the effective concentrations obtained in both aplanatic elliptical and hyperbolic.O presente trabalho consiste em realizar fundamentação teórica de fatores que afetam o desempenho de sistemas de concentração solar, bem como apresentar o aplanático como uma alternativa à concentração solar. É feita uma revisão e conceitos como aberração esférica e coma presentes na maioria dos espelhos e lentes. Em seguida é apresentado o aplanático, que historicamente foi utilizado em dispositivos de precisão como telescópio, e atualmente é utilizado também como concentrador formador de imagem livre de aberração esférica e coma. O trabalho é voltado para fornecer o embasamento teórico para a utilização do aplanático na concentração solar, bem como analisar as diversas configurações dele de acordo com os parâmetros adimensionais que o determinam.     Simple Feedback System for Passive Mode Locked Gyro-Devices at 263 GHz

    A promising new source to generate a periodic series of coherent, ultra-short pulses in the millimeter and submillimeter frequency range is based on the method of passive mode locking [1]. The basic principle is well known from laser physics [2]. A realization for millimeter and sub-millimeter waves consists of an amplifier and a saturable absorber coupled in a feedback loop.     Coherent summation of emission from relativistic Cherenkov sources as a way of production of extremely high-intensity microwave pulses

    For relativistic Cherenkov devices, we investigate the process of high-power microwave pulse generation with its phase correlating to the sharp edge of an e-beam current pulse. Our theoretical consideration is referred to quasi-stationary and superradiative (SR) generation regimes when spontaneous emission of the e-beam edge serves as the seed for the development of further coherent oscillations. Phase correlation of the excited microwave pulses with the characteristics of the current pulse front and/or an initial external electromagnetic pulse has been additionally confirmed by particle-in-cell simulations. Pulse-to-pulse stability of the radiation phase within several percents of the oscillation period makes it possible to arrange multichannel schemes producing mutually coherent microwave pulses. In the experiments that have been carried out, the cathodes of independent generators were powered by identical accelerating pulses from strictly synchronized voltage modulators, or by splitting the pulse from a single powerful modulator. For the 2-ns regime with the power of each Ka-band backward-wave oscillator about 100 MW, we demonstrate quadratic growth of the power density in the interference maximum of the directional diagram. In a short pulse SR regime, with the peak power of 600 MW in a single channel, for a four-channel 2-D array, we attained a 16-fold radiation intensity gain

    A calibrated measure to compare fluctuations of different entities across timescales

However, conclusions based on fluctuations from a single entity can be misleading when used without proper reference to other comparable entities or when examined only on one timescale. Here we introduce a method that uses predictions from a fluctuation scaling law as a benchmark for the observed standard deviations. Differences from the benchmark (residuals) are aggregated across multiple timescales using Principal Component Analysis to reduce data dimensionality. The first component score is a calibrated measure of fluctuations—the reactivityRA of a given entity. We apply our method to activity records from the media industry using data from the Event Registry news aggregator—over 32M articles on selected topics published by over 8000 news outlets. Our approach distinguishes between different news outlet reporting styles: high reactivity points to activity fluctuations larger than expected, reflecting a bursty reporting style, whereas low reactivity suggests a relatively stable reporting style. Combining our method with the political bias detector Media Bias/Fact Check we quantify the relative reporting styles for different topics of mainly US media sources grouped by political orientation.     Los principales parámetros que influyen en la eficiencia de módulo solar orgánico de capa activa P3HT:PCBM, se analizan en este trabajo en condiciones reales de la radiación solar. Los experimentos se llevaron a cabo en la Universidad Federal de Pernambuco en Recife, Pernambuco, Brasil. Por medio del análisis de los resultados se puede observar el comportamiento de los parámetros eléctricos de un módulo orgánico en función de la irradiación y, por tanto, entender cómo funcionaría un sistema que utiliza la electricidad generada a partir de paneles orgánicos.The main parameters that influence the efficiency of organic solar module active layer P3HT:PCBM, are analyzed in this work in real conditions of solar radiation. The experiments were conducted at the Federal University of Pernambuco in Recife, Pernambuco, Brazil.
