    Clinical autonomic nervous system laboratories in Europe: a joint survey of the European Academy of Neurology and the European Federation of Autonomic Societies

The aim of this study was to identify neurology-driven or interdisciplinary clinical ANS laboratories in Europe, describe their characteristics and explore regional differences. Methods: We contacted the European national ANS and neurological societies, as well as members of our professional network, to identify clinical ANS laboratories in each country and invite them to answer a web-based survey. Results: We identified 84 laboratories in 22 countries and 46 (55%) answered the survey. All laboratories perform cardiovascular autonomic function tests, and 83% also perform sweat tests. Testing for catecholamines and autoantibodies are performed in 63% and 56% of laboratories, and epidermal nerve fiber density analysis in 63%. Each laboratory is staffed by a median of two consultants, one resident, one technician and one nurse. The median (interquartile range [IQR]) number of head-up tilt tests/laboratory/year is 105 (49-251). Reflex syncope and neurogenic orthostatic hypotension are the most frequently diagnosed cardiovascular ANS disorders. Thirty-five centers (76%) have an ANS outpatient clinic, with a median (IQR) of 200 (100-360) outpatient visits/year; 42 centers (91%) also offer inpatient care (median 20 [IQR 4-110] inpatient stays/year). Forty-one laboratories (89%) are involved in research activities. We observed a significant difference in the geographical distribution of ANS services among European regions: 11 out of 12 countries from North/West Europe have at least one ANS laboratory versus 11 out of 21 from South/East/Greater Europe (p = 0.021). Methods: We invited 84 European autonomic centers to complete an online survey, recorded the pre-pandemic-to-pandemic percentage of junior participants in the annual congresses of the European Federation of Autonomic Societies (EFAS) and European Academy of Neurology (EAN) and the pre-pandemic-to-pandemic number of PubMed publications on neurological disorders. Results: Forty-six centers answered the survey (55%). Twenty-nine centers were involved in clinical autonomic education and experienced pandemic-related didactic interruptions for 9 (5; 9) months. Ninety percent (n = 26/29) of autonomic educational centers reported a negative impact of the COVID-19 pandemic on education quality, and 93% (n = 27/29) established e-learning models. Both the 2020 joint EAN-EFAS virtual congress and the 2021 (virtual) and 2022 (hybrid) EFAS and EAN congresses marked higher percentages of junior participants than in 2019. Forty-one respondents (89%) were autonomic researchers, and 29 of them reported pandemic-related trial interruptions for 5 (2; 9) months. Since the pandemic begin, almost half of the respondents had less time for scientific writing. Likewise, the number of PubMed publications on autonomic topics showed the smallest increase compared with other neurological fields in 2020-2021 and the highest drop in 2022. Autonomic research centers that amended their trial protocols for telemedicine (38%, n = 16/41) maintained higher clinical caseloads during the first pandemic year. Conclusions: The COVID-19 pandemic had a substantial negative impact on European clinical autonomic education and research.     Direct measurement of the avalanche charge in a resistive plate chambers detector

    Abstract We performed a direct measurement of the avalanche charge with a 10 × 10 cm 2 prototype resistive plate chambers with a 0.2 cm gas gap. We studied its behaviour with respect to the high voltage and its dependence on the presence of the SF 6 in the gas mixture (C 2 H 2 F 4 /i-C 4 H 10 /SF 6 in the ratio 95/4/1 and 96/4/0). The measurements showed a double exponential behaviour of the average avalanche charge with respect to the high voltage. A comparison with other results has been discussed

    Numerosi studi concordano nel rilevare, nei bambini nati pretermine, un aumentato rischio di sviluppare deficit neurologici e difficoltĂ  di natura cognitiva. Il presente studio, che Ăš parte di un progetto di ricerca longitudinale piĂč ampio, indaga lo sviluppo cognitivo, linguistico, comportamentale e socio-emotivo in bambini nati prematuri (N= 20; SG= <33) valutati a 12 e 24 mesi d’etĂ  corretta per prematuritĂ  mediante la somministrazione della Bayley Scales of Infant and Toddler Development - Third Edition (N. Bayley, 2005). È stata inoltre condotta una valutazione dell’assetto psicologico dei genitori in merito al loro vissuto emotivo rispetto alla nascita pretermine con lo scopo di comprendere se e come esso influisca sullo sviluppo cognitivo e comportamentale dei bambini. A tale scopo sono quindi stati somministrati i seguenti questionari: Profile of Mood States (D. McNair, M. Lorr & L.F. Droppleman, 1971) e Parenting Stress Index-Short Form (Abidin, 1995), Child Behavior Checklist for Ages 1 Âœ- 5 (T. M. Achenbach & L.A. Rescorla, 2000).     Paths of cognitive and language development in healthy preterm infants

    Objective: Despite the presence of many studies on difficulties related to premature birth, findings on developmental outcomes are heterogeneous. This could be explained from a biological and environmental point of view, but also from a methodological one. The aims of this study were as follows: assess cognitive and linguistic performance using the BSID-III in a population of healthy preterm infants at 24 and 36 months (corrected age); analyze whether the correction for prematurity should be applied, decide when to stop using corrected age and evaluate possible improvements between 24 and 36 months. Methods: Developmental outcome was assessed at 24 and 36 months (corrected age) with the BSID-III in 75 healthy preterm (GA = 32.5 ± 1.97; BW = 1631.55 ± 453.92) and 69 termborn children (GA = 39.77 ± 1.00; BW = 3298.95 ± 457.27). Results: Preterm infants had significantly lower scores than those of term infants in Cognitive (COG) and Language (LANG REC, LANG EC) scales of the BSID-III at both 24 and 36 months, considering both corrected (CA) and chronological (UCA) age. At 24 months, significant differences between corrected and chronological scores were found for each BSID-III scale, while at 36 months, significant differences between corrected and chronological scores were found for LANG scales. Only the scores in the COG scale were statistically different between 24 and 36 months (F = 4.894, P = 0.009, 2 = 0.075). Considering only the preterm sample at 24 months, the differences between CA and UCA scores in the COG scale were significantly correlated to GA (p = 0.000) and days in hospital (p = 0.002;), while differences between CA and UCA scores in the LANG ESP scale were significantly correlated to GA (p = 0.010), days in hospital (p = 0.001), and birth weight (p = 0.007). At 36 months, no significant correlations were found

    Paediatric anti-N-methyl-D-aspartate receptor encephalitis: The first Italian multicenter case series

    Given the rarity of this condition, especially in children, there is a paucity of large reported paediatric case series of anti-N-methyl-d-aspartate receptor encephalitis.METHODS:To contribute to define the features of this condition, we describe retrospectively a new nationwide case series of 20 children (50% females), referred by 13 Italian centres.RESULTS:Mean age at onset was 8 years (range 3-17). Prodromal symptoms were reported in 31.6%; onset was with neurological symptoms in 70%, and with behavioural/psychiatric disturbances in 30%. Most patients developed a severe clinical picture (90%), and 41% experienced medical complications; children 12-18 years old seemed to be more severe and symptomatic than younger patients. All children received first-line immune therapy; second-line treatment was administered to 45%. Relapses occurred in 15%. At last follow-up (mean 23.9 months, range 5-82), 85% patients had mRS 0-1; this rate was higher among older patients, and in those receiving first immune therapy within 1 month.CONCLUSIONS:Our case series confirms a symptomatologic core of paediatric anti-N-methyl-d-aspartate receptor encephalitis, even though displaying some distinctive features that may be explained by a specific genetic background or by the limited number of patients. The growing incidence of this condition, the relative age-dependent variability of its manifestations, the availability of immunotherapy and the possible better outcome with early treatment impose a high index of clinical suspicion be maintained. In the absence of data suggesting other specific etiologies, paediatricians should consider this diagnosis for children presenting with neurological and/or behavioural or psychiatric disturbances, regardless of age and gender
