    Oxidative status alteration during aerobic-dominant mixed and anaerobic-dominant mixed effort in judokas

    This study aimed to depict the oxidative status variation in judokas during aerobic-dominant mixed effort (AeDME) and anaerobic-dominant mixed effort (AnDME). It is to be expected that the sporting commitment of Judo is a stimulus of oxidative stress leading to the recruitment of antioxidant responses. Blood samples were collected from 17 athletes at rest, immediately after a training session (AeDME) and after a 5-min bout (AnDME). AeDME and AnDME caused significant increases in malondialdehyde (MDA) (p < 0.01 and p < 0.001 respectively) and glutathione (GSH) (p = 0.018 and p < 0.001 respectively). Blood thiol concentrations decreased following AeDME and AnDME (p < 0.001) whilst catalase decreased significantly after AnDME (p = 0.026) only. Uric acid increased significantly after AnDME than after AeDME (p = 0.047) while, conversely, total bilirubin was higher after AnDME than after AeDME (p = 0.02). We may ultimately summarize that AeDME and AnDME caused oxidative stress, higher in AnDME, and some antioxidant response slightly higher in AnDME compared to AeDME. In sports, monitoring of oxidative stress status is recommended as part of the training regimen

    Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P &lt; 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction. Opioid-free discharge analgesia should be adopted routinely

    A New Endophytic Fusarium Oxysporum Gibberellic Acid: Optimization of Production Using Combined Strategies of Experimental Designs and Potency on Tomato Growth under Stress Condition

    This study reports the potential of the endophytic fungi identified as a Fusarium oxysporum to produce gibberellic acid (GA3). The GA3 production was confirmed by high performance liquid chromatography. To improve the production of this phytohormone under solid state fermentation (SSF), successive optimization strategies were used. Firstly, Plackett-Burman design was applied for screening medium components and culture condition. Under the optimized condition, GA3 yield (7.14 g/kg) was 2.62-fold higher than by the use of the initial condition (2.72 g/kg). The concentration of the most influential parameters and their interaction were optimized with a Box-Behnken experimental design. The optimized condition led to a 1.14-fold enhancement in GA3 production, reaching 8.16 g/kg. The GA3 crude extract obtained by SSF was then used to study its ameliorative role on adverse salinity effect on tomato plants (Solanum lycopersicum L.). The interactive effects of different GA3 concentrations were examined on morphological and physiological parameters of tomato plants. The application of GA3 (10-6 M) under salt stress condition (100 mM) was found to improve growth and physiological parameters including plant height, total chlorophyll, starch, and proline contents. The exogenous application of GA3 is a potent strategy to reverse abiotic stress that affect the agricultural productivity and limit plant growth and yield

    Esophageal hirudiniasis: an unusual cause of upper gastrointestinal bleeding

    Leeches are carnivorous, hermaphroditic, segmented worms mainly found in fresh water. The majority of leech attachments are external and short-lasting. Internal surfaces can be involved. Yet, esophageal attachment is very rare. We report a case of a 59-year-old female who presented with hematemesis, melena and chest pain. Upper gastrointestinal endoscopy revealed a round black foreign body in the mid-third of the esophagus identified as a leech. We carefully extracted the worm with a forceps applied to the middle of its body. The diagnosis of leech infestation should be kept in mind when exploring gastrointestinal bleeding in patients living in rural areas and those with poor living conditions

    Overproduction of Glucose Oxidase by Aspergillus tubingensis CTM 507 Randomly Obtained Mutants and Study of Its Insecticidal Activity against Ephestia kuehniella

    In order to enhance the production of glucose oxidase (GOD), random mutagenesis of Aspergillus tubingensis CTM 507 was performed using the chemical and physical mutagens: nitric acid and UV irradiation, respectively. The majority of the isolated mutants showed good GOD production, but only some mutants presented a significant overproduction, as compared with the parent strain. The selected mutants (19 strains), showing an overproduction larger than 200%, are quite stable after three successive subcultures. Among these, six strains revealed an important improvement in submerged fermentation. The insecticidal activity of GOD produced by the wild and the selected mutant strains was evaluated against the third larval instars of E. kuehniella. Mutant strains U11, U12, U20, and U21, presenting the most important effect, displayed an LC50 value of 89.00, 88.51, 80.00, and 86.00 U/cm2, respectively, which was 1.5-fold more important than the wild strain (61 U/cm2). According to histopathology observations, the GOD enzyme showed approximately similar damage on the E. kuehniella midgut including rupture and disintegration of the epithelial layer and cellular vacuolization. The data supports, for the first time, the use of GOD as a pest control agent against E. kuehniella

    Osteopetrosis: a rare case of portal hypertension

    Osteopetrosis is a rare genetic bone disorder characterized by a defect in osteoclasts recruitment and function. Its manifestations are numerous and they mainly include skeletal and dental deformities, cranial nerve entrapment and infections. Over time, osteoclastic expansion invades bone marrow leaving little space for hematopoietic cells. As a result, extramedullary hematopoiesis takes place in the reticular system mainly in the spleen and liver. In these patients, portal hypertension can occur as a result of extramedullary hematopoiesis associated splenomegaly. We are reporting in this article a rare case of spontaneous bacterial peritonitis associated with portal hypertension in a patient with osteopetrosis

    Reliability and Validity of the Arabic Version of the Game Experience Questionnaire: Pilot Questionnaire Study

    Background: Nowadays, digital gaming occupies a central position in the entertainment industry where it has developed into a cherished kind of entertainment in markets all over the world. In addition, it provides other sectors with various social and economic benefits. The Game Experience Questionnaire (GEQ) is a free, quantitative, and comprehensive self-report measure that was developed to assess the player game experience. Despite having been widely used by many research projects in the past, it has not been adapted into Arabic. Furthermore, several components of the scale proved problematic from a psychometric point of view. Therefore, a modified version of the scale is needed to measure the gaming experience of the Arab population. Objective: The aim of this study was to validate and examine the psychometrics of an adapted Arabic version of the GEQ in Tunisia. Methods: A total of 771 volunteer participants completed an online survey, which included an Arabic version of the GEQ, gaming data, and a sociodemographic questionnaire. Subjects were randomized in order to complete two phases of the study: exploratory and confirmatory. The exploratory data were acquired from 360 respondents whose mean age was 23.89 (SD 2.29) years. Out of 360 respondents, 111 (30.8%) were female and 249 (69.2%) were male. Confirmatory data were obtained from the remaining 411 subjects whose mean age was 21.94 (SD 1.80) years. Out of 411 subjects, 169 (41.1%) were female and 242 (58.9%) were male. Results: After the elimination of two items, the exploratory and the confirmatory factor analyses provided an adequate factor structure of the Arabic version of the GEQ. In addition, the internal consistency coefficients suggested the reliability of the instrument. Significant differences were revealed for three subcomponents: flow by age (η2=0.013, P=.002), gender (η2=0.007, P=.02), and game type (η2=0.03, P&lt;.001). For competence (η2=0.01, P=.03) and immersion (η2=0.02, P=.01), significant differences were highlighted by the type of game. The discriminant and convergent validities of the instrument were supported by calculating the average variance extracted (AVE) and comparing the square roots of the AVE values to the correlation coefficients, respectively. Conclusions: The Arabic adapted version of the GEQ is valid and reliable and can be administered to measure the game experience in Arab countries

    The Ethical Compass: Establishing ethical guidelines for research practices in sports medicine and exercise science

    Objective: Research in sports medicine and exercise science has experienced significant growth over recent years. With this expansion, there has been a concomitant rise in ethical challenges specific to these disciplines. While various ethical guidelines exist for numerous scientific fields, a comprehensive set tailored specifically for sports medicine and exercise science is lacking. Aiming to bridge this gap, this paper proposes a comprehensive, updated set of ethical guidelines specifically targeted at researchers in sports medicine and exercise science, providing them with a thorough framework to ensure research integrity. Methods: A collaborative approach was adopted, involving contributions from a diverse group of international experts in the field. A thorough review of existing ethical guidelines was conducted, followed by the identification and detailed examination of 15 specific ethical topics relevant to the discipline. Each topic was discussed in terms of its definition, consequences, and preventive measures. Results: The research in sports medicine and exercise science has grown significantly, bringing to the fore ethical challenges unique to these disciplines. Our comprehensive review identifies 15 key ethical challenges: plagiarism, data falsification, role of artificial intelligence chatbots in academic writing, overstating results, excessive/strategic self-citation, duplicate publications, non-disclosure of conflicts of interest, image manipulation, misuse of peer review, ghost and gift authorship, inadequate data retention, data fabrication, falsification of IRB approvals, lack of informed consent, and unethical human or animal experimentation. For each identified challenge, we propose practical solutions and best practices, enriched by the diverse perspectives of our collaborative international expert panel. This endeavor aims to offer a foundational set of ethical guidelines tailored to the nuanced needs of sports medicine and exercise science, ensuring research integrity and promoting ethical responsibility across these vital fields. Conclusion: This article represents a seminal contribution to the establishment of essential ethical guidelines specifically designed for the fields of sports medicine and exercise science. This article charts a clear course for researchers, clinicians, and policymakers by integrating these ethical principles at the heart of our scholarly and clinical activities. Consequently, it envisions a future where the principles of research integrity and ethical responsibility consistently inform every scientific discovery and every clinical engagement
